The effect of congenital diaphragmatic hernia on the development of left-sided heart structures

Introduction:
Patients with congenital diaphragmatic hernias often have concomitant congenital heart disease (CHD), with small left-sided cardiac structures as a frequent finding. The goal of this study is to evaluate which left-sided heart structures are affected in neonates with congenital diaphragmatic hernias.
Methods:Retrospective review of neonates between May 2007 and April 2015 with a diagnosis of a congenital diaphragmatic hernia was performed. Clinical and echocardiographic data were extracted from the electronic medical record and indexed to body surface area and compared to normative values. Univariable regression models assessed for associations between different variables and length of stay.
Results:Data of 52 patients showed decreased mean z scores for the LVIDd (–3.16), LVIDs (–3.05), aortic annulus (–1.68), aortic sinuses (–2.11), transverse arch (–3.11), and sinotubular junction (–1.47) with preservation of the aorta at the diaphragm compared to age-matched normative data with similar body surface areas. Regression analysis showed a percent reduction in length of stay per 1 mm size increase for LVIDd (8%), aortic annulus (27%), aortic sinuses (18%), sinotubular junctions (20%), and transverse arches (25%).
Conclusions:Patients with congenital diaphragmatic hernias have significantly smaller left-sided heart structures compared to age-matched normative data. Aortic preservation at the diaphragm provides evidence for a mass effect aetiology with increased right-to-left shunting at the fetal ductus resulting in decreased size. Additionally, length of stay appears to be prolonged with decreasing size of several of these structures. These data provide quantitative evidence of smaller left-sided heart structures in patients with congenital diaphragmatic hernias.

An investigation of Illinoian- and Sangamonian-age deposits in the type region for both Pleistocene stages in central Illinois has yielded a palynological record spanning the Illinoian-year during at least the portion of the Sangamonian represented. Sangamonian boundary associated with an interglacial fauna containing Geochelone crassiscutata. The pollen indicates a shift from high Picea and Pinus to deciduous trees, followed by grass and herbaceous taxa, and finally, a return of deciduous trees. This sequence appears to correlate with marine isotopic stages 6 through 5d. Faunal remains are abundant throughout but megafauna are present only in the interglacial section where Geochelone occurs. Neuropsychological Performance of Youth with Secondary Attention-Deficit/Hyperactivity Disorder 6- and 12-Months after Traumatic Brain Injury

The present study compared executive dysfunction among children with attention-deficit/hyperactivity disorder (ADHD) after traumatic brain injury (TBI), also called secondary ADHD (S-ADHD), pre-injury ADHD and children with TBI only (i.e., no ADHD). Youth aged 6–16 years admitted for TBI to five trauma centers were enrolled (n=177) and evaluated with a semi-structured psychiatric interview scheduled on three occasions (within 2 weeks of TBI, i.e., baseline assessment for pre-injury status; 6-months and 12-months post-TBI). This permitted the determination of 6- and 12-month post-injury classifications of membership in three mutually exclusive groups (S-ADHD; pre-injury ADHD; TBI-only). Several executive control measures were administered. Unremitted S-ADHD was present in 17/141 (12%) children at the 6-month assessment, and in 14/125 (11%) children at 12-months post-injury. The study found that children with S-ADHD exhibited deficient working memory, attention, and psychomotor speed as compared to children with pre-injury ADHD. Furthermore, the children with S-ADHD and the children with TBI-only were impaired compared to the children with pre-injury ADHD with regard to planning. No group differences related to response inhibition emerged. Age, but not injury severity, gender, or adaptive functioning was related to executive function outcome. Neuropsychological sequelae distinguish among children who develop S-ADHD following TBI and those with TBI only. Moreover, there appears to be a different pattern of executive control performance in those who develop S-ADHD than in children with pre-injury ADHD suggesting that differences exist in the underlying neural mechanisms that define each disorder, underscoring the need to identify targeted treatment interventions. F. E. Green has corrected errors regarding the provenience and prior determination of the artifactual nature of the ivory semifabricate reported by Saunders et al. (1990) and has brought forth new information regarding the spring deposits of the north bank of the gravel pit at the Clovis site. This statement addresses the contemporaneity of Clovis occupation with deposition of the gray sand and the relation of the gray sand to spring deposits. We offer it in expectation that a better understanding of north-bank stratigraphy and geochronology might result regardless of which of two views of the archaeology of the gray sand eventually prevails.

A carved segment of mammoth ivory recently recognized in a faunal collection from the Clovis type site at Blackwater Draw, New Mexico, reveals for the first time techniques employed by Clovis Paleoindians for working ivory and adds a new trait element shared between the archaeological cultures of the Eurasian Upper Paleolithic and the late Pleistocene Clovis complex of North America.
