People of Chinese ethnicity develop type 2 diabetes mellitus (T2DM) at a younger age and lower body mass index (BMI) than their Caucasian counterparts. Furthermore, Chinese migrants to Westernised countries have an increased risk of metabolic diseases compared to those in their country of origin(1,2). We propose that this increased risk is due to a greater manifestation of metabolic abnormalities in response to altered diet and lifestyle behaviours. Although fasting lipaemia and glycaemia are commonly used to predict risk of CVD and T2DM, assessment of impaired postprandial metabolism has been found to be a more sensitive indicator of risk(3). We hypothesised that Chinese migrants, at risk of T2DM, exhibit impaired postprandial lipid and lipoprotein metabolism compared to Australian-born Caucasian counterparts. Chinese and Caucasian adults at risk of T2DM were recruited to the study in which postprandial lipaemia and glycaemia were monitored following consumption of a high fat and high carbohydrate breakfast meal followed by a mixed, lunch meal. In a nonrandomised acute crossover trial, 15 adults (n = 8 Chinese and n = 7 Caucasian) aged ≥ 18 and ≤ 65 years at risk of T2DM (AUSDRISK score > 12 (median = 14.0, IQR = 3.0)), attended two postprandial test days separated by ≥ 7-day washout period. Test breakfast meals were isocaloric (3.6 MJ), high fat (46% energy from fat, 46% energy from carbohydrates) or high carbohydrate (74% E carbohydrates, 17.5% E fat). Blood samples were collected at baseline (fasting), 180 min and 360 min after consumption of the breakfast meal. The lunch meal (3.7 MJ, 18% E fat, 76% E carbohydrates) was consumed 240 min after baseline. Samples were analysed for lipaemia and glycaemia. Additionally, chylomicron-rich, and VLDL-rich lipoprotein fractions were isolated by sequential ultracentrifugation and chylomicron particle number (apolipoprotein (apo) B48), triacylglycerol (TAG), and total cholesterol were assessed in these fractions. Data were analysed using a mixed between-within-subject analysis of variance. There were no differences in age, and baseline anthropometric measures between groups, apart from the Chinese group exhibiting significantly lower waist circumference and BMI compared to the Caucasian group. There were no differences between groups in blood measures, apart from a higher total- and LDL-cholesterol concentration in the Caucasian compared to the Chinese group (P<0.05). Despite identical fasting TAG concentrations, the Chinese group, compared with the Caucasian group exhibited significantly elevated serum TAG and chylomicron-apo-B48 concentrations at 360 min following both test meals (P<0.01). All other postprandial measures were not different between groups. These findings show that despite having identical or improved fasting glycaemia and lipid profile, the Chinese group exhibited impaired postprandial lipid metabolism which may contributes to their increased risk of metabolic diseases.

This study focuses on analysing the heights of 10,953 Korean men aged 20 to 40 years who were measured during the Joseon dynasty, the Japanese colonialisation period, and the contemporary period, the latter including both North and South Korea. This study thus provides rare long-term statistical evidence on how biological living standards have developed over several centuries, encompassing Confucianism, colonialism, capitalism, and communism. Using error bar analysis of heights for each historical sample period, this study confirms that heights rose as economic performance improved. For instance, economically poorer North Koreans were expectedly shorter, by about 6 cm, than their peers living in the developed South. Similarly, premodern inhabitants of present-day South Korea, who produced a gross domestic product (GDP) per capita below the world average, were about 4 cm shorter than contemporary South Koreans, who have a mean income above the world average. Along similar lines, North Koreans, who have a GDP per capita akin to that of the premodern Joseon dynasty, have not improved much in height. On the contrary, mean heights of North Koreans were even slightly below (by about 2.4 cm) heights of Joseon dynasty Koreans. All in all, the heights follow a U-shaped pattern across time, wherein heights were lowest during the colonial era. Heights bounced back to Joseon dynasty levels during the interwar period, a time period where South Korea benefitted from international aid, only to rise again and surpass even premodern levels under South Korea’s flourishing market economy.

