A multiproxy record from Twin Ponds, VT, is used to reconstruct climatic variability during the late Pleistocene to early Holocene transition. Pollen, ostracodes, δ18O, and lithologic records from 13.5 to 9.0 cal ka BP are presented. Pollen- and ostracode-inferred climatic reconstructions are based on individual species’ environmental preferences and the modern analog technique. Principal components analysis of all proxies highlights the overall warming trend and centennial-scale climatic variability. During the Younger Dryas cooling event (YD), multiple proxies show evidence for cold winter conditions and increasing seasonality after 12.5 cal ka BP. The early Holocene shows an initial phase of rapid warming with a brief cold interval at 11.5 cal ka BP, followed by a more gradual warming; a cool, wet period from 11.2 to 10.8 cal ka BP; and cool, dry conditions from 10.8 to 10.2 cal ka BP. The record ends with steady warming and increasing moisture. Post-YD climatic variability has been observed at other sites in the northeastern United States and points to continued instability in the North Atlantic during the final phases of deglaciation.

Nuts are rich in many nutrients that can benefit multiple cardiometabolic functions, including arterial compliance, blood pressure, inflammation, glucoregulation and endothelial vasodilatation. Impaired vasodilatation may contribute to impaired cognitive performance due to poor cerebral perfusion. The present narrative review examines associations between nut consumption, vascular health and cognitive function. It includes a systematic search which identified seventy-one epidemiological or intervention studies in which effects of chronic nut consumption on blood pressure, glucoregulation, endothelial vasodilator function, arterial compliance, inflammatory biomarkers and cognitive performance were evaluated. Weighted mean changes were estimated where data were available; they indicate that nut consumption reduces blood pressure and improves glucoregulation, endothelial vasodilator function and inflammation, whilst a limited number of studies suggest that nut consumption may also improve cognitive performance. Further clinical trials are warranted to explore relationships between nut consumption, endothelial function and cognitive function.

Background
Treatment guidelines identify few adequately powered trials to guide recommendations for anorexia nervosa.
AimsTo evaluate the effectiveness of three readily available National Health Service treatments for adolescents (aged 12–18 years) with anorexia nervosa.
MethodMulticentre randomised controlled trial of 167 young people comparing in-patient, specialist out-patient and general child and adolescent mental health service (CAMHS) treatment.
ResultsEach group made considerable progress at 1 year, with further improvement by 2 years. Full recovery rates were poor (33% at 2 years, 27% still with anorexia nervosa). Adherence to in-patient treatment was only 50%. Neither in-patient nor specialist out-patient therapy demonstrated advantages over general CAMHS treatment by intention to treat, although some CAMHS out-patients were subsequently admitted on clinical grounds. In-patient treatment (randomised or after out-patient transfer) predicted poor outcomes.
ConclusionsFirst-line in-patient psychiatric treatment does not provide advantages over out-patient management. Out-patient treatment failures do very poorly on transfer to in-patient facilities.

This paper describes how new insight into community nursing know-how or professional artistry was produced through qualitative analysis of 30 transcripts of audio-recorded simulation data. The data were generated by simulated patient assessments of two cases. These simulated assessments were undertaken by a heterogeneous sample of district nurses drawn from a large conurbation in central Scotland in 1995. The existence of multiple sets of rich, dynamic and interactive data about each case provided a sound basis for exploring important and poorly understood aspects of nursing knowledge which are generally regarded as implicit. These aspects of nursing knowledge are particularly challenging for empirical research. The findings described are believed to be of particular interest in their articulation of collaborative approaches to patient assessment. Illustrations from the data are provided, and it is argued that the validity of the findings is enhanced by the unusual nature of the database, as well as the analytical procedures described. The limitations and significance of the findings are also addressed. The empirical analysis is linked to a preliminary discussion of procedural knowledge in nursing practice. The findings are discussed with reference to certain aspects of Foucault's view of power.
