Introduction:
Audits play a critical role in maintaining the integrity of observational cohort data. While previous work has validated the audit process, sending trained auditors to sites (“travel-audits”) can be costly. We investigate the efficacy of training sites to conduct “self-audits.”
Methods:In 2017, eight research groups in the Caribbean, Central, and South America network for HIV Epidemiology each audited a subset of their patient records randomly selected by the data coordinating center at Vanderbilt. Designated investigators at each site compared abstracted research data to the original clinical source documents and captured audit findings electronically. Additionally, two Vanderbilt investigators performed on-site travel-audits at three randomly selected sites (one adult and two pediatric) in late summer 2017.
Results:Self- and travel-auditors, respectively, reported that 93% and 92% of 8919 data entries, captured across 28 unique clinical variables on 65 patients, were entered correctly. Across all entries, 8409 (94%) received the same assessment from self- and travel-auditors (7988 correct and 421 incorrect). Of 421 entries mutually assessed as “incorrect,” 304 (82%) were corrected by both self- and travel-auditors and 250 of these (72%) received the same corrections. Reason for changing antiretroviral therapy (ART) regimen, ART end date, viral load value, CD4%, and HIV diagnosis date had the most mismatched corrections.
Conclusions:With similar overall error rates, findings suggest that data audits conducted by trained local investigators could provide an alternative to on-site audits by external auditors to ensure continued data quality. However, discrepancies observed between corrections illustrate challenges in determining correct values even with audits.

We aimed to quantify the proportion of people receiving care for HIV-infection that are 50 years or older (older HIV patients) in Latin America and the Caribbean between 2000 and 2015 and to estimate the contribution to the growth of this population of people enrolled before (<50yo) and after 50 years old (yo) (⩾50yo). We used a series of repeated, cross-sectional measurements over time in the Caribbean, Central and South American network (CCASAnet) cohort. We estimated the percentage of patients retained in care each year that were older HIV patients. For every calendar year, we divided patients into two groups: those who enrolled before age 50 and after age 50. We used logistic regression models to estimate the change in the proportion of older HIV patients between 2000 and 2015. The percentage of CCASAnet HIV patients over 50 years had a threefold increase (8% to 24%) between 2000 and 2015. Most of the growth of this population can be explained by the increasing proportion of people that enrolled before 50 years and aged in care. These changes will impact needs of care for people living with HIV, due to multiple comorbidities and high risk of disability associated with aging.

Acute respiratory infections (ARI) are the leading cause of death worldwide in children aged <5 years, and understanding contributing factors to their seasonality is important for targeting and implementing prevention strategies. In tropical climates, ARI typically peak during the pre-rainy and rainy seasons. One hypothesis is that rainfall leads to more time spent indoors, thus increasing exposure to other people and in turn increasing the risk of ARI. A case-crossover study design in 718 Bangladeshi children aged <5 years was used to evaluate this hypothesis. During a 3-month period with variable rainfall, rainfall was associated with ARI [odds ratio (OR) 2·97, 95% confidence interval (CI) 1·87–4·70]; some evidence of an increased strength of association as household crowding increased was found (⩾3 people/room, OR 3·31, 95% CI 2·03–5·38), but there was a lack of association in some of the most crowded households (⩾5 to <6 people/room, OR 1·55, 95% CI 0·54–4·47). These findings suggest that rainfall may be increasing exposure to crowded conditions, thus leading to an increased risk of ARI, but that additional factors not captured by this analysis may also play a role.

Objectives.
To better understand the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) colonization or infection in different patient populations, to perform quantitative analysis of MRSA in nasal cultures, and to characterize strains using molecular fingerprinting.
Design.Prospective, multicenter study.
Setting.Eleven different inpatient and outpatient healthcare facilities.
Participants.MRSA-positive inpatients identified in an active surveillance program; inpatients and outpatients receiving hemodialysis; inpatients and outpatients with human immunodeficiency virus (HIV) infection; patients requiring cardiac surgery; and elderly patients requiring long-term care.
Methods.Nasal swab samples were obtained from January 23, 2006, through July 27, 2007; MRSA strains were quantified and characterized by molecular fingerprinting.
Results.A total of 444 nares swab specimens yielded MRSA (geometric mean quantity, 794 CFU per swab; range, 3-15,000,000 CFU per swab). MRSA prevalence was 20% for elderly residents of long-term care facilities (25 of 125 residents), 16% for HIV-infected outpatients (78 of 494 outpatients), 15% for outpatients receiving hemodialysis (31 of 208 outpatients), 14% for inpatients receiving hemodialysis (86 of 623 inpatients), 3% for HIV-infected inpatients (5 of 161 inpatients), and 3% for inpatients requiring cardiac surgery (6 of 199 inpatients). The highest geometric mean quantity of MRSA was for inpatients requiring cardiac surgery (11,500 CFU per swab). An association was found between HIV infection and colonization with the USA300 or USA500 strain of MRSA (P ≤ .001). The Brazilian clone was found for the first time in the United States. Pulsed-field gel electrophoresis patterns for 11 isolates were not compatible with known USA types or clones.
Conclusion.Nasal swab specimens positive for MRSA had a geometric mean quantity of 794 CFU per swab, with great diversity in the quantity of MRSA at this anatomic site. Outpatient populations at high risk for MRSA carriage were elderly residents of long-term care facilities, HIV-infected outpatients, and outpatients receiving hemodialysis.

