Variation in clinical care associated with weekend admission and discharge in psychiatric in-patient units: retrospective case-note review

Background
Questions have been raised regarding differences in the standards of care that patients receive when they are admitted to or discharged from in-patient units at weekends.
AimsTo compare the quality of care received by patients with anxiety and depressive disorders who were admitted to or discharged from psychiatric hospital at weekends with those admitted or discharged during the ‘working week’.
MethodRetrospective case-note review of 3795 admissions to in-patient psychiatric wards in England. Quality of care received by people with depressive or anxiety disorders was compared using multivariable regression analyses.
ResultsIn total, 795 (20.9%) patients were admitted at weekends and 157 (4.8%) were discharged at weekends. There were minimal differences in quality of care between those admitted at weekends and those admitted during the week. Patients discharged at weekends were less likely to be given sufficient notification (48 h) in advance of being discharged (OR = 0.55, 95% CI 0.39–0.78), to have a crisis plan in place (OR = 0.65, 95% CI 0.46–0.92) or to be given medication to take home (OR = 0.45, 95% CI 0.30–0.66). They were also less likely to have been assessed using a validated outcome measure (OR = 0.70, 95% CI 0.50–0.97).
ConclusionsThere is no evidence of a ‘weekend effect’ for patients admitted to psychiatric hospital at weekends, but the quality of care offered to those who were discharged at weekends was relatively poor, highlighting the need for improvement in this area.

Summary
This chapter provides an overview of the use of affect-based interventions to change behavior. Affect is defined in terms of affect proper and affect processing; both of these terms are used regularly in research on affect interventions. The evidence of direct modification of these affect constructs is then reviewed. Based on this evidence, step-by-step guides to techniques focusing on changing two key aspects of affective processing are provided: changing affective attitudes and anticipated affect. The guides to these techniques include typical means of delivery, target audience, behaviors, enabling or inhibiting factors, training and skills required, intensiveness, typical materials needed, and typical examples of implementation. In addition, application of implementation intentions, fear appeals, evaluative conditioning, and exercise games as other ways to change affect as a means to changing behavior are reviewed. Finally, two additional intervention pathways that could have impact on behavior change are reviewed: direct modification of other sources of behavioral influence (e.g., traditional social cognitive factors) in order to overcompensate for the impact of affect and self-regulation of the intensity of the affect experience as a means of inhibiting its impact.

The problem of plasma expansion into a vacuum is revisited with the addition of a finite boundary condition; an electrically insulated surface. As plasma expands towards a charge-accumulating surface, the leading electron cloud charges the surface negatively, which in turn repels electrons and attracts ions. This plasma–surface interaction is shown to result in a feedback process which accelerates the plasma expansion. In addition, we examine the decrease in (negative) surface potential and associated near-surface electron density. To investigate this plasma coupling with an electrically floating surface, we develop an analytic model including four neighbouring plasma regions: (i) undisturbed plasma, (ii) quasi-neutral self-similar expansion, (iii) ion front boundary layer and (iv) electron cloud. A key innovation in our approach is a self-contained analytic approximation of the ion front boundary layer, providing a spatially continuous electric field model for the early phase of bounded plasma expansion.

The combination of sensitivity and large sky coverage of the ALFALFA HI survey has enabled the detection of difficult to observe low mass galaxies in large numbers, including dwarf galaxies overlooked in optical surveys. Three different, but connected, studies of dwarf galaxies from the ALFALFA survey are of particular interest: SHIELD (Survey of HI in Extremely Low-mass Dwarfs), candidate gas-rich ultra-faint dwarf galaxies, and the (Almost) Dark population. SHIELD is a systematic multiwavelength study of all dwarf galaxies from ALFALFA with MHI < 107.2M⊙ and clear optical counterparts. Candidate gas-rich ultra-faint dwarf galaxies extend the dwarf galaxy population to even lower masses. These galaxies are identified as isolated HI clouds with no discernible optical counterpart but subsequent observations reveal that some are extremely faint, gas-dominated galaxies. Leo P, discovered first as an HI detection, and then found to be an actively star-forming galaxy, bridges the gap between these candidate galaxies and the SHIELD sample. The (Almost) Dark sample consists of galaxies whose optical counterparts are overlooked in current optical surveys but which are clear detections in ALFALFA. This sample includes field gas-rich ultra-diffuse galaxies. Coma P, with a peak surface brightness of only ∼26.4 mag arcsec−2 in g’, demonstrates the sort of extreme low surface brightness galaxy that can be discovered in an HI survey.
Utility of Aminocyclopyrachlor for Control of Horsenettle (Solanum carolinense) and Tall Ironweed (Vernonia gigantea) in Cool-Season Grass Pastures

