Reduced attention-driven auditory sensitivity in hallucination-prone individuals

Background
Evidence suggests that auditory hallucinations may result from abnormally enhanced auditory sensitivity.
AimsTo investigate whether there is an auditory processing bias in healthy individuals who are prone to experiencing auditory hallucinations.
MethodTwo hundred healthy volunteers performed a temporal order judgement task in which they determined whether an auditory or a visual stimulus came first under conditions of directed attention (‘attend-auditory’ and ‘attend-visual’ conditions). The Launay–Slade Hallucination Scale was used to divide the sample into high and low hallucination-proneness groups.
ResultsThe high hallucination-proneness group exhibited a reduced sensitivity to auditory stimuli under the attend-auditory condition. By contrast, attention-directed visual sensitivity did not differ significantly between groups.
ConclusionsHealthy individuals prone to hallucinatory experiences may possess a bias in attention towards internal auditory stimuli at the expense of external sounds. Interventions involving the redistribution of attentional resources would have therapeutic benefit in patients experiencing auditory hallucinations.

Preliminary Performance Assessment for Deep Borehole Disposal of High-Level Radioactive Waste

Deep boreholes have been proposed for many decades as an option for permanent disposal of high-level radioactive waste and spent nuclear fuel. Disposal concepts are straightforward, and generally call for drilling boreholes to a depth of four to five kilometers (or more) into crystalline basement rocks. Waste is placed in the lower portion of the hole, and the upper several kilometers of the hole are sealed to provide effective isolation from the biosphere. The potential for excellent long-term performance has been recognized in many previous studies. This paper reports updated results of what is believed to be the first quantitative analysis of releases from a hypothetical disposal borehole repository using the same performance assessment methodology applied to mined geologic repositories for high-level radioactive waste. Analyses begin with a preliminary consideration of a comprehensive list of potentially relevant features, events, and processes (FEPs) and the identification of those FEPs that appear to be most likely to affect long-term performance in deep boreholes. The release pathway selected for preliminary performance assessment modeling is thermally-driven flow and radionuclide transport upwards from the emplacement zone through the borehole seals or the surrounding annulus of disturbed rock. Estimated radionuclide releases from deep borehole disposal of spent nuclear fuel, and the annual radiation doses to hypothetical future humans associated with those releases, are extremely small, indicating that deep boreholes may be a viable alternative to mined repositories for disposal of both high-level radioactive waste and spent nuclear fuel.

Summary
A HISTORICAL OUTLINE OF HONG KONG
Hong Kong is situated at the southeastern tip of the Chinese mainland. Hong Kong Island, Kowloon, the outlying islands, and the New Territories are collectively included as “Hong Kong,” spanning an area of 1,100 km2. Hong Kong became a British colony in 1841 after the Opium War. It began with a population of 3,650 scattered over 20 villages, and 2,000 fishermen living aboard their boats. Hong Kong received an unparalleled influx of immigrants from Guangdong and Shanghai in 1948–1949 after the defeat of the Chinese Nationalist Government. By mid-1950, the population had swelled to 2.2 million, and it currently stands at 6.8 million (Information Services Department of Hong Kong SAR, People's Republic of China, 2002b). In 1997, Hong Kong was re-united with Mainland China as a Special Administrative Region (SAR) under the one-country–two-systems principle. Hong Kong was promised a continuation of its pre-existing capitalistic and political system for fifty years.
ECOLOGICAL FEATURES
Mountains dominate Hong Kong's natural terrain. The main island of Hong Kong and nearby Kowloon peninsula are populated with high-rise residential and office buildings. Rural and farming land in the New Territories are increasingly rebuilt into residential complexes, giving rise to new towns. Hong Kong is linked with good roads, extensive railway and underground networks, and efficient public transport services. Communication and travel within Hong Kong can be made with ease.

Nutrigenomics is the study of how constituents of the diet interact with genes, and their products, to alter phenotype and, conversely, how genes and their products metabolise these constituents into nutrients, antinutrients, and bioactive compounds. Results from molecular and genetic epidemiological studies indicate that dietary unbalance can alter gene–nutrient interactions in ways that increase the risk of developing chronic disease. The interplay of human genetic variation and environmental factors will make identifying causative genes and nutrients a formidable, but not intractable, challenge. We provide specific recommendations for how to best meet this challenge and discuss the need for new methodologies and the use of comprehensive analyses of nutrient–genotype interactions involving large and diverse populations. The objective of the present paper is to stimulate discourse and collaboration among nutrigenomic researchers and stakeholders, a process that will lead to an increase in global health and wellness by reducing health disparities in developed and developing countries.

Background
Two studies tested whether subjects with obsessive-compulsive disorder could successfully use BT STEPS, a computer-aided system, to perform self-assessment for self-treatment of obsessive-compulsive disorder by exposure and ritual prevention.
MethodSubjects were given a self-guiding manual and could use a touch-tone telephone to access computer-controlled Interactive Voice Response interviews at their convenience from home. Using the BT STEPS system, patients rated themselves and worked out a plan for individually tailored self-exposure therapy.
ResultsOutcomes were similar in the two studies. Of the 63 subjects who used BT STEPS, 84% completed the self-assessment module. Most calls were made outside usual office hours. As expected, subjects did not improve merely by completing self-assessment. However, completion of self-assessment predicted later improvement with self-exposure therapy.
ConclusionsMost subjects successfully completed self-assessment using BT STEPS from their homes.

Psychological management is a crucial part of the overall treatment plan involving therapeutic amputations in children. Therapeutic work aims to facilitate the implementation of a coordinated treatment plan taking into account the patient's physical and psychological needs. Psychological contact at the early stage is advocated to minimize the patient's distress and to forestall potentially handicapping emotional disturbances. The paper highlights important issues at different phases of the patient's treatment, including coming to terms with the initial diagnosis, acceptance of the amputation plan, and the final process of rehabilitation and normal life adjustment. Psychological interventions with one adolescent girl treated with therapeutic amputation for osteosarcoma are used to illustrate the therapeutic process.

The experience and perception of effective sources of help of 101 schizophrenic patients were studied. A combination of professional help, social support, and self-coping efforts was listed as helpful. The more experiences patients had with the various sources of help, the better were their outcomes. Premorbid adjustment, exposure to the magnitude and nature of helpful sources, as well as the patients' own ability and motivation to make use of available helpful sources were thought to be important mediating variables for a better outcome. Apart from psychotropic medications, over half of the patients were not using any one of the sources of help mentioned. It remains an important research question whether or not schizophrenic patients may benefit from direct coaching in self-help skills in conjunction with making optimal use of professional and environmental supports.
