Substantial progress has been made in the standardization of nomenclature for paediatric and congenital cardiac care. In 1936, Maude Abbott published her Atlas of Congenital Cardiac Disease, which was the first formal attempt to classify congenital heart disease. The International Paediatric and Congenital Cardiac Code (IPCCC) is now utilized worldwide and has most recently become the paediatric and congenital cardiac component of the Eleventh Revision of the International Classification of Diseases (ICD-11). The most recent publication of the IPCCC was in 2017. This manuscript provides an updated 2021 version of the IPCCC.
The International Society for Nomenclature of Paediatric and Congenital Heart Disease (ISNPCHD), in collaboration with the World Health Organization (WHO), developed the paediatric and congenital cardiac nomenclature that is now within the eleventh version of the International Classification of Diseases (ICD-11). This unification of IPCCC and ICD-11 is the IPCCC ICD-11 Nomenclature and is the first time that the clinical nomenclature for paediatric and congenital cardiac care and the administrative nomenclature for paediatric and congenital cardiac care are harmonized. The resultant congenital cardiac component of ICD-11 was increased from 29 congenital cardiac codes in ICD-9 and 73 congenital cardiac codes in ICD-10 to 318 codes submitted by ISNPCHD through 2018 for incorporation into ICD-11. After these 318 terms were incorporated into ICD-11 in 2018, the WHO ICD-11 team added an additional 49 terms, some of which are acceptable legacy terms from ICD-10, while others provide greater granularity than the ISNPCHD thought was originally acceptable. Thus, the total number of paediatric and congenital cardiac terms in ICD-11 is 367. In this manuscript, we describe and review the terminology, hierarchy, and definitions of the IPCCC ICD-11 Nomenclature. This article, therefore, presents a global system of nomenclature for paediatric and congenital cardiac care that unifies clinical and administrative nomenclature.
The members of ISNPCHD realize that the nomenclature published in this manuscript will continue to evolve. The version of the IPCCC that was published in 2017 has evolved and changed, and it is now replaced by this 2021 version. In the future, ISNPCHD will again publish updated versions of IPCCC, as IPCCC continues to evolve.
Towards establishing no observed adverse effect levels (NOAEL) for different sources of dietary phosphorus in feline adult diets: results from a 7-month feeding study

High dietary phosphorus (P), particularly soluble salts, may contribute to chronic kidney disease development in cats. The aim of the present study was to assess the safety of P supplied at 1 g/1000 kcal (4184kJ) from a highly soluble P salt in P-rich dry format feline diets. Seventy-five healthy adult cats (n 25/group) were fed either a low P control (1·4 g/1000 kcal [4184kJ]; Ca:P ratio 0·97) or one of two test diets with 4 g/1000 kcal (4184 kJ); Ca:P 1·04 or 5 g/1000 kcal (4184kJ); Ca:P 1·27, both incorporating 1 g/1000 kcal (4184 kJ) sodium tripolyphosphate (STPP) – for a period of 30 weeks in a randomised parallel-group study. Health markers in blood and urine, glomerular filtration rate, renal ultrasound and bone density were assessed at baseline and at regular time points. At the end of the test period, responses following transition to a commercial diet (total P – 2·34 g/1000 kcal [4184kJ], Ca:P 1·3) for a 4-week washout period were also assessed. No adverse effects on general, kidney or bone (skeletal) function and health were observed. P and Ca balance, some serum biochemistry parameters and regulatory hormones were increased in cats fed test diets from week 2 onwards (P ≤ 0·05). Data from the washout period suggest that increased serum creatinine and urea values observed in the two test diet groups were influenced by dietary differences during the test period, and not indicative of changes in renal function. The present data suggest no observed adverse effect level for feline diets containing 1 g P/1000 kcal (4184 kJ) from STPP and total P level of up to 5 g/1000 kcal (4184 kJ) when fed for 30 weeks.

Summary
Introduction
The world has an unprecedented opportunity to improve the health and lives of billions of people by adopting practical approaches to meeting the United Nations (UN) Millennium Development Goals. The Task Force on Improving the Lives of Slum Dwellers has identified strategies for managing a projected near doubling of the urban population over the next three decades. We contend that this challenge can be met if local authorities and national governments work closely with the urban poor through open and participatory processes that give them a voice in decisions about the infrastructure and public services that affect their lives. Building on earlier work that aims to reconnect public health and urban planning, we identify four core themes that we consider as transferable best principles, which will be essential in bringing policies and programmes up to scale to meet the 21st-century health challenge of slums and cities: interdisciplinary engagement, gender equality, sustainable development and democratic institution building. By developing policies and programmes that are pro-poor, the health and lives of all of us stand to improve. Indeed, our best hope for reinventing healthy and sustainable communities is to place the urban poor at the very centre of policy formation and investment processes. In this way, the outcomes – healthy populations and sustainable communities – will take care of themselves.
Interdisciplinary scholarship and training
This chapter is written in tribute to our students at Columbia University, and by extension, to all past, current and future students who are committed to answering societies’ big questions (for example, ‘Will the poor always be with us?’ – see Darity, 2003) and solving the world's big problems (for example, achieving the UN Millennium Development Goals). Our ideas have been incubated in the course entitled ‘Interdisciplinary Planning for Health’ that two of us (Mary E. Northridge and Elliott D. Sclar) have co-taught over the past nine years and enhanced by the contributions of successive waves of students. Indeed, the three other coauthors were all former students on this course and conducted apt research used in conceiving our ideas and birthing this chapter (Emily Karpel Kurtz in 2004, Maryann Z. Fiebach in 2005 and Annie Feighery in 2006).

Objective
We hypothesized that young adults would commonly have vitamin D deficiency and that a questionnaire could help identify subjects with the condition.
DesignBetween January and May 2004, we administered a questionnaire to a convenience sample of young adults. We measured each participant’s serum level of 25-hydroxyvitamin D (25(OH)D) using a chemiluminescent assay and defined deficiency as serum 25(OH)D < 16 ng/ml.
Setting and subjectsWe recruited young adults living in Madison, Wisconsin without pre-existing conditions affecting vitamin D and/or Ca metabolism.
ResultsOne hundred and eighty-four adults (mean age 24 years, 53 % women, 90 % Caucasian) participated in the study. Nearly three in four adults (71 %) had 25(OH)D level <30 ng/ml and 26 % were vitamin D-deficient. In multivariate analysis, persons reporting a suntan (OR = 0·24, 95 % CI 0·09, 0·63, P = 0·004), tanning booth use (OR = 0·09, 95 % CI 0·02, 0·43, P = 0·002) and daily ingestion of two or more servings of milk (OR = 0·21, 95 % CI 0·09, 0·48, P < 0·001) were less likely to be deficient. These three questions provided a sensitivity and specificity of 79 % and 78 %, respectively, for the presence of deficiency.
ConclusionsThe questionnaire is moderately useful to identify young adults likely to be vitamin D-deficient. Additional revisions of the questionnaire may improve its ability to predict vitamin D deficiency.
