Unspoken: Verbal Sexual Harassment by Patients in Psychiatry

Aims
Patient-initiated verbal sexual harassment (PIVSH) is common in the healthcare workplace, however institutes often neglect to address it. Objectives: (1) Define extent of PIVSH among staff at South London and the Maudsley Trust (sLaM), (2) Characterise the impact of PIVSH on staff, (3) Understand barriers to reporting PIVSH, (4) Inform policy and training to support staff.
MethodsA questionnaire from Scruggs et al. (2020) was adapted with types of PIVSH on a standardised scale of severity from ‘most’ to ‘least’ harassing. The anonymous, retrospective, online survey was disseminated to sLaM staff via Trust-wide communications, staff networks and Whatsapp groups. Descriptive statistics were used to analyse quantitative data (PIVSH frequency, confidence to respond to PIVSH, reporting practices). Respondents used free text to describe the impact of PIVSH, reasons for not reporting harassment, and views on the role of the Trust and supervisors in addressing PIVSH. Qualitative data were analysed using thematic analysis and externally validated.
Results42 responses were received across staff groups. 95.2% respondents had experienced PIVSH in the last year. 26.2% had formally reported an incident of PIVSH, with only 30.8% stating the report had been actioned by a senior. ‘Less severe’ harassment types were the most common, and the type staff were least confident to address. Five themes were identified in thematic analysis:
1. Nature of PIVSH: Unwanted, covert, influenced by victim demographics, the situation, and motivation of the perpetrator
2. Response to PIVSH: Victim's emotional and practical response, and of the wider MDT
3. Impact on trainee: Personal (desensitisation, feeling unsupported) and professional (time off, moved teams, avoidance of wards)
4. Barriers to action: Practical barriers to reporting (lack of time, complexity) and organisational culture (‘patient unwell’ justification, trivialisation, lack of trust in management)
5. Areas of improvement identified: Written policy on PIVSH clearly communicated to staff and patients; wider cultural changes of zero tolerance to PIVSH; open discussion and reporting, backed up by education and training; formalised support post-PIVSH event
ConclusionThere is a negative impact of PIVSH on staff at sLaM and it is not properly recognised. The NHS is its staff and we cannot afford to neglect their well-being. Action as a result of this survey will include:
1. Creation of a training package with Maudsley Simulation
2. Development of informational posters for clinical spaces
3. Write up-to-date trust policy on PIVSH

This paper presents new water-soluble bio-polyelectrolyte-based nanoparticles, formed from lanthanide-induced polysaccharide aggregates (LIPAs). These new nano-aggregates are formed by coordinating a photoluminescent lanthanide–ligand complex to a single polyelectrolyte [i.e. polyanionic hyaluronic acid (HA)] or to two oppositely charged polyelectrolytes [i.e. HA and polycationic chitosan (CHI)]. We demonstrate that photoluminescent Eu3+–ligand complexes, which are dispersed homogeneously in aqueous solution by the association with water-soluble HA. The polysaccharide supermolecular assembly can be tuned to obtain nanoparticles of different sizes and surface charges. The preparation of stable and water-soluble lanthanide complexes via Eu3+–LIPAs opens opportunities for use of luminescent lanthanides in aqueous environments, for biosensing and bioimaging applications.

Interactions between genetic variants of folate metabolism genes and lifestyle affect plasma homocysteine concentrations in the Boston Puerto Rican population

Objective
To investigate genetic and lifestyle factors and their interactions on plasma homocysteine (Hcy) concentrations in the Boston Puerto Rican population.
DesignCross-sectional study. Plasma concentrations of Hcy, folate, vitamin B12 and pyridoxal phosphate were measured, and genetic polymorphisms were determined. Data on lifestyle factors were collected in interviews.
SettingA population survey of health and nutritional measures.
SubjectsA total of 994 Puerto Rican men and women residing in the Boston metropolitan area.
ResultsSmoking status was positively associated with plasma Hcy. Genetic polymorphisms MTHFR 677C→T, FOLH1 1561C→T, FOLH1 rs647370 and PCFT 928A→G interacted significantly with smoking for Hcy. MTHFR 1298A→C (P = 0·040) and PCFT 928A→G (P = 0·002) displayed significant interactions with alcohol intake in determining plasma Hcy. Subjects with PCFT928GG genotype had significantly higher plasma Hcy concentrations compared with carriers of the A allele (AA+AG; P = 0·030) among non-drinking subjects. When consuming alcohol, GG subjects had lower plasma Hcy levels compared with AA+AG subjects. Physical activity interacted significantly with MTR 2756A→G in determining plasma Hcy (P for interaction = 0·002). Smoking interacted with physical activity for plasma Hcy (P for interaction = 0·023).
ConclusionsSmoking and drinking were associated plasma Hcy concentrations. Genetic variants involved in folate metabolism further modify the effects of lifestyle on plasma Hcy.

Effect of Size, Shape, Composition, and Support Film on Localized Surface Plasmon Resonance Frequency: A Single Particle Approach Applied to Silver Bipyramids and Gold and Silver Nanocubes

Localized surface plasmon resonances (LSPR), collective electron oscillations in nanoparticles, are being heavily scrutinized for applications in chemical and biological sensing, as well as in prototype nanophotonic devices. This phenomenon exhibits an acute dependence on the particle’s size, shape, composition, and environment. The detailed characterization of the structure-function relationship of nanoparticles is obscured by ensemble averaging. Consequently, single-particle data must be obtained to extract useful information from polydisperse reaction mixtures. Recently, a correlated high resolution transmission electron microscopy (HRTEM) LSPR technique has been developed and applied to silver nanocubes. We report here a second generation of experiments using this correlation technique, in which statistical analysis is performed on a large number of single particles. The LSPR dependence on size, shape, material, and environment was probed using silver right bipyramids, silver cubes, and gold cubes. It was found that the slope of the dependence of LSPR peak on size for silver bipyramids increases as the edges become sharper. Also, a plasmon shift of 96 nm was observed between similar silver and gold cubes, while a shift of 26 nm was observed, for gold cubes, between substrates of refractive index (RI) of 1.5 and 2.05.

Summary
INTRODUCTION
Shortness of breath is a common presenting complaint in human immunodeficiency virus (HIV)–infected patients, and the relevant differential diagnosis is broad. Etiologies include both infectious and noninfectious causes related and unrelated to underlying HIV infection and range from the minor (e.g., viral upper respiratory infection) to the life-threatening (e.g., Pneumocystis pneumonia [PCP]). Although the list of potential diseases may seem overwhelming, an understanding of the most commonly encountered pulmonary complications and their characteristic presentations will help narrow the differential diagnosis.
EPIDEMIOLOGY
The rate of bacterial pneumonia is between 5- and 25-fold higher in HIV-infected patients than in the non-HIV infected population. Although its prevalence has declined after the introduction of highly active anti-retroviral therapy (HAART), PCP is still a common acquired immunodeficiency syndrome (AIDS)-defining illness and is seen almost exclusively in immunocompromised persons. Globally, Mycobacterium tuberculosis (TB) pneumonia is the major pulmonary infection complicating the HIV epidemic. HIV-infected patients who have latent TB infection face a 10% annual risk of progressing from latent to active tuberculous disease compared to an approximately 10% lifetime risk in non-HIV-infected individuals. CLINICAL FEATURES
The clinical presentations of common HIV-associated pulmonary infections often overlap. The goal of a thorough history and physical is to narrow the differential diagnosis, recognize constellations of particular symptoms and signs, and guide diagnostic testing and therapy.