We study a retirement savings plan with a default contribution rate of 12 percent of income, which is much higher than previously studied defaults. Twenty-five percent of employees had not opted out of this default 12 months after hire; a literature review finds that the corresponding fraction in plans with lower defaults is approximately one-half. Because only contributions above 12 percent were matched by the employer, 12 percent was likely to be a suboptimal contribution rate for employees. Employees who remained at the 12 percent default contribution rate had average income that was approximately one-third lower than would be predicted from the relationship between salaries and contribution rates among employees who were not at 12 percent. Defaults may influence low-income employees more strongly in part because these employees face higher psychological barriers to active decision making.

Summary
In this chapter, we examine how creativity, intelligence, and wisdom are related in theoretical frameworks and empirical studies. First, definitions of the three constructs are discussed. Although all of them are complex and multifaceted, the relationship between creativity and intelligence has been extensively studied based on their working definitions and models. However, wisdom research remains sparse, and there is a gap between people’s common beliefs and explicit theories of how wisdom is related to intelligence and creativity. Regardless, some common elements of wisdom have been distilled. These include self-awareness, knowledge, and strategies to cope with uncertainty, which can lead to contributions to social goodness. Second, several empirical studies are reviewed. They show some relationships among the three constructs, but the results have been inconsistent. Third, the WICS model, as a cognitive framework, is used to understand the relationships among the three constructs. The model integrated intelligence, creativity, and wisdom, and has also been tested in some fields. Finally, we argue that wisdom, as a distinguishable component of ability, can help determine whether our intelligence and creativity will be harnessed toward benevolence or malevolence.
Effects of prior deployments and perceived resilience on anger trajectories of combat-deployed soldiers

Background
Problematic anger is frequently reported by soldiers who have deployed to combat zones. However, evidence is lacking with respect to how anger changes over a deployment cycle, and which factors prospectively influence change in anger among combat-deployed soldiers.
MethodsReports of problematic anger were obtained from 7298 US Army soldiers who deployed to Afghanistan in 2012. A series of mixed-effects growth models estimated linear trajectories of anger over a period of 1–2 months before deployment to 9 months post-deployment, and evaluated the effects of pre-deployment factors (prior deployments and perceived resilience) on average levels and growth of problematic anger.
ResultsA model with random intercepts and slopes provided the best fit, indicating heterogeneity in soldiers' levels and trajectories of anger. First-time deployers reported the lowest anger overall, but the most growth in anger over time. Soldiers with multiple prior deployments displayed the highest anger overall, which remained relatively stable over time. Higher pre-deployment resilience was associated with lower reports of anger, but its protective effect diminished over time. First- and second-time deployers reporting low resilience displayed different anger trajectories (stable v. decreasing, respectively).
ConclusionsChange in anger from pre- to post-deployment varies based on pre-deployment factors. The observed differences in anger trajectories suggest that efforts to detect and reduce problematic anger should be tailored for first-time v. repeat deployers. Ongoing screening is needed even for soldiers reporting high resilience before deployment, as the protective effect of pre-deployment resilience on anger erodes over time.

The Yellow Sea region is of high global importance for waterbird populations, but recent systematic bird count data enabling identification of the most important sites are relatively sparse for some areas. Surveys of waterbirds at three sites on the coast of southern Jiangsu Province, China, in 2014 and 2015 produced peak counts of international importance for 24 species, including seven globally threatened and six Near Threatened species. The area is of particular global importance for the ‘Critically Endangered’ Spoon-billed Sandpiper Calidris pygmaea (peak count across all three study sites: 62 in spring [2015] and 225 in autumn [2014] and ‘Endangered’ Spotted Greenshank Tringa guttifer (peak count across all three study sites: 210 in spring [2014] and 1,110 in autumn [2015]). The southern Jiangsu coast is therefore currently the most important migratory stopover area in the world, in both spring and autumn, for both species. Several serious and acute threats to waterbirds were recorded at these study sites. Paramount is the threat of large-scale land claim which would completely destroy intertidal mudflats of critical importance to waterbirds. Degradation of intertidal mudflat habitats through the spread of invasive Spartina, and mortality of waterbirds by entrapment in nets or deliberate poisoning are also real and present serious threats here. Collisions with, and displacement by, wind turbines and other structures, and industrial chemical pollution may represent additional potential threats. We recommend the rapid establishment of effective protected areas for waterbirds in the study area, maintaining large areas of open intertidal mudflat, and the urgent removal of all serious threats currently faced by waterbirds here.