Because horsenettle and tall ironweed are difficult to control in cool-season grass pastures, research was conducted in Tennessee and Kentucky in 2010 and 2011 to examine the efficacy of aminocyclopyrachlor on these weeds. Aminocyclopyrachlor was evaluated at 49 and 98 g ai ha−1 alone and in mixtures with 2,4-D amine at 371 and 742 g ae ha−1. Aminopyralid was also included as a comparison treatment at 88 g ai ha−1. Treatments were applied at three POST timings to horsenettle and two POST timings to tall ironweed. By 1 yr after treatment (YAT) horsenettle was controlled 74% with aminocyclopyrachlor plus 2,4-D applied late POST (LPOST) at 98 + 742 g ha−1. By 1 YAT, tall ironweed was controlled ≥ 93% by aminocyclopyrachlor applied early POST (EPOST) or LPOST, at rates as low as 49 g ha−1. Similar control was achieved with aminopyralid applied LPOST. Both aminocyclopyrachlor and aminopyralid were found to reduce horsenettle and tall ironweed biomass the following year. Moreover, all LPOST applications of aminocyclopyrachlor alone or in mixtures with 2,4-D prevented regrowth of tall ironweed at 1 YAT. Based on these studies, a LPOST herbicide application in August or September when soil moisture is adequate is recommended for control of horsenettle and tall ironweed in cool-season grass pastures.

Nations intending to send teams to compete in the equestrian sports at the 2008 Olympic Games in August in Hong Kong were concerned that safe competition would not be possible in this environment. Our objective was to demonstrate that horses could safely compete in the Hong Kong climate, where identifying and treating dehydration and electrolyte losses would be especially important to horse welfare. We hypothesized that evaluating serial changes in body weight, urine specific gravity (USG), urine Na concentration (UNa) and urinary clearance of Na (CLNa) would allow us to identify dehydration and Na deficits. Seventeen horses were housed in air-conditioned stables maintained at 25°C. Vital signs and body weight were recorded twice daily. Twenty-four-hour water consumption, and number and duration of exercise periods, were recorded daily. Haematological and biochemical variables, USG, UNa and CLNa, were measured intermittently in subsets of horses. Environmental conditions were monitored daily. Fifteen horses completed the competition; vital signs and all mean values for haematological and biochemical variables (except a high mean total protein on day 1 after arrival) remained within reference ranges. Daily water intake was (mean ± SE) 38 ± 1.5 l (range 18–74 l). Mean change in body weight during the entire study was − 3 ± 2.4 kg (range − 17 to 14 kg). Horses spent at most 3.5 h outside and at least 20.5 h in the stables daily. The mean value for USG was 1.035 ± 0.0008 and 57% of determinations were >1.035. The mean UNa was 61 ± 8 mmol l− 1 and 57% of values were ≤ 20 mmol l− 1. The mean CLNa was 0.14 ± 0.0.04%, with 76% of determinations being ≤ 0.1%. An 88% completion rate demonstrated that safe competition was possible. Access to climate-controlled stables probably helped to minimize daily fluid and electrolyte losses, and facilitated recovery following exercise. A majority of USG values >1.035, UNa < 20 mmol l− 1 in 29% of determinations, and CLNa < 0.1% in 57% of determinations suggested that some horses may have had insufficient Na intake while in Hong Kong. Monitoring USG, UNa and CLNa may aid in identifying and treating fluid and Na deficits in competition horses.

Experiences of substance-using suicidal males who present frequently to the emergency department

Objective:
This qualitative study investigated the repeated use of the emergency department (ED) by men with a history of suicidal behaviour and substance abuse to understand the needs and barriers to care for this high-risk group. Identification of common themes from interviews with patients and health care workers can serve as a basis for improved ED-based interventions.
Methods:Using semistructured interviews, patients, ED staff and family physicians were asked about needs of the aformentioned group. Twenty-five patients were interviewed and completed questionnaires regarding their substance use, aggression, parasuicidal behaviour, alexithymia and childhood trauma. In addition, 27 staff members were interviewed. Interviews were tape-recorded, transcribed and qualitatively analyzed using an iterative coding process.
Results:Of the 25 patients, 23 (96%) had a mood or anxiety disorder and 18 (75%) had borderline personality disorder. One-half of the patients scored high and another quarter scored moderate on alexithymia testing. The ED was viewed as a last resort despite seeking help. Frustration was felt by both patients and staff regarding difficult communication, especially during an acute crisis.
Conclusion:The ED plays an important role in the provision of care for men with recurrent suicidal behaviour and substance abuse. Some of the diagnoses and problems faced by these patients are beyond the purview of the ED; however, staff can identify mutual goals for crisis interventions, allow for frequent communication and seek to de-escalate situations through the validation of the stress patients are experiencing.
Long-term performance of white clover varieties grown with perennial ryegrass under rotational grazing by sheep with different nitrogen applications

