We studied how patient beliefs regarding the need for antibiotics, as measured by expectation scores, and antibiotic prescribing outcome affect patient satisfaction using data from 2,710 urgent-care visits. Satisfaction was affected by antibiotic prescribing among patients with medium–high expectation scores but not among patients with low expectation scores.

We performed viral culture of nasopharyngeal specimens in individuals aged 79 and older, infected with severe acute respiratory coronavirus virus 2 (SARS-CoV-2), 10 days after symptom onset. A positive viral culture was obtained in 10 (45%) of 22 participants, including 4 (33%) of 12 individuals with improving symptoms. The results of this small study suggest that infectivity may be prolonged among older individuals.

Objective:
The purpose of this study was to investigate differences in the perception of disaster issues between disaster directors and general health care providers in Gyeonggi Province, South Korea.
Methods:The Gyeonggi provincial committee distributed a survey to acute care facility personnel. Survey topics included awareness of general disaster issues, hospital preparedness, and training priorities. The questionnaire comprised multiple choices and items scored on a 10-point Likert scale. We analyzed the discrepancies and characteristics of the responses.
Results:Completed surveys were returned from 43 (67%) of 64 directors and 145 (55.6%) of 261 health care providers. In the field of general awareness, the topic of how to triage in disaster response showed the greatest discrepancies. In the domain of hospital level disaster preparedness, individual opinions varied most within the topics of incident command, manual preparation. The responses to “accept additional patients in disaster situation” showed the biggest differences (> 21 versus 6~10).
Conclusions:In this study, there were disaster topics with discrepancies and concordances in perception between disaster directors and general health care providers. The analysis would present baseline information for the development of better training programs for region-specific core competencies, knowledge, and skills required for the effective response.
Application of the “Plan-Do-Study-Act” Model to Improve Survival after Cardiac Arrest in Korea: A Case Study

Objective:
Low rates of bystander cardiopulmonary resuscitation (CPR) were identified as a shortcoming in the “chain of survival” for out-of-hospital cardiac arrest (OHCA) care in the Korean city of Ansan. This study sought to evaluate the effect of an initiative to increase bystander CPR and quality of out-of-hospital resuscitation on outcome from OHCA. The post-intervention data were used to determine the next quality improvement (QI) target as part of the “Plan-Do-Study-Act” (PDSA) model for QI.
Hypothesis:The study hypothesis was that bystander CPR, return of spontaneous circulation (ROSC), and survival to discharge after OHCA would increase in the post-intervention period.
Methods:This was a retrospective pre/post study. The data from the pre-intervention period were abstracted from 2008–2011 and the post-intervention period from 2012–2013. The effect of the intervention on the odds of ROSC and survival to hospital discharge was determined using a generalized estimating equation to account for confounders and the effect of clustering within medical centers. The analysis was then used to identify other factors associated with outcomes to determine the next targets for intervention in the chain of survival for cardiac arrest in this community.
Results:Rates of documented bystander CPR increased from 13% in the pre-intervention period to 37% in the post-intervention period. The overall rate of ROSC decreased from 18.4% to 14.3% (risk difference −4.1%; 95% CI, −7.1%–1.0%), whereas survival to hospital discharge increased from 3.9% to 5.0% (risk difference 1.1%; 95% CI, −1.8%–3.8%), and survival with good neurologic outcome increased from 0.8% to 1.6% (risk difference 0.8%; 95% CI, −0.8%–2.4%). In multivariable analyses, there was no association between the intervention and the rate of ROSC or survival to hospital discharge. The designated level of the treating hospital was a significant predictor of both survival and ROSC.
Conclusion:In this case study, there were no observed improvements in outcomes from OHCA after the targeted intervention to improve out-of-hospital CPR. However, utilizing the PDSA model for QI, the designated level of the treating hospital was found to be a significant predictor of survival in the post-period, identifying the next target for intervention.

Cognitive dysfunction is a symptomatic domain identified across many mental disorders. Cognitive deficits in individuals with major depressive disorder (MDD) contribute significantly to occupational and functional disability. Notably, cognitive subdomains such as learning and memory, executive functioning, processing speed, and attention and concentration are significantly impaired during, and between, episodes in individuals with MDD. Most antidepressants have not been developed and/or evaluated for their ability to directly and independently ameliorate cognitive deficits. Multiple interacting neurobiological mechanisms (eg, neuroinflammation) are implicated as subserving cognitive deficits in MDD. A testable hypothesis, with preliminary support, posits that improving performance across cognitive domains in individuals with MDD may improve psychosocial function, workplace function, quality of life, and other patient-reported outcomes, independent of effects on core mood symptoms. Herein we aim to (1) provide a rationale for prioritizing cognitive deficits as a therapeutic target, (2) briefly discuss the neurobiological substrates subserving cognitive dysfunction, and (3) provide an update on current and future treatment avenues.
Modeling Tamarisk (Tamarix spp.) Habitat and Climate Change Effects in the Northwestern United States