miRNAs are small, non-coding RNAs that play critical roles in various cellular processes. Although there are several algorithms that can predict the potential candidate genes that are regulated by a miRNA, these algorithms require further experimental validation in order to demonstrate genuine targets of miRNAs. Moreover, most algorithms predict hundreds to thousands of putative target genes for each miRNA, and it is difficult to validate all candidates using the whole 3′-untranslated region (UTR) reporter assay. We report a fast, simple and efficient experimental approach to screening miRNA candidate targets using a 3′-UTR linker assay. Critically, the linker has only a short miRNA regulatory element sequence of approximately 22 base pairs in length and can provide a benefit for screening strong miRNA candidates for further validation using the whole 3′-UTR sequence. Our technique will provide a simplified platform for the high-throughput screening of miRNA target gene validation.

Hereditary mixed polyposis is a genetically heterogeneous, autosomal dominant condition with adenomatous, hyperplastic and juvenile polyps. We conducted a comprehensive clinical evaluation of a large Ashkenazi Jewish family with this phenotype and performed extensive genetic testing. As seen in one previous report, a 40 kb duplication upstream of GREM1 segregated with the polyposis/colon cancer phenotype in this kindred. Our study confirms the association of GREM1 with mixed polyposis and further defines the phenotype seen with this mutation. This gene should be included in the test panel for all Jewish patients with mixed polyposis and may be considered in any Ashkenazi patient with unexplained hereditary colon cancer when mutations in other hereditary colon cancer genes have been ruled out.

Compound semiconductors belong to the most important materials for optoelectronic applications. Many of them exhibit favorable optical properties, such as a direct energy band gap (in contrast to silicon) and high-absorption coefficients over a wide spectral range. Moreover, varying the composition of the compound or substituting some of its elements often allows for controlled band gap engineering and optimization for specific applications. Because many compound semiconductors enable efficient conversion of light into electricity and vice versa, they are commonly used materials for optoelectronic devices.

We describe the pension plan features of the states and the largest cities and counties in the U.S. Unlike in the private sector, defined benefit (DB) pensions are still the norm in the public sector. However, a few jurisdictions have shifted toward defined contribution (DC) plans as their primary savings plan, and fiscal pressures are likely to generate more movement in this direction. Holding fixed a public employee's work and salary history, we show that DB retirement income replacement ratios vary greatly across jurisdictions. This creates large variation in workers’ need to save for retirement in other accounts. There is also substantial heterogeneity across jurisdictions in the savings generated in primary DC plans because of differences in the level of mandatory employer and employee contributions. One notable difference between public and private sector DC plans is that public sector primary DC plans are characterized by required employee or employer contributions (or both), whereas private sector plans largely feature voluntary employee contributions that are supplemented by an employer match. We conclude by applying lessons from savings behavior in private sector savings plans to the design of public sector plans.

