Using junior doctors to improve patient care: Creating a clinic to monitor the physical health of patients prescribed clozapine

Introduction
In the United Kingdom, compliance with guidelines on physical health monitoring of patients prescribed clozapine is poor. Our community team established a ‘clozapine clinic’, led by junior doctors, to monitor the physical health of this population.
AimsThe aims of this audit were:
– to ascertain levels of compliance with guidelines on the physical health monitoring of patients taking clozapine;
– to compare the current level of compliance with that prior to the establishment of the clinic.
MethodsEleven standards were drawn from National Institute for Health and Care Excellence guidelines and the Maudsley Prescribing Guidelines in Psychiatry.
Three audit cycles were conducted: two prior to the establishment of the clinic and one after. In each cycle, searches of patient records were conducted and blood results were reviewed. This was supplemented by telephone calls to general practitioners to ensure a complete data set.
Analysis was conducted in Microsoft Excel™ and changes between the cycles were analysed using a two-tailed Z-score.
ResultsEach audit cycle included 28–30 patients. In the current cycle compliance levels varied between 66% (annual ECG recording) and 100% (monthly full blood count). The average compliance level was 73% across all standards. This represents an overall improvement on previous audit cycles. Since the clinic was established there has been a statistically significant improvement in compliance with annual monitoring of weight (P = 0.147), body mass index (P = 0.0178), and ECG monitoring (P = 0.0244).
ConclusionsImprovements in the care of a vulnerable population may be achieved through setting clear standards, regular audit, and harnessing the leadership and enthusiasm of junior doctors.

Background
Major depressive disorder (MDD) is a common and disabling condition with well-established heritability and environmental risk factors. Gene–environment interaction studies in MDD have typically investigated candidate genes, though the disorder is known to be highly polygenic. This study aims to test for interaction between polygenic risk and stressful life events (SLEs) or childhood trauma (CT) in the aetiology of MDD.
MethodThe RADIANT UK sample consists of 1605 MDD cases and 1064 controls with SLE data, and a subset of 240 cases and 272 controls with CT data. Polygenic risk scores (PRS) were constructed using results from a mega-analysis on MDD by the Psychiatric Genomics Consortium. PRS and environmental factors were tested for association with case/control status and for interaction between them.
ResultsPRS significantly predicted depression, explaining 1.1% of variance in phenotype (p = 1.9 × 10−6). SLEs and CT were also associated with MDD status (p = 2.19 × 10−4 and p = 5.12 × 10−20, respectively). No interactions were found between PRS and SLEs. Significant PRSxCT interactions were found (p = 0.002), but showed an inverse association with MDD status, as cases who experienced more severe CT tended to have a lower PRS than other cases or controls. This relationship between PRS and CT was not observed in independent replication samples.
ConclusionsCT is a strong risk factor for MDD but may have greater effect in individuals with lower genetic liability for the disorder. Including environmental risk along with genetics is important in studying the aetiology of MDD and PRS provide a useful approach to investigating gene–environment interactions in complex traits.

Background
Sleep loss produces abnormal increases in reward seeking but the mechanisms underlying this phenomenon are poorly understood. The present study examined the influence of one night of sleep deprivation on neural responses to a monetary reward task in a sample of late adolescents/young adults.
MethodUsing a within-subjects crossover design, 27 healthy, right-handed late adolescents/young adults (16 females, 11 males; mean age 23.1 years) underwent functional magnetic resonance imaging (fMRI) following a night of sleep deprivation and following a night of normal sleep. Participants' recent sleep history was monitored using actigraphy for 1 week prior to each sleep condition.
ResultsFollowing sleep deprivation, participants exhibited increased activity in the ventral striatum (VS) and reduced deactivation in the medial prefrontal cortex (mPFC) during the winning of monetary reward, relative to the same task following normal sleep conditions. Shorter total sleep time over the five nights before the sleep-deprived testing condition was associated with reduced deactivation in the mPFC during reward.
ConclusionsThese findings support the hypothesis that sleep loss produces aberrant functioning in reward neural circuitry, increasing the salience of positively reinforcing stimuli. Aberrant reward functioning related to insufficient sleep may contribute to the development and maintenance of reward dysfunction-related disorders, such as compulsive gambling, eating, substance abuse and mood disorders.
No increased risk of psychological/behavioral disorders in siblings of women with hyperemesis gravidarum (HG) unless their mother had HG

