Somatic multicomorbidity and disability in patients with psychiatric disorders in comparison to the general population: a quasi-epidemiological investigation in 54,826 subjects from 40 countries (COMET-G study)

Background
The prevalence of medical illnesses is high among patients with psychiatric disorders. The current study aimed to investigate multi-comorbidity in patients with psychiatric disorders in comparison to the general population. Secondary aims were to investigate factors associated with metabolic syndrome and treatment appropriateness of mental disorders.
MethodsThe sample included 54,826 subjects (64.73% females; 34.15% males; 1.11% nonbinary gender) from 40 countries (COMET-G study). The analysis was based on the registration of previous history that could serve as a fair approximation for the lifetime prevalence of various medical conditions.
ResultsAbout 24.5% reported a history of somatic and 26.14% of mental disorders. Mental disorders were by far the most prevalent group of medical conditions. Comorbidity of any somatic with any mental disorder was reported by 8.21%. One-third to almost two-thirds of somatic patients were also suffering from a mental disorder depending on the severity and multicomorbidity. Bipolar and psychotic patients and to a lesser extent depressives, manifested an earlier (15–20 years) manifestation of somatic multicomorbidity, severe disability, and probably earlier death. The overwhelming majority of patients with mental disorders were not receiving treatment or were being treated in a way that was not recommended. Antipsychotics and antidepressants were not related to the development of metabolic syndrome.
ConclusionsThe finding that one-third to almost two-thirds of somatic patients also suffered from a mental disorder strongly suggests that psychiatry is the field with the most trans-specialty and interdisciplinary value and application points to the importance of teaching psychiatry and mental health in medical schools and also to the need for more technocratically oriented training of psychiatric residents.
Cannabidiol modulates contextual fear memory consolidation in animals with experimentally induced type-1 diabetes mellitus

Objectives:
In view of the neuroprotective characteristic of cannabidiol (CBD) and its beneficial action on aversive memory in non-diabetic animals, we aimed to investigate in animals with experimentally induced type-1 diabetes mellitus (T1DM) whether CBD treatment would be able to impair the contextual fear memory consolidation, its generalisation and whether the effect would be lasting. We also investigated the CBD effect on anxiety-like responses.
Methods:After T1DM induction, animals received single or more prolonged treatment with CBD and were submitted to the contextual fear conditioning test. As expression of activity-regulated cytoskeletal-associated (Arc) protein is necessary for memory consolidation, we evaluated its expression in the dorsal hippocampus (DH). For evaluating anxiety-related responses, animals were submitted to the elevated plus maze test (EPMT), in which the time and number of entries in the open arms were used as anxiety index.
Results:A single injection of CBD impaired the contextual fear memory consolidation and its generalisation, which was evaluated by exposing the animal in a neutral context. This single injection was able to reduce the elevated expression of Arc in the DH from these animals. Interestingly, more prolonged treatment with CBD also impaired the persistence of context-conditioned fear memory and induced an anxiolytic-like effect, as the treated group spent more time in the open arms of the EPMT.
Conclusion:CBD interferes with contextual fear memory and the dosage regimen of treatment seems to be important. Moreover, we cannot rule out the involvement of emotional aspects in these processes related to fear memory.
Intake of ultra-processed foods is associated with inflammatory markers in Brazilian adolescents

Objective:
To evaluate the association of the consumption of foods of the ultra-processed group (UPF) with inflammatory markers in the adolescent population in Northeastern Brazil.
Design:A cross-sectional population-based study. Food consumption was evaluated using two 24-h dietary recalls using the NOVA classification for food processing levels. The following inflammatory markers were evaluated: adiponectin, IL-6, IL-8, C-reactive protein (CRP) and TNF-α. Multivariate linear regression was used to investigate the association between the percentage of UPF energy contribution and inflammatory markers.
Setting:São Luís, Maranhão, Brazil.
Participants:The sample consisted of 391 male and female adolescents, aged from 17 to 18 years.
Results:The average daily energy consumption by adolescents was 8032·9 kJ/d, of which 26·1 % originated from UPF. The upper tertile (T3) of UPF consumption presented higher intake of simple carbohydrates, lipids, saturated fat, and Na and lower protein intake. Individuals in T3 presented higher serum leptin and CRP levels (P < 0·05). Adolescents with UPF energy consumption ≥30·0 % (tertile 3 of UPF) had a 79 % (exp (0·58) = 1·79) increase in IL-8 levels when compared with adolescents in tertile 1 of UPF (P = 0·013).
Conclusions:The association between the consumption of UPF, poor quality diet and pro-inflammatory markers have important harmful effects that can be observed as early as in adolescence.

