While there is increasing recognition among archaeologists of the extent to which non-agricultural societies have managed their terrestrial ecosystems, the traditional management of marine ecosystems has largely been ignored. In this paper, we bring together Indigenous ecological knowledge, coastal geomorphological observations, and archaeological data to document how Northwest Coast First Nations cultivated clams to maintain and increase productivity. We focus on “clam gardens,” walled intertidal terraces constructed to increase bivalve habitat and productivity. Our survey and excavations of clam gardens in four locations in British Columbia provide insights into the ecological and social context, morphology, construction, and first reported ages of these features. These data demonstrate the extent of traditional maricultural systems among coastal First Nations and, coupled with previously collected information on terrestrial management, challenge us to broaden our definition of “forager” as applied to Northwest Coast peoples. This study also highlights the value of combining diverse kinds of knowledge, including archaeological data, to understand the social and ecological contexts of traditional management systems.

The rising rate of obesity has been blamed on increased consumption of sugar-sweetened soft drinks, such as carbonated sodas, which fail to satisfy hunger. The objective of the present study was to compare the effect on appetite and energy intake of a sugar-sweetened beverage (cola) and a chocolate milk drink, matched for energy content and volume. It was hypothesised that chocolate milk may be more satiating because of its protein content. Twenty-two healthy young men (age 23 (sd 1·8) years) of normal weight (BMI 22·2 (sd 1·5) kg/m2) were recruited to the randomised cross-over study. Visual analogue scales were used to record subjective appetite ratings every 30 min on each of two test days. A drink of 500 ml cola or chocolate milk (900 kJ) was ingested 30 min before an ad libitum lunch. Satiety and fullness were significantly greater (P = 0·0007, P = 0·0004, respectively) 30 min after chocolate milk than after cola. Ratings of prospective consumption and hunger were significantly greater after cola than after chocolate milk, both immediately after preload intake (P = 0·008, P = 0·01, respectively) and 30 min afterwards (P = 0·004, P = 0·01, respectively). There was no significant difference (P = 0·42) in ad libitum lunch intake after ingestion of chocolate milk (3145 (sd 1268) kJ) compared with cola (3286 (sd 1346) kJ). The results support the hypothesis that sweetened soft drinks are different from milk products in their impact on short-term hunger and satiety, although differences in subjective appetite scores were not translated into differences in energy intake.

Summary
Although disorders of the coagulation system are uncommon in pregnancy, the two main hazards, haemorrhage and thromboembolism, are leading causes of direct maternal death in the UK and worldwide. The fetus may be seriously affected by some bleeding disorders and by the consequences of major maternal haemorrhage or thromboembolism. Gynaecologists may encounter abnormal haemostasis as a cause of intractable menorrhagia or perioperative haemorrhage, and thromboembolism is a significant risk for women undergoing major gynaecological surgery. As more is discovered about the coagulation system, it is becoming apparent that some women are at increased risk of haemorrhage or thrombosis. This book, which brings together various aspects of haemostasis in relation to obstetrics and gynaecology, will be useful to candidates preparing for the MRCOG examination and may provide some practical guidance for doctors who only occasionally encounter these conditions. Good working relationships and close liaison between obstetricians and gynaecologists, anaesthetists, haematologists, and the blood transfusion service are essential to ensure the best outcome for women with disturbances of haemostasis or thrombosis. This book brings together various aspects of haemostasis in relation to obstetrics and gynaecology to provide practical guidance to clinicians, who may only occasionally encounter these problems. Emphasis is placed on the need for good working relationships and close liaison between obstetricians and gynaecologists, anaesthetists, haematologists and blood transfusion services. The book is primarily designed to provide a comprehensive summary for candidates preparing for the Part 2 MRCOG examination, and as such covers the RCOG curriculum for haemorrhage and thrombosis. It is also a valuable guide for all healthcare professionals working in the field.

