Feasibility of regional center telehealth visits utilizing a rural research network in people with Parkinson’s disease

Background:
Impaired motor and cognitive function can make travel cumbersome for People with Parkinson’s disease (PwPD). Over 50% of PwPD cared for at the University of Arkansas for Medical Sciences (UAMS) Movement Disorders Clinic reside over 30 miles from Little Rock. Improving access to clinical care for PwPD is needed.
Objective:To explore the feasibility of remote clinic-to-clinic telehealth research visits for evaluation of multi-modal function in PwPD.
Methods:PwPD residing within 30 miles of a UAMS Regional health center were enrolled and clinic-to-clinic telehealth visits were performed. Motor and non-motor disease assessments were administered and quantified. Results were compared to participants who performed at-home telehealth visits using the same protocols during the height of the COVID pandemic.
Results:Compared to the at-home telehealth visit group (n = 50), the participants from regional centers (n = 13) had similar age and disease duration, but greater disease severity with higher total Unified Parkinson’s disease rating scale scores (Z = −2.218, p = 0.027) and lower Montreal Cognitive Assessment scores (Z = −3.350, p < 0.001). Regional center participants had lower incomes (Pearson’s chi = 21.3, p < 0.001), higher costs to attend visits (Pearson’s chi = 16.1, p = 0.003), and lived in more socioeconomically disadvantaged neighborhoods (Z = −3.120, p = 0.002). Prior research participation was lower in the regional center group (Pearson’s chi = 4.5, p = 0.034) but both groups indicated interest in future research participation.
Conclusions:Regional center research visits in PwPD in medically underserved areas are feasible and could help improve access to care and research participation in these traditionally underrepresented populations.

This study examines the performance of low-cost, low-power GNSS positioning systems for glacier monitoring in high-latitude environments. We compare the positioning performance of co-located low-cost u-blox ZED-F9P GNSS units (a few hundred USDs) and survey-grade Trimble R10 units (> $10,000 USD) under stationary (on land) and dynamic (on glacier) conditions near Terra Nova Bay, Antarctica. Low-cost and survey-grade systems yield almost identical error magnitudes under short (3 m), medium (34 km) and long (390 km) baseline kinematic-positioning scenarios. We further examined the efficacy of low-cost GNSS for glaciological applications by installing four u-blox and two Trimble receivers on Priestley Glacier to observe tide-modulated ice flexure. All receivers successfully detected subtle tidal oscillations with amplitudes < 3 cm, consistent with the predicted phasing from a tide model. These experiments offer a strong rationale for the widespread use of low-cost receivers to expand and densify GNSS monitoring networks, both in Antarctica and in glaciated regions worldwide.

Introduction:
Gait, balance, and cognitive impairment make travel cumbersome for People with Parkinson’s disease (PwPD). About 75% of PwPD cared for at the University of Arkansas for Medical Sciences’ Movement Disorders Clinic reside in medically underserved areas (MUAs). Validated remote evaluations could help improve their access to care. Our goal was to explore the feasibility of telemedicine research visits for the evaluation of multi-modal function in PwPD in a rural state.
Methods:In-home telemedicine research visits were performed in PwPD. Motor and non-motor disease features were evaluated and quantified by trained personnel, digital survey instruments for self-assessments, digital voice recordings, and scanned and digitized Archimedes spiral drawings. Participant’s MUA residence was determined after evaluations were completed.
Results:Twenty of the fifty PwPD enrolled resided in MUAs. The groups were well matched for disease duration, modified motor UPDRS, and Montreal Cognitive assessment scores but MUA participants were younger. Ninety-two percent were satisfied with their visit, and 61% were more likely to participate in future telemedicine research. MUA participants traveled longer distances, with higher travel costs, lower income, and education level. While 50% of MUA participants reported self-reliance for in-person visits, 85% reported self-reliance for the telemedicine visit. We rated audio-video quality highly in approximately 60% of visits in both groups. There was good correlation with prior in-person research assessments in a subset of participants.
Conclusions:In-home research visits for PwPD in MUAs are feasible and could help improve access to care and research participation in these traditionally underrepresented populations.

