Household food insufficiency, health status and emergency healthcare utilisation among children with and without special healthcare needs

Objective:
To compare exposure to household food insufficiency and the relationship between household food insufficiency and both health status and emergency healthcare utilisation among children with and without special healthcare needs (SHCN).
Design:Analysing pooled data from the 2016–2017 iterations of the National Survey of Children’s Health, we conducted multivariate logistic regressions on household food insufficiency, health status and emergency healthcare utilisation. We assessed interactions between household food insufficiency and children’s SHCN status in our models of health status and utilisation.
Setting:United States.
Participants:Parents of a nationally representative sample of non-institutionalised children (aged 0–17 years).
Results:Children with SHCN were more likely to experience household food insufficiency (70 v. 56 %), non-excellent health status (67 v. 28 %) and emergency healthcare utilisation (32 v. 18 %) than other children. Household food insufficiency was associated with 37 % (children with SHCN) and 19 % (children without SHCN) reductions in the likelihood of having excellent health. Household food insufficiency was associated with a roughly equal (16–19 %) increase in the likelihood of emergency department utilisation across groups.
Conclusions:Compared with other children, children with SHCN have an elevated risk of exposure to household food insufficiency and experiencing greater reductions in health status when exposed.

Background
Anxiety disorders are common, and cognitive–behavioural therapy (CBT) is a first-line treatment. Candidate gene studies have suggested a genetic basis to treatment response, but findings have been inconsistent.
AimsTo perform the first genome-wide association study (GWAS) of psychological treatment response in children with anxiety disorders (n = 980).
MethodPresence and severity of anxiety was assessed using semi-structured interview at baseline, on completion of treatment (post-treatment), and 3 to 12 months after treatment completion (follow-up). DNA was genotyped using the Illumina Human Core Exome-12v1.0 array. Linear mixed models were used to test associations between genetic variants and response (change in symptom severity) immediately post-treatment and at 6-month follow-up.
ResultsNo variants passed a genome-wide significance threshold (P=5×10–8) in either analysis. Four variants met criteria for suggestive significance (P<5×10–6) in association with response post-treatment, and three variants in the 6-month follow-up analysis.
ConclusionsThis is the first genome-wide therapygenetic study. It suggests no common variants of very high effect underlie response to CBT. Future investigations should maximise power to detect single-variant and polygenic effects by using larger, more homogeneous cohorts.

Background
We previously reported an association between 5HTTLPR genotype and outcome following cognitive–behavioural therapy (CBT) in child anxiety (Cohort 1). Children homozygous for the low-expression short-allele showed more positive outcomes. Other similar studies have produced mixed results, with most reporting no association between genotype and CBT outcome.
AimsTo replicate the association between 5HTTLPR and CBT outcome in child anxiety from the Genes for Treatment study (GxT Cohort 2, n = 829).
MethodLogistic and linear mixed effects models were used to examine the relationship between 5HTTLPR and CBT outcomes. Mega-analyses using both cohorts were performed.
ResultsThere was no significant effect of 5HTTLPR on CBT outcomes in Cohort 2. Mega-analyses identified a significant association between 5HTTLPR and remission from all anxiety disorders at follow-up (odds ratio 0.45, P = 0.014), but not primary anxiety disorder outcomes.
ConclusionsThe association between 5HTTLPR genotype and CBT outcome did not replicate. Short-allele homozygotes showed more positive treatment outcomes, but with small, non-significant effects. Future studies would benefit from utilising whole genome approaches and large, homogenous samples.

Summary
Abstract
In this chapter, you will learn about the unique challenges of learning game design, the necessary multidisciplinary makeup of learning game design teams, and ways to improve team efficiency and effectiveness through communication. Learning games combine content and context to create a meaningful interaction between players’ experience and learning. They often employ an experiential learning strategy and have been called “designed experiences” (Squire, 2006). When learning games are viewed in this light, designing them becomes quite a challenge for several reasons: 1) many variables must be manipulated to achieve the right kind of learning experience at the right time; 2) learning game design has characteristics of ill-structured problem solving; 3) as an ill-structured problem, it requires learning game designers with a high level of expertise; and 4) the solution will require input from multiple disciplines. Having a highly skilled multidisciplinary design team raises another set of challenges including the development of a shared mental model. Research has shown that when team members think similarly, they are more likely to work effectively together (Cannon-Bowers & Salas, 1998; Guzzo & Salas, 1995; Hackman, 1990). When team members understand their differences and take measures to leverage them, learning game design teams are strengthened, leading to a more efficient and effective design process. Research indicates that multidisciplinary learning game design team members think differently about: 1) design goals; 2) authenticity requirements; 3) feedback design; 4) the integration of fun within the learning experience; 5) term definition; and 6) documentation contents. Current design models do not include steps to mitigate these differences and to build a team’s shared mental model. Therefore, we provide specific actions that should be integrated into a learning game design model to support the critical and necessary communications among learning game design team members.