Summary
Massive obstetric haemorrhage is usually due to placenta praevia, premature placental separation or postpartum causes and is often associated with coagulopathy. Most maternal deaths associated with ectopic pregnancy, which is the most common cause of death in early pregnancy, are due to haemorrhage. Many women who have died from ectopic pregnancy presented with symptoms of urinary or gastrointestinal conditions. The main causes of antepartum haemorrhage are placenta praevia and placental abruption. A high incidence of false positive diagnosis of placenta praevia is associated with second-trimester transabdominal ultrasound scans. The most common cause of primary postpartum haemorrhage is uterine atony. The signs and symptoms of secondary postpartum haemorrhage include a rising pulse rate, falling blood pressure, pallor, sweating, restlessness and oliguria. Haemorrhage can occur suddenly and unexpectedly in any woman. However, some women are at increased risk of bleeding.

Composite Ag-SiO2 thin films were deposited to examine the stability of materials with high resistivity above 5000 μΩ-cm. We found that the resistivity increases exponentially with SiO2 volume fractions larger than 0.50 which is consistent with a tunneling conductivity mechanism. In order to obtain a broad composition range, these films were deposited on a stationary substrate placed above Ag and SiO2 sputtering sources. This configuration allowed compositions ranging from 0.8 to 56.3% SiO2 to be deposited on the same sample. Resistance measurements were made using a four-point probe and a profilometer was used to measure thickness. Predicted values of thickness and composition were obtained by calibrating deposition rates from the separate sources, and were verified using Rutherford Backscattering Spectroscopy. Electron microscopy analysis revealed Ag agglomerating on the surface of the film. Because of the high mobility of Ag, the films should be capped to prevent Ag agglomeration. The results of this research demonstrate that high-resistivity thin films can be grown using Ag-SiO2 composites.

Each of the 13 essays in this volume considers an aspect of female participation in the paid or unpaid labor force of the antebellum American south. The work of ordinary women is the theme that unites the essays, work the editors in their introduction note was often unacknowledged due to prevailing and evolving attitudes about women's proper work and the role of the male head of household as the family breadwinner. The essays vary widely in their scope, but share a search for ingenious sources of information, a search necessitated by the invisibility of women in official and more conventional sources. The topics range from Native American female makers and sellers of baskets to antebellum female iron manufacturing workers, from coastal Savannah slave women participants in produce markets to western Virginia businesswomen, from Richmond prostitutes to New Orleans nuns.

Economic historians have to respond favorably when sociologist–feminist scholar Christine Bose early in her text writes, “This book is intended to provide a historical perspective on contemporary issues that all too often are analyzed only in terms of the present” (p. 3). She returns frequently to this theme, stressing that female participation in the labor force began long before the late 1960s. Of course, numerous economic historians have noted that such participation began long before 1900 and their work, unsurprisingly, exhibits stronger understanding of historical economic conditions than does Women in 1900. Bose's intent, however, is not to study women throughout U.S. history; rather, she analyzes data on 29,673 women included in the Public Use Sample of the 1900 census to re-estimate female labor-force participation, and determine the effect of gender, race, ethnicity, and class on that participation. Her most valuable contribution comes through matching her sample observations with county economic data obtained through the Inter-University Consortium for Political and Social Research (ICPSR) at the University of Michigan. She uses these data to generate what she calls contextual variables, basically regional and urban or rural location of the sample respondent, and average female manufacturing wage and population characteristics of the respondent's county.

Motivational interviewing is a client centred behavioural therapy for addictive behaviours. It is an intervention designed to help all addicts, not just those ready to change. It is therefore suitable for use as an opportunistic intervention for clients whose main reason for contact may not be their addiction. A pilot randomized controlled trial of home-based motivational interviewing by a specially trained midwife to help pregnant smokers reduce their habit was performed in Glasgow from February 1997 to January 1998. Did motivational interviewing take place? All 171 counselling interviews from 48 intervention clients were audio-taped. Forty-nine interviews from 13 randomly selected clients were transcribed for content analysis. A rating scale established for feedback to trainee psychologists was used by three experienced analysts. Thirty-two interviews were scored independently to validate the rating scale in this setting. More than 75% of interviews showed satisfactory motivational interviewing. Therapist utterances were motivational, and client responses included many self-motivational statements. Few episodes of client resistance were recorded. Rating took 160 mins per half hour interview. This instrument provided a valid measure of intervention quality for a randomized controlled trial. It would not be practical to document process outside a research environment.
