56 Genetic Models for Long-Term Neurocognitive Outcomes in Pediatric Medulloblastoma and Traumatic Brain Injury

Objective:
Children who suffer from brain insults (i.e., traumatic brain injury (TBI), chemotherapy and radiation treatment for brain tumors) are susceptible to late-emerging cognitive sequelae. Even with similar neurological risk variables, variability in long-term cognitive outcomes remains an area of investigation for researchers of acquired brain injury. Given the potential for genetic factors to influence response to chemoradiation, researchers have examined associations between germline, inherited, single nucleotide polymorphisms (SNPs), and neurocognitive outcomes for cancer survivors. Children who sustain an uncomplicated mild TBI generally recover without long-term neuropsychological consequences. However, TBI survivors have overlapping mechanism categories with cancer survivors through secondary injury variables that can be influenced by genomic variation (e.g., oxidative stress and neuroinflammation). Furthermore, the study of genomic vulnerability is limited in heterogenous groups of pediatric TBI survivors. This study aims to identify associations between genotype and long-term neurocognitive outcomes for acquired brain injury survivors by utilizing machine learning to uncover pathophysiological similarities and differences between groups.
Participants and Methods:Fourteen brain tumor survivors, 139 traumatic brain injury survivors, and 63 healthy, age-matched controls completed the Letter N-back task to obtain performances on core neurocognitive skills (attention, working memory, and processing speed). Ten targeted genotypes were examined across five pathophysiological pathways (neurotransmission, oxidative stress, neuroinflammation, plasticity, growth and repair, and folate metabolism). Data were trained and tested utilizing three regression machine learning models. Mean estimated error and R2 were generated for each neurocognitive outcome. A feature importance score for models with positive variance was generated to determine how predictive a given SNP is for neurocognitive outcomes.
Results:Genotype only accounted for a small amount of variance in cognitive outcomes when all clinical groups were combined. The mean absolute error for the best-fitting models from analyses where all groups were combined decreased when groups were examined separately; however, the differences in model R2 values were not significant. The relationship between brain tumor survivors and processing speed performance depended on genotype. Two SNPs had positive feature importance at the interaction level (rs58225473 and rs1801394). These SNPs are located on the CACNB2 and MTR genes and have functional consequences for neurotransmission and folate metabolism. Models of traumatic brain injury survivors did not explain positive variance and could not be examined for feature importance. Additionally, even when removing the only mechanism of action that should not be relevant for TBI survivors (folate metabolism polymorphisms), the TBI models still did not explain positive variance.
Conclusions:Findings of the importance of two key SNPs on MTR and CACNB2 genes align with recent systematic reviews, which found associations between these polymorphisms and neuropsychological outcomes in more than one group or cohort of pediatric cancer survivors. Models for TBI survivors were limited by the heterogeneity of the group and ceiling effects on performance. An understanding of genetic vulnerabilities influenced by treatment and injury-related factors in acquired brain injury will inform our understanding of the developing and recovering childhood brain. The current study is an initial contribution to this goal and highlights the utility of machine learning methodology for future studies that examine the influence of genetic heterogeneity in pediatric acquired brain injury.
Trends in BMI of Indonesian adults between 1993 and 2014: a longitudinal population-based study

Objective:
To examine the trajectories of BMI in Indonesian adults from 1993 to 2014, investigating different patterns by sex and birth cohort.
Design:Longitudinal study: secondary data analysis of the Indonesian Family Life Survey, a large-scale population-based longitudinal study, had their height and weight measured up to five times throughout the 21-year study period (1993–2014). The change in BMI across time was estimated using group-based trajectory models, then differences by sex and birth cohort were investigated using random effect (mixed) models.
Setting:Thirteen out of twenty-seven provinces in Indonesia.
Participants:Indonesian adults aged 19 years and older (n 42 537) were included in the analysis.
Results:Mean BMI in adults increased between 1993 (21·4 kg/m2) and 2014 (23·5 kg/m2). The group-based trajectory model found three distinct groups with mean BMI increasing more rapidly in the most recent time periods. The first group (56·7 % of participants) had a mean BMI entirely within the normal weight range; the second group (34·7 %) started in the normal weight category and were obese, on average by the end of the study period; and the third group (8·6 %) were always in the obese category, on average. The shape of these three trajectories differed by gender (P < 0·001) and birth cohort (P < 0·001).
Conclusions:The mean BMI among Indonesian adults has increased between 1993 and 2014, driven by those in the most recent birth cohorts. Our findings support the urgent need for targeted overweight and obesity prevention and intervention programmes in Indonesia.