Tamarisk species are shrubs or small trees considered by some to be among the most aggressively invasive and potentially detrimental exotic plants in the United States. Although extensively studied in the southern and interior west, northwestern (Oregon, Washington, and Idaho) distribution and habitat information for tamarisk is either limited or lacking. We obtained distribution data for the northwest, developed a habitat suitability map, and projected changes in habitat due to climate change in a smaller case study area using downscaled climate data. Results show extensive populations of tamarisk east of the Cascade Mountains. Despite the perceived novelty of tamarisk in the region, naturalized populations were present by the 1920s. Major population centers are limited to the warmest and driest environments in the central Snake River Plain, Columbia Plateau, and Northern Basin and Range. Habitat suitability model results indicate that 21% of the region supports suitable tamarisk habitat. Less than 1% of these areas are occupied by tamarisk; the remainder is highly vulnerable to invasion. Although considerable uncertainty exists regarding future climate change, we project a 2- to 10-fold increase in highly suitable tamarisk habitat by the end of the century. Our habitat suitability maps can be used in “what if” exercises as part of planning, detection, restoration, management, and eradication purposes.
Patterns of fruit and vegetable availability and price competitiveness across four seasons are different in local food outlets and supermarkets

Objective
To explore the effect of seasonality on fruit and vegetable availability and prices across three outlet types (farmers’ markets, roadside stands and conventional supermarkets).
DesignCross-sectional survey of geographically clustered supermarkets, farmers’ markets and roadside stands. Enumerators recorded the availability and lowest price for eleven fruits and eighteen vegetables in each season of 2011.
SettingPrice data were collected at retail outlets located in central and eastern North Carolina.
SubjectsThe sample consisted of thirty-three supermarkets, thirty-four farmers’ markets and twenty-three roadside stands.
ResultsOutside the local harvest season, the availability of many fruits and vegetables was substantially lower at farmers’ markets and roadside stands compared with supermarkets. Given sufficient availability, some items were significantly cheaper (P<0·05) at direct retail outlets in the peak season (e.g. cantaloupe cost 36·0 % less at roadside stands than supermarkets), while others were significantly more expensive (e.g. carrots cost 137·9 % more at farmers’ markets than supermarkets). Although small samples limited statistical power in many non-peak comparisons, these results also showed some differences by item: two-thirds of fruits were cheaper at one or both direct outlets in the spring and autumn, whereas five of eighteen vegetables cost more at direct retail year-round.
ConclusionsCommonly consumed fruits and vegetables were more widely available at supermarkets in central and eastern North Carolina than at direct retail outlets, in each season. Contingent on item availability, price competitiveness of the direct retail outlets varied by fruit and vegetable. For many items, the outlets compete on price in more than one season.

Activity patterns of greater horseshoe bats Rhinolophus ferrumequinum were investigated at caves in Cheddar (south-west England) during the hibernation season. An ultrasound detector and datalogger were used to monitor and record the number of echolocation calls in a single cave. Activity of R. ferrumequinum remained largely nocturnal throughout winter, and the mean time of activity over 24 h was 88 to 369 min after sunset. There was an increase in diurnal activity from late May to early June, probably because bats remained active after foraging at dawn towards the end of the hibernation season. Visits to the cave did not increase bat activity. Cave air temperature reflected external climatic temperature, although there was variation in cave temperature and its range within and among caves. Individual R. ferrumequinum are usually dispersed in caves in regions where temperature fluctuations correlate with climatic variations in temperature. There was a positive correlation between the number of daily bat passes monitored by the bat detector and datalogger (= daily activity) and cave temperature. Nocturnal activity may be sometimes associated with winter feeding. Neither date nor barometric pressure had a significant effect on daily activity. Activity patterns largely reflected the findings from individual R. ferrumequinum studied by telemetry (Park, 1998), in that bat activity increased with cave and climatic temperatures, and the temporal pattern of activity remained consistently nocturnal throughout winter, starting at dusk.

Legal education is only beginning to make use of the technological capabilities of computers as a medium for instructional design. In this article the authors show the applicability of computer-assisted instruction to law through programmed instructional techniques. They trace the two decades of development of computer-assisted instruction (CAI) in law, the use and misuse of the computer's capabilities, the impact of CAI on law student attitudes and performance, and the positive student endorsement. They carefully analyze the computer's capacity for use in memory drills, tutorial sessions, and simulation exercises. They do not offer an unrestricted endorsement of the computer's capabilities but hope for the continuing development of CAI in the legal education process.
