A practical risk calculator for suicidal behavior among transitioning U.S. Army soldiers: results from the Study to Assess Risk and Resilience in Servicemembers-Longitudinal Study (STARRS-LS)

Background
Risk of suicide-related behaviors is elevated among military personnel transitioning to civilian life. An earlier report showed that high-risk U.S. Army soldiers could be identified shortly before this transition with a machine learning model that included predictors from administrative systems, self-report surveys, and geospatial data. Based on this result, a Veterans Affairs and Army initiative was launched to evaluate a suicide-prevention intervention for high-risk transitioning soldiers. To make targeting practical, though, a streamlined model and risk calculator were needed that used only a short series of self-report survey questions.
MethodsWe revised the original model in a sample of n = 8335 observations from the Study to Assess Risk and Resilience in Servicemembers-Longitudinal Study (STARRS-LS) who participated in one of three Army STARRS 2011–2014 baseline surveys while in service and in one or more subsequent panel surveys (LS1: 2016–2018, LS2: 2018–2019) after leaving service. We trained ensemble machine learning models with constrained numbers of item-level survey predictors in a 70% training sample. The outcome was self-reported post-transition suicide attempts (SA). The models were validated in the 30% test sample.
ResultsTwelve-month post-transition SA prevalence was 1.0% (s.e. = 0.1). The best constrained model, with only 17 predictors, had a test sample ROC-AUC of 0.85 (s.e. = 0.03). The 10–30% of respondents with the highest predicted risk included 44.9–92.5% of 12-month SAs.
ConclusionsAn accurate SA risk calculator based on a short self-report survey can target transitioning soldiers shortly before leaving service for intervention to prevent post-transition SA.

Summary
Philip Rieff's (1922– 2006) intellectual and cultural inheritance derives from a variety of sources. For example, he was not a religious Jew and mentions of the Holocaust hardly make an appearance in his early work. Yet increasingly “Auschwitz” takes on great moral and cultural significance in his late writings. Rieff made his intellectual name with an intellectual biography of Sigmund Freud, co- written with his first wife Susan Sontag. It still stands as a major achievement, even if his own attitude toward Freud moved from admiration to disapproval over the years. As a sociologist, Rieff was influenced by Emile Durkheim's emphasis on the relationship between a society's cohesion and its religious ethos, not to mention Max Weber's writings on authority and his idea of modernity as intertwined with secularization, a special preoccupation of modern German thought. The philosophy of culture hammered out by Friedrich Nietzsche was also of fundamental importance for Rieff's later work, particularly, of course, for his “death of God” anti- theology. Besides that Rieff was steeped in the advanced art and culture of the twentieth century. In this he was a product of an eclectic and pluralist modernist culture.
But as a theorist of culture, Rieff was indifferent to the philosophical tradition of Greece as it has shaped modern thought, particularly German thought. There's no evidence that Rieff accepted Martin Heidegger's version of Western thought as Seinsvergessenheit (the forgetting of Being). Though he admired Hannah Arendt as “one of the most trustworthy guides to our time and its past,” he never really responded to her claim that the history of Western political ideas was the story of the loss of authentic political thinking. And unlike another (adopted) American conservative, Leo Strauss, with whom he is sometimes compared, Rieff did not choose Athens over Jerusalem, rationality over revelation, natural law over God's commandments. Unlike both Strauss and Arendt, he was largely dismissive of politics, which he considered to be the pursuit of crass material interests and power grabbing. Still, in his late writings, he identified the Hellenic origins of the West with what he called the “first world,” one founded on “taboo” and guided by “fate.” The third world, which we the living allegedly now inhabit, sees the world in terms of the “primacy of possibility” (aka POP), and it is guided by “rules” that regulate the roles the self continually tries on and sheds.

It is easy to forget that interest in the intellectual history of the US South only fully emerged in the post-1960s years. Though there have been no outstanding southern philosophers or philosophers of the South to focus on, there have been plenty of talented literary and cultural critics and political and social thinkers, as well as historians, political scientists, and sociologists, whose work has significantly shaped the idea of the South and who thus deserve the interest of the intellectual historian. Works as varied as Clement Eaton's The Freedom-of-Thought Struggle in the Old South (1940) Rollin Osterweis's Romanticism and Nationalism in the Old South (1949), and William R. Taylor's Cavalier and Yankee (1961) were early examples of the effort to identify the intellectual and cultural issues central to the history of the region. By exploring the literary history of the South in the interwar years, what came to be called the Southern Renaissance, post-1945 literary historians such as Louis D. Rubin and Lewis Simpson undoubtedly also influenced those who tried to make sense of the life of the mind below the Mason–Dixon line.

The tradition of civil disobedience in America seems to be in pretty good health. Recent examples, including the Occupy movement, disruption of the functioning of abortion clinics, and even the release of classified government documents by Edward Snowden and Chelsea Manning, suggest that it is alive and kicking. Yet a consideration of these examples also re-enforces how fuzzy the edges and undefined the essential core of civil disobedience are. Indeed, a main achievement of Lewis Perry's book under review here is to emphasize that what we now consider the definitive traits of civil disobedience—respect for the law in principle, willingness to accept punishment for violating an unjust law, and a commitment to nonviolence—have rarely all been present when civil disobedience has been engaged in. Perhaps Perry's minimalist description of civil disobedience is the best we can do: “the national heritage of resistance to unjust laws.”

