We evaluated whether universal chlorhexidine bathing (decolonization) with or without COVID-19 intensive training impacted COVID-19 rates in 63 nursing homes (NHs) during the 2020–2021 Fall/Winter surge. Decolonization was associated with a 43% lesser rise in staff case-rates (P < .001) and a 52% lesser rise in resident case-rates (P < .001) versus control.
Derivation and external validation of a prediction model for pneumococcal urinary antigen test positivity in patients with community-acquired pneumonia

Objective:
Derive and externally validate a prediction model for pneumococcal urinary antigen test (pUAT) positivity.
Methods:Retrospective cohort study of adults admitted with community-acquired pneumonia (CAP) to 177 U.S. hospitals in the Premier Database (derivation and internal validation samples) or 12 Cleveland Clinic hospitals (external validation sample). We utilized multivariable logistic regression to predict pUAT positivity in the derivation dataset, followed by model performance evaluation in both validation datasets. Potential predictors included demographics, comorbidities, clinical findings, and markers of disease severity.
Results:Of 198,130 Premier patients admitted with CAP, 27,970 (14.1%) underwent pUAT; 1962 (7.0%) tested positive. The strongest predictors of pUAT positivity were history of pneumococcal infection in the previous year (OR 6.99, 95% CI 4.27–11.46), severe CAP on admission (OR 1.76, 95% CI 1.56–1.98), substance abuse (OR 1.57, 95% CI 1.27–1.93), smoking (OR 1.23, 95% CI 1.09–1.39), and hyponatremia (OR 1.35, 95% CI 1.17–1.55). Negative predictors included IV antibiotic use in past year (OR 0.65, 95% CI 0.52–0.82), congestive heart failure (OR 0.72, 95% CI 0.63–0.83), obesity (OR 0.71, 95% CI 0.60–0.85), and admission from skilled nursing facility (OR 0.60, 95% CI 0.45–0.78). Model c-statistics were 0.60 and 0.67 in the internal and external validation cohorts, respectively. Compared to guideline-recommended testing of severe CAP patients, our model would have detected 23% more cases with 5% fewer tests.
Conclusion:Readily available data can identify patients most likely to have a positive pUAT. Our model could be incorporated into automated clinical decision support to improve test efficiency and antimicrobial stewardship.

The purpose of this study was to examine possible pathways by which genetic risk associated with externalizing is transmitted in families. We used molecular data to disentangle the genetic and environmental pathways contributing to adolescent externalizing behavior in a sample of 1,111 adolescents (50% female; 719 European and 392 African ancestry) and their parents from the Collaborative Study on the Genetics of Alcoholism. We found evidence for genetic nurture such that parental externalizing polygenic scores were associated with adolescent externalizing behavior, over and above the effect of adolescents’ own externalizing polygenic scores. Mediation analysis indicated that parental externalizing psychopathology partly explained the effect of parental genotype on children’s externalizing behavior. We also found evidence for evocative gene-environment correlation, whereby adolescent externalizing polygenic scores were associated with lower parent–child communication, less parent–child closeness, and lower parental knowledge, controlling for parental genotype. These effects were observed among participants of European ancestry but not African ancestry, likely due to the limited predictive power of polygenic scores across ancestral background. These results demonstrate that in addition to genetic transmission, genes influence offspring behavior through the influence of parental genotypes on their children’s environmental experiences, and the role of children’s genotypes in shaping parent–child relationships.

