Feeding after congenital heart surgery: a mixed-methods study of the caregiver experience

Objectives:
Feeding difficulties after congenital heart surgery are a common concern for caregivers of children with CHD. Insight into the intricacies of their experience is lacking. With a better understanding, healthcare providers can continue to optimize the approach and support mechanisms for these families. This study will explore the psychosocial impacts on caregivers, define barriers to care, and identify areas to improve their care.
Study Design:This mixed-methods study combined semi-structured interviews with surveys. Purposive sampling targeted caregivers of a child who underwent heart surgery and was discharged with alternative enteral feeding access. A hybrid inductive-deductive methodology was used to analyse interview transcripts. Survey scores were compared to interview content for concordance.
Results:Fifteen interviews were conducted with socio-demographically diverse caregivers. Feeding difficulties were often identified as their greatest challenge, with the laborious feeding schedule, sleep deprivation, and tube management being common contributors. Most caregivers described feeling overwhelmed and worried. Time-intensive feeding schedules and lack of appropriate childcare options precluded caregivers’ ability to work. Barriers to care included imperfect feeding education, proximity of specialist clinics, and issues with medical supply companies. Caregiver proposals for improved care addressed easing the transition home, improving emotional support mechanisms, and intensifying feeding therapy for expedited tube removal.
Conclusion:This study describes the psychosocial toll on the caregiver, typical barriers to care, and ideas for improved provision of care. These themes and ideas can be used to advance the family-centered approach to feeding difficulties after heart surgery.

Biosensors based on lipid membranes promise an inexpensive and versatile platform for application in many fields of molecular sensing. An extensive review of the applications for tethered membranes was reported in the July 2006 MRS Bulletin [A.N. Parikh and J.T. Groves, Materials science of supported lipid membranes. MRS Bull.31(8), 507 (2006)]. The commercial use to which tethered lipid membranes have been applied has been limited by their stability under long-term storage. This report describes a novel membrane construct that is stable at room temperature for months, eliminates the mobile lipid phase present in lipid bilayers, and is robust against detergents under conditions that would destroy a lipid bilayer.
Gene silencing using micro-RNA designed hairpins

During RNA interference (RNAi), long dsRNA is processed to ∼21 nt duplexes, short interfering RNAs (siRNAs), which silence genes through a mRNA degradation pathway. Small temporal RNAs (stRNAs) and micro-RNAs (miRNAs) are ∼21 nt RNAs that are processed from endogenously encoded hairpin-structured precursors, and function to silence genes via translational repression. Here we report that synthetic hairpin RNAs that mimic siRNAs and miRNA precursor molecules can target a gene for silencing, and the mechanism of silencing appears to be through mRNA degradation and not translational repression. The sequence and structural configuration of these RNAs are important, and even slight modification in structure can affect the silencing activity of the hairpins. Furthermore, these RNAs are active when expressed by DNA vectors containing polymerase III promoters, opening the possibility for new approaches in stable RNAi-based loss of function studies.

In vitro selection was used to sample snRNA-related sequences for ribozyme activities, and several 2′,5′-branch-forming ribozymes were isolated. One such ribozyme is highly dependent upon an 11-nt motif that contains a conserved U6 snRNA sequence (ACAGAGA-box) known to be important for pre-mRNA splicing. The ribozyme reaction is similar to the first step of splicing in that an internal 2′-hydroxyl of an unpaired adenosine attacks at the 5′-phosphate of a guanosine. It differs in that the leaving group is diphosphate rather than a 5′ exon. The finding that lariat formation can be accomplished by a small RNA with sequences related to U6 snRNA indicates that the RNA available in the spliceosome may be involved in RNA-catalyzed branch formation.

The SRm160/300 splicing coactivator, which consists of the serine/arginine (SR)-related nuclear matrix protein of 160 kDa and a 300-kDa nuclear matrix antigen, functions in splicing by promoting critical interactions between splicing factors bound to pre-mRNA, including snRNPs and SR family proteins. In this article we report the isolation of a cDNA encoding the 300-kDa antigen and investigate the activity of it and SRm160 in splicing. Like SRm160, the 300-kDa antigen contains domains rich in alternating S and R residues but lacks an RNA recognition motif; the protein is accordingly named “SRm300.” SRm300 also contains a novel and highly conserved N-terminal domain, several unique repeated motifs rich in S, R, and proline residues, and two very long polyserine tracts. Surprisingly, specific depletion of SRm300 does not prevent the splicing of pre-mRNAs shown previously to require SRm160/300. Addition of recombinant SRm160 alone to SRm160/300-depleted reactions specifically activates splicing. The results indicate that SRm160 may be the more critical component of the SRm160/300 coactivator in the splicing of SRm160/300-dependent pre-mRNAs.
PUF60: A novel U2AF65-related splicing activity

We have identified a new pyrimidine-tract binding factor, PUF, that is required, together with U2AF, for efficient reconstitution of RNA splicing in vitro. The activity has been purified and consists of two proteins, PUF60 and the previously described splicing factor p54. p54 and PUF60 form a stable complex in vitro when cotranslated in a reaction mixture. PUF activity, in conjunction with U2AF, facilitates the association of U2 snRNP with the pre-mRNA. This reaction is dependent upon the presence of the large subunit of U2AF, U2AF65, but not the small subunit U2AF35. PUF60 is homologous to both U2AF65 and the yeast splicing factor Mud2p. The C-terminal domain of PUF60, the PUMP domain, is distantly related to the RNA-recognition motif domain, and is probably important in protein–protein interactions.

In the report of the Association's Committee on War-time Services occurs the following passage: “It seems to the Committee that the customary individualism of the profession is a luxury that cannot be unimpaired in war-time; political scientists must not go through the war with a business-as-usual attitude toward research and critical writing. The crises upon the nation and awaiting the nation demand that the profession recognize priorities in its scholarly work…. Students, mature and immature, should know what men of affairs consider to be the more crucial issues … The Committee … does ask … that the profession be given leadership in determining what to do ….”
The Research Committee of the Association considered this challenge and sought an answer from those members of the profession who had temporarily left their academic halls and plunged into the war effort in Washington. This group gave generously of their time and thought to the matter. The Committee's own function became merely that of a reporter or synthesizer of the views thus expressed. It is this synthesis which this statement incorporates. The suggestions are deliberately not attributed to any one individual. In the first place, many suggestions were made by more than one person; in the second place, the total pattern is even more intriguing than the individual suggestions.
