Sex-dependent differences in vulnerability to early risk factors for posttraumatic stress disorder: results from the AURORA study

Background
Knowledge of sex differences in risk factors for posttraumatic stress disorder (PTSD) can contribute to the development of refined preventive interventions. Therefore, the aim of this study was to examine if women and men differ in their vulnerability to risk factors for PTSD.
MethodsAs part of the longitudinal AURORA study, 2924 patients seeking emergency department (ED) treatment in the acute aftermath of trauma provided self-report assessments of pre- peri- and post-traumatic risk factors, as well as 3-month PTSD severity. We systematically examined sex-dependent effects of 16 risk factors that have previously been hypothesized to show different associations with PTSD severity in women and men.
ResultsWomen reported higher PTSD severity at 3-months post-trauma. Z-score comparisons indicated that for five of the 16 examined risk factors the association with 3-month PTSD severity was stronger in men than in women. In multivariable models, interaction effects with sex were observed for pre-traumatic anxiety symptoms, and acute dissociative symptoms; both showed stronger associations with PTSD in men than in women. Subgroup analyses suggested trauma type-conditional effects.
ConclusionsOur findings indicate mechanisms to which men might be particularly vulnerable, demonstrating that known PTSD risk factors might behave differently in women and men. Analyses did not identify any risk factors to which women were more vulnerable than men, pointing toward further mechanisms to explain women's higher PTSD risk. Our study illustrates the need for a more systematic examination of sex differences in contributors to PTSD severity after trauma, which may inform refined preventive interventions.

Background
Several hypotheses may explain the association between substance use, posttraumatic stress disorder (PTSD), and depression. However, few studies have utilized a large multisite dataset to understand this complex relationship. Our study assessed the relationship between alcohol and cannabis use trajectories and PTSD and depression symptoms across 3 months in recently trauma-exposed civilians.
MethodsIn total, 1618 (1037 female) participants provided self-report data on past 30-day alcohol and cannabis use and PTSD and depression symptoms during their emergency department (baseline) visit. We reassessed participant's substance use and clinical symptoms 2, 8, and 12 weeks posttrauma. Latent class mixture modeling determined alcohol and cannabis use trajectories in the sample. Changes in PTSD and depression symptoms were assessed across alcohol and cannabis use trajectories via a mixed-model repeated-measures analysis of variance.
ResultsThree trajectory classes (low, high, increasing use) provided the best model fit for alcohol and cannabis use. The low alcohol use class exhibited lower PTSD symptoms at baseline than the high use class; the low cannabis use class exhibited lower PTSD and depression symptoms at baseline than the high and increasing use classes; these symptoms greatly increased at week 8 and declined at week 12. Participants who already use alcohol and cannabis exhibited greater PTSD and depression symptoms at baseline that increased at week 8 with a decrease in symptoms at week 12.
ConclusionsOur findings suggest that alcohol and cannabis use trajectories are associated with the intensity of posttrauma psychopathology. These findings could potentially inform the timing of therapeutic strategies.

Background
This is the first report on the association between trauma exposure and depression from the Advancing Understanding of RecOvery afteR traumA(AURORA) multisite longitudinal study of adverse post-traumatic neuropsychiatric sequelae (APNS) among participants seeking emergency department (ED) treatment in the aftermath of a traumatic life experience.
MethodsWe focus on participants presenting at EDs after a motor vehicle collision (MVC), which characterizes most AURORA participants, and examine associations of participant socio-demographics and MVC characteristics with 8-week depression as mediated through peritraumatic symptoms and 2-week depression.
ResultsEight-week depression prevalence was relatively high (27.8%) and associated with several MVC characteristics (being passenger v. driver; injuries to other people). Peritraumatic distress was associated with 2-week but not 8-week depression. Most of these associations held when controlling for peritraumatic symptoms and, to a lesser degree, depressive symptoms at 2-weeks post-trauma.
ConclusionsThese observations, coupled with substantial variation in the relative strength of the mediating pathways across predictors, raises the possibility of diverse and potentially complex underlying biological and psychological processes that remain to be elucidated in more in-depth analyses of the rich and evolving AURORA database to find new targets for intervention and new tools for risk-based stratification following trauma exposure.