Objective:
This project aimed to quantify and compare Massachusetts and Georgia public school districts’ 2017–2018 winter-storm-related Twitter unplanned school closure announcements (USCA).
Methods:Public school district Twitter handles and National Center for Education Statistics data were obtained for Georgia and Massachusetts. Tweets were retrieved using Twitter application programming interface. Descriptive statistics and regression analyses were conducted to compare the rates of winter-storm-related USCA.
Results:Massachusetts had more winter storms than Georgia during the 2017–2018 winter season, but Massachusetts school districts posted winter-storm-related USCA at a 60% lower rate per affected day (adjusted rate ratio, aRR = 0.40, 95% confidence intervals, CI: 0.30, 0.52) than Georgia school districts after controlling for the student enrollments and Twitter followers count per Twitter account. A 10-fold increase in followers count was correlated with a 118% increase in USCA rate per affected day (aRR = 2.18; 95% CI: 1.74, 2.75). Georgia school districts had a higher average USCA tweet rate per winter-storm-affected day than Massachusetts school districts. A higher number of Twitter followers was associated with a higher number of USCA tweets per winter-storm-affected day.
Conclusion:Twitter accounts of school districts in Massachusetts had a lower tweet rate for USCA per winter-storm-affected days than those in Georgia.
Bacterial coinfection in influenza pneumonia: Rates, pathogens, and outcomes

Background:
Evidence from pandemics suggests that influenza is often associated with bacterial coinfection. Among patients hospitalized for influenza pneumonia, we report the rate of coinfection and distribution of pathogens, and we compare outcomes of patients with and without bacterial coinfection.
Methods:We included adults admitted with community-acquired pneumonia (CAP) and tested for influenza from 2010 to 2015 at 179 US hospitals participating in the Premier database. Pneumonia was identified using an International Classification of Disease, Ninth Revision, Clinical Modification (ICD-9-CM) algorithm. We used multiple logistic and gamma-generalized linear mixed models to assess the relationships between coinfection and inpatient mortality, intensive care unit (ICU) admission, length of stay, and cost.
Results:Among 38,665 patients hospitalized with CAP and tested for influenza, 4,313 (11.2%) were positive. In the first 3 hospital days, patients with influenza were less likely than those without to have a positive culture (10.3% vs 16.2%; P < .001), and cultures were more likely to contain Staphylococcus aureus (34.2% vs 28.2%; P = .007) and less likely to contain Streptococcus pneumoniae (24.9% vs 31.0%; P = .008). Of S. aureus isolates, 42.8% were methicillin resistant among influenza patients versus 53.2% among those without influenza (P = .01). After hospital day 3, pathogens for both groups were similar. Bacterial coinfection was associated with increased odds of in-hospital mortality (aOR, 3.00; 95% CI, 2.17–4.16), late ICU transfer (aOR, 2.83; 95% CI, 1.98–4.04), and higher cost (risk-adjusted mean multiplier, 1.77; 95% CI, 1.59–1.96).
Conclusions:In a large US inpatient sample hospitalized with influenza and CAP, S. aureus was the most frequent cause of bacterial coinfection. Coinfection was associated with worse outcomes and higher costs.

Previous studies have reported the basic reproduction number (R0) of coronavirus disease from publicly reported data that lack information such as onset of symptoms, presence of importations or known super-spreading events. Using data from the Republic of Korea, we illustrated how estimates of R0 can be biased and provided improved estimates with more detailed data. We used COVID-19 contact trace system in Korea, which can provide symptom onset date and also serial intervals between contacted people. The total R0 was estimated as 2.10 (95% confidence interval (CI) 1.84–2.42). Also, early transmission of COVID-19 differed by regional or social behaviours of the population. Regions affected by a specific church cluster, which showed a rapid and silent transmission under non-official religious meetings, had a higher R0 of 2.40 (95% CI 2.08–2.77).

