We describe system verification tests and early science results from the pulsar processor (PTUSE) developed for the newly commissioned 64-dish SARAO MeerKAT radio telescope in South Africa. MeerKAT is a high-gain ( ${\sim}2.8\,\mbox{K Jy}^{-1}$ ) low-system temperature ( ${\sim}18\,\mbox{K at }20\,\mbox{cm}$ ) radio array that currently operates at 580–1 670 MHz and can produce tied-array beams suitable for pulsar observations. This paper presents results from the MeerTime Large Survey Project and commissioning tests with PTUSE. Highlights include observations of the double pulsar $\mbox{J}0737{-}3039\mbox{A}$ , pulse profiles from 34 millisecond pulsars (MSPs) from a single 2.5-h observation of the Globular cluster Terzan 5, the rotation measure of Ter5O, a 420-sigma giant pulse from the Large Magellanic Cloud pulsar PSR $\mbox{J}0540{-}6919$ , and nulling identified in the slow pulsar PSR J0633–2015. One of the key design specifications for MeerKAT was absolute timing errors of less than 5 ns using their novel precise time system. Our timing of two bright MSPs confirm that MeerKAT delivers exceptional timing. PSR $\mbox{J}2241{-}5236$ exhibits a jitter limit of $<4\,\mbox{ns h}^{-1}$ whilst timing of PSR $\mbox{J}1909{-}3744$ over almost 11 months yields an rms residual of 66 ns with only 4 min integrations. Our results confirm that the MeerKAT is an exceptional pulsar telescope. The array can be split into four separate sub-arrays to time over 1 000 pulsars per day and the future deployment of S-band (1 750–3 500 MHz) receivers will further enhance its capabilities.

Introduction: A critical component for successful implementation of any innovation is an organization's readiness for change. Competence by Design (CBD) is the Royal College's major change initiative to reform the training of medical specialists in Canada. The purpose of this study was to measure readiness to implement CBD among the 2019 launch disciplines. Methods: An online survey was distributed to program directors of the 2019 CBD launch disciplines one month prior to implementation. Questions were developed based on the R = MC2 framework for organizational readiness. They addressed program motivation to implement CBD, general capacity for change, and innovation-specific capacity. Questions related to motivation and general capacity were scored using a 5-point scale of agreement. Innovation-specific capacity was measured by asking participants whether they had completed 33 key pre-implementation tasks (yes/no) in preparation for CBD. Bivariate correlations were conducted to examine the relationship between motivation, general capacity and innovation specific capacity. Results: Survey response rate was 42% (n = 79). A positive correlation was found between all three domains of readiness (motivation and general capacity, r = 0.73, p < 0.01; motivation and innovation specific capacity, r = 0.52, p < 0.01; general capacity and innovation specific capacity, r = 0.47, p < 0.01). Most respondents agreed that successful launch of CBD was a priority (74%). Fewer felt that CBD was a move in the right direction (58%) and that implementation was a manageable change (53%). While most programs indicated that their leadership (94%) and faculty and residents (87%) were supportive of change, 42% did not have experience implementing large-scale innovation and 43% indicated concerns about adequate support staff. Programs had completed an average of 72% of pre-implementation tasks. No difference was found between disciplines (p = 0.11). Activities related to curriculum mapping, competence committees and programmatic assessment had been completed by >90% of programs, while <50% of programs had engaged off-service rotations. Conclusion: Measuring readiness for change aids in the identification of factors that promote or inhibit successful implementation. These results highlight several areas where programs struggle in preparation for CBD launch. Emergency medicine training programs can use this data to target additional implementation support and ongoing faculty development initiatives.

