A novel algorithm for classification of interatrial communications within the oval fossa in the newborn: Results from the Copenhagen Baby Heart Study

Background:
An interatrial communication is present in most neonates. The majority are considered the “normal” patency of the oval foramen, while a minority are abnormal atrial septal defects. Differentiation between the two with transthoracic echocardiography may be challenging, and no generally accepted method of classification is presently available. We aimed to develop and determine the reliability of a new classification of interatrial communications in newborns.
Methods and Results:An algorithm was developed based on echocardiographic criteria from 495 newborns (median age 11[8;13] days, 51.5% females). The algorithm defines three main categories: patency of the oval foramen, atrial septal defect, and no interatrial communication as well as several subtypes. We found an interatrial communication in 414 (83.6%) newborns. Of these, 386 (93.2%) were categorised as patency of the oval foramen and 28 (6.8%) as atrial septal defects.
Echocardiograms from another 50 newborns (median age 11[8;13] days, 36.0% female), reviewed by eight experts in paediatric echocardiography, were used to assess the inter- and intraobserver variation of classification of interatrial communications into patency of the oval foramen and atrial septal defect, with and without the use of the algorithm. Review with the algorithm gave a substantial interobserver agreement (kappa = 0.66), and an almost perfect intraobserver agreement (kappa = 0.82). Without the use of the algorithm, the interobserver agreement between experienced paediatric cardiologists was low (kappa = 0.20).
Conclusion:A new algorithm for echocardiographic classification of interatrial communications in newborns produced almost perfect intraobserver and substantial interobserver agreement. The algorithm may prove useful in both research and clinical practice.

Background
Inflammation and immune activation have been implicated in the pathogenesis of severe mental disorders and cardiovascular disease (CVD). Despite high level of comorbidity, many studies of the immune system in severe mental disorders have not systematically taken cardiometabolic risk factors into account.
MethodsWe investigated if inflammatory markers were increased in schizophrenia (SCZ) and affective (AFF) disorders independently of comorbid CVD risk factors. Cardiometabolic risk factors (blood lipids, body mass index and glucose) and CVD-related inflammatory markers CXCL16, soluble interleukin-2 receptor (sIL-2R), soluble CD14 (sCD14), macrophage inhibitory factor and activated leukocyte cell adhesion molecule (ALCAM) were measured in n = 992 patients (SCZ, AFF), and n = 647 healthy controls. We analyzed the inflammatory markers before and after controlling for comorbid cardiometabolic risk factors, and tested for association with psychotropic medication and symptom levels.
ResultsCXCL16 (p = 0.03) and sIL-2R (p = 7.8 × 10−5) were higher, while sCD14 (p = 0.05) were lower in patients compared to controls after controlling for confounders, with significant differences in SCZ for CXCL16 (p = 0.04) and sIL-2R (p = 1.1 × 10−5). After adjustment for cardiometabolic risk factors higher levels of sIL-2R (p = 0.001) and lower sCD14 (p = 0.002) remained, also in SCZ (sIL-2R, p = 3.0 × 10−4 and sCD14, p = 0.01). The adjustment revealed lower ALCAM levels (p = 0.03) in patients. We found no significant associations with psychotropic medication or symptom levels.
ConclusionThe results indicate that inflammation, in particular enhanced T cell activation and impaired monocyte activation, are associated with severe mental disorders independent of comorbid cardiometabolic risk factors. This suggests a role of novel pathophysiological mechanisms in severe mental disorders, particularly SCZ.

Mental health research funding priorities in high-income countries must balance longer-term investment in identifying neurobiological mechanisms of disease with shorter-term funding of novel prevention and treatment strategies to alleviate the current burden of mental illness. Prioritising one area of science over others risks reduced returns on the entire scientific portfolio.