The majority of Galliformes are ground dwelling, many live in forests, and c. 25% are on the IUCN Red List. The Djibouti francolin Francolinus ochropectus is a Critically Endangered galliform endemic to only two areas of relict Juniperus procera forest in Djibouti. This study assessed population status and habitat condition in the species' stronghold in the Forêt du Day during the post-breeding season. Line transect distance sampling was used to survey the francolin, recording visual encounters and calls. Canopy and understorey vegetation were sampled across the study area at 150-m intervals using 400-m2 quadrats. Interviews were conducted in all adjacent villages to obtain information about francolin sightings, forest use and capacity for community-based conservation. Distance was used to generate francolin population density estimates. A geographical information system and generalized linear modelling were used to determine predictors of francolin presence and juniper condition. The Distance model estimated francolin density to be 38–94 km-2. Within the Forêt du Day this is equivalent to a population of 285–705 individuals. The presence of juveniles in the samples suggests that the effective population size may be lower and therefore, although this is the first estimate of Djibouti francolin density using standard survey methodology, it should be interpreted cautiously. Juniper condition in the Forêt du Day is poor. The healthiest forest is 50% dead. Francolins are more abundant where tree cover is high. This cover now mostly consists of Buxus hildebrandtii, which appears to have mostly replaced the original juniper. In areas of high tree cover, grazing intensity is significantly negatively correlated with francolin presence. Anthropogenic influences on juniper health and francolin decline are mediated through the large number of cows grazing in the forest. We recommend an ecosystem approach to conservation of the forest, with additional species-specific protection measures for the francolin and juniper.

The CRESST-II direct Dark Matter search is located in the Gran Sasso underground laboratories, Italy. CaWO4 crystals are used as scintillating targets for WIMP (weakly interacting massive particle) interactions. They are operated as cryogenic calorimeters in combination with a second cryogenic detector used to measure the scintillation light produced in the target crystal. For each particle interaction, the combination of phonon and light signals provides an event by event discrimination which allows to distinguish known particles (alphas, betas, gammas, neutrons) from the expected signal of WIMPs. A major upgrade of the setup comprises modifications of the shielding, installation of a muon-veto, and new read out electronics, as well as a new detector-support structure to accommodate up to 33 detector modules, i.e. 10 kg of target mass. The experiment was thereafter successfully commissioned in 2007. Data obtained during this commissioning phase from 2 detector modules are presented here. Combining the data collected with these two detector modules with data from one single module obtained during the CRESST-I phase, the experiment could already place a limit of ~6 × 10-7 pb for the spin independent WIMP-nucleon scattering cross section at a WIMP mass of ~60 GeV/c2.

EURECA (European Underground Rare Event Calorimeter Array) is an astro-particle physics facility aiming to directly detect galactic dark matter. The Laboratoire Souterrain de Modane has been selected as host laboratory. The EURECA collaboration unites CRESST, EDELWEISS and the Spanish-French experiment ROSEBUD, thus concentrating and focussing effort on cryogenic detector research in Europe into a single facility. EURECA will use a target mass of up to one ton, enough to explore WIMP – nucleon scalar scattering cross sections in the region of 10-9 – 10-10 picobarn. A major advantage of EURECA is the planned use of more than just one target material (multi target experiment for WIMP identification).

The SMC represents an exciting opportunity to observe the direct results of tidal interactions on star birth. One of the best indicators of recent star birth activity is the presence of significant numbers of High-Mass X-ray Binaries (HMXBs) — and the SMC has them in abundance! We present results from nearly 10 years of monitoring these systems plus a wealth of other ground-based optical data. Together they permit us to build a picture of a galaxy with a mass of only a few percent of the Milky Way but with a more extensive HMXB population. However, as often happens, new discoveries lead to some challenging puzzles — where are the other X-ray binaries (e.g., black hole systems) in the SMC? And why do virtually all the SMC HMXBs have Be star companions? The evidence arising from these extensive optical observations for this apparently unusual stellar evolution are discussed.