We offer definitions of important terms related to neurodevelopmental disorders through unifying research data, medical and other viewpoints. This may improve understanding, clinical practice and development of neurodevelopmental disorder pathways within adult mental health services.

The University of Arkansas for Medical Sciences (UAMS), like many rural states, faces clinical and research obstacles to which digital innovation is seen as a promising solution. To implement digital technology, a mobile health interest group was established to lay the foundation for an enterprise-wide digital health innovation platform. To create a foundation, an interprofessional team was established, and a series of formal networking events was conducted. Three online digital health training models were developed, and a full-day regional conference was held featuring nationally recognized speakers and panel discussions with clinicians, researchers, and patient advocates involved in digital health programs at UAMS. Finally, an institution-wide survey exploring the interest in and knowledge of digital health technologies was distributed. The networking events averaged 35–45 attendees. About 100 individuals attended the regional conference with positive feedback from participants. To evaluate mHealth knowledge at the institution, a survey was completed by 257 UAMS clinicians, researchers, and staff. It revealed that there are opportunities to increase training, communication, and collaboration for digital health implementation. The inclusion of the mobile health working group in the newly formed Institute for Digital Health and Innovation provides a nexus for healthcare providers and researches to facilitate translational research.

The Kirchhoff approximation is widely used to describe the scatter of elastodynamic waves. It simulates the scattered field as the convolution of the free-space Green’s tensor with the geometrical elastodynamics approximation to the total field on the scatterer surface and, therefore, cannot be used to describe nongeometrical phenomena, such as head waves. The aim of this paper is to demonstrate that an alternative approximation, the convolution of the far-field asymptotics of the Lamb’s Green’s tensor with incident surface tractions, has no such limitation. This is done by simulating the scatter of a critical Gaussian beam of transverse motions from an infinite plane. The results are of interest in ultrasonic nondestructive testing.
Using electron backscatter diffraction to measure full crystallographic orientation in Antarctic land-fast sea ice

We have mapped the full crystallographic orientation of sea ice using electron backscatter diffraction (EBSD). This is the first time EBSD has been used to study sea ice. Platelet ice is a feature of sea ice near ice shelves. Ice crystals accumulate as an unconsolidated sub-ice platelet layer beneath the columnar ice (CI), where they are subsumed by the advancing sea–ice interface to form incorporated platelet ice (PI). As is well known, in CI the crystal preferred orientation comprises dominantly horizontal c-axes, while PI has c-axes varying between horizontal and vertical. For the first time, this study shows the a-axes of CI and PI are not random. Misorientation analysis has been used to illuminate the possible drivers of these alignments. In CI the misorientation angle distribution from random pairs and neighbour pairs of grains are indistinguishable, indicating the distributions are a consequence of crystal preferred orientation. Geometric selection during growth will develop the a-axis alignment in CI if ice growth in water is fastest parallel to the a-axis, as has previously been hypothesised. In contrast, in PI random-pair and neighbour-pair misorientation distributions are significantly different, suggesting mechanical rotation of crystals at grain boundaries as the most likely explanation.
Orientation contrast imaging of microstructures in rocks using forescatter detectors in the scanning electron microscope

We have developed a system using ‘forescatter detectors’ for backscattered imaging of specimen surfaces inclined at 50–80° to the incident beam (inclined-scanning) in the SEM. These detectors comprise semiconductor chips placed below the tilted specimen. Forescatter detectors provide an orientation contrast (OC) image to complement quantitative crystallographic data from electron backscatter patterns (EBSP). Specimens were imaged using two detector geometries and these images were compared to those collected with the specimen surface normal to the incident beam (normal-scanning) using conventional backscattered electron detector geometries and also to an automated technique, orientation imaging microscopy (OIM). When normal-scanning, the component of the BSE signal relating to the mean atomic number (z) of the material is an order of magnitude greater than any OC component, making OC imaging in polyphase specimens almost impossible. Images formed in inclined-scanning, using forescatter detectors, have OC and z-contrast signals of similar magnitude, allowing OC imaging in polyphase specimens.
OC imaging is purely qualitative, and by repeatedly imaging the same area using different specimen-beam geometries, we found that a single image picks out less than 60% of the total microstructural information and as many as 6 combined images are required to give the full data set. The OIM technique is limited by the EBSP resolution (1–2°) and subsequently misses a lot of microstructural information. The use of forescatter detectors is the most practical means of imaging OC in tilted specimens, but it is also a powerful tool in its own right for imaging microstructures in polyphase specimens, an essential asset for geological work.
Slip systems and critical resolved shear stress in pyrite: an electron backscatter diffraction (EBSD) investigation