Background Attention-deficit/hyperactivity disorder (ADHD) is among the most common psychiatric disorders of childhood that often persists into adulthood and old age. Yet ADHD is currently underdiagnosed and undertreated in many European countries, leading to chronicity of symptoms and impairment, due to lack of, or ineffective treatment, and higher costs of illness.
Methods The European Network Adult ADHD and the Section for Neurodevelopmental Disorders Across the Lifespan (NDAL) of the European Psychiatric Association (EPA), aim to increase awareness and knowledge of adult ADHD in and outside Europe. This Updated European Consensus Statement aims to support clinicians with research evidence and clinical experience from 63 experts of European and other countries in which ADHD in adults is recognized and treated.
Results Besides reviewing the latest research on prevalence, persistence, genetics and neurobiology of ADHD, three major questions are addressed: (1) What is the clinical picture of ADHD in adults? (2) How should ADHD be properly diagnosed in adults? (3) How should adult ADHDbe effectively treated?
Conclusions ADHD often presents as a lifelong impairing condition. The stigma surrounding ADHD, mainly due to lack of knowledge, increases the suffering of patients. Education on the lifespan perspective, diagnostic assessment, and treatment of ADHD must increase for students of general and mental health, and for psychiatry professionals. Instruments for screening and diagnosis of ADHD in adults are available, as are effective evidence-based treatments for ADHD and its negative outcomes. More research is needed on gender differences, and in older adults with ADHD.

Cell suspension cultures of white clover (Trifolium repens L. ‘Regal Ladino’) were treated with 2,4-D [(2,4-dichlorophenoxy)acetic acid], 2,4-DB [4-(2,4-dichlorophenoxy)butyric acid], 2,4,5-T [(2,4,5-trichlorophenoxy)acetic acid], 2,4,5-TP [2-(2,4,5-trichlorophenoxy) propionic acid], and 2,4,5-TB [4-(2,4,5-trichlorophenoxy)butyric acid]. Cell population densities were monitored throughout the treatment period and herbicide remaining in the cells and culture medium was extracted and quantified at the termination of the treatment. Herbicide tolerance increased in cell populations which were conditioned by pretreatment with 2,4-DB and 2,4,5-T. However, 2,4-DB was rapidly degraded by all cell populations regardless of pretreatment. White clover cell suspension cultures treated with 2,4-D, 2,4-DB, 2,4,5-T, 2,4,5-TP and 2,4,5-TB metabolized the homologs, according to the following sequence: butyric acidåpropionic acidåacetic acid homologs. There was no difference in the rate of degradation of similar homologs of the 2,4-dichloro- and 2,4,5-trichloro- analogs.
Defining pathogenic verocytotoxin-producing Escherichia coli (VTEC) from cases of human infection in the European Union, 2007–2010

During 2007–2010, 13 545 confirmed human verocytotoxin (VT)-producing Escherichia coli (VTEC) infections were reported in the European Union, including 777 haemolytic uraemic syndrome (HUS) cases. Clinical manifestations were reported for 53% of cases, 64% of which presented with diarrhoea alone and 10% with HUS. Isolates from 85% of cases were not fully serotyped and could not be classified on the basis of the Karmali seropathotype concept. There is no single or combination of phenotypic or genetic marker(s) that fully define ‘pathogenic’ VTEC. Isolates which contain the vtx2 (verocytotoxin 2) gene in combination with the eae (intimin-encoding) gene or aaiC (secreted protein of enteroaggregative E. coli) and aggR (plasmid-encoded regulator) genes have been associated with a higher risk of more severe illness. A molecular approach targeting genes encoding VT and other virulence determinants is thus proposed to allow an assessment of the potential severity of disease that may be associated with a given VTEC isolate.

The performance of 47 patients with Parkinson's disease on a battery of tests of cognition, motor function, disability and mood was compared with the performance of 47 healthy control subjects who were matched to the patients on the basis of age, sex and pre-morbid IQ. An increased prevalence of impairment over a range of cognitive functions was observed in the Parkinson's disease patients as compared with their matched controls. The differences between the Parkinson's disease patients and controls could not be accounted for by factors such as depressed mood, effects of medication or motor impairment. Our findings are discussed in relation to the methodology of previous studies in this area and to the need for a comprehensive clinico-pathological longitudinal study.

