The mental health of Australians bereaved during the first two years of the COVID-19 pandemic: a latent class analysis

Background
The COVID-19 pandemic disrupted many areas of life, including culturally accepted practices at end-of-life care, funeral rites, and access to social, community, and professional support. This survey investigated the mental health outcomes of Australians bereaved during this time to determine how these factors might have impacted bereavement outcomes.
MethodsAn online survey indexing pandemic and bereavement experiences, levels of grief, depression, anxiety, and health, work, and social impairment. Latent class analysis (LCA) was used to identify groups of individuals who shared similar symptom patterns. Multinomial regressions identified pandemic-related, loss-related, and sociodemographic correlates of class membership.
Results1911 Australian adults completed the survey. The LCA identified four classes: low symptoms (46.8%), grief (17.3%), depression/anxiety (17.7%), and grief/depression/anxiety (18.2%). The latter group reported the highest levels of health, work, and social impairment. The death of a child or partner and an inability to care for the deceased due to COVID-19 public health measures were correlated with grief symptoms (with or without depression and anxiety). Preparedness for the person's death and levels of pandemic-related loneliness and social isolation differentiated all four classes. Unemployment was associated with depression/anxiety (with or without grief).
ConclusionsCOVID-19 had profound impacts for the way we lived and died, with effects that are likely to ricochet through society into the foreseeable future. These lessons learned must inform policymakers and healthcare professionals to improve bereavement care and ensure preparedness during and following future predicted pandemics to prevent negative impacts.
Collaborative team dynamics and scholarly outcomes of multidisciplinary research teams: A mixed-methods approach

Introduction:
Impactful, transdisciplinary scientific discoveries are created by teams of researchers spanning multiple disciplines, but collaboration across disciplines can be challenging. We examined how team dynamics and collaboration are related to successes and barriers faced by teams of researchers from multiple disciplines.
Methods:A mixed-methods approach was used to examine 12 research teams granted multidisciplinary pilot awards. Team members were surveyed to assess their team dynamics and individual views about transdisciplinary research. Forty-seven researchers (59.5%) responded, including two to eight members from each funded team. Associations were examined between collaborative dynamics and scholarly product outcomes, including manuscripts, grant proposals, and awarded grants. One member from each team was selected for an in-depth interview to contextualize and extend information about collaborative processes, successes, and barriers to performing transdisciplinary research.
Results:Quality of team interactions was positively associated with achievement of scholarly products (r = 0.64, p = 0.02). Satisfaction with team members (r = 0.38) and team collaboration scores (r = 0.43) also demonstrated positive associations with achievement of scholarly products, but these were not statistically significant. Qualitative results support these findings and add further insight into aspects of the collaborative process that were particularly important to foster success on multidisciplinary teams. Beyond scholarly metrics, additional successes from the multidisciplinary teams were identified through the qualitative portion of the study including career development and acceleration for early career researchers.
Conclusions:Both the quantitative and qualitative study results indicate that effective collaboration is critical to multidisciplinary research team success. Development and/or promotion of team science-based trainings for researchers would promote these collaborative skills.

Summary
Transformation is required in complex food systems to bring about global food security for a well-nourished world population while meeting climate-related challenges. The key is to identify the best levers to achieve change. To this end, food-system transformation has four major interlocking elements: (1) rerouting systems and livelihoods into new trajectories; (2) addressing climate impacts, thereby reducing risks; (3) tackling new environmental issues, for example by reimagining diets and value chains, to lessen emissions; and (4) realigning the ’enablers of change’, such as policies, regulation, finance, and innovation. Eleven specific, concrete actions are proposed to attain these four objectives, with explanations of the goal of each action, the mechanisms to accomplish it, targeted geographic areas, and key stakeholders. Achieving food-system transformation will require annual investments of US$850 billion from now until 2050, with private-sector finance helping to fill current gaps.

Background
Evidence on the long-term comparative effectiveness of posttraumatic stress disorder (PTSD) psychotherapies in adults remains unknown. Therefore, we performed an extensive network meta-analysis of randomised controlled trials (RCTs) to determine the comparative effectiveness of psychotherapies for people diagnosed with PTSD.
MethodsA comprehensive search was conducted in Cochrane library, Embase, Medline-OVID, PubMed, Scopus, and Psych-Info until March 2021. Studies on the effectiveness of cognitive processing therapy (CPT), cognitive therapy (CT), eye movement desensitisation reprocessing (EMDR), narrative exposure therapy (NET), prolonged exposure (PE), cognitive behavioural therapy (CBT), present-centred therapy (PCT), brief eclectic psychotherapies (BEP), psychodynamic therapy (PDT) or combination therapies compared to no treatment (NT) or treatment as usual (TAU) in adults with PTSD were included. Frequentist and Bayesian approaches were used for analysis in R-software.
ResultsWe included 98 RCTs with 5567 participants from 18 897 studies. CPT, EMDR, CT, NET, PE, CBT, and PCT were significant to reduce PTSD symptoms (SMD range: −1.53 to −0.75; Certainty: very low to high) at immediate post-treatment and ranked accordingly. Longitudinal analysis found EMDR (1.02) and CPT (0.85) as the significant therapies with large effect size in short-term and long-term follow-up, respectively. NET and CPT showed higher proportion of loss of PTSD diagnosis (RR range: 5.51–3.45) while there were no significant psychotherapies for retention rate compared to NT.
Conclusions:Our findings provide evidence for improving current guidelines and informing clinical decision-making for PTSD management. However, the best PTSD treatment plan should be tailored to patients' needs, characteristics, and clinician expertise.

