An outbreak of hepatitis A virus infection in a secondary school in England with no undetected asymptomatic transmission among students

In June 2019 the Health Protection Team in Yorkshire and Humber, England, was notified of cases of hepatitis A virus (HAV) infection in staff at a secondary school. Investigation revealed that an earlier case worked as a food handler in the school kitchen. Indirect transmission through food from the canteen was considered the most likely route of transmission. Cases were described according to setting of exposure. Oral fluid was obtained from students for serological testing. Environmental investigations were undertaken at settings where food handling was considered a potential transmission risk. Thirty-three confirmed cases were linked to the outbreak. All of those tested (n = 31) shared the same sequence with a HAV IB genotype. The first three cases were a household cluster and included the index case for the school. A further 19 cases (16 students, 3 staff) were associated with the school and consistent with indirect exposure to the food handler. One late onset case could not be ruled out as a secondary case within the school and resulted in vaccination of the school population. Five cases were linked to a bakery where a case from the initial household cluster worked as a food server. No concerns about hygiene standards were noted at either the school or the bakery. Oral fluid samples taken at the time of vaccination from asymptomatic students (n = 219, 11–16 years-old) showed no evidence of recent or current infection. This outbreak included household and foodborne transmission but limited (and possibly zero) person-to-person transmission among secondary school students. Where adequate hygiene exists, secondary transmission within older students may not occur.

We report key learning from the public health management of the first two confirmed cases of COVID-19 identified in the UK. The first case imported, and the second associated with probable person-to-person transmission within the UK. Contact tracing was complex and fast-moving. Potential exposures for both cases were reviewed, and 52 contacts were identified. No further confirmed COVID-19 cases have been linked epidemiologically to these two cases. As steps are made to enhance contact tracing across the UK, the lessons learned from earlier contact tracing during the country's containment phase are particularly important and timely.

Objective:
To determine whether there were changes in the prevalence or healthiness of products carrying claims post-implementation of Standard 1.2.7: Nutrition, Health and Related Claims in the Australia New Zealand Food Standards Code.
Design:Observational survey of claims on food packages in three categories: non-alcoholic beverages, breakfast cereals and cereal bars. Nutrient profiling was applied to products to determine their eligibility to carry health claims under Standard 1.2.7. The Standard came into effect in 2013. The proportion of products carrying claims and the proportion of those not meeting the nutrient profiling criteria were calculated. A comparative analysis was conducted to determine changes between 2011 and 2016.
Setting:Three large metropolitan stores from the three major supermarket chains in Sydney, Australia were surveyed in 2011 and 2016.
Participants:All claims on all available products in 2016 (n 1737). Nutrition composition and ingredients were collected from the packaging.
Results:Overall in 2016, 76 % of products carried claims and there were 7367 claims identified in the three food categories. Of products in 2016 with health claims, 34 % did not meet nutrient profiling criteria. These may breach Standard 1.2.7. Comparison of 2011–2016 showed a significant increase in the number of products carrying claims (66 v. 76 %, P < 0·001).
Conclusions:The proportion of products carrying claims that do not meet nutrient profiling and consumers’ tendency to infer health benefits from nutrition content claims warrants the regulation of all claims using the nutrient profiling. This will ensure consumers are not misled by claims on unhealthy food products.
Nutrient composition of Australian fast-food and fast-casual children’s meals available in 2016 and changes in fast-food meals between 2010 and 2016

Objective:
A quarter of Australian children are overweight or obese. Research conducted in 2010 found that fast-food children’s meals were energy-dense and nutrient-poor. Since then, menu labelling and self-regulation of marketing have been introduced in Australia. The present study aimed to: (i) investigate the nutrient composition of children’s meals offered at fast-food chains; (ii) compare these with children’s daily requirements and recommendations and the food industry’s own criteria for healthier children’s meals; and (iii) determine whether results have changed since last investigated in 2010.
Design:An audit of nutrition information for fast-food children’s meals was conducted. Meals were compared with 30 % (recommended contribution for a meal) and 100 % of children’s daily recommendations and requirements. A comparative analysis was conducted to determine if the proportion of meals that exceeded meal requirements and recommendations, and compliance with the food industry’s own criteria, changed between 2010 and 2016.
Setting:Large Australian fast-food chains.
Participants:All possible children’s meal combinations.
Results:Overall, 289 children’s meals were included. Most exceeded 30 % of daily recommendations and requirements for a 4-year-old’s energy, saturated fat, sugars and Na. Results were also substantial for 8- and 13-year-olds, particularly for Na. When compared with mean energy and nutrient contents from 2010, there were minimal changes overall.
Conclusions:Children’s meals can provide excess energy, saturated fat, sugar and Na to children’s diets. Systematic reformulation of energy, saturated fat, sugars and Na would improve the nutrient composition of the meals.
How effective is food industry self-substantiation of food–health relationships underpinning health claims on food labels in Australia?