Hyperemesis gravidarum (HG), severe nausea and vomiting of pregnancy, is characterized by prolonged maternal stress, undernutrition and dehydration. Maternal stress and malnutrition of pregnancy are linked to poor neonatal outcome and associated with poor adult health, and we recently showed that in utero exposure to HG may lead to increased risks of psychological and behavioral disorders in the offspring. In addition, we have shown familial aggregation of HG, which is strong evidence for a genetic component to the disease. In this study, we compare the rates of psychological and behavioral disorders in 172 adults with and 101 adults without a sibling with HG. The rate of emotional/behavioral disorders is identical (15%) in both groups. The results suggest that the etiology of HG is not likely to include genetic factors associated with emotional and behavioral disorders. In addition, this study provides evidence that the increased incidence of psychological/behavioral disorders among offspring of women with HG is attributable to the HG pregnancy itself, rather than to confounding genetic factors linked to HG.

Hyperemesis gravidarum (HG), severe nausea and vomiting of pregnancy, is characterized by long-term maternal stress, undernutrition and dehydration. While maternal stress and malnutrition of pregnancy are linked to poor neonatal outcome and associated with poor adult health, long-term outcome of fetal exposure to HG has never been explored. The purpose of this study is to determine whether long-term emotional and behavioral diagnoses may be associated with fetal exposure to HG. Emotional and behavioral diagnoses of adults born of a pregnancy complicated by HG were compared to diagnoses from non-exposed controls. Offspring exposed to HG in utero were significantly more likely to have a psychological and behavioral disorder (OR = 3.6, P < 0.0001) with diagnoses primarily of depression, bipolar disorder and anxiety. In utero exposure to HG may lead to increased risks of psychological and behavioral disorders in the offspring.

A unique atomic force microscope-based local thermal-mechanical analysis (LTA) technique was used to study the influence of room temperature aging on viscoelastic properties of ethylene-methacrylic (E/MAA) acid ionomers. This approach permits easy access to structural relaxation effects on viscoelasticity at a short aging time, for instance, before the occurrence of secondary crystallization differential scanning calorimetry (DSC) melting peak. A Burger model along with finite element method yields quantitative analysis of viscoelastic properties versus the aging time. Creep curves were obtained with LTA after various times of aging at room temperature upon cooling from the melt. Measurements were carried out at both 30 and 70 °C. The results reveal the effects of structural relaxation upon aging in the ion-rich amorphous region, the influence of secondary crystallites on the viscoelastic properties, and shed light on the processes associated with aging in E/MAA ionomers.

Summary
A new mechanism for oblique wave resonance
Despite the large body of research concerned with the near wake of a circular cylinder, the far wake, which extends beyond about 100 diameters downstream, is relatively unexplored, especially at low Reynolds numbers. We have recently shown that the structure of the far wake is exquisitely sensitive to free-stream noise, and is precisely dependent on the frequency and scale of the near wake; indeed it is shown that the presence of extremely low-amplitude peaks in the free-stream spectrum, over a remarkably wide range of frequencies, are sufficient to trigger an “oblique wave resonance” in the far wake.
We show, in the upper photograph of Fig. 1, a nonlinear interaction between oblique shedding waves generated from upstream (to the left) and 2–D waves amplified downstream from free-stream disturbances (in the central region). We use the “smoke-wire” technique (placed 50 diameters down-stream), and the wake is viewed in planview, with flow to the right. This two-wave interaction triggers a third wave, namely an “oblique resonance wave” at a large oblique angle, to grow through nonlinear effects (in the right half of the photograph), in preference to the original two waves. If smoke is introduced 100 diameters downstream, in the lower photograph (under slightly different conditions), then all that is seen is a set of such large-angle oblique resonance waves.