The X-ray binary population of the SMC is very different from that of the Milky Way consisting, with one exception, entirely of transient pulsating Be/neutron star binaries. We have now been monitoring these SMC X-ray pulsars for over 10 years using the Rossi X-ray Timing Explorer with observations typically every week. The RXTE observations have been complemented with surveys made using the Chandra observatory. The RXTE observations are non-imaging but enable detailed studies of pulsing sources. In contrast, Chandra observations can provide precise source locations and detections of sources at lower flux levels, but do not provide the same timing information or the extended duration light curves that RXTE observations do. We summarize the results of these monitoring programs which provide insights into both the differences between the SMC and the Milky Way, and the details of the accretion processes in X-ray pulsars.

Summary
Bacterial protein toxins are powerful biological poisons normally associated with impairment of cellular function and/or cellular death. The wide spectrum of physiological processes and cell types that are affected by bacterial products also includes bone tissue and bone cells. It has been known for many years that bacterial infection or exposure to certain toxins can lead to pathological bone disorders, most commonly, those associated with abnormal or excessive bone loss, such as periodontal disease (reviewed by Henderson and Nair, 2003). However, in most cases the bone-resorbing factors involved in these effects remain part of, or associated with, the bacterial surface. For example, the bone-resorbing effects of endotoxin, a component of lipopolysaccharide, are well established, although for the most part this action appears to be indirect, being dependent on the production of pro-inflammatory cytokines (IL-1, TNFα) from other cell types (Nair et al., 1996; Henderson and Nair 2003). In contrast, the effects of bacterial protein toxins on the cellular constituents of bone remain largely unknown. For simplicity, this review will focus only on bacterial toxins, in particular, those toxins that interfere with key signalling processes that have direct relevance to bone cell differentiation and function. However, a brief overview of the general biology of bone cells is necessary before discussing the mechanisms of toxin action and specific signal transduction pathways in bone.

Objective:
The objective of this quality standard is to optimize the treatment of bacteremia in hospitalized patients by ensuring that the antibiotic given is appropriate in terms of the blood culture susceptibility of the pathogen. Although this standard may appear to be minimal in scope, it is needed because appropriate antimicrobial treatment is not given in 5% to 17% of cases. To implement the standard, physicians, pharmacists, and microbiologists will need to devise a coordinated strategy.
Options:We considered criteria for appropriate dosing, most cost-effective selection, proper antibiotic levels in serum, least toxicity, narrowest spectrum, specific clinical indications, and optimal duration of treatment. All these criteria were rejected as the basis for the standard because they were too controversial and too difficult to be applied by a nonphysician chart reviewer. In contrast, the selection of an antibiotic to which the pathogen is sensitive is a non-controversial criterion and easy for a chart reviewer to apply.
Outcomes:The standard is designed to reduce the incidence of adverse outcomes of septicemia such as renal failure, prolonged hos-pitalization, and death.
Evidence:Several well-designed clinical trials without randomization as well as case-controlled studies have confirmed the benefit of using an antibiotic that is appropriate in light of the susceptibility of the isolate in blood culture. Prospective, randomized, placebo-controlled trials are not available.
Values:Our premise is that the presence of bacteremia is a risk factor for serious adverse outcomes. We also believe that the administration of antibiotics must always be guided by the susceptibility report for the pathogen(s) obtained from blood cultures. This concern is more critical for pathogens from the blood than for those from most other body sites. We had evidence that susceptibility reports for pathogens from positive blood cultures were not always used properly. We used group discussion to reach a consensus among the members of the Quality Standards Subcommittee.
Benefits, Harms, and Costs:Through the implementation of this standard, at least 5% of bacteremias could be treated more appropriately. An unknown number of deaths would likely be prevented, and mortality from bacteremia treated inappropriately would probably be reduced. The primary undesirable feature of the standard is an increased workload of pharmacists and microbiologists.
Recommendations:Treatment of bacteremia with an antibiotic that is appropriate in terms of the pathogen's blood-culture susceptibility is a minimal standard of care for all patients.
Validation:We consulted more than 50 experts in infectious diseases from the fields of medicine, surgery, pediatrics, obstetrics and gynecology, nursing, epidemiology, pharmacology, and government. In addition, the methods for its implementation were reviewed by the American Society of Hospital Pharmacists and were tested by one of the members of the Quality Standards Subcommittee.