Background
Studies suggest that alcohol consumption and alcohol use disorders have distinct genetic backgrounds.
MethodsWe examined whether polygenic risk scores (PRS) for consumption and problem subscales of the Alcohol Use Disorders Identification Test (AUDIT-C, AUDIT-P) in the UK Biobank (UKB; N = 121 630) correlate with alcohol outcomes in four independent samples: an ascertained cohort, the Collaborative Study on the Genetics of Alcoholism (COGA; N = 6850), and population-based cohorts: Avon Longitudinal Study of Parents and Children (ALSPAC; N = 5911), Generation Scotland (GS; N = 17 461), and an independent subset of UKB (N = 245 947). Regression models and survival analyses tested whether the PRS were associated with the alcohol-related outcomes.
ResultsIn COGA, AUDIT-P PRS was associated with alcohol dependence, AUD symptom count, maximum drinks (R2 = 0.47–0.68%, p = 2.0 × 10−8–1.0 × 10−10), and increased likelihood of onset of alcohol dependence (hazard ratio = 1.15, p = 4.7 × 10−8); AUDIT-C PRS was not an independent predictor of any phenotype. In ALSPAC, the AUDIT-C PRS was associated with alcohol dependence (R2 = 0.96%, p = 4.8 × 10−6). In GS, AUDIT-C PRS was a better predictor of weekly alcohol use (R2 = 0.27%, p = 5.5 × 10−11), while AUDIT-P PRS was more associated with problem drinking (R2 = 0.40%, p = 9.0 × 10−7). Lastly, AUDIT-P PRS was associated with ICD-based alcohol-related disorders in the UKB subset (R2 = 0.18%, p < 2.0 × 10−16).
ConclusionsAUDIT-P PRS was associated with a range of alcohol-related phenotypes across population-based and ascertained cohorts, while AUDIT-C PRS showed less utility in the ascertained cohort. We show that AUDIT-P is genetically correlated with both use and misuse and demonstrate the influence of ascertainment schemes on PRS analyses.
The impact of Livestock Manure Control Policy on human leptospirosis in Republic of Korea using interrupted time series analysis

Leptospirosis is a zoonotic disease that the pathogen can be transmitted to humans through the excretions of infected animals. In the Republic of Korea, the Livestock Manure Control Act was enforced in September 2007 to improve underground water hygiene. The objective of this study was to evaluate the impact of Livestock Manure Control Policy on the incidence and the trend of human leptospirosis. An interrupted time series analysis using the monthly incidence of leptospirosis was conducted based on data derived from the Korean National Surveillance System between January 1999 and January 2015. We used a Spearman correlation method to compare the level of leptospirosis incidence decrease between the metropolitan cities and rural provinces. The annual incidence of leptospirosis in South Korea decreased by 33% after policy enforcement of the policy. A significant change in the slope of human leptospirosis cases was observed after the policy enforcement (β = −0·09, P < 0·001). Moreover, we detected a clear association between the size of the rice paddy fields and the decrease in leptospirosis incidence in provinces (r = 0·817, P = 0·01). This study shows that the Livestock Manure Control Policy had significantly reduced human leptospirosis incidence in the Republic of Korea, in particular, in rural regions.
Primary tumour characteristics predict the invasiveness of lymph node metastases in papillary thyroid carcinoma patients

Objective:
This study evaluated whether primary tumour characteristics are associated with specific features of metastatic lymph nodes in papillary thyroid carcinoma patients.
Method:A retrospective review of 411 patients with pathologically diagnosed cervical lymph node metastasis was conducted.
Results:A metastatic lymph node focus size of at least 2 mm was independently associated with a primary tumour size of at least 1 cm (hazard ratio 1.962) and with male sex (hazard ratio 1.947). A number of at least five lymph node metastases was independently associated with a primary tumour size of at least 1 cm (hazard ratio 2.863), extrathyroidal extension (hazard ratio 1.737) and male sex (hazard ratio 1.689). Extranodal extension was independently associated with a primary tumour size of at least 1 cm (hazard ratio 2.288), extrathyroidal extension (2.201) and male sex (hazard ratio 1.733).
Conclusion:Primary papillary thyroid carcinoma characteristics are related to the pathological features of lymph node metastases.

We report on the results of CCD photometric observations of the open cluster NGC 2539. Eight new variable stars have been found in the observed field of this cluster. However, no γ Doradus-type variability was found among the member stars.