Objective
The Food Standards Code regulates health claims on Australian food labels. General-level health claims highlight food–health relationships, e.g. ‘contains calcium for strong bones’. Food companies making claims must notify Food Standards Australia New Zealand (FSANZ) and certify that a systematic literature review (SLR) substantiating the food–health relationship has been conducted. There is no pre- or post-notification assessment of the SLR, potentially enabling the food industry to make claims based on poor-quality research. The present study assessed the rigour of self-substantiation.
DesignFood–health relationships notified to FSANZ were monitored monthly between 2013 and 2017. These relationships were assessed by scoping published literature. Where evidence was equivocal/insufficient, the relevant government food regulatory agency was asked to investigate. If not investigated, or the response was unsatisfactory, the project team conducted an independent SLR which was provided to the government agency.

ResultsThere were sixty-seven relationships notified by thirty-eight food companies. Of these, thirty-three relationships (52 %) from twenty companies were deemed to have sufficient published evidence. Four were excluded as they originated in New Zealand. Three relationships were removed before investigations were initiated. The project initiated twenty-seven food–health relationship investigations. Another six relationships were withdrawn, and three relationships were awaiting government assessment.
ConclusionsTo ensure that SLR underpinning food–health relationships are rigorous and reduce regulatory enforcement burden, pre-market approval of food–health relationships should be introduced. This will increase consumer and public health confidence in the regulatory process and prevent potentially misleading general-level health claims on food labels.
Monitoring the changes to the nutrient composition of fast foods following the introduction of menu labelling in New South Wales, Australia: an observational study

Objective
The present study examined the energy (kilojoule) content of Australian fast-food menu items over seven years, before and after introduction of menu board labelling, to determine the impact of the introduction of the legislation.
DesignAnalysis of the median energy contents per serving and per 100g of fast-food menu items. Change in energy content of menu items across the years surveyed and differences in energy content of standard and limited-time only menu items were analysed.
SettingFive of Australia’s largest fast food chains: Hungry Jack’s, KFC, McDonald’s, Oporto and Red Rooster.
SubjectsAll standard and limited-time only menu items available at each fast-food chain, collected annually for seven years, 2009–2015.
ResultsAlthough some fast-food chains/menu item categories had significant increases in the energy contents of their menus at some time points during the 7-year period, overall there were no significant or systematic decreases in energy following the introduction of menu labelling (P=0·19 by +17 kJ/100 g, P=0·83 by +8 kJ/serving). Limited-time only items were significantly higher in median energy content per 100 g than standard menu items (+74 kJ/100 g, P=0·002).
ConclusionsWhile reformulation across the entire Australian fast-food supply has the potential to positively influence population nutrient intake, the introduction of menu labelling legislation in New South Wales, Australia did not lead to reduced energy contents across the five fast-food chains. To encourage widespread reformulation by the fast-food industry and enhance the impact of labelling legislation, the government should work with industry to set targets for reformulation of nutrient content.

An economic evaluation of inpatient treatment versus day hospital care for psychiatric patients

Objectives: To provide a methodology for the examination of costs and clinical outcomes in two distinct care settings for psychiatric patients inpatient and day hospitals. The major emphasis is on the relationship between costs and outcomes in the two care regimes.
Method: The study is a retrospective cost-effectiveness analysis. People living in Sector B catchment area in the Mid-Western Health Board who were admitted to inpatient care, or treated as day hospital patients, between June 1st 1994 -February 28th 1995 are eligible for inclusion in the study. Information on resource use and clinical outcome is available for 92 of these patients.
Results: The average weekly cost of care for mentally ill patients in the inpatient setting is over twice the level of the cost of care for people attending the day hospital facility. Pay costs and hotel costs are higher in the inpatient facility. Day hospital care is also more cost-effective than inpatient care, when account is taken of the relationship between cost and clinical outcomes.
Conclusion: The study supports the general literature view of the superiority of community care settings for certain categories of mentally ill people. However, the absence of randomisation in the study, incomplete data, and the retrospective nature of the analysis suggests that caution is needed in the interpretation of the results.

