Evaluation of an experiential training program in patient-centered outcomes and comparative effectiveness research for diverse researcher communities and health care organizations

Background/Objective:
The goal of the Patient-Centered Outcomes Research Partnership was to prepare health care professionals and researchers to conduct patient-centered outcomes and comparative effectiveness research (CER). Substantial evidence gaps, heterogeneous health care systems, and decision-making challenges in the USA underscore the need for evidence-based strategies.
Methods:We engaged five community-based health care organizations that serve diverse and underrepresented patient populations from Hawai’i to Minnesota. Each partner nominated two in-house scholars to participate in the 2-year program. The program focused on seven competencies pertinent to patient-centered outcomes and CER. It combined in-person and experiential learning with asynchronous, online education, and created adaptive, pragmatic learning opportunities and a Summer Institute. Metrics included the Clinical Research Appraisal Inventory (CRAI), a tool designed to assess research self-efficacy and clinical research skills across 10 domains.
Results:We trained 31 scholars in 3 cohorts. Mean scores in nine domains of the CRAI improved; greater improvement was observed from the beginning to the midpoint than from the midpoint to conclusion of the program. Across all three cohorts, mean scores on 52 items (100%) increased (p ≤ 0.01), and 91% of scholars reported the program improved their skills moderately/significantly. Satisfaction with the program was high (91%).
Conclusions:Investigators that conduct patient-centered outcomes and CER must know how to collaborate with regional health care systems to identify priorities; pose questions; design, conduct, and disseminate observational and experimental research; and transform knowledge into practical clinical applications. Training programs such as ours can facilitate such collaborations.

Background
Cognitive impairments are well-established features of psychotic disorders and are present when individuals are at ultra-high risk for psychosis. However, few interventions target cognitive functioning in this population.
AimsTo investigate whether omega-3 polyunsaturated fatty acid (n−3 PUFA) supplementation improves cognitive functioning among individuals at ultra-high risk for psychosis.
MethodData (N = 225) from an international, multi-site, randomised controlled trial (NEURAPRO) were analysed. Participants were given omega-3 supplementation (eicosapentaenoic acid and docosahexaenoic acid) or placebo over 6 months. Cognitive functioning was assessed with the Brief Assessment of Cognition in Schizophrenia (BACS). Mixed two-way analyses of variance were computed to compare the change in cognitive performance between omega-3 supplementation and placebo over 6 months. An additional biomarker analysis explored whether change in erythrocyte n−3 PUFA levels predicted change in cognitive performance.
ResultsThe placebo group showed a modest greater improvement over time than the omega-3 supplementation group for motor speed (ηp2 = 0.09) and BACS composite score (ηp2 = 0.21). After repeating the analyses without individuals who transitioned, motor speed was no longer significant (ηp2 = 0.02), but the composite score remained significant (ηp2 = 0.02). Change in erythrocyte n-3 PUFA levels did not predict change in cognitive performance over 6 months.
ConclusionsWe found no evidence to support the use of omega-3 supplementation to improve cognitive functioning in ultra-high risk individuals. The biomarker analysis suggests that this finding is unlikely to be attributed to poor adherence or consumption of non-trial n−3 PUFAs.

Summary
Delmer Daves’ 1950 film Broken Arrow is credited in histories of the Western genre with inaugurating a cycle of “pro-Indian” (or “liberal” or “adult”) Westerns that began in the early 1950s and lasted until the early 1960s. Based on true events, the film follows former Army Scout Tom Jeffords (played by James Stewart) as he is initiated into the Chiricahua Apache tribe led by the respected chief Cochise (Jeff Chandler). Overcoming bad actors on both sides of the racial divide and the murder of his Apache wife, Sonseeahray (Debra Paget), Jeffords ultimately brokers a peace treaty between the Apache and the U.S. Government.
Though Broken Arrow was not the first post-war Western to take a compassionate view of American Indians—consider John Ford's Fort Apache from 1948 and She Wore a Yellow Ribbon from 1949—the film initiated a wave of Indian-themed films in the 1950s and 60s, the majority of which, as Edward Buscombe has observed, offer a positive interpretation of Indian history and culture (within, of course, the limits of generic constraints). Like most Westerns of the period, Broken Arrow takes liberties with its representation of people and events from American history. This includes the portrayal of its Indian protagonist, Cochise, who is, among other things, played by a Caucasian actor—in accordance with Hollywood's standard casting practices.
Broken Arrow and the films that followed it, including titles like Apache (Robert Aldrich, 1954), Broken Lance (Edward Dmytryk, 1954), White Feather (Robert D. Webb, 1955), Run of the Arrow (Samuel Fuller, 1957), Flaming Star (Don Siegel,1960), and Cheyenne Autumn (John Ford, 1964), featured sympathetic, and at times heroic, portrayals of Indian characters as well as attempts to portray Indian customs with a greater degree of authenticity and detail. In Broken Arrow the Apache are depicted, accurately, as residing in wikiups rather than in tipis. The narrative includes the depiction of Apache customs, including the girls’ puberty rite and the social dance, which is performed by members of the White Mountain Apache Tribe of Arizona's Fort Apache Reservation. The Apache characters also speak in standard (if formal) English, a significant change from Indians speaking in broken, pidgin English, as was conventional at the time.