Accountability and adaptive performance under uncertainty: A long-term view

Accountability pressures are a ubiquitous feature of social systems: virtually everyone must answer to someone for something. Behavioral research has, however, warned that accountability, specifically a focus on being responsible for outcomes, tends to produce suboptimal judgments. We qualify this view by demonstrating the long-term adaptive benefits of outcome accountability in uncertain, dynamic environments. More than a thousand randomly assigned forecasters participated in a ten-month forecasting tournament in conditions of control, process, outcome or hybrid accountability. Accountable forecasters outperformed non-accountable ones. Holding forecasters accountable to outcomes (“getting it right”) boosted forecasting accuracy beyond holding them accountable for process (“thinking the right way”). The performance gap grew over time. Process accountability promoted more effective knowledge sharing, improving accuracy among observers. Hybrid (process plus outcome) accountability boosted accuracy relative to process, and improved knowledge sharing relative to outcome accountability. Overall, outcome and process accountability appear to make complementary contributions to performance when forecasters confront moderately noisy, dynamic environments where signal extraction requires both knowledge pooling and individual judgments.
Developing expert political judgment: The impact of training and practice on judgmental accuracy in geopolitical forecasting tournaments

The heuristics-and-biases research program highlights reasons for expecting people to be poor intuitive forecasters. This article tests the power of a cognitive-debiasing training module (“CHAMPS KNOW”) to improve probability judgments in a four-year series of geopolitical forecasting tournaments sponsored by the U.S. intelligence community. Although the training lasted less than one hour, it consistently improved accuracy (Brier scores) by 6 to 11% over the control condition. Cognitive ability and practice also made largely independent contributions to predictive accuracy. Given the brevity of the training tutorials and the heterogeneity of the problems posed, the observed effects are likely to be lower-bound estimates of what could be achieved by more intensive interventions. Future work should isolate which prongs of the multipronged CHAMPS KNOW training were most effective in improving judgment on which categories of problems.
Life expectancy, mortality risks and cause of death in patients with serious mental illness in South East London: a comparison between 2008–2012 and 2013–2017

Background
People with serious mental illness (SMI) have a significantly shorter life expectancy than the general population. This study investigates whether the mortality rate in this group has changed over the last decade.
MethodsUsing Clinical Record Interactive Search software, we extracted data from a large electronic database of patients in South East London. All patients with schizophrenia, schizoaffective disorder or bipolar disorder from 2008 to 2012 and/or 2013 to 2017 were included. Estimates of life expectancy at birth, standardised mortality ratios and causes of death were obtained for each cohort according to diagnosis and gender. Comparisons were made between cohorts and with the general population using data obtained from the UK Office of National Statistics.
ResultsIn total, 26 005 patients were included. In men, life expectancy was greater in 2013–2017 (64.9 years; 95% CI 63.6–66.3) than in 2008–2012 (63.2 years; 95% CI 61.5–64.9). Similarly, in women, life expectancy was greater in 2013–2017 (69.1 years; 95% CI 67.5–70.7) than in 2008–2012 (68.1 years; 95% CI 66.2–69.9). The difference with general population life expectancy fell by 0.9 years between cohorts in men, and 0.5 years in women. In the 2013–2017 cohorts, cancer accounted for a similar proportion of deaths as cardiovascular disease.
ConclusionsRelative to the general population, life expectancy for people with SMI is still much worse, though it appears to be improving. The increased cancer-related mortality suggests that physical health monitoring should consider including cancer as well.

