Development of a model to predict antidepressant treatment response for depression among Veterans

Background
Only a limited number of patients with major depressive disorder (MDD) respond to a first course of antidepressant medication (ADM). We investigated the feasibility of creating a baseline model to determine which of these would be among patients beginning ADM treatment in the US Veterans Health Administration (VHA).
MethodsA 2018–2020 national sample of n = 660 VHA patients receiving ADM treatment for MDD completed an extensive baseline self-report assessment near the beginning of treatment and a 3-month self-report follow-up assessment. Using baseline self-report data along with administrative and geospatial data, an ensemble machine learning method was used to develop a model for 3-month treatment response defined by the Quick Inventory of Depression Symptomatology Self-Report and a modified Sheehan Disability Scale. The model was developed in a 70% training sample and tested in the remaining 30% test sample.
ResultsIn total, 35.7% of patients responded to treatment. The prediction model had an area under the ROC curve (s.e.) of 0.66 (0.04) in the test sample. A strong gradient in probability (s.e.) of treatment response was found across three subsamples of the test sample using training sample thresholds for high [45.6% (5.5)], intermediate [34.5% (7.6)], and low [11.1% (4.9)] probabilities of response. Baseline symptom severity, comorbidity, treatment characteristics (expectations, history, and aspects of current treatment), and protective/resilience factors were the most important predictors.
ConclusionsAlthough these results are promising, parallel models to predict response to alternative treatments based on data collected before initiating treatment would be needed for such models to help guide treatment selection.

Background
Fewer than half of patients with major depressive disorder (MDD) respond to psychotherapy. Pre-emptively informing patients of their likelihood of responding could be useful as part of a patient-centered treatment decision-support plan.
MethodsThis prospective observational study examined a national sample of 807 patients beginning psychotherapy for MDD at the Veterans Health Administration. Patients completed a self-report survey at baseline and 3-months follow-up (data collected 2018–2020). We developed a machine learning (ML) model to predict psychotherapy response at 3 months using baseline survey, administrative, and geospatial variables in a 70% training sample. Model performance was then evaluated in the 30% test sample.
Results32.0% of patients responded to treatment after 3 months. The best ML model had an AUC (SE) of 0.652 (0.038) in the test sample. Among the one-third of patients ranked by the model as most likely to respond, 50.0% in the test sample responded to psychotherapy. In comparison, among the remaining two-thirds of patients, <25% responded to psychotherapy. The model selected 43 predictors, of which nearly all were self-report variables.
ConclusionsPatients with MDD could pre-emptively be informed of their likelihood of responding to psychotherapy using a prediction tool based on self-report data. This tool could meaningfully help patients and providers in shared decision-making, although parallel information about the likelihood of responding to alternative treatments would be needed to inform decision-making across multiple treatments.

Palynological, loss-on-ignition, and X-ray fluorescence data from a 5.25 m sediment core from a mangrove forest at the mouth of the Shark River Estuary in the southwestern Everglades National Park, Florida were used to reconstruct changes occurring in coastal wetlands since the mid-Holocene. This multi-proxy record contains the longest paleoecological history to date in the southwestern Everglades. The Shark River Estuary basin was formed ~ 5700 cal yr BP in response to increasing precipitation. Initial wetlands were frequently-burned short-hydroperiod prairies, which transitioned into long-hydroperiod prairies with sloughs in which peat deposits began to accumulate continuously about 5250 cal yr BP. Our data suggest that mangrove communities started to appear after ~ 3800 cal yr BP; declines in the abundance of charcoal suggested gradual replacement of fire-dominated wetlands by mangrove forest over the following 2650 yr. By ~ 1150 cal yr BP, a dense Rhizophora mangle dominated mangrove forest had formed at the mouth of the Shark River. The mangrove-dominated coastal ecosystem here was established at least 2000 yr later than has been previously estimated.

Photon Management for Near-Total Solar Absorption in Hematite Photoanodes

Using first-principles full-field electromagnetic simulations, we demonstrate that near-perfect above-band-gap solar absorption can be achieved in nanostructured, ultra-thin-film iron oxide photoanodes for photoelectrochemical (PEC) water splitting. In our designed core-shell nanocone structures, all regions of hematite (α-iron oxide) are away from the interface between hematite and water by a minimum distance of less than the hole diffusion length in hematite, which is assumed to be no greater than 20nm. The optical absorption in our structure corresponds to a photocurrent density of 12.5mA/cm2 if one assumes an air mass 1.5 solar spectrum and a unity absorbed photon-to-current efficiency (APCE) for all wavelengths in that spectrum. Our photon management strategy eliminates the trade-off between optical absorption and carrier collection as commonly found in conventional designs of PEC cells, and variants of the strategy are generally applicable to other material systems.