Formula 127 glass has been developed to immobilize ICPP zirconia calcine. This glass has been prepared remotely on a laboratory scale basis with radioactive zirconia calcine retrieved after ten years of storage from Bin Set 2. The aqueous leachability of the glass produced was investigated and compared through application of standard leach tests with that of Formula 127 glass prepared with simulated calcine. Solid state properties of the glasses prepared with actual and simulated calcines were also compared through application of electron spectroscopy for chemical analysis (ESCA) and scanning electron microscopy (SEM) with energy dispersive X-ray (EDS).

Ten Landrace × (Landrace × Large White) young female pigs were allocated to each of four treatments. The experiment was arranged as a 2 × 2 factorial design, the variables being oestradiol benzoate v. ethyl oleate and boar v. visual testing. The oestradiol benzoate (or ethyl oleate) was administered at 140 days of age, the treatment consisting of six injections, over a 3-day period, of 0·5 ml of either ethyl oleate or a solution of 0·4 mg/ml β-oestradiol-3-benzoate in ethyl oleate. Subsequently the gilts were either inspected visually each day or taken to a vasectomized boar for oestrus detection. All gilts were mated at the second oestrous period, and slaughtered on the 20th day of gestation. Those gilts receiving exogenous oestrogen attained puberty significantly earlier (P<0·01) than the control gilts. Of the 20 gilts treated with oestrogen, 12 (60%) attained puberty within 5 days of the start of treatment. No significant difference was found between boar testing and visual testing, although receptivity to mating appeared to be lower for visually tested gilts. There are no significant differences between treatments in ovulation rate, conception rate or embryo survival.

Ten Landrace × (Landrace × Large White) young female pigs (gilts) were allocated to each of three growth rate treatments so that they attained 73 kg live weight at 135, 160 or 190 days of age. At 73 kg live weight the gilts were introduced to a vasectomized boar for 30 min each day, and were subsequently mated at the second oestrous period. The interval from boar introduction to puberty was 15·2 and 19·0 days respectively for gilts having first boar contact at 190 and 160 days of age, these intervals being significantly shorter (P<0·05) than the 34·3 days for the 135-day group. However, the mean ages at puberty of the 135- and 160-day groups (169·4 and 178·2 days respectively) were not significantly different but these gilts were significantly younger at puberty than the 190-day group (205·3 days). There were no significant differences between treatments for gilt live weight at puberty or first litter size.

Fifty-three Landrace × (Landrace × Large White) young female pigs (gilts) were fed ad libitum from selection at 20 kg live weight until mating at the second oestrous period. All gilts were subjected to a change of environment at 55 kg live weight, and the male was introduced daily from 64 kg live weight. Gilts were mated twice at second heat, and slaughtered on the 20th day of gestation. There was a range in age at puberty from 135 to 235 days (mean 178·9 ± 3·4 days), and in weight at puberty from 70 to 125 kg (mean 90·5 ±2·3 kg), indicating that there is a potential in some animals, at least, to attain sexual maturity at a very young age. Neither gilt age norweight at puberty significantly affected ovulation rate at second oestrus, conception rate or embryo survival during the first 20 days of gestation.
Gilts sired by one boar reached puberty significantly earlier (P < 0·05) than did the offspring of three other boars.

Forty litters were weaned from their Landrace × Landrace × Large White dams at 2-day intervals to give a range of lactation lengths from 4 to 42 days. A significant (P<0·001) negative curvilinear relationship was observed between lactation length and the interval from weaning to first oestrus. This period was increased by 4·2 days with the reduction of lactation from 42 to 4 days. The relationship between lactation length and the interval from farrowing to remating was significant (P<0·001) and positively linear over the whole range of observations. A decrease of 1 day in lactation length reduced this interval by 0·91 days. Very short lactations were associated with reduced litter size at the following farrowing; sows weaned after lactation lengths between 4 and 21 days had an average litter size of 9–6 piglets born per litter in the next parity, whereas sows weaned following lactation lengths between 21 and 42 days had an average of 12·7 piglets born in the next parity (P<0·01).