Three experiments were carried out to investigate the dry matter production of nine white clover (Trifolium repens L.) varieties grown with the same variety of perennial ryegrass (Lolium perenne L.) and subjected to rotational grazing by sheep over a period of 10 years. The experiments differed only in the nitrogen fertilizer added: no nitrogen, 100 kg/ha per year and 200 kg/ha per year. Dry matter (DM) yields of white clover and perennial ryegrass components of the sward were recorded at approximately 6-week intervals during the growing season.
Without applied nitrogen, 10-year mean white clover annual DM yields ranged from 2·89–4·54 t/ha. Mean clover content for all varieties ranged from 31–50% for the first 9 years, declining to 20% in the 10th year. The most consistently high yielding variety, AberVantage, gave yields greater than 3·5 t/ha in every year. Similar results were observed for the variety Aran. The 10-year mean annual DM yield of the perennial ryegrass companion (variety Talbot) ranged from 4·41–5·57 t/ha depending on the white clover variety in the mixture. Variation in perennial ryegrass DM yield was of a similar range and pattern as for white clover.
In the experiment in which 100 kg N/ha per year was added, 10-year mean white clover annual DM yields ranged from 2·27–3·87 t/ha. The varieties AberVantage and Aran showed yields that fell below 3 t/ha per year in only 1 or 2 years, respectively. Mean clover content for all varieties ranged from 26–38% for the first 9 years, declining to 17·5% in the 10th year. There was no consecutive annual decline in white clover DM yields. The 10-year mean annual DM yields of the perennial ryegrass companion ranged from 6·03–7·57 t/ha.
With applied nitrogen fertilizer of 200 kg N/ha per year (Experiment 3), 10-year mean white clover annual DM yields ranged from 1·21–2·67 t/ha. Mean clover content ranged from 12–27% for the 10 years. AberVantage and Aran showed yields consistently above 2 t/ha. No general decline in white clover DM yields was observed although some varieties showed consecutive annual reductions in years 8, 9 and 10. The 10-year mean annual DM perennial ryegrass yields ranged from 7·27–8·68 t/ha.
The results show that, under rotational grazing by sheep, white clover can give reliably high yields over a 10-year period. With moderate to high applications of nitrogen fertilizer white clover yields were lower than without nitrogen but they were consistent and sufficient to make a considerable impact on forage quality. There was no evidence for a general decline in clover content until the 10th year of the experiment and no cyclical pattern of annual variation in white clover yield was observed. The implications of these results for the role of white clover in UK agriculture are discussed.

A vital component to nanoparticle science will be the three dimensional (3-D) characterization of both structure and chemistry of these nanoparticles on their supports at the nanometer scale and below. to achieve this goal, quantitative Z-contrast and atomic resolution will provide essential information about their structure. Z-contrast imaging is ideal for imaging these large Z nanoparticles on low Z supports. in this proceedings, we present a quantitative Z-contrast method to determine number of atoms and a few examples of a combination of electron microscopy methods to gain structural insights into supported nanoparticle, such as Pt on different support materials, PtRu5 on C and Pt-Sn on SiO2.
A relatively new and powerful method is to determine the number of atoms in a nanoparticle, by very high angle annular dark-field (HAADF) imaging or Z-contrast technique [1, 2]. We have shown that quantification of the absolute image intensity from very HAADF microscopy will provide the number of atoms in very small particles of high atomic number to ±2 atoms for Re6 nanoparticles supported on carbon [3].

The Medium-l Program of the Michelson Doppler Imager (MDI) instrument on board SOHO provides continuous observations of oscillation modes of angular degree, l, from 0 to ∼ 300. The initial results show that the noise in the Medium-l oscillation power spectrum is substantially lower than in ground-based measurements. This enables us to detect lower amplitude modes and, thus, to extend the range of measured mode frequencies. The MDI observations also reveal the asymmetry of oscillation spectral lines. The line asymmetries agree with the theory of mode excitation by acoustic sources localized in the upper convective boundary layer. The sound-speed profile inferred from the mean frequencies gives evidence for a sharp variation at the edge of the energy-generating core. In a thin layer just beneath the convection zone, helium appears to be less abundant than predicted by theory. Inverting the multiplet frequency splittings from MDI, we detect significant rotational shear in this thin layer.

Our aim, in this paper, is to study a class of functions which occurs in pure integer programming, and to investigate conditions under which discrete subadditive functions belong to that class. The inspiration for the paper was the problem of classifying discrete metrics used in pattern recognition, while the methods of proof of the main theorem are those of pure integer programming.