In this article, we propose a causal relationship between a region's communist revolutionary legacy before 1949 and the variation in private sector development after 1949. In the case of Zhejiang, the pre-1949 revolutionary experience led to the power struggle between two elite groups, the guerrilla cadre group and the southbound cadre group, in the province after 1949. As the weak side, guerrilla cadres were willing to protect local economic interests in exchange for local popular support, which improved their odds of political survival. As a result, in contrast with counties where the guerrilla forces were historically weak, counties with strong guerrilla forces before 1949 saw significantly more robust private sector development throughout much of the Mao and post-Mao periods. In this article we provide preliminary historical and statistical evidence to support this hypothesis.

Spectacular economic growth in China suggests the ruling Chinese Communist Party (CCP) has somehow gotten it right. A key hypothesis in both economics and political science is that the CCP's cadre evaluation system, combined with China's geography-based governing logic, has motivated local administrators to compete with one another to generate high growth. We raise a number of theoretical and empirical challenges to this claim. Using a new biographical database of Central Committee members, a previously overlooked feature of CCP reporting, and a novel Bayesian method that can estimate individual-level correlates of partially observed ranks, we find no evidence that strong growth performance was rewarded with higher party ranks at any of the postreform party congresses. Instead, factional ties with various top leaders, educational qualifications, and provincial revenue collection played substantial roles in elite ranking, suggesting that promotion systems served the immediate needs of the regime and its leaders, rather than encompassing goals such as economic growth.

Summary
The political elite was an early focus of Western analysis of the People’s Republic of China, partly because information about the elite was relatively available in the opaque political system. As China opened up, information about all aspects of Chinese politics became increasingly available, thus decreasing the relative importance of elite studies in China. In this chapter, we first argue that elite studies continue to be important and necessary for understanding authoritarian regimes like China. Further, we introduce a dataset of Central Committee (CC) members that combines traditional elite studies with new coding and statistical methodologies. The data are used to trace several basic characteristics of the CC through the entire history of the Chinese Communist Party (CCP), and to measure the relative influence of Hu Yaobang and Hua Guofeng in the CC. We further discuss the potential of this dataset for providing more systematic evidence of how elite characteristics and elite conflict affect policy and political outcomes in China.
Elite Studies in Authoritarian Regimes
At a time when China was sealed off from the rest of the world, Western scholars of China relied mainly on the official press and in some cases military sources from Taiwan to make scholarly inferences. As the heroic deeds and – at times – the “errors” of senior leaders were often the foci of official press coverage, Western scholars naturally made extensive use of this relatively abundant source of information, thus creating a heavy emphasis on the elite. Today, China scholars have a wide range of sources available to them, ranging from internal government documents to interviews to survey instruments. The proliferation of new sources has given rise to the study of grassroots-level political phenomena in China and a decline in the relative importance of elite studies. However, the authoritarian nature of the Chinese regime provides compelling reason to continue elite studies.

Can one man dominate the Chinese Communist Party? This has been a much debated issue in the field of Chinese politics. Using a novel database that tracks the biographies of all Central Committee (CC) members from 1921 to 2007, we derive a measure of top CCP leaders' factional strength in the CC. We show that Mao could not maintain a commanding presence in the Party elite after the Eighth Party Congress in 1956, although the Party chairman enjoyed a prolonged period of consolidated support in the CC at a time when the CCP faced grave external threats. No Chinese leader, not even Mao himself, could regain the level of influence that he had enjoyed in the late 1940s. Our results, however, do not suggest that a “code of civility” has developed among Chinese leaders. The Cultural Revolution saw the destruction of Liu Shaoqi's faction. Although violent purges ended after the Cultural Revolution, Chinese leaders continued to promote followers into the CC and to remove rivals' followers.
Magnetron Sputtering for II-VI Solar Cells: Thinning the CdTe

Magnetron sputtering (MS) of CdTe and related II-VI materials facilitates low energy ion and electron bombardment that promotes good film growth at substrate temperatures well below those needed for other physical vapor deposition methods. MS also provides good control of deposition rates while allowing scale-up to large areas. In this paper we review the use of MS for deposition of polycrystalline thin films of CdS, CdTe and related materials for solar cells with a focus on reducing the thickness. We relate the deposition conditions and plasma properties determined by Langmuir probe measurements to some of the materials properties of the films through spectroscopic ellipsometry and high resolution electron microscopy. For cells with CdTe layers from 0.35 to 2.5 μm, we have done a first-order optimization of chloride treatment conditions and back contact structure. We discuss the influence of CdTe thickness and post-deposition processing on the efficiency, open-circuit voltage, short-circuit current, and fill factor and show that 10% efficient cells can be fabricated with 0.5 μm of CdTe.

In this paper, various types of defects (both threading dislocation and misfit dislocations) in strained Si (sSi) have been analyzed by transmission electron microscopy (TEM). Germanium upper-diffusion has been studied by scanning transmission electron microscopy (STEM) for strained Si on SiGe/SOI. SGOI-devices processed using an optimized thermal budget show minimal Ge diffusion and minimal process related defects. Correlation between the device performance (such as leakage current and reliability) and structural information found in TEM has been established.
