Comparison of antibiotic prescribing between physicians and advanced practice clinicians

We compared antibiotic prescribing rates for respiratory conditions in a national sample of outpatient visits from 2010 to 2018 between physicians and advanced practice clinicians (APCs). APCs prescribed antibiotics more frequently than physicians (58% vs 52%), but there were no differences in selection of guideline recommended first-line agents between specialties.

Summary
The challenges of post-atrocity recovery are massive and manifold, but perhaps the most fundamental is the capacity for previously divided groups to return to an emotional place of sustainable, harmonious coexistence. This requires efforts to break down the stereotypes and barriers of “tribalism,” including conflicting characterizations of victims and perpetrators. Yet even the best intentioned of such efforts often prove inadequate in the face of three common obstacles: (1) lack of clarity about the objectives of reconciliation, (2) application of a cookie-cutter reconciliation plan, and (3) an ineffective process used to design the plan. Thus, in this chapter we introduce four crucial objectives for promoting integrative dynamics, describe a system for operationalizing those objectives to fit the particular post-atrocity context, and illuminate key negotiation principles to ensure that the reconciliation design process itself moves toward a productive outcome.

Time constraints have been suggested as a potential driver of antibiotic overuse for acute respiratory tract infections. In this cross-sectional analysis of national data from visits to offices and emergency departments, we identified no statistically significant association between antibiotic prescribing and the duration of visits for acute respiratory tract infections.

Models of products and design processes are key to interacting with engineering designs and managing the processes by which they are developed. In practice, companies maintain networks of many interrelated models which need to be synthesised in the minds of their users when considering issues that cut across them. This article considers how information from product and design process models can be integrated with a view to help manage these complex interrelationships. A framework highlighting key issues surrounding model integration is introduced and terminology for describing these issues is developed. To illustrate the framework and terminology, selected modelling approaches that integrate product and process information are discussed and organised according to their levels and forms of integration. Opportunities for further work to advance integrated modelling in engineering design research and practice are discussed.

Psychotic disorders continue to be among the most disabling and scientifically challenging of all mental illnesses. Accumulating research findings suggest that the etiologic processes underlying the development of these disorders are more complex than had previously been assumed. At the same time, this complexity has revealed a wider range of potential options for preventive intervention, both psychosocial and biological. In part, these opportunities result from our increased understanding of the dynamic and multifaceted nature of the neurodevelopmental mechanisms involved in the disease process, as well as the evidence that many of these entail processes that are malleable. In this article, we review the burgeoning research literature on the prodrome to psychosis, based on studies of individuals who meet clinical high risk criteria. This literature has examined a range of factors, including cognitive, genetic, psychosocial, and neurobiological. We then turn to a discussion of some contemporary models of the etiology of psychosis that emphasize the prodromal period. These models encompass the origins of vulnerability in fetal development, as well as postnatal stress, the immune response, and neuromaturational processes in adolescent brain development that appear to go awry during the prodrome to psychosis. Then, informed by these neurodevelopmental models of etiology, we turn to the application of new research paradigms that will address critical issues in future investigations. It is expected that these studies will play a major role in setting the stage for clinical trials aimed at preventive intervention.

In this paper we consider the family of rational maps of the complex plane given by \[z^2+\frac{\lambda}{z^2}\] where $\lambda$ is a complex parameter. We regard this family as a singular perturbation of the simple function $z^2$. We show that, in any neighborhood of the origin in the parameter plane, there are infinitely many open sets of parameters for which the Julia sets of the corresponding maps are Sierpinski curves. Hence all of these Julia sets are homeomorphic. However, we also show that parameters corresponding to different open sets have dynamics that are not conjugate.
