Outbreak of hepatitis B and hepatitis C virus infections associated with a cardiology clinic, West Virginia, 2012–2014

Objective:
To stop transmission of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections in association with myocardial perfusion imaging (MPI) at a cardiology clinic.
Design:Outbreak investigation and quasispecies analysis of HCV hypervariable region 1 genome.
Setting:Outpatient cardiology clinic.
Patients:Patients undergoing MPI.
Methods:Case patients met definitions for HBV or HCV infection. Cases were identified through surveillance registry cross-matching against clinic records and serological screening. Observations of clinic practices were performed.
Results:During 2012–2014, 7 cases of HCV and 4 cases of HBV occurred in 4 distinct clusters among patients at a cardiology clinic. Among 3 case patients with HCV infection who had MPI on June 25, 2014, 2 had 98.48% genetic identity of HCV RNA. Among 4 case patients with HCV infection who had MPI on March 13, 2014, 3 had 96.96%–99.24% molecular identity of HCV RNA. Also, 2 clusters of 2 patients each with HBV infection had MPI on March 7, 2012, and December 4, 2014. Clinic staff reused saline vials for >1 patient. No infection control breaches were identified at the compounding pharmacy that supplied the clinic. Patients seen in clinic through March 27, 2015, were encouraged to seek testing for HBV, HCV, and human immunodeficiency virus. The clinic switched to all single-dose medications and single-use intravenous flushes on March 27, 2015, and no further cases were identified.
Conclusions:This prolonged healthcare-associated outbreak of HBV and HCV was most likely related to breaches in injection safety. Providers should follow injection safety guidelines in all practice settings.

A new method of electromagnetic sounding in resistive electrical environments has been developed for use in lunar exploration. It is applicable to the study of terrestrial glaciers and ice sheets. A horizontal electric dipole antenna on the ground is used to transmit power at frequencies of 1, 2, 4, 8, 16 and 32 MHz. A set of orthogonal receiving coils is mounted on a vehicle which traverses away from the transmitter. Field strength is recorded as a function of distance. Waves which travel above the surface interfere with waves from the subsurface, generating interference patterns which can be used to determine the dielectric constant, the loss tangent, and depth to reflecting horizons.
The technique was tested on the Athabasca Glacier in western Canada. At 1, 2 and 4 MHz the ice was found to have a dielectric constant of about 3.3, a loss tangent (tan δ) which is roughly inversely proportional to frequency giving values of f tan δ in the range 0.25 to 0.35 (where f is in MHz). These values correspond well with the known properties of ice near 0° C, which is a temperature typical of temperate glaciers. It has been possible to determine the depth of the ice but results are not always consistent with previous seismic and gravity surveys and with drilling. At frequencies of 16 and 32 MHz, scattering is the dominant feature of the results. At 8 MHz there is a transition from clear-cut interference patterns to the scattering patterns. From these findings, we suggest that the Athabasca Glacier has a large number of dielectric scatterers with dimensions less than about 35 m, probably due in large part to crevasses.

Strontium-isotopic ratios of dated corals have been obtained from submerged reefs formed during Quaternary glacial periods off the Hawaiian islands. These data, combined with data from deep-sea sediments, tightly constrain the secular variation of marine 87Sr/86Sr for the past 800,000 yr. Although long-term trends are apparent, no significant (>0.02‰), rapid (<100,000 yr) excursions in 87Sr/86Sr were resolved nor did we observe any samples with 87Sr/86Sr greater than that of modern seawater. Strontium in mollusks from elevated marine terraces formed during interglacial periods on the southern California coast show resolvable and consistent variations in 87Sr/86Sr which, when compared to the trend of Quaternary marine 87Sr/86Sr, can be used to infer uplift rates and define approximate ages for the higher terraces. The Sr-isotope age estimates indicate that uplift rates vary among crustal blocks and were not necessarily constant with time. No contrast in Sr-isotopic ratios between similar-age Hawaiian and California fossils was observed, confirming that any change in marine 87Sr/86Sr from glacial to interglacial periods must be small. A realistic appraisal of the potential of Sr-isotope stratigraphy for chronometric applications in the Quaternary suggests that the technique will be limited to relatively coarse distinctions in age.

