A theory-based behavioural change communication intervention to decrease the provision of water to infants under 6 months of age in the Republic of Guinea

Objective:
In many countries, the provision of water in the early months of a baby’s life jeopardises exclusive breast-feeding (EBF). Using a behavioural theory, this study assessed the impact of a behaviour change intervention on mothers’ intention to act and, in turn, on the water provision in addition to breast milk to their infants under 6 months of age (IU6M) in two regions of Guinea.
Design:A quasi-experimental design. Data on individual and environmental factors of the theoretical framework, sociodemographic and outcomes were collected using validated questionnaires before and after the intervention. The outcomes examined were the intention to provide water to IU6M, the provision of water and EBF. Path analyses were performed to investigate pathways by which psychosocial and environmental factors influenced the water provision in addition to breast milk.
Setting:Four health centres were assigned randomly to each study’s arm (one control/CG and one intervention group/IG per region).
Participants:The sample included 300 mothers of IU6M: 150 per group.
Results:In IG, the proportion of mothers providing water decreased from 61 % to 29 % before and after the intervention (P < 0·001), while no difference was observed in CG (P = 0·097). The EBF rate increased in IG (from 24·0 % to 53·8 %, P < 0·001) as opposed to CG (36·7 % to 45·9 %, P = 0·107). An association (P < 0·001) between the intention and the behaviour was observed in both groups.
Conclusions:An intervention developed using a sound framework reduces the provision of water among IU6M and improves EBF.

Objective
To investigate associations between multimodal analgesia and post-operative pain among patients undergoing transoral robotic surgery for oropharyngeal squamous cell carcinoma.
MethodsRecords of patients who underwent surgery from 5 September 2012 to 30 November 2016 were abstracted. Associations were assessed using multivariable analysis.
ResultsA total of 216 patients (mean age of 59.1 years, 89.4 per cent male) underwent transoral robotic surgery (92.6 per cent were human papilloma virus positive, 87.5 per cent had stage T1–T2 tumours, and 82.9 per cent had stage N0–N1 nodes). Gabapentin (n = 86) was not associated with a reduction in severe pain. Ibuprofen (n = 72) was administered less often in patients with severe pain. Gabapentin was not associated with increased post-operative sedation (p = 0.624) and ibuprofen was not associated with increased bleeding (p = 0.221). Post-operative opioid usage was not associated with surgical duration, pharyngotomy, bilateral neck dissections, tumour stage, tumour size, subsite or gabapentin.
ConclusionScheduled low-dose gabapentin was not associated with improved pain control or increased respiratory depression. Ibuprofen was not associated with an increased risk of bleeding and may be under-utilised.

Mislabeling of an Invasive Vine (Celastrus orbiculatus) as a Native Congener (C. scandens) in Horticulture

The horticultural industry is an important source of invasive ornamental plant species, which is part of the motivation for an increased emphasis on using native alternatives. We were interested in the possibility that plants marketed in the midwestern United States as the native Celastrus scandens, or American bittersweet, were actually the difficult-to-distinguish invasive Celastrus orbiculatus (oriental bittersweet) or hybrids of the two species. We used nuclear microsatellite DNA loci to compare the genetic identities of 34 plants from 11 vendors with reference plants from wild populations of known species identity. We found that 18 samples (53%) were mislabeled, and 7 of the 11 vendors sold mislabeled plants. Mislabeled plants were more likely to be purchased through Internet or phone order shipments and were significantly less expensive than accurately labeled plants. Vendors marketed mislabeled plants under five different cultivar names, as well as unnamed strains. Additionally, the most common native cultivar, ‘Autumn Revolution,’ displays reproductive characteristics that diverge from the typical C. scandens, which could be of some concern. The lower price and abundance of mislabeled invasive plants introduces incentives for consumers to unknowingly contribute to the spread of C. orbiculatus. Revealing the potential sources of C. orbiculatus is critical for controlling further spread of the invasive vine and limiting its impact on C. scandens populations.

Introduction
An unprecedented outbreak of Ebola virus diseases (EVD) occurred in West Africa from March 2014 to January 2016. The French Institute for Public Health implemented strengthened surveillance to early identify any imported case and avoid secondary cases.
MethodsFebrile travellers returning from an affected country had to report to the national emergency healthcare hotline. Patients reporting at-risk exposures and fever during the 21st following day from the last at-risk exposure were defined as possible cases, hospitalised in isolation and tested by real-time polymerase chain reaction. Asymptomatic travellers reporting at-risk exposures were considered as contact and included in a follow-up protocol until the 21st day after the last at-risk exposure.
ResultsFrom March 2014 to January 2016, 1087 patients were notified: 1053 were immediately excluded because they did not match the notification criteria or did not have at-risk exposures; 34 possible cases were tested and excluded following a reliable negative result. Two confirmed cases diagnosed in West Africa were evacuated to France under stringent isolation conditions. Patients returning from Guinea (n = 531; 49%) and Mali (n = 113; 10%) accounted for the highest number of notifications.
ConclusionNo imported case of EVD was detected in France. We are confident that our surveillance system was able to classify patients properly during the outbreak period.
Competitive Interactions of Garlic Mustard (Alliaria petiolata) and Damesrocket (Hesperis matronalis)

Competitive interactions between native plants and nonnative, invasive plant species have been extensively studied; however, within degraded landscapes, the effect of interspecific interactions among invasive plants is less explored. We investigated a competitive interaction between two sympatric, invasive mustard species that have similar life history strategies and growth forms: garlic mustard and damesrocket. Greenhouse experiments using a full range of reciprocal density ratios were conducted to investigate interspecific competition. Garlic mustard had a negative effect on the final biomass, number of leaves, and relative growth rate in height of damesrocket. Survival of damesrocket was not negatively affected by interspecific competition with garlic mustard; however, garlic mustard showed higher mortality because of intraspecific competition. These results indicated that although garlic mustard has been observed to be the dominant species in this landscape, it may not completely outcompete damesrocket in all situations. Studies of invasive species in competition are important in degraded landscapes because this is the common situation in many natural areas.

