A citywide boil water notice necessitated an alternative solution for treating contaminated water. We report our experience using portable reverse osmosis machines to treat the municipal water to provide purified water to patient care areas where non-sterile water was needed, preventing interruptions in services like elective surgeries.

Tape rolls are often used for multiple patients despite recommendations by manufacturers for single-patient use. We developed a survey to query Health Care Personnel about their tape use practices and beliefs and uncovered behaviors that put patients at risk for hospital-acquired infections due to tape use.

This manuscript addresses a critical topic: navigating complexities of conducting clinical trials during a pandemic. Central to this discussion is engaging communities to ensure diverse participation. The manuscript elucidates deliberate strategies employed to recruit minority communities with poor social drivers of health for participation in COVID-19 trials. The paper adopts a descriptive approach, eschewing analysis of data-driven efficacy of these efforts, and instead provides a comprehensive account of strategies utilized. The Accelerate COVID-19 Treatment Interventions and Vaccines (ACTIV) public–private partnership launched early in the COVID-19 pandemic to develop clinical trials to advance SARS-CoV-2 treatments. In this paper, ACTIV investigators share challenges in conducting research during an evolving pandemic and approaches selected to engage communities when traditional strategies were infeasible. Lessons from this experience include importance of community representatives’ involvement early in study design and implementation and integration of well-developed public outreach and communication strategies with trial launch. Centralization and coordination of outreach will allow for efficient use of resources and the sharing of best practices. Insights gleaned from the ACTIV program, as outlined in this paper, shed light on effective strategies for involving communities in treatment trials amidst rapidly evolving public health emergencies. This underscores critical importance of community engagement initiatives well in advance of the pandemic.

Background: Medical tape is one of the most ubiquitous resources in the hospital. Although tape is advertised by manufacturers as a single patient-use item, half-used rolls are a common sight in hospitals. Tape is often manipulated by un-sanitized and ungloved hands and comes in close contact with patient skin. Medical tape has the potential to be a source of hospital-acquired infection as it has been documented to be colonized with pathogens ranging from MRSA to Rhizopus. Despite infection risk, currently the only clinical guidelines of tape use are outlined in the Centers for Medicare & Medicaid Services guidance for Hemodialysis patients issued in 2008 that requires “tape should be dedicated to a single patient and discarded after use” as hemodialysis patients are at higher risk of infection. However, there is a lack of standards in the practice of tape use across hospital systems. Methods: To understand the current practices and beliefs of tape use at our institution, we developed a standardized survey to query individuals from various roles (RN, Physician, Patient Care Technicians, respiratory therapists, phlebotomists) across all patient care areas at a 746-bed academic, tertiary care center. Results: 52 units were surveyed, including 225 employees. Qualitative analysis revealed a wide variety of uses for medical tape for patient care, with venipuncture, securing IVs, and wound dressings being the most common. Only 1.4% of individuals reported single use of tape rolls. 54% of individuals reported tape use behaviors that carry an elevated risk for inoculation of pathogens. 70% of individuals reported that tape was discarded after the patient was discharged from their respective area. These practices did not change across procedure-heavy areas such as the Emergency Department or the Operating Rooms, in fact only 22% of individuals surveyed reported single use of tape in these areas. Beliefs about tape use varied: 95% of individuals agreed that a roll of tape could be used multiple times on a single patient, and 52% of individuals agreed that a roll of tape could be used on multiple patients. Conclusions: Tape use practices varied across hospital units, indicating the need for standardized policies for tape use and storage. Beliefs about tape not being a single-use item were consistent across the hospital and suggest that education and culture change efforts are needed to decrease the risk for hospital-acquired infections from improper medical tape use.