Summary
Introduction
‘The Structure of Simple General Equilibrium Models,’ published in 1965, has been a popular paper, although it was not my first article. I will first explain as to why this paper was written and then I shall make some remarks about general equilibrium models as used in the theory of International Trade.
Early in the 1960s, at the University of Rochester, we hired Robert Fogel who was finishing his graduate work at Johns Hopkins University. At Rochester, our new department was led by our new Chairman, Lionel Mckenzie. Fogel was at the early stage of his career with focus on economic history. He wanted to use basic economic theory in his work, which at that time, focused on explaining the importance of the development of railroads and how it contributed to the growth in the United States in the mid-nineteenth century while the West was being developed. This was a novel idea among economists studying economic history, but was not generally welcomed by scholars of History. Lionel developed our department by hiring young scholars who would focus on areas such as Labor, International Trade, or other applied areas, and would also expose how basic economic theory was essential in explaining major results in applied areas. Lionel asked me to explain to Bob some of the essentials of General Equilibrium Theory that might be useful to Bob's research. I agreed, explaining to Bob that I would try to make it simple for him and making it simpler would also help me gain a better insight of the theory.
Simplifications
As ascertained by my paper, I thought that traditional approaches to the subject matter found in GE theory were more complicated than need be. Apart from similar questions that mathematical theorists were most concerned about, such as, assumptions leading to existence, economists would be more interested in observing causes behind a new equilibrium with spillover effects. The mathematical formulations are preoccupied with establishing conditions that support assumptions for ensuring existence of competitive markets. It simultaneously produces well-defined equilibria that are stable, perhaps unique, and those which reveal interesting normative properties. Economists have always been more interested in applying how changes that may take place in one market would cause new equilibria to be established in other markets, that is, general equilibrium as opposed to partial equilibrium.

Objectives:This study is the first to examine the effect of bereavement of a first-degree family member on subsequent morbidity over a 10-year follow-up period.
Methods:A sample of bereaved subjects (n = 72) were compared with a control group (n = 80) recruited in the same period with respect to morbidity experience during follow-up. Morbidity events were ascertained from the subject themselves, their health care providers and these sources were also compared.
Results:Bereavement was associated with an elevated total burden of illness as well as with mental health and circulatory system categories diagnosed according to the International Classification of Diseases - Clinically Modified (ICD-9) classification system. The elevation ranged from approximately 20% for any illness to 60–100% among circulatory system disorders. Although in an earlier study there was a downregulation of T-cell function in the bereaved during the first 8 weeks, there was no evidence that the bereavement was associated with increased morbidity in the respiratory or immune system ICD-9 categories long-term.
Conclusions:Past epidemiological research has indicated that bereavement of a close family member is associated with adverse health consequences of a generalised morbidity. Our study suggests an increase in mental health and circulatory system effects in particular. Further research is required to determine whether other systems are also affected by bereavement.

Summary
Macroeconomic models of economies in the growth process are most often highly aggregated. There is no doubt that many variations at the detailed microeconomic level can yield the same overall macrolevels of growth, perhaps remaining fairly constant at high levels for years if not decades. However, the smooth aggregate levels may disguise rather interesting churning activity at the microlevel, with compositional changes that are not accidental and may, indeed, help to maintain high overall levels of growth. Here I wish mainly to consider two examples of this phenomenon, one focused on international trade and the other on technology.
INTERNATIONAL TRADE AND CHURNING AT THE MICROLEVEL
By definition, a country that engages in international trade forsakes the kind of balanced growth associated with autarky, with local production responding to local demand. However, in simple textbook models of international trade that limit production and consumption to two commodities, it is possible to consider a balanced expansion in the composition of a country's production levels, with aggregate growth rates matched by those in each sector. What is missing in this account is the potential of international trade to allow (or force) a country to produce only a narrow range of products for the world market while consuming a wide variety of commodities, some requiring higher capital–labor ratios than found in home production and others that would utilize more labor-intensive techniques than endowment ratios available at home. Furthermore, economic growth usually entails rising levels of per capita income, often accompanied by net investment (either from local savings or from foreign sources) and perhaps by improvements in technology.
Thermal Metalorganic Chemical Vapor Deposition of Ti-Si-N Films for Diffusion Barrier Applications