What effect does political instability in the form of a potential secession from a political union have on business formation? Using newly collected data on business creation, we show that entrepreneurial activity in Ireland in the late nineteenth century was much lower than Scotland, and this divergence fluctuated over time. Several factors may have contributed to this, but we argue that political uncertainty about the prospect of a devolved government in Ireland played a role. The effects were most acute in the North of Ireland, the region that was most concerned by potential changes.
Information brokerage in Caribbean coral reef governance networks

Poor connectivity between diverse resource users and complex wider governance networks is a challenge in environmental governance. Organizations that ‘broker’ interactions among these relationships are expected to improve governance outcomes. Here, we used semi-structured interviews and social network analysis to identify actors in positions to broker coral reef-related information to and from resource users and to assess the performance of these brokers. Representatives (n = 262) of actor groups were interviewed, including local and national government, non-governmental organizations (NGOs), community organizations and resource user groups from 12 communities across four Caribbean countries, to map information-sharing networks and to identify brokers. Broker performance was assessed through separate interviews with coral reef resource users (n = 545). The findings show that marine NGOs were the highest-functioning brokers. Where such local-level organizations were absent, government agencies in reef management roles acted as brokers, but their performance was lower. Actors in brokerage positions did not always effectively share information, with broker performance being positively correlated with network brokerage scores. The results further our understanding of the roles of brokers in different governance contexts. Identifying those in brokerage positions and supporting their roles in connecting local resource users to wider governance networks could encourage functional brokerage and enhance reef management outcomes.

Background
Positive symptoms are a useful predictor of aggression in schizophrenia. Although a similar pattern of abnormal brain structures related to both positive symptoms and aggression has been reported, this observation has not yet been confirmed in a single sample.
MethodTo study the association between positive symptoms and aggression in schizophrenia on a neurobiological level, a prospective meta-analytic approach was employed to analyze harmonized structural neuroimaging data from 10 research centers worldwide. We analyzed brain MRI scans from 902 individuals with a primary diagnosis of schizophrenia and 952 healthy controls.
ResultsThe result identified a widespread cortical thickness reduction in schizophrenia compared to their controls. Two separate meta-regression analyses revealed that a common pattern of reduced cortical gray matter thickness within the left lateral temporal lobe and right midcingulate cortex was significantly associated with both positive symptoms and aggression.
ConclusionThese findings suggested that positive symptoms such as formal thought disorder and auditory misperception, combined with cognitive impairments reflecting difficulties in deploying an adaptive control toward perceived threats, could escalate the likelihood of aggression in schizophrenia.

How do government policies and practices affect struggles over collective identity and struggles over land? Examining the interconnections among collective identity struggles, land struggles and state policies and practices in post-apartheid South Africa, this paper argues that the government's contradictory policies and ambivalent practices have aggravated collective struggles over the boundaries of belonging. Specifically, the differing definitions of community set forth in traditional leadership, land tenure and land restitution policies exacerbate existing divisions among ‘communities’ concurrently subject to these policies and create practical policy dilemmas for decision-makers. This paper illustrates the interplay between public policies and collective identity struggles through close examination of struggles among the Barokologadi ba ga Maotwe, a so-called traditional community. The Barokologadi case underscores the necessity of attending to these interactions.

Summary
Introduction
Plate tectonics is the kinematic theory that describes the large-scale motions and events of the outermost shell of the solid Earth in terms of the relative motions and interactions of large, rigid, interlocking fragments of lithosphere called tectonic plates. Plates form and disappear incrementally over time as a result of tectonic processes. There are currently about a dozen major plates on the surface of the Earth, and many minor ones. The present-day configuration of tectonic plates is illustrated inFigure 7.1. As the interlocking plates move relative to each other, they interact at plate boundaries, where adjacent plates collide, diverge, or slide past each other. The interactions of plates result in a variety of observable surface phenomena, including the occurrence of earthquakes and the formation of large-scale surface features such as mountains, sedimentary basins, volcanoes, island arcs, and deep ocean trenches. In turn, the appearance of these phenomena and surface features indicates the location of plate boundaries. For a detailed review of the theory of plate tectonics, consult Wessel and Müller (2007).
A plate-tectonic reconstruction is the calculation of positions and orientations of tectonic plates at an instant in the history of the Earth. The visualization of reconstructions is a valuable tool for understanding the evolution of the systems and processes of the Earth's surface and near subsurface. Geological and geophysical features may be “embedded” in the simulated plates, to be reconstructed along with the plates, enabling a researcher to trace the motions of these features through time.

Twenty-five patients with mild mental retardation were compared with 26 schizophrenic patients of normal intelligence. The patients with retardation had suffered an earlier onset and were less likely to have been married or employed. However, the clinical phenomena exhibited by the two groups, as elicited by a SADS–L interview, were very similar. Thus, standardised interviews and criteria for schizophrenia can be readily applied to those with an IQ between 50 and 70.

This paper describes the context and nature of an assemblage of forty-four Palaeolithic hand-axes from a Roman religious site at Witham, Essex. The hand-axes are considered to have been derived from several sources, and it is suggested that the Romano-British occupants of the site deliberately selected them for their shape and placed them in the bottom of two large man-made depressions. In the light of stone axe finds on continental temple sites, and of classical Roman texts and traditions, the possibility arises that the Witham finds may have represented ‘thunderbolts’ in the worship of Jupiter or a local Celtic equivalent.