In his recent lecture on “ Airship Mooring and Handling,” Captain Butcher refers to certain experiments carried out with the use of models in connection with three-wire mooring. It is, perhaps, useful to add some further details of these special experiments, which were conducted at the National Physical Laboratory.
Mention was made by Captain Butcher of the original or “ fixed ” three-wire system (see Fig. 1), which originated from the emergency mooring of R.9. Its defects lie in the occasional slackening and subsequent impulsive tightening of one or more of the mooring cables when the airship is riding in gusty weather. In the particular modifications introduced by the N.P.L., breakdown due to this contingency is eliminated.
The roles of actuaries in UK life offices: changes and challenges

We examine the roles of actuaries in UK life offices, along with trends, challenges to and opportunities for actuaries. We carry out an analysis of senior roles in life offices, a questionnaire survey and interviews with relevant senior personnel. We find that actuaries occupy many important roles in life offices and are regarded as having good industry knowledge and technical skills, especially in financial modelling. There are fewer executive directors and more non-executive directors of life offices who are actuaries compared with the position in 1990. A higher proportion of reserved roles is outsourced to consultants than was the case in 1990. Only a small number of Actuarial Function Holders are directors. Actuaries are more siloed than was the case in the past, although actuaries are well represented in the finance and risk functions of many offices. Although actuarial work in connection with the preparation for Solvency II will decline, there will be important ongoing requirements for actuaries following Solvency II implementation. We also see opportunities for actuaries in four areas: in risk management, in financial analysis and management based on Solvency II and international financial reporting standards, in connection with “big data”, and in product development and the customer proposition. There are implications for the examination syllabus, continuing professional development and research.

The human malaria parasite, Plasmodium falciparum, is currently being actively studied by molecular biologists. It is hoped that the use of recombinant DNA techniques in this area will give new insights into the biology of the organism and, at the same time, provide new approaches to diagnosis and vaccine development.
Our own studies employ the blood stages of the parasite and cover three main areas: enzymes of importance in parasite metabolism; antigens of potential use in a subunit vaccine; and repetitive DNA as a probe able to distinguish genetically different isolates of P. falciparum and as a species-specific diagnostic tool in human and mosquito infections.

The genus Sequoia has existed since Mesozoic times, and early Tertiary Redwoods occurred practically throughout the northern hemisphere. Today, the Coast Redwood (Sequoia sempervirens) is the only living species of the genus, and it is restricted to the coastal environments of California and the southern extremity of Oregon. Climatic factors determine the outer limits of Redwood distribution, while edaphic, biotic, and hydrologic, variables influence the occurrence of Redwood within the climatic range.
Human use of Redwood forests has altered the environmental conditions under which the Sequoia now grows. Logging has exposed the forest floor to increased light and higher temperatures, while in some places it has exposed the soil to accelerated erosion. Recent floods in the major watersheds of north-western California have been particularly disastrous in undercutting large trees along the river banks. Increased degradation rates on upper slopes have resulted in delivery of coarse sediments to floodplains, with possibly deleterious effects on living old-growth Redwoods.

Background
People with Down's syndrome (DS) are at high risk for developing dementia in middle age. The biological basis for this is unknown. It has been proposed that non-demented adults with DS may undergo accelerated brain ageing.
MethodWe used volumetric magnetic resonance imaging (MRI) and manual tracing to compare brain anatomy and ageing in 39 non-demented adults with DS and 42 healthy controls.
ResultsIndividuals with DS had significant differences in brain anatomy. Furthermore, individuals with DS had a significantly greater age-related reduction in volume of frontal, temporal and parietal lobes, and a significantly greater age-related increase in volume of peripheral cerebrospinal fluid (CSF).
ConclusionsNon-demented adults with DS have differences in brain anatomy and ‘accelerated’ ageing of some brain regions. This may increase their risk for age-related cognitive decline and Alzheimer's disease (AD).
Psychosocial predictors of vulnerability to distress in the year following heart transplantation