Fumonisin B1 (FB1) is a mycotoxin produced by Fusarium verticillioides, a fungus that commonly contaminates maize. Sex-related effects of FB1 have been observed with respect to carcinogenicity in rodents, to performances in pigs and immunosuppression in mice. In the present study the sex-related effect of FB1 on the pig immune response was determined. Female and castrated male piglets received for 28d either control feed or feed contaminated with 8mg FB1/kgfeed in the form of F. verticillioidesculture material. At day 7 and day 21, animals were immunised subcutaneously with a Mycoplasma agalactiae vaccine. Ingestion of FB1-contaminated feed significantly decreased weight gain in males but had no effect in females. No sex-related difference was observed in biochemical parameters, but a higher level of creatinine was noted in toxin-treated animals. FB1 also altered the pig immune response in a sex-specific manner. In males, ingestion of FB1-contaminated feed significantly decreased specific antibody levels after vaccination as well as the mRNA expression level of IL-10. In females, the toxin has no effect on specific antibodies or on cytokine mRNA levels. The results of the present study indicatethat FB1 is immunosuppressive in pigs. The magnitude of this FB1-induced immunosuppression is highly dependent on sex, with males being more susceptible than females.

Summary
Side-channel analysis (SCA) or information leakage analysis (ILA), refers to a new and emerging type of cryptanalysis that uses leaked side-channel information from a cryptographic device to determine the secret key. The traditional cryptographic model consists of two parties, Alice and Bob, who wish to communicate secretly over an insecure channel. To protect against a potential eavesdropper, cryptographic algorithms are used to ensure the secrecy of the communication. In this traditional model, the security of the system relies solely on the secrecy of the key and the mathematical properties of the cryptographic algorithm used.
However, in a more practical scenario the communicating parties are mostly computing devices, such as personal computers, smart cards or mobile phones. On such devices, a cryptographic algorithm is implemented and executed whenever a cryptographic protocol requires its computation. Although these devices operate in potentially hostile environments and are subject to attacks themselves, they are supposed to protect the secret key. Furthermore, such devices can act as a source of information. Depending on the data they process, they might consume different amounts of power, emit different amounts of electromagnetic emanations, produce different types of noises, need different running times or output different error messages; see Figure IV.1.

In a prospective randomized study in children, we compared caudal bupivacaine–clonidine with bupivacaine–morphine to evaluate whether clonidine can be used as an alternative to morphine in caudal anaesthesia. Caudal anaesthesia was administered in 36 children undergoing orchidopexy, hernia repair or circumcision, using 1.5 mL kg−1 bupivacaine 0.18% with either 1 μg kg−1 clonidine (group 1) or 30 μg kg−1 morphine (group 2). Haemodynamic and respiratory parameters, anaesthetic requirements, recovery time and pain score were monitored for 24 h. Eleven children in group 1 and nine children in group 2 did not need any supplementary systemic analgesics throughout the 24-h observation period. Mean (±SD) duration of analgesia in the remaining patients was 6.3 h (±3.3 h) in group 1 and 7.1 h (±3.4 h) in group 2 (P=0.43). Recovery time after anaesthesia was significantly longer in group 1 (16.6±8.8 min) than in group 2 (11.5±4.7 min) (P< 0.05). We conclude that analgesia provided by 1 μg kg−1 clonidine added to caudal bupivacaine is comparable with that provided by 30 μg kg−1 caudal morphine with bupivacaine. Clonidine at this low dose did not cause respiratory depression.

Certain R2Co17 and R2 T14 B systems (R = a rare earth, T = Fe or Co) are of significance for use in the fabrication of high energy magnets. Improvements in these systems as high energy magnet materials are effected by partially replacing Co or T by appropriate d transition metals. A description is given of the magnetic behavior of a number of systems based on R2 Co17 and R2T14 B. Also, an account is given of recent work dealing with the systems RCo4 B and PrCo4-xFex B. The latter materials have large magnetizations and high Curie temperatures. However, they are not useful permanent magnet materials because they exhibit planar anisotropy.
Results of some Auger spectroscopy studies of R2 Fe14 B systems (R = Pr,Nd or Er) are presented. This work shows the intergranular region to be rich in B, O and R. This region is undoubtedly non-magnetic, a feature which is most probably of significance in regard to the observed coercivity of R2 Fe14 B magnets.

A number of drugs capable of mono-amine oxidase inhibition have been marketed with the claim that they alleviate depressive illness. One of these is tranylcypromine (‘Parnate’), which has won notoriety on account of the severe headache experienced by those receiving the drug when they also eat ripe cheese (Lancet, 1964). In chemical structure, it is related to amphetamine. Amphetamine is a drug of addiction, and addiction to tranylcypromine was therefore to be expected. A preliminay notice of the present patient has already been published (Le Gassicke, 1962).
