Challenges and lessons learned from the Pediatric Heart Network Normal Echocardiogram Database study

Background:
The Pediatric Heart Network Normal Echocardiogram Database Study had unanticipated challenges. We sought to describe these challenges and lessons learned to improve the design of future studies.
Methods:Challenges were divided into three categories: enrolment, echocardiographic imaging, and protocol violations. Memoranda, Core Lab reports, and adjudication logs were reviewed. A centre-level questionnaire provided information regarding local processes for data collection. Descriptive statistics were used, and chi-square tests determined differences in imaging quality.
Results:For the 19 participating centres, challenges with enrolment included variations in Institutional Review Board definitions of “retrospective” eligibility, overestimation of non-White participants, centre categorisation of Hispanic participants that differed from National Institutes of Health definitions, and exclusion of potential participants due to missing demographic data. Institutional Review Board amendments resolved many of these challenges. There was an unanticipated burden imposed on centres due to high numbers of echocardiograms that were reviewed but failed to meet submission criteria. Additionally, image transfer software malfunctions delayed Core Lab image review and feedback. Between the early and late study periods, the proportion of unacceptable echocardiograms submitted to the Core Lab decreased (14 versus 7%, p < 0.01). Most protocol violations were from eligibility violations and inadvertent protected health information disclosure (overall 2.5%). Adjudication committee reviews led to protocol changes.
Conclusions:Numerous challenges encountered during the Normal Echocardiogram Database Study prolonged study enrolment. The retrospective design and flaws in image transfer software were key impediments to study completion and should be considered when designing future studies collecting echocardiographic images as a primary outcome.
Exercise restriction is not associated with increasing body mass index over time in patients with anomalous aortic origin of the coronary arteries

Anomalous aortic origin of the coronary arteries is associated with exercise-induced ischaemia, leading some physicians to restrict exercise in patients with this condition. We sought to determine whether exercise restriction was associated with increasing body mass index over time. From 1998 to 2015, 440 patients ⩽30 years old were enrolled into an inception cohort. Exercise-restriction status was documented in 143 patients. Using linear mixed model repeated-measures regression, factors associated with increasing body mass index z-score over time, including exercise restriction and surgical intervention as time-varying covariates, were investigated. The 143 patients attended 558 clinic visits for which exercise-restriction status was recorded. The mean number of clinic visits per patient was 4, and the median duration of follow-up was 1.7 years (interquartile range (IQR) 0.5–4.4). The median age at first clinic visit was 10.3 years (IQR 7.1–13.9), and 71% (101/143) were males. All patients were alive at their most recent follow-up. At the first clinic visit, 54% (78/143) were exercise restricted, and restriction status changed in 34% (48/143) during follow-up. The median baseline body mass index z-score was 0.2 (IQR 0.3–0.9). In repeated-measures analysis, neither time-related exercise restriction nor its interaction with time was associated with increasing body mass index z-score. Surgical intervention and its interaction with time were associated with decreasing body mass index z-score. Although exercise restriction was not associated with increasing body mass index over time, surgical intervention was associated with decreasing body mass index z-score over time in patients with anomalous aortic origin of the coronary arteries.
Aerobic and Cognitive Exercise (ACE) Pilot Study for Older Adults: Executive Function Improves with Cognitive Challenge While Exergaming

Dementia cases are increasing worldwide; thus, investigators seek to identify interventions that might prevent or ameliorate cognitive decline in later life. Extensive research confirms the benefits of physical exercise for brain health, yet only a fraction of older adults exercise regularly. Interactive mental and physical exercise, as in aerobic exergaming, not only motivates, but has also been found to yield cognitive benefit above and beyond traditional exercise. This pilot study sought to investigate whether greater cognitive challenge while exergaming would yield differential outcomes in executive function and generalize to everyday functioning. Sixty-four community based older adults (mean age=82) were randomly assigned to pedal a stationary bike, while interactively engaging on-screen with: (1) a low cognitive demand task (bike tour), or (2) a high cognitive demand task (video game). Executive function (indices from Trails, Stroop and Digit Span) was assessed before and after a single-bout and 3-month exercise intervention. Significant group × time interactions were found after a single-bout (Color Trails) and after 3 months of exergaming (Stroop; among 20 adherents). Those in the high cognitive demand group performed better than those in the low cognitive dose condition. Everyday function improved across both exercise conditions. Pilot data indicate that for older adults, cognitive benefit while exergaming increased concomitantly with higher doses of interactive mental challenge. (JINS, 2015, 21, 768–779)
The effect of shear flow on the rotational diffusion of a single axisymmetric particle

Understanding the orientation dynamics of anisotropic colloidal particles is important for suspension rheology and particle self-assembly. However, even for the simplest case of dilute suspensions in shear flow, the orientation dynamics of non-spherical Brownian particles are poorly understood. Here we analytically calculate the time-dependent orientation distributions for non-spherical axisymmetric particles confined to rotate in the flow–gradient plane, in the limit of small but non-zero Brownian diffusivity. For continuous shear, despite the complicated dynamics arising from the particle rotations, we find a coordinate change that maps the orientation dynamics to a diffusion equation with a remarkably simple ratio of the enhanced rotary diffusivity to the zero shear diffusion: $D_{eff}^{r}/D_{0}^{r}=(3/8)(p-1/p)^{2}+1$, where $p$ is the particle aspect ratio. For oscillatory shear, the enhanced diffusion becomes orientation dependent and drastically alters the long-time orientation distributions. We describe a general method for solving the time-dependent oscillatory shear distributions and finding the effective diffusion constant. As an illustration, we use this method to solve for the diffusion and distributions in the case of triangle-wave oscillatory shear and find that they depend strongly on the strain amplitude and particle aspect ratio. These results provide new insight into the time-dependent rheology of suspensions of anisotropic particles. For continuous shear, we find two distinct diffusive time scales in the rheology that scale separately with aspect ratio $p$, as $1/D_{0}^{r}p^{4}$ and as $1/D_{0}^{r}p^{2}$ for $p\gg 1$. For oscillatory shear flows, the intrinsic viscosity oscillates with the strain amplitude. Finally, we show the relevance of our results to real suspensions in which particles can rotate freely. Collectively, the interplay between shear-induced rotations and diffusion has rich structure and strong effects: for a particle with aspect ratio 10, the oscillatory shear intrinsic viscosity varies by a factor of ${\approx}2$ and the rotational diffusion by a factor of ${\approx}40$.
