Comparing the effectiveness and predictors of cognitive behavioural therapy-enhanced between patients with various eating disorder diagnoses: a naturalistic study

Cognitive behaviour therapy-enhanced (CBT-E) is an effective treatment for non-underweight patients with eating disorders. Its efficacy and effectiveness is investigated mostly among transdiagnostic samples and remains unknown for binge eating disorder. The aim of the present study was to assess several treatment outcome predictors and to compare effectiveness of CBT-E among adult out-patients with bulimia nervosa (n=370), binge eating disorder (n=113), and those with a restrictive food pattern diagnosed with other specified feeding and eating disorders (n=139). Effectiveness of CBT-E was assessed in routine clinical practice in a specialised eating disorders centre. Eating disorder pathology was measured with the EDEQ pre- and post-treatment, and at 20 weeks follow-up. Linear mixed model analyses with fixed effect were performed to compare treatment outcome among the eating disorder groups. Several predictors of treatment completion and outcome were examined with a regression analysis. No predictors for drop-out were found, except the diagnosis of bulimia nervosa. Eating disorder pathology decreased among all groups with effect sizes between 1.43 and 1.70 on the EDE-Q total score. There were no differences in remission rates between the three groups at end of treatment or at follow-up. Eating disorder severity at baseline affected treatment response. The results can be generalised to other specialised treatment centres. No subgroup of patients differentially benefited from CBT-E supporting the transdiagnostic perspective for the treatment of eating disorders. Longer-term follow-up data are necessary to measure persistence of treatment benefits.

Amazona lilacina is a threatened species endemic to Ecuador, existing across a patchwork of mangroves, lowland coastal forests, agricultural and community owned land. The species was described in 2014 and listed as ‘Endangered’ on the IUCN Red List, however, full assessment of the population was lacking. Using a combination of field observations, roost surveys and community questionnaires, conducted over the last 20 years, we provide up-to-date information on the species’ Extent of Occurrence, estimate its global population size, and evaluate its level of threat. Our results suggest the species occurs across an area of 19,890 km2 in three distinct geographically isolated subpopulations. Roost surveys across the range estimate the minimum remaining population at 741–1,090 individuals and we present evidence to suggest a 60% decline over the past 19 years in one part of the species’ range. We conducted community questionnaires with 427 people from 52 communities. The presence of pet parrots was reported in 37 communities, including 17 communities which reported pet A. lilacina. From this we predict that over half of all communities within our study area keep parrots as pets and at least 96 communities keep A. lilacina. Our findings justify an IUCN Red Listing of at least ‘Endangered’ for this species and highlight the need for conservation support. In order to assess population health in more detail, further research is required to assess genetic diversity and roost dynamics, and to identify areas that may be important for feeding and nesting throughout the range. As many of these areas are likely to overlap with community owned land, we suggest that future conservation actions should revolve around, and be led by, these communities.

An experiment was conducted to test the hypothesis that meat products have digestible indispensable amino acid scores (DIAAS) >100 and that various processing methods will increase standardised ileal digestibility (SID) of amino acids (AA) and DIAAS. Nine ileal-cannulated gilts were randomly allotted to a 9 × 8 Youden square design with nine diets and eight 7-d periods. Values for SID of AA and DIAAS for two reference patterns were calculated for salami, bologna, beef jerky, raw ground beef, cooked ground beef and ribeye roast heated to 56, 64 or 72°C. The SID of most AA was not different among salami, bologna, beef jerky and cooked ground beef, but was less (P < 0·05) than the values for raw ground beef. The SID of AA for 56°C ribeye roast was not different from the values for raw ground beef and 72°C ribeye roast, but greater (P < 0·05) than those for 64°C ribeye roast. For older children, adolescents and adults, the DIAAS for all proteins, except cooked ground beef, were >100 and bologna and 64°C ribeye roast had the greatest (P < 0·05) DIAAS. The limiting AA for this age group were sulphur AA (beef jerky), leucine (bologna, raw ground beef and cooked ground beef) and valine (salami and the three ribeye roasts). In conclusion, meat products generally provide high-quality protein with DIAAS >100 regardless of processing. However, overcooking meat may reduce AA digestibility and DIAAS.

Radio-glaciological parameters from the Moore’s Bay region of the Ross Ice Shelf, Antarctica, have been measured. The thickness of the ice shelf in Moore’s Bay was measured from reflection times of radio-frequency pulses propagating vertically through the shelf and reflecting from the ocean, and is found to be 576 ± 8 m. Introducing a baseline of 543 ± 7m between radio transmitter and receiver allowed the computation of the basal reflection coefficient, R, separately from englacial loss. The depth-averaged attenuation length of the ice column, 〈L〉 is shown to depend linearly on frequency. The best fit (95% confidence level) is 〈L(ν)〉= (460±20) − (180±40)ν m (20 dB km−1), for the frequencies ν = [0.100–0.850] GHz, assuming no reflection loss. The mean electric-field reflection coefficient is
(1.7 dB reflection loss) across [0.100–0.850] GHz, and is used to correct the attenuation length. Finally, the reflected power rotated into the orthogonal antenna polarization is <5% below 0.400 GHz, compatible with air propagation. The results imply that Moore’s Bay serves as an appropriate medium for the ARIANNA high-energy neutrino detector.