The amphipod Ericthonius didymus Krapp-Schickel, 2013 is reported for the first time outside its type locality in European waters, from the French Atlantic coast and from the Azores, Portugal. In particular, a large population was recorded in Arcachon, France. Molecular analyses were made to confirm the validity of three closely related Ericthonius species: E. didymus, E. convexus and E. pugnax. An identification key is provided, distinguishing Ericthonius species into three groups, based on the presence or absence of stridulating ridges on coxa 2 and posterodistal lobe on basis of male pereopod 5.
Modulation of the intestinal microbiota and the metabolites produced by the administration of ice cream and a dietary supplement containing the same probiotics

The aim of the present work was to compare the capacity to modulate the intestinal microbiota and the production of metabolites after 14 d administration of a commercial dietary supplement and a manufactured ice cream, both containing the same quantity of inulin and the same viable counts of Lactobacillus acidophilus LA-5 and Bifidobacterium animalis BB-12, using the Simulator of the Human Intestinal Microbial Ecosystem (SHIME®) model. Samples of the colonic contents were evaluated microbiologically by real-time quantitative PCR (qRT-PCR) and next-generation sequencing and chemically by the production of SCFA (acetate, propionate and butyrate) and ammonium ions ($\text{NH}_4^ + $). Statistical analyses were carried out for all the variables using the two-way ANOVA followed by the Tukey multiple comparisons test (P < 0·05) for metabolite production, qRT-PCR and the bioinformatics analysis for microbiota diversity. Dietary supplement and ice cream were able to deliver the probiotic L. acidophilus and B. animalis to the simulated colon and modulate the microbiota, increasing beneficial micro-organisms such as Bifidobacterium spp., Bacteroides spp. and Faecalibacterium spp. for dietary supplement administration, and Lactobacillus spp. for ice cream supplementation. However, the ice cream matrix was probably more favourable for the maintenance of the metabolic activity of the probiotics in the SHIME® model, due to the larger amounts of acetate, propionate, butyrate and ammonium ions obtained after 14 d of supplementation. In conclusion, both ways of probiotic supplementation could be efficient, each with its own particularities.

Neospora caninum is an apicomplexan parasite strongly related to reproductive problems in cattle. The neosporosis control is not well established and several fronts are under development, predominantly based on immune protection, immunomodulation and chemotherapy. The use of anti-malarial drugs as therapeutic sources has, in theory, considerable potential for any apicomplexan. Drugs such as methylene blue (MB) and pyrimethamine (Pyr) represent therapeutic options for malaria; thus, their use for neosporosis should be assessed. In this work, we tested the effects of MB and Pyr on N. caninum proliferation and clearance, using LacZ-tagged tachyzoites. The drugs inhibited at nanomolar dosages and its combination demonstrated an antagonistic interaction in proliferation assays, according to the Chou and Talalay method for drug combination index. However, the drug combination significantly improved the parasite in vitro clearance. The repositioning of well-established drugs opens a short-term strategy to obtain low-cost therapeutics approaches against neosporosis.

Aripiprazole for Patients with Schizophrenia and Schizoaffective Disorder: An Open-Label, Randomized, Study Versus Haloperidol