A suite of experimentally deformed single-crystal pyrite samples has been investigated using electron backscatter diffraction (EBSD). Single crystals were loaded parallel to <100> or <110> and deformed at a strain rate of 10-5 s-1, confining pressure of 300 MPa and temperatures of 600°C and 700°C. Although geometrically (Schmid factor) the {001}<100> slip system should not be activated in <100> loaded samples, lattice rotation and boundary trace analyses of the distorted crystals indicate this slip system is easier to justify. Determination of 75 MPa as the critical resolved shear stress (CRSS) for {001}<100> activation, in the <110> loaded crystals, suggests a crystal misalignment of ~5—15° in the <100> loaded crystals would be sufficient to activate the {001}<100> slip system. Therefore, {001}<100> is considered the dominant slip system in all of the single-crystal pyrite samples studied. Slip-system analysis of the experimentally deformed polycrystalline pyrite aggregates is consistent with the single-crystal findings, with the exception that {001}<11̄> also appears to be important, although less common than the {001}<100> slip system. The lack of crystal preferred orientation (CPO) development in the polycrystalline pyrite aggregates can be accounted for by the presence of two independent symmetrically equivalent slip systems more than satisfying the von Mises criterion.

To material scientists the term texture means the crystallographic orientation of grains in a polycrystal. In contrast, geologists use the term more generally to refer to the spatial arrangement or association of mineral grains in a rock. In this contribution we are concerned with the materials science definition. There are several established techniques available for the determination of crystallographic textures in rocks. It has also been realised that the scanning electron microscope (SEM) is applicable to the study of crystallographic textures via the electron channelling (EC) effect. This provides an image of mineral/rock microstructure (via orientation contrast), as well as a means of accurately indexing their crystal orientations (via electron channelling patterns, ECP). Both types of EC image result from the relationship between incident electron beam and crystal structure, and the subsequent modulation of the backscattered electron (BSE) emission signal according to Bragg's Law. It is a simple matter to switch between the two imaging modes. A related effect, electron backscattering, provides only the diffraction patterns, but has superior spatial resolution and pattern angles.
Due to crystal symmetry restrictions, there is only a limited range of ECP configurations possible for any mineral. Individual patterns can therefore be identified by comparison with the complete ‘ECP-map’. The location of an individual pattern within the map area is determined by spherical angles, the exact definition of which depends on the type of fabric diagram (e.g. inverse pole figure, pole figure or orientation distribution function). Originally, these angles were measured manually. A computer program (CHANNEL) has been developed which uses a digitisation approach to pattern recognition, derives the required fabric diagrams and also constructs ECP-maps from standard crystal data (i.e. unit cell parameters etc.).
The combination of SEM/EC and CHANNEL dramatically facilitates the study of crystal textures in minerals and rocks, making statistical crystallographic analysis from individual orientations a practicality. The following example applications are considered: (1) crystal structure representation of the Al2SiO5 polymorph system; (2) local crystal texture relationships (epitaxial nucleation) between andalusite and sillimanite grains; (3) bulk rock crystal textures of quartzites; and (4) physical properties (e.g. elastic constants and seismic velocities) determined from bulk rock texture.

Dust particles in an ice core from East Rongbuk Glacier on the northern slope of Qomolangma (Mount Everest; 28°01′ N, 86°58′ E; 6518 m a.s.l.), central Himalaya, have been identified as mica using a combination of scanning electron microscope-based techniques and energy-dispersive X-ray spectroscopy to identify the elements present, and electron backscatter diffraction to identify the crystal type. This technique for identifying individual crystalline dust particles in samples of glacial ice could be especially useful in the future for identifying water-soluble crystals in ice, for studying the strain history (glaciotectonics) of basal ice or in studies of ice–mica composites used as analogs of quartz-mica rocks.
Bereavement, multimorbidity and mortality: a population-based study using bereavement as an indicator of mental stress

