Hypothermia Management, an Evaluation of a Novel Lightweight System.

Introduction:
Accidental hypothermia remains an important contributory factor to the mortality of trauma patients in both civilian and military environments. As a component of the ‘lethal triad’ it poses a significant problem in patients at risk of hemorrhage from traumatic injuries. Systems used to mitigate hypothermia in the prehospital environment must strike a balance between weight: size ratio and optimal performance.
Method:This study compared three hypothermia mitigation systems; two leading products and the novel Xtract™SR Heatsaver, over a three-day trial period. Seven subjects were placed in a closed system, held at around 0°C, to promote the onset of mild hypothermia. Individuals with a tympanic temperature recording of < or = 35°C were placed into one of the three systems. Recordings of aural temperature and a numerical perceived comfort score were made every 15-20 minutes to assess rate of rewarming and subject’s perceptions of the process. An additional study was carried out by an experienced consultant in military and civilian emergency medicine, on day three of the trial, to determine the ease of clinical assessment of individuals placed inside the Xtract™SR Heatsaver prototype.
Results:On all three days, subjects placed in the Xtract™SR Heatsaver recovered from their hypothermic state faster than those placed in the other systems. Clinical assessment could easily be performed on a patient placed in the Xtract™SR Heatsaver system.
Conclusion:Results demonstrate that the new Xtract™SR Heatsaver system is superior with regards to reducing heat loss, increasing patient comfort and allowing for clinical assessment. The study also highlights the importance of the use of adjuncts such as heat cell blankets and insulation matts alongside hypothermia mitigation systems deployed in the prehospital environment. Furthermore, data gathered provides scope for future research into nuances surrounding the effects and onset of hypothermia.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

A fine-grained, up to 3-m-thick tephra bed in southwestern Saskatchewan, herein named Duncairn tephra (Dt), is derived from an early Pleistocene eruption in the Jemez Mountains volcanic field of New Mexico, requiring a trajectory of northward tephra dispersal of ~1500 km. An unusually low CaO content in its glass shards denies a source in the closer Yellowstone and Heise volcanic fields, whereas a Pleistocene tephra bed (LSMt) in the La Sal Mountains of Utah has a very similar glass chemistry to that of the Dt, supporting a more southerly source. Comprehensive characterization of these two distal tephra beds along with samples collected near the Valles caldera in New Mexico, including grain size, mineral assemblage, major- and trace-element composition of glass and minerals, paleomagnetism, and fission-track dating, justify this correlation. Two glass populations each exist in the Dt and LSMt. The proximal correlative of Dt1 is the plinian Tsankawi Pumice and co-ignimbritic ash of the first ignimbrite (Qbt1g) of the 1.24 Ma Tshirege Member of the Bandelier Tuff. The correlative of Dt2 and LSMt is the co-ignimbritic ash of Qbt2. Mixing of Dt1 and Dt2 probably occurred during northward transport in a jet stream.

Nivation is a collective noun identifying a set of geomorphic processes, comprised of an indeterminate number of elements and of unknown relative importance, for which there is little likelihood of ever producing a precise definition. Instead, attention should first be directed towards the relationships between snow-packs and individual geomorphic processes. The relationship between freezing amplitude in the bedrock, snow cover, and aspect at an Arctic site in northern Norway and an alpine site in the Front Range, Colorado, U.S.A. is complex. Comparison of field data and laboratory criteria permit several conflicting interpretations. If oscillations across 0°C, regardless of freezing amplitude, are critical, the alpine site is potentially a more active freeze-thaw weathering regime, with a primary springtime peak and a secondary fall peak. If a freezing amplitude of –5°C is required for effective freeze-thaw weathering then the alpine site is largely inactive and the Arctic active (but with only a single fall peak). Chemical weathering is much more important at snow-patch sites than has traditionally been recognized. Mass wasting at colluvial sites subject to snow patches is dominated by interaction between overland flow and solifluction when the site is unvegetated, and by solifluction when it is vegetated. Given contemporary knowledge of snow and glacial geomorphology, there appears to be no threshold, only differences of intensity. Resolution of the disruptive mechanism associated with bedrock freezing and its constraining temperature and moisture requirements is the most pressing present problem in the field of snow geomorphology.

Summary
ABSTRACT
Changing patterns of tooth wear have been used extensively to obtain information about the lifestyle and culture of pre-historic and modern humans. The assessment of tooth wear in previous anthropological studies has been largely based on quantitative analysis of wear indices and qualitative analysis of micrographs. Wear indices are simple to use and can be sensitive tools, but there is a lack of international standardization in their use. Micrographic assessment of pits and scratch marks on the worn surfaces of teeth can assist in dietary reconstruction of humans, but this approach has low reliability and high observer error. This review will provide an update on a new wear index and novel nano-techniques that hold promise for improving the analysis of tooth wear. Recently, a new wear index, termed the Basic Erosive Wear Examination index, has been proposed as a standardized universal tool for diagnosing erosive tooth wear. However, its value seems to be limited when assessing the dentitions of populations, in whom tooth wear occurs predominantly by attrition and abrasion. Optical techniques involving scanning confocal microscopy combined with fractal analysis can provide an objective assessment of the worn surface. Other nanotechnology-based methods, such as nanohardness measurements, nano-computed tomography and mass spectrometry, can be also useful in physical and chemical characterization of both sound and worn teeth, but these techniques are limited to use in vitro. A combined assessment of the worn dentition using all of these techniques promises to provide the best holistic approach to analyse tooth wear.