Introduction: Aripiprazole, a dopamine D2 receptor partial agonist, has also partial agonist activity at serotonin (5-HT)1A receptors and antagonist activity at 5-HT2A receptors.
Methods: In this 8-week, multicenter, randomized, parallel-group, open-label, flexible-dose study, patients diagnosed with schizophrenia or schizoaffective disorder were randomized to aripiprazole 15–30 mg/day or haloperidol 10–15 mg/day.
Results: Patients treated with both aripiprazole and haloperidol improved from baseline in Positive and Negative Syndrome Scale total, positive, and negative scores as well as in Clinical Global Impressions scores (all P<.001). At the end of the study, the percentage of patients classified as responders—according to ≥40% reduction in the Positive and Negative Syndrome Scale negative subscale score—was significantly higher in the aripiprazole group (20%) than in the haloperidol group (0%) (P<.05). Additionally, a higher number of patients receiving haloperidol required more anticholinergic medications (P<.001) than aripiprazole-treated patients, whereas more aripiprazole (45.5%) than haloperidol-treated patients (12.9%) required benzodiazepines (P=.002). At endpoint, rates of preference of medication were higher in the aripiprazole group (63.2%) than in the haloperidol group (21.7%), as expressed by patients and caregivers (P=.001).
Conclusion: Aripiprazole and haloperidol had similar efficacy in terms of reduction of overall psychopathology. Although aripiprazole has been demonstrated to be superior concerning negative symptoms and in terms of tolerability (extrapyramidal symptoms) and preferred by patients and caregivers than haloperidol, significantly more aripiprazole-treated patients required benzodiazepines.
Differential effects of reduced protein diets on fatty acid composition and gene expression in muscle and subcutaneous adipose tissue of Alentejana purebred and Large White × Landrace × Pietrain crossbred pigs

The present study assessed the effect of pig genotype (fatty v. lean) and dietary protein and lysine (Lys) levels (normal v. reduced) on intramuscular fat (IMF) content, subcutaneous adipose tissue (SAT) deposition, fatty acid composition and mRNA levels of genes controlling lipid metabolism. The experiment was conducted on sixty intact male pigs (thirty Alentejana purebred and thirty Large White × Landrace × Pietrain crossbred), from 60 to 93 kg of live weight. Animals were divided into three groups fed with the following diets: control diet equilibrated for Lys (17·5 % crude protein (CP) and 0·7 % Lys), reduced protein diet (RPD) equilibrated for Lys (13·2 % CP and 0·6 % Lys) and RPD not equilibrated for Lys (13·1 % CP and 0·4 % Lys). It was shown that the RPD increased fat deposition in the longissimus lumborum muscle in the lean but not in the fatty pig genotype. It is strongly suggested that the effect of RPD on the longissimus lumborum muscle of crossbred pigs is mediated via Lys restriction. The increase in IMF content under the RPD was accompanied by increased stearoyl-CoA desaturase (SCD) and PPARG mRNA levels. RPD did not alter backfat thickness, but increased the total fatty acid content in both lean and fatty pig genotype. The higher amount of SAT in fatty pigs, when compared with the lean ones, was associated with the higher expression levels of ACACA, CEBPA, FASN and SCD genes. Taken together, the data indicate that the mechanisms regulating fat deposition in pigs are genotype and tissue specific, and are associated with the expression regulation of the key lipogenic genes.
Environmental factors – spatial and temporal variation of chironomid communities in oceanic island streams (Azores archipelago)

Freshwater systems on volcanic oceanic islands have very particular characteristics as a result of their geological origins, relatively small size, distances from source areas for colonizers, and distinct catchment morphology. These factors result in freshwater communities that are clearly distinct from continental systems. Chironomid spatial and temporal distribution was investigated in chironomid assemblages at 21 sites across the Azores Archipelago. Results using PERMANOVA, a permutational multivariate analysis of variance, indicated significant longitudinal differences in assemblages but none between islands or over time.
Links between hydromorphological and physicochemical variables and the community assemblage were assessed using DISTLM, a linear model for distance-based multivariate analysis. The percentage of variation explained by hydromorphological factors (31%) was slightly higher than that explained by physicochemical (28%) factors. Descriptors of land use (forest area, scrub area, natural area of catchment) and stream slope were found to be the best environmental predictors of chironomid assemblages in Azores. Physicochemical variables such as temperature, pH, nitrite, iron and conductivity were the principal drivers of change in chironomid composition in stream locations. Headwater sites, characterized by lower temperature, acid to neutral pH values, low conductivity, nutrient and metal concentrations were dominated by Rheocricotopus atripes. Mid-section sites, located in agricultural areas were dominated by Thienemanniella clavicornis. Urbanized lower reaches were characterized by higher temperature, pH, conductivity and nutrient levels and were dominated by Cricotopus sp. These results give essential information that allows us to predict the response of different chironomid species to hydromorphological and physicochemical gradients across the archipelago's streams contributing providing the basis for the development of tools for the implementation of the Water Frame Directive.