Background
Mental stress is associated with higher mortality, but it remains controversial whether the association is causal or a consequence of a higher physical disease burden in those with a high mental stress load. Understanding causality is important when developing targeted interventions. We aimed to estimate the effect of mental stress on mortality by performing a ‘natural’ experiment using spousal bereavement as a disease-independent mental stressor.
MethodsWe followed a population-based matched cohort, including all individuals in Denmark bereaved in 1997–2014, for 17 years. Prospectively recorded register data were obtained for civil and vital status, 39 mental and physical diagnoses, and socioeconomic factors.
ResultsIn total, 389 316 bereaved individuals were identified and 137 247 died during follow-up. Bereaved individuals had higher all-cause mortality than non-bereaved references in the entire study period. The relative mortality in the bereaved individuals was highest shortly after the loss (adjusted hazard ratio (aHR), first month: 2.50, 95% confidence interval (CI) 2.37–2.63; aHR, 6–12 months: 1.38, 95% CI 1.34–1.42). The excess mortality rate associated with bereavement rose with increasing number of physical diseases (1.33 v. 7.00 excess death per 1000 person-months for individuals with 0 v. ⩾3 physical conditions during the first month) and was exacerbated by the presence of mental illness. The excess mortality among bereaved individuals was primarily due to death from natural causes.
ConclusionsBereavement was associated with increased short-term and long-term mortality, even after adjustment for morbidities, which suggests that mental stress may play a causal role in excess mortality.
Microstructure, phase content, and thermal stability of a cast Co–Cr dental alloy after porcelain sintering cycles using electron backscatter diffraction

Phase maps of Co–Cr alloys bonded to dental porcelain cycled through an incremental number of porcelain firings at two separate thicknesses (0.5 and 1 mm) were analyzed. Bulk hexagonal close-packed (hcp) phase vol% of the alloy was found to increase with the number of porcelain firings for both 0.5 and 1 mm specimens. At the metal-porcelain interface, a uniform fine-grained hcp phase was observed. The depth and grain size of this hcp layer increased with the number of porcelain firings with the thicker specimens undergoing more substantial growth and transformation. Simple heat transfer modeling of the specimens during heat treatment cycles indicated that the thicker specimen had more time at high temperature to affect the face-centered cubic to hcp phase transformation. Therefore, the amount of porcelain firings and the thickness of the alloy should be considered and kept to a minimal when manufacturing metal-porcelain restoration.

We describe aspects of the life history of Erynnis propertius (Scudder and Burgess) (Lepidoptera: Hesperiidae) by examining several populations over multiple years. We focused on peripheral populations of this species because they are isolated, are threatened by habitat loss, and may play an important role in driving poleward range expansion under increasing regional temperature. Our findings show that the annual larval growth rate does not vary directly with temperature, adult flight phenology and the timing of key resources respond to average daytime temperatures in spring, and population-density patterns among sites are robust over years across a broad region near the species' northern range limit. In addition, we provide descriptions of all larval instars for this species. This fundamental information about the biology, timing, and abundance of this species will facilitate further experimental study and improved assessment of its conservation status.

A fire-mediated recruitment bottleneck provides a possible explanation for the coexistence of trees and grasses in mesic savannas. The key element of this hypothesis is that saplings are particularly vulnerable to fire because they are small enough to be top-killed by grass fires, but unlike juveniles, they take several years to recover their original size. This limits the number of recruits into the adult size classes. Thus savanna vegetation may be maintained by a feedback whereby fire restricts the density of adult trees and allows a grass layer to develop, which provides fuel for subsequent fires. Here, we use results from a landscape-scale fire experiment in tropical Australia, to explore the possible existence of a recruitment bottleneck. This experiment compared tree recruitment and survival over 4 y under regimes of no fire, annual early and annual late dry-season fire. Stem mortality decreased with increasing stem height in the fire treatments but not in the unburnt treatment. Tree recruitment was 76–84% lower in the fire treatments than the unburnt treatment. Such fire-induced stem loss of saplings and reduced recruitment to the canopy layer in this eucalypt savanna are consistent with the predictions of the fire-mediated recruitment bottleneck hypothesis.