Summary
The design and development of learning games is a field of endeavor that crosses the boundaries of several disciplines. A successful computer game for learning (a.k.a. training game, educational game, serious game, instructional game) must weave together the skills and decision making of instructional system designers, game designers, software developers, story developers, assessment professionals, educational scientists, graphic artists, subject matter experts, instructors, and more. While prior publications have addressed theoretical issues from the perspective of one or two of these fields, and some publications have explored practical design and development issues, none has addressed the central methodological issues of how to produce an effective game with an explicit multidisciplinary approach. This book draws on the expertise and experience of a number of active participants in the field of learning games to provide practical guidelines and recommendations from a multidisciplinary perspective to assist you in your efforts to create effective learning games.
Definition of Learning Games
Before we dive into the issues surrounding the design of learning games, we must clarify what we mean by learning game. The literature reveals no consistent, widely accepted definition of learning games, although they are often classified as a subset of serious games for nonentertainment purposes (Sawyer & Smith, 2008). We prefer to distinguish learning games as having certain characteristics. In this book, we define learning games as games intentionally designed to help the player meet instructional goals while actively interacting with and being engaged and immersed in the experience. The learning game provides an environment in which the player must demonstrate a level of understanding of a body of knowledge or skills (expressed as learning objectives) to achieve game goals by solving challenges posed in the environment (e.g., identifying or resolving problems, completing tasks). The type of learning games we refer to require a significant design effort to achieve the learning goals.

Summary
Abstract
What should we look for in a game? How well do existing games embody good instructional and gaming design principles? How do we put those design principles into practice? The following chapters will review several games of different genres and domains. For each game, there are two chapters. The first is an impartial review of the game and the second is a description of the methods and lessons learned from the game developer’s perspective. In this chapter, we introduce each game briefly, discuss the criteria used to review the games, and summarize some of the key points you should look for in these and other games.
Introduction
Now that we’ve introduced a number of key issues and approaches to consider when designing a learning game, let’s put it all in context. The following chapters review seven different games, each focused on a different approach to training or a different training use.

Abstract
The Computer-based Corpsman Training System (CBCTS) was developed by ECS, Inc. for the U.S. Army Research, Development and Engineering Command. Game design elements complement the instructional design elements to produce an award-winning learning game. Notable design features include a well-designed tutorial, opportunities for decision making, time to reflect and replay a scenario, and implicit and explicit feedback. While game and instructional elements work very well together in CBCTS, suggestions are made in this chapter to increase instructional guidance to gain learning efficiencies without jeopardizing gameplay. These suggestions will benefit all learning game designers striving to improve their own games. Game designers are cautioned that additional elements may increase the design and development resource requirements, and instructional and gameplay trade-offs have to be considered. Some of these trade-offs are briefly addressed.
Introduction
The Computer-based Corpsman Training System (CBCTS) is a learning game that provides combat corpsmen realistic training to prepare them to apply their skills in a combat situation. CBCTS was developed by ECS, Inc. for the U.S. Army Research, Development and Engineering Command (RDECOM). The game supports training for Navy combat medics who are assigned to the U.S. Marine Corps. CBCTS is used at the Army Medical Department (AMEDD) Center and School as part of the curriculum to prepare combat medics.

Objective
The emphasis in school nutrition policy has been on vending and competitive items. Our study was designed to characterize and quantify the amount and source of other foods and beverages on school campuses.
DesignA cross-sectional observational study was conducted using a specially designed objective nutrition observation system.
SettingOne low-income school district in southern California with six elementary and two middle schools.
SubjectsData were not collected from individual children. A total of 4033 students, 42 % of whom were Hispanic/Latino, 26 % African American and 21 % non-Hispanic white, were observed across school settings.
ResultsData were collected continuously from 9 January 2008 to 16 June 2010. Healthy foods had, per serving, total energy ≤732 kJ (≤175 kcal), total fat content ≤35 %, total saturated fat ≤10 %, sugar less ≤15 g, sodium <200 mg and trans-fat ≤0·5 g. Healthy beverages were only 100 % juice or water, and unflavoured non-fat, 1 %, 2 % milk and soya or rice milk. The system had high inter-rater reliability (r = 0·78 to 0·99), percentage agreement (83 % to 100 %) and test–retest reliability (r = 0·81 to 0·98). Significantly more unhealthy foods and beverages than healthy items were observed on all campuses (P < 0·001). An average of 1·26 (sd 0·46) items per student per week was found with an average of 0·86 (sd 0·34) unhealthy items per child per week.
ConclusionsThere were substantial amounts of unhealthy foods and beverages brought onto campuses for classroom rewards, celebrations and fundraising that should be targeted for intervention.