Summary
In this chapter we present a natural open system approach to organizational control, as opposed to dominant approaches, which can be typified as either rational system approaches, closed system approaches (Scott,1987), or both. Our aim is to promote scholarly understanding of organizational control by drawing on a wider range of possible insights than dominant approaches do.
Two scholarly traditions, both with rather strong foci on certain aspects of control, have been prominent in the field. The first tradition, represented by agency theory and rational choice models of human behavior (Eisenhardt, 1989), focuses on rational choice of control mechanisms by managers, with the underlying assumption that control choices are driven by effectiveness and efficiency concerns. This focus typifies a rational system approach. The second tradition, represented by bureaucracy studies, draws on agency theory and the critical management tradition (for an overview of this tradition see Delbridge, Chapter 4) to focus on reactions to managerial control by those subjected to it. The studies in this tradition have mainly highlighted the constraining, harnessing, distrust-signaling perceptions of control. Both traditions can be typified as closed system approaches, since factors outside the organization, such as organizational environments, are not included in the analysis of employee reactions.
We propose to move from these rather narrow rational and closed system approaches to a more encompassing understanding of factors involved in shaping managerial choices and reactions of employees.

Objective.
To investigate an outbreak of healthcare-associated Burkholderia cepacia complex (BCC) primary bloodstream infections (BCC-BSI).
Design and Setting.Case-crossover study in a public hospital, a university hospital and a private hospital in Rio de Janeiro, Brazil, from March 2006 to May 2006.
Patients.Twenty-five patients with BCC-BSI.
Design.After determining the date BCC-BSI symptoms started for each patient, 3 time intervals of data collection were defined, each one with a duration of 3 days: the case period, starting just before BCC-BSI symptoms onset; the control period, starting 6 days before BCC-BSI symptoms onset; and the washout period, comprising the 3 days between the case period and the control period. Exposures evaluated were intravascular solutions and invasive devices and procedures. Potential risk factors were identified by using the McNemar χ2 adjusted test. Cultures of samples of potentially contaminated solutions were performed. BCC strain typing was performed by pulsed-field gel electrophoresis using Spel.
Results.The statistical analysis revealed that the use of bromopride and dipyrone was associated with BCC-BSI. A total of 21 clinical isolates from 17 (68%) of the 25 patients and an isolate obtained from the bromopride vial were available for strain typing. Six pulsotypes were detected. A predominant pulsotype (A) accounted for 11 isolates obtained from 11 patients (65%) in the 3 study hospitals.
Conclusion.Our investigation, using a case-crossover design, of an outbreak of BCC-BSI infections concluded it was polyclonal but likely caused by infusion of contaminated bromopride. The epidemiological finding was validated by microbiological analysis. After recall of contaminated bromopride vials by the manufacturer, the outbreak was controlled.
Exposure to zinc deficiency in fetal and postnatal life determines nitric oxide system activity and arterial blood pressure levels in adult rats

We had previously shown that prenatal exposure to Zn-deficient diets induces an increase in blood pressure and impairs renal function in adult rats. The aim of the present study was to investigate if moderate Zn restriction during early growth periods, fetal life and lactation would induce impairment in the vascular and renal NO system and alterations in plasma lipid profile. We also investigated if these effects persisted into adult life, even when a Zn-replete diet was provided after weaning. Pregnant rats were fed control (30 parts per million (ppm)) or low (8 ppm) Zn diets throughout gestation up to weaning. Afterwards, male offspring from low-Zn mothers were assigned to low- or control-Zn diets during 60 d. Male offspring from control mothers were fed a control diet. Animals exposed to Zn restriction showed low birth weight, increased systolic blood pressure and serum TAG levels, and decreased glomerular filtration rate in adulthood. Zn restriction induced a decrease in vascular and renal NO synthase activity and a reduced expression of the endothelial NO synthase isoform in aorta. A control-Zn diet during post-weaning growth returned TAG levels to normal but was unsuccessful in normalising systolic blood pressure, glomerular filtration rate or NO system activity in Zn-deficient offspring. Zn restriction during fetal life, lactation and/or post-weaning growth induced alterations in the vascular and renal NO system and in lipid metabolism that could contribute to the programming of hypertension and renal dysfunction in adulthood.

