OBJECTIVE
Adult ventilator-associated event (VAE) definitions include ventilator-associated conditions (VAC) and subcategories for infection-related ventilator-associated complications (IVAC) and possible ventilator-associated pneumonia (PVAP). We explored these definitions for children.
DESIGNRetrospective cohort
SETTINGPediatric, cardiac, or neonatal intensive care units (ICUs) in 6 US hospitals
PATIENTSPatients ≤18 years old ventilated for ≥1 day
METHODSWe identified patients with pediatric VAC based on previously proposed criteria. We applied adult temperature, white blood cell count, antibiotic, and culture criteria for IVAC and PVAP to these patients. We matched pediatric VAC patients with controls and evaluated associations with adverse outcomes using Cox proportional hazards models.
RESULTSIn total, 233 pediatric VACs (12,167 ventilation episodes) were identified. In the cardiac ICU (CICU), 62.5% of VACs met adult IVAC criteria; in the pediatric ICU (PICU), 54.2% of VACs met adult IVAC criteria; and in the neonatal ICU (NICU), 20.2% of VACs met adult IVAC criteria. Most patients had abnormal white blood cell counts and temperatures; we therefore recommend simplifying surveillance by focusing on “pediatric VAC with antimicrobial use” (pediatric AVAC). Pediatric AVAC with a positive respiratory diagnostic test (“pediatric PVAP”) occurred in 8.9% of VACs in the CICU, 13.3% of VACs in the PICU, and 4.3% of VACs in the NICU. Hospital mortality was increased, and hospital and ICU length of stay and duration of ventilation were prolonged among all pediatric VAE subsets compared with controls.
CONCLUSIONSWe propose pediatric AVAC for surveillance related to antimicrobial use, with pediatric PVAP as a subset of AVAC. Studies on generalizability and responsiveness of these metrics to quality improvement initiatives are needed, as are studies to determine whether lower pediatric VAE rates are associated with improvements in other outcomes.

Since the 1970s scholars in China have identified mo 貘 as the ancient name for the giant panda (Ailuropoda melanoleuca). I concur with this identification and I trace the source of the modern misidentification of mo as the Malayan tapir (Tapirus indicus) to the article by Jean Pierre Abel-Rémusat published in 1824. Abel-Rémusat based his identification on woodblock drawings of the mo depicted as the quadripartite animal first described by Bo Juyi in the ninth century: elephant trunk, rhinoceros eyes, cow tail, tiger paws. Xu Shen (ca. 55–ca. 149) in the Shuowen jiezi compared mo to the bear, as did all descriptions of mo before Bo Juyi. Bo Juyi's description reflects new ideas about mo in medieval culture, and cannot be used as evidence of the animal named mo in early China. As a consequence of Abel-Rémusat's mistaken identification – which was immediately accepted in Western zoology – the word mo lost its original meaning and became the word for tapir in modern Chinese and Japanese. Examination of textual and zooarchaeological evidence confirms the giant panda as the original referent of mo. Although the tapir inhabited the region of China in prehistory there is no evidence of the tapir in China in historical times.

The work of historians in providing new editions of primary documents, and other aids to research, has tended to go largely unsung, yet is crucial to scholarship, as providing the very foundations on which further enquiry can be based. The essays in this volume, conversely, celebrate the achievements in this field by a whole generation of medievalists, of whom the honoree, David Smith, is one of the most distinguished. They demonstrate the importance of such editions to a proper understanding and elucidation of a number of problems in medieval ecclesiastical history, ranging from thirteenth-century forgery to diocesan administration, from the church courts to the cloisters, and from the English parish clergy to the papacy. Summary
The natural experts were vital participants in the intellectual and spiritual ferment of the Warring States period. They looked for order in natural phenomena and related this order to human activity in numerologically based classifications. The natural experts embraced the magico-religious elements of Warring States culture. Varieties of divination, demonology, charms, incantations, and magico-religious operations (exorcism, propitiation) found a place in their practices and texts; in short, the natural experts were also occultists. While correlative cosmology is a significant historical outcome, other aspects of Warring States astrology and the calendar should not be overlooked. Astrology remained closely connected to religious ideas about Heaven and the spirit world, whose influence was manifested in celestial and other phenomena. Divination with turtle and milfoil remained important in the religious life of the Warring States elite. The bamboo-strip divination record from Baoshan tomb 2, Hubei, documents routine divination to obtain judgments from the spirits and their approval of sacrificial offerings.

Fundamental changes occurred in chinese civilization between the fifth century B.C.E. and the second century C.E., roughly corresponding to the Warring States (which officially commences in 453 B.C.E.) and the Ch'in and Han empires (Han rule ends in 220 C.E.). The emergence and maturation of philosophical speculation and of new sociopolitical models have traditionally constituted the ground on which other elements of the contemporary civilization have been drawn. As a consequence, the nature of religious traditions during this period has been poorly understood and insufficiently studied; first, because the documentation preserved in received literature overwhelmingly reflects the politico-philosophical leanings of an elite social stratum and thus offers only a partial, biased view of the range of religious belief and practice; and second, because modern research on Warring States, Ch'in, and Han religion by and large relies on the viewpoints expressed in the received record. Whereas the study of Buddhism and, more recently, of religious Taoism and popular religion is thriving among an ever-growing number of scholars of the history of Chinese religion, the ancient period of Warring States, Ch'in, and Han is still dominated by scholars engaged in the study of the philosophical tradition. The idea that religious belief during the period was tempered by philosophical reason is a widely shared assumption.

The bamboo-slip medical manuscript entitled Yinshu (Pulling Book) from Zhangjiashan tomb 247, Hubei (burial dated ca. mid-second cen¬tury B.C.), includes a passage that uses the analogy of the bellows attested in received literature in Laozi V. In Yinshu the analogy is placed at the head of a macrobiotic technique. This article discusses the technique and argues that the bellows analogy most likely developed as part of the Warring States medical tradition of macrobiotic hygiene; that is, the bellows analogy in Laozi V was borrowed from medicine.

This article adduces additional evidence to substantiate the author's ideas regarding the cosmic board shih published in Early China 4. The most essential points in the discussion concern first the relation between the appearance of the Big Dipper on the cosmic board and the role of the Big Dipper in ancient cosmological belief and calendrical theory; and second, the influence of the cosmic board on religious belief and practice beginning in the Han period. Contrary to Cullen's opinion that the expression “main-cay of heaven” which denotes the Big Dipper is merely a figure of speech, passages from a variety of previn and Han texts show that this term accurately reflects the conception of the Big Dipper in ancient cosmological belief. The development of the cosmic board and the system of divination associated with it are best understood in light of this belief and of the major role played by the Big Dipper in ancient calendriil theory. The influence on Han popular belief of the celestial spirits associated with the cosmic board is attested in the Lun Heng. The presence of several cosmic board divination texts dating to the Han and six Dynasties period in the Tao Tsang are evidence of the absorption of the cosmic board system of divinaion into Taoism at that time. In Taoist hagiography the traditional legend of the transmission of cosmic board divination to the Yellow God is also explicitly linked to the Taoist technique of pacing the Dipper, lese facts, combined with a Buddhist description of a mandala modeled on the design of the cosmic board, npy document the role of the cosmic board in Chinese religion.