Our main approach to the synthesis and study of hybrid organic/inorganic materials involves incorporating nano-size inorganic polyhedral oligomeric silsesquioxane (POSS) clusters into various polymeric resins. A typical POSS cluster is a discrete silicon and oxygen framework solubilized with organic groups and contains a single reactive site. This lone site of reactivity is used to covalently attach the inorganic macromers pendent to a polymer backbone without causing any crosslinking. This strategy permits the synthesis of melt processable, linear hybrid polymers containing pendent inorganic clusters, and allows us to study the effect these clusters have on chain motion, polymer properties and morphology. The synthesis of norbornenyl-based (POSS) macromers, their ring opening metathesis copolymerizations with varying amounts of norbornene, and analysis of the effect of the pendent POSS group is presented. Ring opening metathesis polymerization permits the easy synthesis of both random and diblock copolymers. Transmission electron microscopy (TEM) clearly images POSS-rich domains against the POSS-free regions. Major differences in polymer morphology are observed as the amount of inorganic POSS is varied, between random and diblock copolymers, as well as between polymers that differ only in the solubilizing cycloalkyl groups on the POSS cluster.

A time-series CCD photometric survey of variable stars in open clusters has been in progress using 1.8-m telescope at the Bohyunsan Optical Astronomy Observatory (BOAO) in Korea. We present preliminary results of time series photometry for the intermediate-age open cluster NGC 2301.

High-purity tungsten was prepared by the self-propagating high-temperature synthesis (SHS) process from a mixture of CaO · WO3 and Mg. The complete reduction of CaO · WO3 required a 33% excess of magnesium over the stoichiometric molar ratio Mg/CaO · WO3 of 3: 1. The MgO and CaO in the product were leached with an HCl solution. The product tungsten had a purity of 99.980% which was higher than that of the reactants. The high purity results because the nontungsten reactants and products are volatilized by the high temperatures generated during the rapid exothermic SHS reaction and are dissolved during HCl leaching of the product.

Compositional ordering is observed in essentially all IIIN semiconductor alloys. The mechanism leading to the specific ordered structures formed is only now beginning to be understood. It is intimately related to the atomic-scale physical processes occurring on the surface during epitaxial growth, specifically surface reconstruction and the attachment of atoms at steps. This paper will emphasize the use of surface analytic techniques to better understand the ordering processes during organometallic vapor phase epitaxial growth of GalnP. Surface photo absorption has been used to determine, for the first time, that the surface reconstruction is (2×4)-like during growth of GalnP with Cu-Pt ordering. By increasing the growth temperature and/or decreasing the V/Ill ratio the concentration of phosphorus dimers characteristic of the (2×4) structure is observed to decrease in tandem with a decrease in the degree of order measured in the epitaxial layers. This suggests that formation of the (2×4) reconstruction is necessary for formation of the Cu-Pt ordered structure.
Atomic force microscopy was used to examine the step structure. For exactly (001)-oriented substrates, growth at low V/Ill ratios produces islands surrounded by single monolayer steps. Increasing the V/Ill ratio leads to the formation of bilayer steps approximately 6 Å high. A similar change in the structure of steps is observed in the vicinal regions for substrates misoriented to produce [110] steps. This phenomenon is interpreted in terms of the stabilization, at high V/Ill ratios, of the (2×2) reconstruction on the (111)B regions of the bilayer steps. This may also have an effect on formation of the Cu-Pt ordered structure.
By changing the substrate temperature or the input partial pressure of the phosphorus precursor, the degree of order and, hence, the bandgap energy of GalnP, can be modulated during growth. This has allowed the production of a variety of single and double heterostructures. The heterostructures produced by changing the temperature from 740 to 620°C and from 620 to 520°C are abrupt. The heterostructures formed by changing the phosphine flow rate are not. The ordered structure in the lower layer persists into the upper layer. This may be an indication of the importance of the step structure.