This research communication was designed to evaluate the effects of different levels of diet restriction on the composition and ethanol stability (MES) of raw bovine milk. This research was carried out using three electronic databases: Scopus, Pubmed and Web of Science. The main inclusion criteria were: (i) original research, (ii) use of alcohol (ethanol) test as a method to assess milk stability, (iii) measure different levels of feed restriction and (iv) allow access to the raw data of articles. Of the nine publications that addressed the subject filtered by the systematic review, seven fitted the selection criteria and were selected to perform the meta-analysis. Feed restriction (reduction of 20, 30, 40 and 50% of the dietary dry matter offered) decreased (P < 0.01) milk yield (−18%), ethanol stability (−5%), acidity (−4%), protein (−3%) and lactose (−2%) concentrations, but did not affect the values of pH, density, fat and total solids concentrations, nor somatic cell count. The correlation between milk yield and MES was low but positive and numerically higher in the control group compared with the restriction group. The milk of cows fed the control diet presented greater ethanol stability (76.5%) compared with milk of cows fed the restrictive diet (72.8%). This decrease by up to 4 percentage units due to restriction levels ranging from 20 to 50% of diet intake may cause limitations in milk processing at the dairy industry, increasing milk rejection.

The 1717 Christmas flood is one of the most catastrophic storm surges the Frisian coast (Netherlands and Germany) has ever experienced. With more than 13,700 casualties it is the last severe storm surge with a death toll of this order. At the same time, little is known about the hydrodynamic conditions and the morphological effects associated with this storm surge.

In this study, 41 potential dyke failures in the Province of Groningen (Netherlands) associated with the 1717 Christmas flood were systematically reconstructed and mapped by using historical maps and literature and by analysing the recent topography in search of typical pothole structures and sediment fans. The dimensions of the sediment fans as derived from the topography show a good accordance with the dimensions documented by vibracore profiles, direct push tests and electrical resistivity tomography data taken at three fieldwork sites. Moreover, the fan dimensions closely agree with the dimensions as simulated using a process-based morphodynamic numerical model for one of the three sites, the village of Wierhuizen. Consequently, the recent topography is still indicative for the locations and dimensions of dyke failures and sediment fans associated with the 1717 Christmas flood. Considering the large number of detected dyke failures (41) and the large dimensions of the potholes and particularly of the sediment fans up to a few hundred metres wide and up to 0.7 m thick, this study proves significant morphological effects of the 1717 Christmas flood on the mainland of the Province of Groningen.

Based on the numerical simulation approach and the comparison with field data and field observations, a maximum seaward water level of 5 m NAP for the dyke failure at Wierhuizen during the Christmas flood can be derived. A similar maximum water level is indicated for the two other fieldwork sites Vierhuizen and Kohol, which is in good agreement with the maximum storm surge level of 4.62 m NAP historically documented for the city of Emden located almost 50 km to the east of Wierhuizen.

The results of the current study demonstrate that the reconstruction of historical dyke failures based on (i) historical sources, (ii) recent lidar/high-resolution topographical data, (iii) multi-proxy sedimentary field data and (iv) hydro- and morphodynamic numerical simulations is a highly promising approach to derive hydrodynamic conditions and the morphological onshore response of the 1717 Christmas flood in the Province of Groningen. This knowledge is essential to improve our understanding of extreme storm surge dynamics, their influence on the coastal landscape and the associated hazards for the coastal population.

Agri-environmental schemes involving organic farming or set-aside management aim at promoting biodiversity and restoring ecosystem functioning in agrarian landscapes. Application of pesticides in these crop fields is strongly regulated facilitating the spread of weeds but also allowing for the establishment of endangered herbs and a variety of animals. Recent studies found gastropods and earthworms to be legitimate dispersers of seeds of wild plants. We assumed that both groups also play a significant role in the spread and establishment of wild plants within crop fields. Therefore, we are conducting a series of experiments in three different study systems on (1) the role of earthworms and gastropods as dispersers of rare herbs and weeds in an organic rye field in Germany, (2) the seed feeding behavior of gastropods of plants sown in fallow ground in Switzerland, and (3) weed dispersal in irrigated rice fields by golden apple snails in the Philippines.