OPEN FRACTURE IN CHILDREN
Introduction
Motor vehicle accidents and falls from height account for the majority of open fractures in children. They differ from open fractures in adults in that children have greater potential for healing due to the thicker periosteum. Infection rates are also lower in children compared to adults. Open fractures in children with closed physes should receive the same treatment as in adults.
Epidemiology
Open fractures have been reported to account for 9% of fractures treated at a pediatric tertiary trauma center. Most studies show a preponderance of boys. The tibia and forearm are the areas most frequently involved.
Clinical Features
The modified Gustilo classification system is used to classify open fractures in children (Table 47.1). The overall rate of infection following open fracture in children is reported as 3%. By type, infection occurs in 2% of type I, 2% of type II, and 8% of type III fractures.
Although community-acquired methicillin-resistant Staphylococcus aureus (MRSA) is of increasing concern, no studies have been published demonstrating superior efficacy of vancomycin, clindamycin, or other antibiotics over cefazolin for open fracture. Indiscriminate use of second-tier agents may lead to increased resistance. Therefore, in the absence of a cephalosporin allergy, cefazolin is recommended as first-line prophylaxis.
Table 47.2 summarizes important clinical features of open fractures in children.

Summary
Epidural injections have been used to treat low-back and radicular pain for the past 100 years; these injections have included corticosteroids for the past 50 years. As the technique has evolved and become more widespread, epidural steroid injections have become an important part of the armamentarium in treating low-back pain, sciatica, and, to a lesser degree, neck pain.
The solution to back pain continues to vex physicians and their patients. Most patients with acute disc herniations will improve without surgery (1); however, even those who recover without invasive therapy often suffer pain and disability in the meantime. This may lead to lost wages and diminished productivity. Although epidural steroid injections are not a cure for the root cause of pain, they may reduce or eliminate the pain in the short and intermediate term (2), allowing rehabilitation to proceed and a speedier return to normal activities (3). Even though occasional patients experience permanent, complete relief from epidural steroid injections, one cannot advise patients that they will achieve lasting relief purely from the injection. The goal is to improve symptoms, which will allow a more normal lifestyle and facilitate other modes of therapy (4).
This chapter reviews the relevant anatomy for epidural injections, history of the technique, medications, practical steps, and other relevant information of epidural injections. A variety of routes will be discussed.

INTRODUCTION
Flouroscopically directed selective nerve root blocks (SNRBs) were initially described by Macnab in patients with “negative disc exploration” (1). Macnab described needle tip position, reproduction of radicular pain, confirmation of placement with injection of contrast to produce a neurogram, and anesthetization of the affected nerve with lidocaine. The technique has not changed significantly since 1971.
The utility of an SNRB is reflected in the name. It should be selective and, as such, tends to have significant diagnostic and prognostic power in evaluating patients with unclear radicular etiologies. The injection, when combined with a steroid, can be therapeutic as well. In the lumbar spine, if therapeutic benefit is the goal, a transforaminal epidural steroid injection is of more utility with more volume able to be delivered to the affected level with reflux into the epidural space.
In our clinical practice, lumbar SNRBs are performed in order to pinpoint the affected levels in patients with unclear clinical symptoms or discordant symptoms that are discordant with the magnetic resonance images (MRI). It has also been shown to have significant prognostic importance in identifying the level for surgical intervention and in identifying those patients who will benefit from surgery (2,3). Cervical selective nerve root blocks (CNRB) are performed under similar clinical circumstances. Thoracic SNRBs are rarely performed but can have important diagnostic and therapeutic utility in uncommon clinical settings.

This study investigates the interface between speech and gesture in second language (L2) narration within Slobin's (2003) thinking-for-speaking (TFS) framework as well as with respect to McNeill's (1992, 2005) growth point (GP) hypothesis. Specifically, our interest is in whether speakers shift from a first language (L1) to a L2 TFS pattern as manifested in the GP of narrations they produce. The data are drawn from the narrations of an animated cartoon story produced by advanced L2 speakers of Korean (L1 English) and L2 speakers of English (L1 Korean). Korean is a verb-framed language (Talmy, 2000) that conflates path of motion on the verb and expresses manner, if at all, through separate lexical items, onomatopoeia, or gesture alone. English is a satellite-framed language (Talmy, 2000) that encodes manner on verbs, expresses path through satellite phrases, and synchronizes manner-path conflated gestures with manner verbs when manner is in focus. The typological difference between the languages presents significant challenges for the L2 speakers, who, despite their high level of proficiency in their respective L2s, appear to retain their L1 TFS patterns.