I much prefer the audience not to know that there's a director. That's my general thesis in regard to directing.
Delmer DavesDelmer Daves is the property of those who can enjoy stylistic conviction in an intellectual vacuum. The movies of Delmer Daves are fun of a very special kind. Call it Camp or call it Corn. The director does not so much transcend his material as mingle with it.

Jean-Pierre Coursodon, American DirectorsIt is tempting, in introducing a book of this nature, to declare at the outset something like, “Delmer Daves is the best filmmaker you've never heard of!” Alas, we cannot resort to such rhetoric—not because Delmer Daves is not a great, critically overlooked filmmaker, but because you likely have heard of him. Daves is remembered principally as a maker of Westerns, and of two Westerns in particular: Broken Arrow (1950) and 3:10 to Yuma (1957). The former, about an Army scout who brokers a peace treaty between the American government and the Chiricahua Apache, offered viewers a comparatively nuanced, sympathetic depiction of Native American culture, and in so doing helped to transform how American Indians are represented in cinema. The latter, a stylish, psychological frontier drama about a poor farmer who agrees to help escort a dangerous outlaw to the train that will take him to prison, is frequently considered among the best Westerns of the 1950s, and was even subject to a high-profile remake in 2007. As these synopses suggest, however, Broken Arrow and 3:10 to Yuma are quite different films, and are remembered for different reasons: the former for its sociocultural significance, the latter for its style. This perceived difference, it turns out, matters a great deal.
Making the jump from discussing individual films to discussing filmmakers normally requires us to detect consistency, not difference. Identifying recurring features of style or theme across multiple films implies the underlying presence of an impelling, organizing personality: an artist who imbues his work with his own vision of the world.

Summary
From the late 1930s up until the 1960s, the western was one of the most popular movie genres in the United States, and westerns routinely ranked among the highest earning films in any given year. This period, often referred to as the genre's golden age, saw the release of celebrated westerns such as Stagecoach (John Ford, 1939), Red River (Howard Hawks, 1948), High Noon (Fred Zinnemann, 1952), Shane (George Stevens, 1953), The Searchers (John Ford, 1956), Rio Bravo (Howard Hawks, 1959), The Magnificent Seven (John Sturges, 1960), along with countless others. In the 1940s and 1950s, westerns consistently made up one-quarter of all films produced in the United States, roughly one hundred westerns a year. Even into the 1960s, moviegoers could reliably expect twenty new westerns annually. This was a time when overall film production levels in Hollywood were dropping, such that westerns still made up about 15 percent of movies playing in American cinemas. Beginning in the late 1960s, however, after nearly thirty years of being one of Hollywood's most reliably popular products, the western experienced a precipitous decline. This fall was reflected in not only a dramatic decrease in the number of films being made, but also a more negative critical response to the genre by both critics and filmmakers. Production of westerns fell throughout the 1960s and 1970s, and by the 1980s the western had faded away as a popular genre. Despite a number of small revivals in the ensuing decades – most notably in the early and mid-1990s, after the releases of Dances with Wolves (Kevin Costner, 1990) and Unforgiven (Clint Eastwood, 1992) – this state of affairs remains largely unchanged. “The genre … is more or less dead,” proclaimed the critic Scott Eyman in 2014, “except when a powerful director or star gets an urge to make a vanity western.”
The narrative of the Hollywood western's rise and fall is as well known as it is lamented. It also raises a number of questions, which scholarship on the genre has spent more than a half-century debating. Where did the western originate in the first place? What made the western so popular with audiences in the mid-twentieth century? And how was a genre that once dominated our movie screens relegated to the sidelines of popular culture?

A simple hydrothermal route to the eulytite phase of bismuth germanium oxide (E-BGO: Bi4(GeO4)3) that required no post-processing has been developed. The E-BGO material was isolated from a mixture of bismuth nitrate pentahydrate and a slight excess of germanium oxide in water under hydrothermal conditions (185 °C for 24 h). The resultant materials were characterized by powder x-ray diffraction, scanning electron microscopy, transmission electron microscopy, and luminescence measurements to verify the particle's phase (eulytite), morphology, size, and response to a variety of excitation energy sources, respectively. Photoluminescence spectroscopic response from E-BGO pellets indicated that the samples exhibited a strong emission peak consistent with an x-ray induced luminescence of a E-BGO single crystal (500 nm excited at 285 nm). Cathodoluminescent properties of the E-BGO displayed a broadband spectrum with a maximum at 487 nm. The growth process was consistent with a standard Oswald ripening and LaMer growth processes.