Do abundance–occupancy relationships exist in cetaceans?

A positive relationship between the number of locations where a species occurs and the average density of individuals across those locations has been found in a wide variety of taxa and has been described as one of the most general and widespread relationships in macro-ecology. However, exceptions to this general rule have been found and this study tested whether abundance–occupancy relationships exist within the cetacean community of the west coast of Scotland. Data were collected in 2003–2006 and occupancy rates were calculated and compared to two density indices (relative density of groups per km2 surveyed and relative density of individuals per km2 surveyed) for four cetacean species (harbour porpoise, bottlenose dolphin, common dolphin and minke whale). Significant positive intraspecific abundance–occupancy relationships were found for both relative density of groups per km2 and relative density of individuals per km2 for two out of the four cetacean species tested (harbour porpoise and minke whale). When the relationships between the different species were compared, all four were found to conform to the same interspecific relationship when relative density of groups was used as the density index. However, some species were found to conform to different relationships when relative density of individuals was used as the density index, potentially due to differences in social structure between cetacean species. These relationships mean that when cetaceans are at a higher density within an area, they also occupy a greater number of locations and vice versa. The existence of positive abundance–occupancy relationships in cetaceans has a number of potential implications for their conservation and management. In particular, it means that when a potential impact is likely to positively or negatively affect the size of the range of a species or population, such as noise pollution or climate change, it is likely to also affect the species' or population's abundance in the same direction. It also has implications for the design and extent of protected areas, such as marine protected areas; as such relationships could be used to determine the area required to maintain a viable population size.

Summary
Introduction
To date, nearly all vegetation studies in New Zealand have been carried out in pristine to semi-natural systems. Thus, urban ecology in New Zealand is in its infancy as compared with the centuries of observation, documentation and mapping of vegetation, biotopes and natural history in urban areas of Europe (Gilbert,1989; Breuste et al., 1998; Sukopp, 2002; Breuste, Chapter 21; Florgård, Chapter 22; Wittig, Chapter 30) and 30-plus years of study in North America (Zipperer and Guntenspergen, Chapter 17). The relatively few studies of urban vegetation in the New World have typically focused on remnant natural systems enveloped by residential and commercial dwellings (Airola and Buchholz, 1984; Rudnicky and McDonnell, 1989; Kuschel, 1990; Molloy, 1995; McDonnell et al., 1997). Accordingly, there is a distinction to be made between the ecology of remnant primary ecosystems (those that retain at least some thread of biological and pedological continuity with the primeval system) and the ecology of synthetic or spontaneous recombinant systems on anthropogenic substrates (i.e. most vegetation in cities and towns). In New Zealand, we know quite a lot about natural forest, wetland and grassland vegetation, whether in National Parks or as remnants in cities (Wardle, 1991); but little about recombinant communities of cultural landscapes (that is, human-inhabited landscapes in the sense of Nassauer (1997)). They have been traditionally shunned in New Zealand because they almost totally comprise exotic, planted and/or weedy species.

The combination of Raman spectroscopy and Secondary Ion Mass Spectrometry can improve understanding of the chemistry of the glass alteration process. Formic and acetic acids play an important role in the alteration of museum glass objects placed in a humid atmosphere. Raman spectroscopy indicates that the soda-rich glass structure is modified differently when exposed to a humid versus a humid and polluted atmosphere at 60°C. Formic acid was not formed from soda-rich glass in the presence of carbon dioxide, high humidity and light.

‘A beacon of excellence in healthcare design’… ‘a symbol of civic pride in our community’ … ‘a “state of the art” building’ … ‘a building which could be magnificently innovative and visionary in its time, location and conception’. These phrases, taken from the ‘Aspirational Statement’ that introduces the competition brief, should have left no one in any doubt that the clients for the proposed new integrated care centre in Kentish Town were looking for something new and different. With hindsight, it is easy to see why Allford Hall Monaghan Morris's (AHMM) unusual design [1a–c] won the competition.
Randomised trial of thyroxine to prevent postnatal depression in thyroid-antibody-positive women

Background
Women who are positive for thyroid antibodies in early gestation are prone to post-partum depression, apparently independent of thyroid dysfunction, as measured by serum levels of free thyroxine, free triodothyroxine and thyroid-stimulating hormone. This finding may be due to infrequent monitoring of thyroid function, because hyperthyroidism, hypothyroidism and combinations of both may occur post-partum.
AimsTo test the hypothesis that stabilising thyroid function post-partum by administering daily thyroxine reduces the rate of occurrence and severity of associated depression.
MethodIn a randomised double-blind placebo-controlled trial, 100 of thyroxine or placebo was given daily to 446 thyroid-antibody-positive women (342 of whom were compliant) from 6 weeks to 6 month spost-partum, assessing their psychiatric and thyroid status at 4-weeklyintervals.
ResultsThere was no evidence that thyroxine had any effect on the occurrence of depression. The 6-month period prevalence of depression was similar to that reported previously.
ConclusionsThe excess of depression in thyroid-antibody-positive women in the post-partum period is not corrected by daily administration of thyroxine.