In Camus’ The Plague, Father Paneloux delivers two sermons to the population of oran. neither sermon practically helps the people and each has a different theological foundation. Camus uses a theodicy dilemma to illuminate the limits and dangers of theology (and all disciplines that can linger only in the theoretical, philosophy and science included). in this paper, i will briefly describe both sermons and the event that triggers a change in theology for Father Paneloux. From this, i will develop a “metaphysical flow chart” that will track the rise of the Absurd, the Father's reaction, and his attempt to deal with it. After illuminating the absurd and the resulting revolt against it, i will offer several possible theological foundations that Camus may have been comfortable with, or even pointed to as actually being helpful. This also helps the reader understand Camus’ rather difficult statements about not believing in god and not being an atheist.
PART I
After delivering several short sermons of instructions - these sermons appear to be essentially public service announcements - Father Paneloux crafts a theological response to the plague based in Augustine and justice. After witnessing the suffering and death of a child, he retools and crafts a second sermon based in Tertullian and the mystery of god with an “all or nothing” demand. in the end, in one of Camus’ most brilliant word uses, Paneloux dies “a doubtful case.” it is both possible that he died of plague and that he died a heretic in the eyes of Rome.
First, who was Father Paneloux? we know quite a bit about him from Camus. He was: Roman Catholic, Jesuit, well-informed on Augustine, Pied-Noir, “militant” (which i read as orthodox), a large man built to preach, intelligent, meticulous, introverted, and deeply devoted to his faith and humanity.
As the central religious character in The Plague, he is no strawman. situated in north Africa, he is a fine representative of French Catholicism and Camus is well-equipped to use him in the narrative. with a Jesuit education and north African in the tradition of Augustine, Paneloux is a solid representation of the Christian response to the plague (and all it stands for in the novel: the absurd, fascism, nazism, literal plague).

Summary
Obstructive sleep apnea (OSA) is a serious and chronic condition affecting as many as 15–20 million American adults. Resulting sleep deprivation has been linked to motor vehicle and workplace accidents. The incidence of OSA is increasing with the obesity epidemic, and it is increasingly recognized as a mediator of cardiovascular disease including atrial fibrillation, stroke, myocardial infarction, and sudden cardiac death. The importance of appropriate diagnosis and timely treatment thus cannot be overstated.
Obstructive sleep apnea manifests by repeated episodes of apnea or hypopnea during sleep. During deeper levels of sleep, especially that characterized by rapid eye movement (REM), there is loss of the normal tone of the pharyngeal and tongue muscles that keep the pharynx open, resulting in collapse of the oropharyngeal and nasopharyngeal airway. In the majority of the population this decrease in airway diameter is clinically insignificant. However, in OSA patients the varied degree of airway obstruction can have clinical consequences. Narrowing of the airway causes increased velocity of inspiratory airflow in the pharynx, causing decreased intraluminal pressure, further tissue collapse, and increased airway obstruction (Bernoulli's principle). In instances of complete airway obstruction, the patient will experience apnea, a cessation of breathing for at least 10 seconds. Incomplete obstruction may result in hypopnea, a reduction in airflow with associated oxygen desaturation, which is more common. Each apnea or hypopnea episode continues until the patient awakens to a more shallow level of sleep, which results in a recovery of pharyngeal muscle tone and recovery of airway integrity. The more frequent the apnea and hypopnea, the more fragmented the sleep, which results in greater sleep deprivation due to the lack of adequate REM activity.

The United States, along with other nations and international organizations, has developed an elaborate system of ethical norms and legal rules to govern biomedical research using human subjects. These policies govern research that might provide direct health benefits to participants and research in which there is no prospect for participant health benefits. There has been little discussion, however, about how well these rules would apply to research designed to improve participants’ capabilities or characteristics beyond the goal of good health. When mentioned at all in the literature, this so-called enhancement research, as opposed to research aimed at diagnosing, preventing, curing, or treating illnesses or medical conditions, is usually dismissed without explanation.

Standardized Screening and Assessment of Older Emergency Department Patients: A Survey of Implementation in Quebec*

Cost-effective methods have been developed to help busy emergency department (ED) staff cope with the growing number of older patients, including quick screening and assessment tools to identify those at high risk and note their specific needs. This survey, from a sample of key informants from all EDs (n=111) in the province of Quebec (participation rate of 88.2%), investigated the implementation of these tools and barriers to implementation. Questionnaires (administered either by telephone or by self-completion) included characteristics of the ED, characteristics of the respondent, use of tools, and method of implementation. Barriers to the implementation of these tools included lack of resources for screening and follow-up, misunderstandings of the difference between screening and assessment tools, and need for adaptation of the tools to the local context. Education of staff and pre-implementation adaptation and testing are needed for successful implementation.

We consider a network traffic model consisting of an infinite number of sources linked to a server. Sources initiate transmissions to the server at Poisson time points. The duration of each transmission has a heavy-tailed distribution. We show that suitable scalings of the traffic process converge to a totally skewed stable Lévy motion in Skorohod space, equipped with the Skorohod M1 topology. This allows us to prove a heavy-traffic theorem for a single-server fluid model.

We agree with Barsalou's claim about the importance of perceptual symbols in a theory of abstract concepts. Yet we maintain that the richness of many abstract concepts arises from the metaphorical mapping of recurring patterns of perceptual, embodied experience to provide essential structure to these abstract ideas.