This study examines psychological symptomatology in a cohort of 72 heart transplant recipients followed longitudinally during their first year post-transplant. In keeping with research on other domains of life stressors and illnesses, a central study goal was to identify pre-transplant and perioperative psychosocial factors associated with increased vulnerability to, and maintenance of, elevated psychological distress levels post-transplant. Average anxiety and depression levels, but not anger–hostility symptoms, were substantially elevated in the early post-transplant period, relative to normative data. Average symptom levels improved significantly over time, although one-third of the sample continued to have high distress levels at all follow-up assessments. Recipients with any of seven psychosocial characteristics at initial interview were particularly susceptible to continued high average distress levels over time: a personal history of psychiatric disorder prior to transplant; younger age; lower social support from their primary family caregiver; exposure to recent major life events involving loss; poor self-esteem; a poor sense of mastery; and use of avoidance coping strategies to manage health problems. Recipients without such factors showed improvement in average distress levels across the assessment period. These effects were stronger for anxiety than depressive symptoms, with the exception of a sizeable relationship between loss events and subsequent depression. The findings suggest that clinical interventions designed to minimize prolonged emotional distress post-transplant need to be closely tailored to heart recipients' initial psychosocial assets and liabilities.

Background
Individuals with Down's syndrome (DS) are at high risk of developing Alzheimer's disease (AD). However, few studies have investigated brain anatomy in DS individuals with AD.
MethodWe compared whole brain anatomy, as measured by volumetric magnetic resonance imaging (MRI), in DS individuals with and without AD. We also investigated whether volumetric differences could reliably classify DS individuals according to AD status. We used volumetric MRI and manual tracing to examine regional brain anatomy in 19 DS adults with AD and 39 DS adults without AD.
ResultsDS individuals with AD had significantly smaller corrected volumes bilaterally of the hippocampus and caudate, and right amygdala and putamen, and a significantly larger corrected volume of left peripheral cerebrospinal fluid (CSF), compared to DS individuals without AD. The volume of the hippocampus and caudate nucleus correctly categorized 92% and 92% respectively of DS individuals without AD, and 75% and 80% respectively of DS individuals with AD.
ConclusionsDS individuals with AD have significant medial temporal and striatal volume reductions, and these may provide markers of clinical AD.
Sesame seed products contaminated with Salmonella: three outbreaks associated with tahini

In November 2002, the first of three outbreaks of Salmonella Montevideo infection in Australia and New Zealand was identified in New South Wales, Australia. Affected persons were interviewed, and epidemiologically linked retail outlets inspected. Imported tahini was rapidly identified as the source of infection. The contaminated tahini was recalled and international alerts posted. A second outbreak was identified in Australia in June–July 2003 and another in New Zealand in August 2003. In a total of 68 S. Montevideo infections, 66 cases were contacted. Fifty-four (82%) reported consumption of sesame seed-based foods. Laboratory analyses demonstrated closely related PFGE patterns in the S. Montevideo isolates from human cases and sesame-based foods imported from two countries. On the basis of our investigations sesame-based products were sampled in other jurisdictions and three products in Canada and one in the United Kingdom were positive for Salmonella spp., demonstrating the value of international alerts when food products have a wide distribution and a long shelf life. A review of the controls for Salmonella spp. during the production of sesame-based products is recommended.

This paper outlines our current approach to utilize infrared reflectance spectroscopy for thin film measurement in the semiconductor industry. The multi-layer thickness and doping concentration of IC wafers can be determined by a single angle, unpolarized infrared reflectance measurement performed using Fourier transform infrared spectrometer. A computer algorithm, which matches theoretical tc measured infrared reflectance spectra, was successfully employed to determine multiple thin film properties.

Alloying transition metal trialuminides such as Al3Nb with appropriate ternary elements may stabilize the ordered cubic Ll2 structure relative to the DO22 structure. This may enhance the symmetry related contributions to plastic flow, thereby aiding in overcoming the low ductility in the trialuminide systems. The present investigation was directed towards exploring the potential for obtaining new ternary Al3Nb-based intermetallics with the Ll2 structure. The effects of various ternary elements on the phase stability of A13Nb were studied by SEM, XRD, and EPMA. Results of the A13 Nb-X phase relationship studies are presented. Preliminary microhardness results on the ternary DO22 Al3Nb-X phases are also reported. The predictive capabilities of Pettifor's structural stability scheme are evaluated in the light of the present investigation.