Structurally disordered refractory ternary films such as titanium silicon nitride (Ti-Si-N) have potential as advanced diffusion barriers in future ULSI metallization schemes. Here we present results on purely thermal metalorganic chemical vapor deposition (CVD) of Ti-Si-N. At temperatures between 300 and 450°C, tetrakis(diethylamido)titanium (TDEAT), silane, and ammonia react to grow Ti-Si-N films with Si contents of 0-20 at.%. Typical impurity contents are 5-10 at.%H and 0.5 to 1.5 at.% C, with no O or other impurities detected in the bulk of the film. Although the film resistivity increases with increasing Si content, it remains below 1000 μΩ-cm for films with less than 5 at.% Si. These films are promising candidates for advanced diffusion barriers.

In the title of Paul Krugman's stimulating paper I would have been tempted to replace ‘versus’ connecting ‘regionalism’ and ‘multilateralism’ with an ‘and’. Herein, I think, hangs a tale. Those who view the emergence of regional efforts to establish free-trading areas, especially in Europe and North America, as a threat to the more than forty years of efforts of GATT to lower trade barriers on a multilateral basis are driven to compare the benefits of multilateralism with the potential or actual welfare-reducing possibilities of discriminatory regional arrangements. To my knowledge few economists would do an 180° turn on the comparison to argue that regional efforts raise welfare whereas multilateral reductions in trade impediments lower real incomes. ‘Regionalism and multilateralism’ reflects a different point of view: in today's world regionalism and multilateralism can coexist. Indeed, given the strong inclinations of certain regions to pursue closer economic associations among small groups of countries, there is an even more important role for GATT and international consultations to ensure that trading blocs join others in a multilateral effort to reduce obstacles to trade. Choosing between the two is not the option.
Krugman's Chapter 3 is based on a paper that is the third of a sequence inaugurated at a conference in Tel Aviv in 1989 (Krugman, 1991a). In that paper the Krugman touch is well illustrated by his analysis of the effect on world welfare of a systematic joining of countries into trading blocs, with each such bloc imposing an optimal tariff on imports. As the number of blocs decreases (with each getting larger), so does world welfare, with such welfare racing a minimum when there are three such blocs.

TESTING TRADE THEORIES: A COMMENT
I have been asked to focus particular attention on Stern's paper. Stern has, I think, provided us with an excellent, comprehensive survey of trade theories, together with the results and the difficulties of testing these theories. I shall therefore limit my comments to some of the issues he raises concerning the directions in which we should now try to move on both the theoretical and empirical levels.
The present state of trade theory is far from satisfactory. Tests of the Heckscher–Ohlin factor-proportions theory have shown that, at least in its simple form, it does not perform very well in predicting the commodity composition and direction of trade. As yet, however, we do not have a well worked out theory to replace it. We have lots of bits and pieces with which to amend the Heckscher–Ohlin approach, but we do not know how significant each part is or how the parts are tied together. Consequently, most of us, in presenting the pure-theory side of trade to our students, still spend considerable time carefully building up the simple Heckscher–Ohlin–Samuelson trade model and then examining such theorems as the Stopler–Samuelson proposition, the Rybczynski effect, and the factor-price equalization theorem. The appeal of the model, of course, is that it enables us to relate price changes all the way back to the distribution of income.