Electro-migration of Zinc during current-assisted pressure sintering of β-Zn4Sb3 – effect of process parameters and its influence on the thermoelectric properties

Compaction of β-Zn4Sb3 was carried out by current-assisted short term sintering under pressure (STS) using different material and process parameters. Surface Seebeck mapping (PSM) of the compacted specimens (along the uniaxial pressing direction) shows a wide gradation ranging from ∼40–180 μV/K due to electro-migration of Zinc along the current direction. The wide distribution of S corresponds to a mixture of Zn-Sb phases [1] which arise depending on the extent of Zinc migration during the STS process. Variation in the material and process parameters (average particle size, heating rate, compaction time/temperature) results in different spatial distribution of S. Measurements of electrical (σ), thermal (κ) conductivities and Seebeck (S) coefficients between room temperature and 523 K were carried out on two specimens having different average S values and distributions as observed by the PSM. The results indicate an increase in the lattice thermal conductivity (κL) and subsequent lower ZT in the specimens compared to the reported values for β-Zn4Sb3.

Mental health and health service use among post-national service veterans: results from the 2007 Adult Psychiatric Morbidity Survey of England

Background
There is concern surrounding the psychological health and uptake of treatment services among veterans of the UK Armed Forces.
MethodData from a cross-sectional, nationally representative sample were used to compare health outcomes and treatment seeking among 257 post-national service veterans aged 16–64 years and 504 age and sex frequency-matched non-veterans living in the community in England. Early leavers (<4 years service) were compared with longer serving veterans.
ResultsMale veterans reported more childhood adversity and were more likely to have experienced a major trauma in adulthood than non-veterans. There was no association between any measure of mental health and veteran status in males, except reporting more violent behaviours [adjusted odds ratio (aOR) 1.44, 95% confidence interval (CI) 1.01–2.06]. In females, a significant association was found between veteran status and ever having suicidal thoughts (aOR 2.82, 95% CI 1.13–7.03). No differences in treatment-seeking behaviour were identified between veterans and non-veterans with any mental disorder. Early service leavers were more likely to be heavy drinkers (aOR 4.16, 95% CI 1.08–16.00), to have had suicidal thoughts (aOR 2.37, 95% CI 1.21–4.66) and to have self-harmed (aOR 12.36, 95% CI 1.61–94.68) than longer serving veterans.
ConclusionsThe findings of this study do not suggest that being a veteran is associated with adversity in terms of mental health, social disadvantage or reluctance to seek treatment compared with the general population. Some evidence implies that early service leavers may experience more mental health problems than longer-serving veterans.

Trematodes are ubiquitous elements of coastal ecosystems that commonly modify the phenotype of their invertebrate hosts, often with ramifications to higher levels of ecological organization. In this context, trematode infections have been suggested to reduce the consumption of the herbivorous gastropod Littorina littorea (L.) (Mollusca: Gastropoda), in turn affecting the composition of the macroalgal community on which the snail grazes. Here, we examine the effect of two species of trematodes, Renicola roscovita and Himasthla elongata, on L. littorea's consumption in two outdoor microcosm experiments offering the snails two different ephemeral green algae species as a food source. Our results show that, irrespective of the species of parasite and food source, infection decreases consumption: uninfected snails consumed up to 65% more macroalgal biomass than infected snails. Aside from infection status, gender and size also influenced the snails' consumption rate significantly. The differing histopathological impacts of the two species of trematodes on the hosts' gonad–digestive gland complex (in which the parasites reside), suggests that parasitic castration is a likely mechanism for the reduced energy demand of infected periwinkles. Together with existing evidence, our investigation suggests that trematodes in general depress the grazing activity of L. littorea, and that the resulting community regulation occurs throughout the snails' distributional range.
Maximum Entropy Method Charge Density Distributions of Novel Thermoelectric Clathrates

Recently materials with promising thermoelectric properties were discovered among the clathrates. Transport data has indicated that these materials have some of the characteristics of a good thermoelectric, namely a low thermal conductivity and a high electrical conductivity. Based on synchrotron powder and conventional single crystal x-ray diffraction data we have determined the charge density distribution in Sr8Ga16Ge3O using the Maximum Entropy Method. The MEM density shows clear evidence of guest atom rattling, and this contributes to the reduction of the thermal conductivity. Analysis of the charge distribution reveals that Sr8Ga16Ge30 contains mixed valence alkaline earth guest atoms. The Sr atoms in the small cavities are, as expected, doubly positively charged, whereas the Sr atoms in the large cavities appear negatively charged. The MEM density furthermore suggests that the Ga and Ge atoms may not be randomly disordered on the framework sites as found in the conventional leastsquares refinements.