Background: Various organizations and universities have developed competencies for health professionals and other emergency responders. Little effort has been devoted to the integration of these competencies across health specialties and professions. The American Medical Association Center for Public Health Preparedness and Disaster Response convened an expert working group (EWG) to review extant competencies and achieve consensus on an educational framework and competency set from which educators could devise learning objectives and curricula tailored to fit the needs of all health professionals in a disaster.
Methods: The EWG conducted a systematic review of peer-reviewed and non–peer reviewed published literature. In addition, after-action reports from Hurricane Katrina and relevant publications recommended by EWG members and other subject matter experts were reviewed for congruencies and gaps. Consensus was ensured through a 3-stage Delphi process.
Results: The EWG process developed a new educational framework for disaster medicine and public health preparedness based on consensus identification of 7 core learning domains, 19 core competencies, and 73 specific competencies targeted at 3 broad health personnel categories.
Conclusions: The competencies can be applied to a wide range of health professionals who are expected to perform at different levels (informed worker/student, practitioner, leader) according to experience, professional role, level of education, or job function. Although these competencies strongly reflect lessons learned following the health system response to Hurricane Katrina, it must be understood that preparedness is a process, and that these competencies must be reviewed continually and refined over time. The Golden-cheeked Warbler Dendroica chrysoparia is a federally endangered Neotropical migrant that inhabits montane pine-oak forests in Mexico and northern Central America during the non-breeding season. Although it is known that Golden-cheeked Warblers are closely associated with ‘encino’ oaks (evergreen or holm oak) such as Quercus sapotifolia, Q. eliptica and Q. elongata, which have shiny, narrow, elliptical, or oblong leaves, quantitative habitat targets are useful for effectively incorporating this information into conservation planning and forest management practices. We analysed data on wintering Golden-cheeked Warblers collected during the non-breeding season in Honduras from 1996 to 1998 to identify quantitative targets for habitat conditions for this species. Data on warbler abundance were collected using line transect surveys located in montane pine-oak forests in a stratified-random fashion. Habitat data were collected at five 0.04 ha plots on these same transects and the averaged values used as predictors of Golden-cheeked Warbler abundance. We found that Golden-cheeked Warblers were strongly associated with the basal area of encino oaks and density of ‘roble’ oaks, such as Q. segoviensis, Q. purulhana and Q. rugosa, which have large, lobed leaves. Density of Golden-cheeked Warblers peaked at ≈ 5.6 m2 ha–1 basal area of encino and ≈7 roble oaks ha–1. These values can be used to identify quantitative habitat targets that can be directly incorporated into forest management practices to ensure that these activities maintain habitat conditions necessary for their use by Golden-cheeked Warblers.

Summary
In recent years, Hannah Arendt has been criticized for the way she characterized the people of sub-Saharan Africa as they faced invasion and occupation by European imperial powers. Such criticism has become more frequent, even as her “boomerang” thesis in The Origins of Totalitarianism (1951) – the idea that the European imperial experience in Africa was among the main factors contributing to the emergence of totalitarianism in Europe – has received renewed attention, even from academic historians. Thus her critique of imperialism places her among the forerunners of anticolonial thought and of postcolonial studies, while her attitude toward sub-Saharan African cultures makes her sound eerily like the very European settler population she was condemning.
Some defend Arendt by suggesting that what she did was to capture the racist mentality of the white settlers in southern Africa through an act of narrative ventriloquism. By entering into the consciousness of the early Boer settlers and Europeans generally, she revealed, according to Seyla Benhabib, the “temptation of regression to a condition in which everything is possible.” Yet even before Chapter 7 (“Race and Bureaucracy”) in Origins, where Arendt took up this topic directly, she wrote that the eighteenth-century European “enthusiasm for the diversity in which the all-present identical nature of man and reason could find expression” met a stern test when it was “faced with tribes which, as far as we know, never had found by themselves any adequate expression of human reason or human passion in either cultural deeds or popular customs, and which had developed human institutions only to a very low level.” Her view here clearly characterized sub-Saharan Africans not as in- or subhuman but as culturally underdeveloped. In the words Sankar Muthu has used to describe the capacity that anti-imperialist European thinkers variously attributed to non-European peoples, Arendt denied much, if any, “cultural agency” to black Africans.

It is tempting to think that we have heard just about all we want or need to know about race. As the above quotes indicate, modern notions of race have always revolved around the faculty of vision, with supplementary contributions from other senses such as hearing, as Arendt notes in a tacit allusion to one mark of Jewish difference—the way they sounded when concentrated in urban settings. Yet two very recent works—Mark M. Smith's How Race Is Made and Anne C. Rose's Psychology and Selfhood in the Segregated South—have much to teach us about how race has “worked”, particularly in the twentieth-century South but also, by implication, in the United States in general. Both works assume that, historically, race is no mere add-on to the self, a kind of externality that, once detected, can be relatively easily excised. Rather, it stands right at the heart of personal and group identity in a nation where race and ethnicity continue to assume surprising new shapes and forms.