Oceanic islands freshwater systems are unique due to their volcanic origin, oceanic situation, catchment morphology and the presence of distinct freshwater communities when compared with continental systems. This study provides an update of Azorean chironomid fauna records, distribution data and includes biogeographical comments, based on collections covering a range of freshwater habitats on several islands over 2005 to 2008. Island species area-relationships or ISAR was performed as a descriptive model for species accumulation patterns in each island in relation to their area. Six new chironimid species records are given for the Azores archipelago: Orthocladiinae:Orthocladius (Eudactylocladius) fuscimanus (Kieffer, 1908), Pseudorthocladius (Pseudorthocladius) curtistylus (Goetghebuer, 1921), Synorthocladius semivirens (Kieffer, 1909), Parachaetocladius abnobaeus (Wülker, 1959); Chironominae:Chironomus (Chironomus) annularis (Meigen, 1818), Parachironomus tenuicaudatus (Malloch, 1915). As a result, the Azorean chironomid fauna has increased from 38 to 44 species, spread across 33 genera. Most recorded chironomids are primarily of western Palaearctic origin; Nearctic, Oriental and Neotropical regions are comparatively poorly represented. A checklist is provided, incorporating and updating previous records. Chirononidae distribution pattern among the islands is analyzed and island species area-relationship (ISAR) analyses, performed to provide a descriptive model for species accumulation patterns in each island, indicated a positive correlation between species richness (number of species) and the area of the Azorean islands, supporting MacArthur and Wilson’s equilibrium theory of island biogeography. Chironomidae could be promising for searching candidate bioindicators to answer the demand of Water Frame Directive.

Arraiolos tapestries are probably one of the richest artistic Portuguese expressions in terms of textile heritage. It is difficult to date the beginning of the production of rugs in Arraiolos (Southern Portugal), but they were already produced in the late 1600's as they are listed in the inventories of Portuguese aristocratic households in the beginning of the 18th century. Sensitive detection techniques play an increasing role in the chemical investigation of historical objects since the knowledge derived from the chemical composition of materials is of upmost importance for textile conservation and restoration purposes. Textiles deteriorate due to natural causes like heat, radiation, mechanical stress, moisture, microbiological and enzymatic attack. Deterioration of materials causes breakdown of the molecular structure and results in a loss of strength, extensibility and durability, discoloration and fading which affects the appearance of the textiles.
Healthcare-Associated Infections Among Neonates in Brazil

Objective:
To describe the epidemiology of healthcare-associated infections (HAIs) among neonates.
Design:Prospective surveillance of HAIs was conducted during 2 years. Infections beginning within 48 hours of birth were defined as HAIs of maternal origin. Death occurring during an active episode of HAI was considered related to HAI.
Setting:Seven neonatal units located in three Brazilian cities.
Patients:All admitted neonates were included and observed until discharge.
Results:Twenty-two percent of 4,878 neonates had at least one HAI. The overall incidence density was 24.9 per 1,000 patient-days, and 28.1% of all HAIs were maternally acquired. HAI rates ranged from 12.3% in the group with a birth weight (BW) of more than 2,500 g to 51.9% in the group with a BW of 1,000 g or less. The main HAIs were bloodstream infection (BSI) and pneumonia. Coagulase-negative staphylococci, Enterobacter species, Staphylococcus aureus, and Klebsiella pneumoniae were the main pathogens. Forty percent of all deaths were related to HAI. Central venous catheter (CVC)-associated BSIs per 1,000 CVC-days ranged from 17.3 (BW, 1,501 to 2,500 g; device utilization [DU], 0.11) to 34.9 (BW, ≤ 1,000 g; DU, 34.92). Ventilator-associated pneumonia per 1,000 ventilator-days ranged from 7.0 (BW, ≤ 1,000 g; DU, 0.34) to 9.2 (BW, 1,001 to 1,500 g; DU, 0.14).
Conclusions:The high proportion of HAIs of maternal origin highlights perinatal care issues in Brazil and the need to improve the diagnosis of neonatal HAIs. The very low BW group and device-associated infections should be priorities for prevention strategies in this population.