A 14 year old boy with scapuloperoneal muscular atrophy, pes cavus, areflexia and distal sensory loss (Davidenkow syndrome) is described. Nerve conduction velocities were diminished. Sural nerve biopsy demonstrated a reduction in the number of myelinated fibers and early “onion-bulb” formation. These observations support the hypothesis that the scapuloperoneal amyotrophy associated with distal sensory loss may represent a variant of type I hereditary motor sensory neuropathy.

Although scholarly investigation of the Book of Mormon has increased significantly over the last three decades, only a tiny portion of that effort has been focused on the theological or doctrinal content of this central volume of Latter-day Saints (LDS) scripture. This article identifies three inclusios which promise definitions of the doctrine or gospel of Jesus Christ and proposes a cumulative methodology to explain how these definitions work. This approach reveals a consistently presented, six-part formula defining ‘the way’ by which mankind can qualify for eternal life. In this way the article provides a starting point for scholarly examinations of the theological content of this increasingly influential religious text.
While the names of the six elements featured in Mormon's gospel will sound familiar to students of the New Testament, the meanings he assigns to these may differ substantially from traditional Christian discourse in ways which make Mormon's characterisation of the gospel or doctrine of Christ unique. (1) Faith is understood primarily as action displaying complete trust or reliance on Christ and the power of his atonement. (2) Repentance requires turning away from one's own way and humbly submitting – by covenant – to the way of the Lord. (3) Water baptism is then the prescribed sign of that covenant a repentant person gives in witnessing both to God and to the world that she has repented and undertaken to follow Christ in all things. (4) The baptism of fire and of the Holy Ghost brings the remission of sins in a spiritual rebirth to the repentant individual at such time as God judges her repentance to be true. It also provides converts with a direct witness of the Father and of the Son and of the promises of salvation for those who follow this gospel – as they may be led by the continuing guidance of the Holy Ghost. (5) But only those who endure to the end in this way will (6) receive salvation in the kingdom of God.
The overall pattern suggested is a dialogue between man and God, who initially invites all people to trust in Christ and repent. Those who respond by repenting and seeking baptism will be visited by fire and by the Holy Ghost, which initiates a lifelong interaction, leading the convert day by day in preparation for the judgement, at which she may finally be invited to enter the kingdom of God.

A new ternary phase, Er2Cu10.9Ga6.1, has been found in the Er-Cu-Ga phase diagram. X-ray powder diffraction data were successfully refined based on a rhombohedral Th2Zn17 structure type (space group R-3m, No. 166) with cell parameters a=8.6627(1) Å, c=12.6824(2) Å, and Z=3. Er2Cu10.9Ga6.1 exhibits a paramagnetic Curie-Weiss behaviour down to 2 K with a trivalent character of Er atoms.

Under more general assumptions than those usually made in the sequential analysis literature, a variable-sample-size-sequential probability ratio test (VPRT) of two simple hypotheses is found that maximizes the expected net gain over all sequential decision procedures. In contrast, Wald and Wolfowitz [25] developed the sequential probability ratio test (SPRT) to minimize expected sample size, but their assumptions on the parameters of the decision problem were restrictive. In this article we show that the expected net-gain-maximizing VPRT also minimizes the expected (with respect to both data and prior) total sampling cost and that, under slightly more general conditions than those imposed by Wald and Wolfowitz, it reduces to the one-observation-at-a-time sequential probability ratio test (SPRT). The ways in which the size and power of the VPRT depend upon the parameters of the decision problem are also examined.

Summary
INTRODUCTION
Although the vast majority of cardiac diseases relate to the presence of atherosclerosis in the vessels supplying blood and oxygen to the heart, there are certain conditions in which immunological events play an important role. This chapter will concentrate on the immunological aspects of these diseases. We will also discuss the animal models associated with each of these disorders with particular emphasis on whether these models help us to understand further the immunological aspects of these conditions.
RHEUMATIC FEVER
Acute rheumatic fever (ARF) is a delayed, nonsuppurative sequela of a pharyngeal infection with the group A streptococcus. A latent period of two to three weeks follows the initial streptococcal pharyngitis. The latent period remains the same for each individual patient in the event of a recurrence. This suggests that the patient has already been exposed to more than one streptococcal infection in the past. The onset of disease is usually characterized by an acute febrile illness, which may manifest itself in one of three classical ways: (1) The patient may present with migratory arthritis predominantly involving the large joints of the body. (2) There may also be concomitant clinical and laboratory signs of carditis and valvulitis, or carditis and valvulitis may be the only signs of an acute episode. (3) There may be involvement of the central nervous system, manifesting itself as Sydenham's chorea. The clinical episodes are self-limiting but damage to the valves may be chronic and progressive, resulting in cardiac decompensation and death.

The star formation history of the Magellanic Clouds, including the old and intermediate-age star formation events, can be studied reliably and in detail through color-magnitude diagrams reaching the oldest main sequence turnoffs. This paper reviews our current understanding of the Magellanic Clouds' star formation histories and discusses the impact of this information on general studies of galaxy formation and evolution.
