Background
It is unknown how much variation in adult mental health problems is associated with differences between societal/cultural groups, over and above differences between individuals.
MethodsTo test these relative contributions, a consortium of indigenous researchers collected Adult Self-Report (ASR) ratings from 16 906 18- to 59-year-olds in 28 societies that represented seven culture clusters identified in the Global Leadership and Organizational Behavioral Effectiveness study (e.g. Confucian, Anglo). The ASR is scored on 17 problem scales, plus a personal strengths scale. Hierarchical linear modeling estimated variance accounted for by individual differences (including measurement error), society, and culture cluster. Multi-level analyses of covariance tested age and gender effects.
ResultsAcross the 17 problem scales, the variance accounted for by individual differences ranged from 80.3% for DSM-oriented anxiety problems to 95.2% for DSM-oriented avoidant personality (mean = 90.7%); by society: 3.2% for DSM-oriented somatic problems to 8.0% for DSM-oriented anxiety problems (mean = 6.3%); and by culture cluster: 0.0% for DSM-oriented avoidant personality to 11.6% for DSM-oriented anxiety problems (mean = 3.0%). For strengths, individual differences accounted for 80.8% of variance, societal differences 10.5%, and cultural differences 8.7%. Age and gender had very small effects.
ConclusionsOverall, adults' self-ratings of mental health problems and strengths were associated much more with individual differences than societal/cultural differences, although this varied across scales. These findings support cross-cultural use of standardized measures to assess mental health problems, but urge caution in assessment of personal strengths.

This series of four studies was designed to clarify the underlying dimensionality and psychological well-being correlates of the major extant measures of the maximization tendency: the Maximization Scale (MS; Schwarz et al., 2002) and the Maximization Tendency Scale (MTS; Diab et al., 2008). Four studies using psychometric and factor analysis, item response theory (IRT), and an experimental manipulation all supported the following conclusions. The MS does measure three separate factors as postulated by its authors, but only two of them (alternative search and decisional difficulty) are correlated with each other and (negatively) with indices of well-being as postulated by the scale authors; high standards, the third factor, correlated strongly with the MTS, and both of these were strongly correlated with positive indices of well-being (optimism and happiness) and functioning (e.g., self-esteem and self-efficacy). The high standards subscale and MTS were related to analytical decision making style, while alternative search and decision difficulty were related to the regret-based decision making style and to procrastination. The IRT analysis indicated serious weaknesses in the measurement capabilities of existing scales, and the findings of the experimental study confirmed that alternative search and decision difficulty are related to the maximization tendency while high standards and MTS are not.Implications for further research and scale development are discussed.

We present the Maximization Inventory, which consists of three separate scales: decision difficulty, alternative search, and satisficing. We show that the items of the Maximization Inventory have much better psychometric properties when compared to the original Maximization Scale (Schwartz et al., 2002). The satisficing scale is a new addition to the study of maximization behavior, and we demonstrate that this scale is positively correlated with positive adaptation, whereas the decision difficulty and alternative search scales are positively correlated with nonproductive decisional behavior. The Maximization Inventory was then compared to previous maximization scales and, while the decision difficulty and alternative search scales are positively correlated with similar previous constructs, the satisficing scale offers a dimension entirely different from maximization.

Approximately, 1.7 million individuals in the United States have been infected with SARS-CoV-2, the virus responsible for the novel coronavirus disease-2019 (COVID-19). This has disproportionately impacted adults, but many children have been infected and hospitalised as well. To date, there is not much information published addressing the cardiac workup and monitoring of children with COVID-19. Here, we share the approach to the cardiac workup and monitoring utilised at a large congenital heart centre in New York City, the epicentre of the COVID-19 pandemic in the United States.

Background
Item 9 of the Patient Health Questionnaire-9 (PHQ-9) queries about thoughts of death and self-harm, but not suicidality. Although it is sometimes used to assess suicide risk, most positive responses are not associated with suicidality. The PHQ-8, which omits Item 9, is thus increasingly used in research. We assessed equivalency of total score correlations and the diagnostic accuracy to detect major depression of the PHQ-8 and PHQ-9.
MethodsWe conducted an individual patient data meta-analysis. We fit bivariate random-effects models to assess diagnostic accuracy.
Results16 742 participants (2097 major depression cases) from 54 studies were included. The correlation between PHQ-8 and PHQ-9 scores was 0.996 (95% confidence interval 0.996 to 0.996). The standard cutoff score of 10 for the PHQ-9 maximized sensitivity + specificity for the PHQ-8 among studies that used a semi-structured diagnostic interview reference standard (N = 27). At cutoff 10, the PHQ-8 was less sensitive by 0.02 (−0.06 to 0.00) and more specific by 0.01 (0.00 to 0.01) among those studies (N = 27), with similar results for studies that used other types of interviews (N = 27). For all 54 primary studies combined, across all cutoffs, the PHQ-8 was less sensitive than the PHQ-9 by 0.00 to 0.05 (0.03 at cutoff 10), and specificity was within 0.01 for all cutoffs (0.00 to 0.01).
ConclusionsPHQ-8 and PHQ-9 total scores were similar. Sensitivity may be minimally reduced with the PHQ-8, but specificity is similar.