Background
Schizophrenia patients have markedly elevated prevalence of diabetes compared with the general population. However, risk of mortality and diabetes-related complications among schizophrenia patients with co-occurring diabetes is understudied.
AimsWe investigated whether schizophrenia increased the risk of overall mortality, complications and post-complication mortality in people with diabetes.
MethodThis population-based, propensity-score matched (1:10) cohort study identified 6991 patients with incident diabetes and pre-existing schizophrenia and 68 682 patients with incident diabetes only between 2001 and 2016 in Hong Kong using a medical record database of public healthcare services. Association between schizophrenia and all-cause mortality was examined with a Cox proportional hazards model. Effect of schizophrenia on first-year complication occurrence following diabetes diagnosis and post-complication mortality rates were evaluated.
ResultsSchizophrenia was associated with increased all-cause mortality (adjusted hazards ratio [aHR] 1.11, 95% CI 1.05–1.18), particularly among men and older age groups. Schizophrenia patients with diabetes had higher metabolic complication rate (aHR 1.99, 95% CI 1.63–2.42), lower microvascular complication rate (aHR 0.75, 95% CI 0.65–0.86) and comparable macrovascular complication rate (aHR 0.93, 95% CI 0.85–1.03), relative to patients with diabetes only. Among patients with diabetes complications, schizophrenia was associated with elevated all-cause mortality after macrovascular (aHR 1.19, 95% CI 1.04–1.37) and microvascular (aHR 1.33, 95% CI 1.08–1.64) complications. Gender-stratified analyses revealed that a significant effect of schizophrenia on heightened post-complication mortality was observed in men only.
ConclusionsSchizophrenia patients with co-occurring diabetes are at increased risk of excess mortality, including post-complication mortality. Further research identifying effective interventions is warranted to optimise diabetes-related outcomes in this vulnerable population.

Background: Certain nursing home (NH) resident care tasks have a higher risk for multidrug-resistant organisms (MDRO) transfer to healthcare personnel (HCP), which can result in transmission to residents if HCPs fail to perform recommended infection prevention practices. However, data on HCP-resident interactions are limited and do not account for intrafacility practice variation. Understanding differences in interactions, by HCP role and unit, is important for informing MDRO prevention strategies in NHs. Methods: In 2019, we conducted serial intercept interviews; each HCP was interviewed 6–7 times for the duration of a unit’s dayshift at 20 NHs in 7 states. The next day, staff on a second unit within the facility were interviewed during the dayshift. HCP on 38 units were interviewed to identify healthcare personnel (HCP)–resident care patterns. All unit staff were eligible for interviews, including certified nursing assistants (CNAs), nurses, physical or occupational therapists, physicians, midlevel practitioners, and respiratory therapists. HCP were asked to list which residents they had cared for (within resident rooms or common areas) since the prior interview. Respondents selected from 14 care tasks. We classified units into 1 of 4 types: long-term, mixed, short stay or rehabilitation, or ventilator or skilled nursing. Interactions were classified based on the risk of HCP contamination after task performance. We compared proportions of interactions associated with each HCP role and performed clustered linear regression to determine the effect of unit type and HCP role on the number of unique task types performed per interaction. Results: Intercept-interviews described 7,050 interactions and 13,843 care tasks. Except in ventilator or skilled nursing units, CNAs have the greatest proportion of care interactions (interfacility range, 50%–60%) (Fig. 1). In ventilator and skilled nursing units, interactions are evenly shared between CNAs and nurses (43% and 47%, respectively). On average, CNAs in ventilator and skilled nursing units perform the most unique task types (2.5 task types per interaction, Fig. 2) compared to other unit types (P < .05). Compared to CNAs, most other HCP types had significantly fewer task types (0.6–1.4 task types per interaction, P < .001). Across all facilities, 45.6% of interactions included tasks that were higher-risk for HCP contamination (eg, transferring, wound and device care, Fig. 3). Conclusions: Focusing infection prevention education efforts on CNAs may be most efficient for preventing MDRO transmission within NH because CNAs have the most HCP–resident interactions and complete more tasks per visit. Studies of HCP-resident interactions are critical to improving understanding of transmission mechanisms as well as target MDRO prevention interventions.

The growth of the longitudinal beam–plasma instability in the presence of an inhomogeneous background

We study the longitudinal stability of beam–plasma systems in the presence of a density inhomogeneity in the background plasma. Previous works have focused on the non-relativistic regime where hydrodynamical models are used to evolve pre-existing Langmuir waves within inhomogeneous background plasmas. Here, for the first time we study the problem with kinetic equations in a fully relativistic way. We do not assume the existence of Langmuir waves, and we focus on the rate and the mechanism by which waves are excited in such systems from an initial perturbation. We derive the structure of the unstable modes and compute an analytical approximation for their growth rates. Our computation is limited to dilute and cold beams, and shows an excellent agreement with particle-in-cell simulations performed using the SHARP code. We show that, due to such an inhomogeneity, the virulent beam–plasma instabilities in the intergalactic medium are not suppressed but their counterparts in the solar wind can be suppressed as evidenced by propagating type-III solar radio bursts.

Recent studies have shown that for suitable initial conditions both super- and sub-Chandrasekhar mass carbon-oxygen white dwarf mergers produce explosions similar to observed SNe Ia. The question remains, however, how much fine tuning is necessary to produce these conditions. We performed a large set of SPH merger simulations, sweeping the possible parameter space. We find trends for merger remnant properties, and discuss how our results affect the viability of our recently proposed sub-Chandrasekhar merger channel for SNe Ia.
