Precision Medicine is an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle. Autoimmune diseases are those in which the body’s natural defense system loses discriminating power between its own cells and foreign cells, causing the body to mistakenly attack healthy tissues. These conditions are very heterogeneous in their presentation and therefore difficult to diagnose and treat. Achieving precision medicine in autoimmune diseases has been challenging due to the complex etiologies of these conditions, involving an interplay between genetic, epigenetic, and environmental factors. However, recent technological and computational advances in molecular profiling have helped identify patient subtypes and molecular pathways which can be used to improve diagnostics and therapeutics. This review discusses the current understanding of the disease mechanisms, heterogeneity, and pathogenic autoantigens in autoimmune diseases gained from genomic and transcriptomic studies and highlights how these findings can be applied to better understand disease heterogeneity in the context of disease diagnostics and therapeutics.

SHEA endorses adhering to the recommendations by the CDC and ACIP for immunizations of all children and adults. All persons providing clinical care should be familiar with these recommendations and should routinely assess immunization compliance of their patients and strongly recommend all routine immunizations to patients. All healthcare personnel (HCP) should be immunized against vaccine-preventable diseases as recommended by the CDC/ACIP (unless immunity is demonstrated by another recommended method). SHEA endorses the policy that immunization should be a condition of employment or functioning (students, contract workers, volunteers, etc) at a healthcare facility. Only recognized medical contraindications should be accepted for not receiving recommended immunizations.

We study stimulated generation – the transfer of energy from balanced flows to existing internal waves – using an asymptotic model that couples barotropic quasi-geostrophic flow and near-inertial waves with $\text{e}^{\text{i}mz}$ vertical structure, where $m$ is the vertical wavenumber and $z$ is the vertical coordinate. A detailed description of the conservation laws of this vertical-plane-wave model illuminates the mechanism of stimulated generation associated with vertical vorticity and lateral strain. There are two sources of wave potential energy, and corresponding sinks of balanced kinetic energy: the refractive convergence of wave action density into anti-cyclones (and divergence from cyclones); and the enhancement of wave-field gradients by geostrophic straining. We quantify these energy transfers and describe the phenomenology of stimulated generation using numerical solutions of an initially uniform inertial oscillation interacting with mature freely evolving two-dimensional turbulence. In all solutions, stimulated generation co-exists with a transfer of balanced kinetic energy to large scales via vortex merging. Also, geostrophic straining accounts for most of the generation of wave potential energy, representing a sink of 10 %–20 % of the initial balanced kinetic energy. However, refraction is fundamental because it creates the initial eddy-scale lateral gradients in the near-inertial field that are then enhanced by advection. In these quasi-inviscid solutions, wave dispersion is the only mechanism that upsets stimulated generation: with a barotropic balanced flow, lateral straining enhances the wave group velocity, so that waves accelerate and rapidly escape from straining regions. This wave escape prevents wave energy from cascading to dissipative scales.

Korscheltellus gracilis (Grote) is a pest of red spruce and balsam fir roots in the forests of the northeastern United States and southeastern Canada. The larvae feed below ground on woody and non-woody plant tissues for 2 years before maturing. It was found at 18 localities across New England and Nova Scotia during the summers of 1987 and 1988. On Whiteface Mt., New York, and Mt. Moosilauke, New Hampshire, larvae were absent to uncommon at low (500 m) and high (1300 m) altitudes, but common to abundant in red spruce – balsam fir associations at 700, 900, and 1100 m. During September 1987, larval populations were estimated at 56 000 and 312 000 per hectare in soils at 900 m on Whiteface and Moosilauke, respectively. Dense populations occurred locally, e.g. 27 larvae were recovered from a single 0.25-m2 soil pit at 900 m on Moosilauke. By June 1988, larval numbers on Whiteface and Moosilauke had declined by 51% and 72%, respectively. Seedlings and mature trees of both spruce and fir were damaged by larval feeding on bark and cambium. Seedlings were girdled; mature trees had wounds paralleling the root axes.

Thermal Plasma Chemical Vapor Deposition of Superhard Nanostructured Si-C-N Coatings

A triple torch plasma reactor was used to synthesize Si—C—N composite films via the thermal plasma chemical vapor deposition process. The argon-nitrogen plasma provided atomic nitrogen to carbon- and silicon-based reactants, which were injected through a central injection probe and ring configuration. Films were deposited with variations of the total nitrogen flow through the torches (1.5-4.5slm), reactant mixture (silicon tetrachloride and acetylene or hexamethyldisilazane) and substrate material (silicon and molybdenum). Micro X-ray diffraction was used to determine that both α-Si3N4 and β-Si3N4 were dominant in most of the depositions. Composites of silicon nitride and silicon carbide were synthesized on molybdenum. The bonding of amorphous phases was investigated using Fourier transform infrared spectroscopy, which indicated the presence of N—H, CHx and CΞ in various films. Indentation tests on the polished film cross-sections determined that large variations in hardness and elastic modulus existed for minor changes in film composition. Correlations between indentation results and scanning electron and optical microscope images showed that the mechanical properties greatly depend on the film morphology; the denser, smoother, and more crystalline films tended to display enhanced mechanical properties.

The structure of germanium thin films prepared on glass by plasma enhanced chemical vapor deposition was characterized by Raman spectroscopy, atomic force microscopy (AFM) and field emission scanning electron microscopy (SEM). Crystallinity, surface roughness, and grain size were measured as functions of film thickness and deposition temperature. Grain nucleation was apparent for films as thin as 10 nm. Over the thickness range studied, grain size increased with film thickness, whereas average surface roughness started to increase with film thickness, but then remained fairly constant at approximately 1 nm for a film thickness greater than 25 nm.

The eruptive variable V838 Mon was discovered on Jan 6, 2002. Due to a subsequent phase of almost constant brightness and a spectral appearance which is unlike classical novae, speculations have been made about its nature. Either it was a very peculiar, slow nova defining a new class, an eruptive event in an evolved star as in the case of Sakurai’s Object but in a much earlier phase, or something completely different.

Investigations describing the utilization pattern and comparing the outcome from emergency and mass casualty situations are limited by the lack of a reliable and valid patient classification system. In this study we briefly describe the use of APACHE (Acute Physiology and Chronic Health Evaluation), a physiologically based classification system for measuring severity of illness in groups of critically ill patients, as a tool in comparing outcomes of 1437 ICU admissions from eight European and five American hospitals. Because of the successful results from this pilot effort, we believe that APACHE could be used to compare the performance of hospitals in an emergency or mass casualty situation.

Fasting plasma triglycerides and cholesterol, as well as cholesterol in very low density, low density and high density lipoproteins, were measured in 250 MZ and 264 DZ adult male twin pairs. A new method was used to choose an estimate of genetic variance. This includes an estimate of genetic variance for use when the total variances of MZ and DZ twins are unequal. DZ twins had greater total variance for cholesterol. When genetic variance was estimated by subtracting the within-MZ-sets mean square from the within-DZ-sets mean square, all of the lipids had significant estimates of genetic variance; however, when genetic variance was estimated by a method designed to correct bias due to unequal total variances of MZ and DZ twins, only triglycerides had significant genetic variance. The heritability of plasma triglycerides was calculated to be 0.68.