Background
Different diagnostic interviews are used as reference standards for major depression classification in research. Semi-structured interviews involve clinical judgement, whereas fully structured interviews are completely scripted. The Mini International Neuropsychiatric Interview (MINI), a brief fully structured interview, is also sometimes used. It is not known whether interview method is associated with probability of major depression classification.
AimsTo evaluate the association between interview method and odds of major depression classification, controlling for depressive symptom scores and participant characteristics.
MethodData collected for an individual participant data meta-analysis of Patient Health Questionnaire-9 (PHQ-9) diagnostic accuracy were analysed and binomial generalised linear mixed models were fit.
ResultsA total of 17 158 participants (2287 with major depression) from 57 primary studies were analysed. Among fully structured interviews, odds of major depression were higher for the MINI compared with the Composite International Diagnostic Interview (CIDI) (odds ratio (OR) = 2.10; 95% CI = 1.15–3.87). Compared with semi-structured interviews, fully structured interviews (MINI excluded) were non-significantly more likely to classify participants with low-level depressive symptoms (PHQ-9 scores ≤6) as having major depression (OR = 3.13; 95% CI = 0.98–10.00), similarly likely for moderate-level symptoms (PHQ-9 scores 7–15) (OR = 0.96; 95% CI = 0.56–1.66) and significantly less likely for high-level symptoms (PHQ-9 scores ≥16) (OR = 0.50; 95% CI = 0.26–0.97).
ConclusionsThe MINI may identify more people as depressed than the CIDI, and semi-structured and fully structured interviews may not be interchangeable methods, but these results should be replicated.

Summary
OVERVIEW
Fragmentation poses one of the greatest threats to freshwater fish biodiversity (Nilsson et al., 2005; Reidy-Liermann et al., 2012). Whereas damming of large rivers is perhaps the most obvious form of fragmentation (e.g., Nilsson et al., 2005), smaller, semipermeable barriers such as road crossings (Perkin & Gido, 2012) or water withdrawals that dry sections of a river network (Falke et al., 2011) also pose a conservation challenge. In glacial regions, lakes that are naturally connected through waterways are increasingly being isolated by summer evaporation and groundwater loss (Baki et al., 2012). Climate and land-use changes also isolate populations in headwater reaches by increasing temperatures (Rahel et al., 1996) or drying of streams (Falke et al., 2011) in downstream reaches. Finally, barriers can form when the occurrence of a species, such as a large predator, inhibits the movement of prey through a dispersal corridor (Fraser et al., 1995). This severing of connectivity in aquatic habitats affects species persistence through multiple stressors (Chapters 4 and 6) including limiting dispersal necessary to fulfil important life stages, exacerbating negative species interactions, and inhibiting recolonisation following disturbance. Barriers to movement isolate small populations leading to reduced genetic diversity (Chapter 16) and potentially compromise long-term population persistence (e.g. Wofford et al., 2005).
In this chapter, we discuss how fragmentation disrupts dispersal and migration of freshwater fishes and the long-term consequences for population diversity and stability. We begin with a global overview of the problem followed by a review of theoretical and empirical methods for quantifying the effects of fragmentation on population viability. We conclude with a discussion of conservation challenges along with future research and management recommendations. The primary tenet of our review is that persistence of species in fragmented systems is dependent on the nature of barriers to dispersal and ecological traits of species, particularly their ability to complete critical life-history stages within fragmented habitats (Figure 10.1). We often refer to the terms fragmentation, isolation and connectivity. Whereas there are instances where these might be used interchangeably, we consider fragmentation to represent habitats that have been partitioned into smaller habitats and by extension result in smaller populations. The terms connectivity and isolation refer to the ability or lack of ability, respectively, of fishes to disperse into or out of particular habitats.
Effect of Hybrid stage 1 procedure on ventricular function in infants with hypoplastic left heart syndrome*

Objective
The effect of Hybrid stage 1 palliation for hypoplastic left heart syndrome on right ventricular function is unknown. We sought to compare right ventricular function in normal neonates and those with hypoplastic left heart syndrome before Hybrid palliation and to assess the effect of Hybrid palliation on right ventricular function, using the right ventricular myocardial performance index and the ratio of systolic and diastolic durations.
MethodsWe carried out a retrospective review of echocardiographic data on 23 infants with hypoplastic left heart syndrome who underwent Hybrid palliation and 35 normal controls. Data were acquired before Hybrid and after Hybrid palliation – post 1, 0–4 days; post 2, 1 week; post 3, 2–3 weeks; post 4, 1–1.5 months following Hybrid palliation.
ResultsMyocardial performance index and ratio of systolic and diastolic durations were higher in the pre-Hybrid hypoplastic left heart syndrome group (n=23) – 0.47±0.16 versus 0.25±0.07, p<0.001; 1.59±0.44 versus 1.09±0.14, p<0.0001 – compared with controls (n=35). There was no significant change in the myocardial performance index at any of the post-Hybrid time points. Ratio of systolic and diastolic durations increased significantly 2 weeks after Hybrid – post 3: 2.08±0.62 and post 4: 2.21±0.45 versus pre: 1.59±0.44, p=0.043 and 0.003. There were no significant differences in parameters between sub-groups of infants who died (n=10) and survivors (n=13).
ConclusionsRight ventricular myocardial performance index and ratio of systolic and diastolic durations were significantly higher in infants with hypoplastic left heart syndrome before intervention compared with controls. The ratio of systolic and diastolic durations increased significantly 2 weeks after Hybrid palliation. Our data suggest that infants with hypoplastic left heart syndrome have right ventricular dysfunction before intervention, which worsens over 2 weeks after Hybrid palliation.

Historians, facing “an interconnected crisis of content, method and audience,” have attempted to resolve this crisis by calling upon “a new inter-disciplinary approach to the study and understanding of human memory” (Thelen, 1989a:B1). I believe that such an interdisciplinary approach can shed much light on the phenomenon of ethnicity. I shall attempt to demonstrate this point by examining a number of recent works dealing with the historical identity of the English and with “tribalism” (cultural identity) in southern Africa. Then I shall attempt to apply some of the insights gained to interviews I conducted in Sankuru Sub-Region (Kasai Oriental Region, Republic of Zaire) in 1970.
According to Thelen, the crisis of content in the discipline of history arose out of the need to publish or perish. Scholars have researched and written about increasingly specialized topics. “Common questions about how people make sense of their pasts go unasked” (Thelen 1989a:B1).

Background
Depression is expensive to treat, but providing ineffective treatment is more expensive. Such is the case for many patients who do not respond to antidepressant medication.
AimsTo assess the cost-effectiveness of cognitive–behavioural therapy (CBT) plus usual care for primary care patients with treatment-resistant depression compared with usual care alone.
MethodEconomic evaluation at 12 months alongside a randomised controlled trial. Cost-effectiveness assessed using a cost-consequences framework comparing cost to the health and social care provider, patients and society, with a range of outcomes. Cost-utility analysis comparing health and social care costs with quality-adjusted life-years (QALYs).
ResultsThe mean cost of CBT per participant was £910. The difference in QALY gain between the groups was 0.057, equivalent to 21 days a year of good health. The incremental cost-effectiveness ratio was £14 911 (representing a 74% probability of the intervention being cost-effective at the National Institute of Health and Care Excellence threshold of £20 000 per QALY). Loss of earnings and productivity costs were substantial but there was no evidence of a difference between intervention and control groups.
ConclusionsThe addition of CBT to usual care is cost-effective in patients who have not responded to antidepressants. Primary care physicians should therefore be encouraged to refer such individuals for CBT.

We present an update of the ‘key points’ from the Antarctic Climate Change and the Environment (ACCE) report that was published by the Scientific Committee on Antarctic Research (SCAR) in 2009. We summarise subsequent advances in knowledge concerning how the climates of the Antarctic and Southern Ocean have changed in the past, how they might change in the future, and examine the associated impacts on the marine and terrestrial biota. We also incorporate relevant material presented by SCAR to the Antarctic Treaty Consultative Meetings, and make use of emerging results that will form part of the Intergovernmental Panel on Climate Change (IPCC) Fifth Assessment Report.
