Background:
Spinal muscular atrophy (SMA) is a devastating rare disease that affects individuals regardless of ethnicity, gender, and age. The first-approved disease-modifying therapy for SMA, nusinursen, was approved by Health Canada, as well as by American and European regulatory agencies following positive clinical trial outcomes. The trials were conducted in a narrow pediatric population defined by age, severity, and genotype. Broad approval of therapy necessitates close follow-up of potential rare adverse events and effectiveness in the larger real-world population.
Methods:The Canadian Neuromuscular Disease Registry (CNDR) undertook an iterative multi-stakeholder process to expand the existing SMA dataset to capture items relevant to patient outcomes in a post-marketing environment. The CNDR SMA expanded registry is a longitudinal, prospective, observational study of patients with SMA in Canada designed to evaluate the safety and effectiveness of novel therapies and provide practical information unattainable in trials.
Results:The consensus expanded dataset includes items that address therapy effectiveness and safety and is collected in a multicenter, prospective, observational study, including SMA patients regardless of therapeutic status. The expanded dataset is aligned with global datasets to facilitate collaboration. Additionally, consensus dataset development aimed to standardize appropriate outcome measures across the network and broader Canadian community. Prospective outcome studies, data use, and analyses are independent of the funding partner.
Conclusion:Prospective outcome data collected will provide results on safety and effectiveness in a post-therapy approval era. These data are essential to inform improvements in care and access to therapy for all SMA patients.

Consider low-speed potential flow past a ship modelled as a semi-infinite two-dimensional body with constant draught. Is it possible to design the hull in such a way as to eliminate the waves produced downstream of the ship? In 1977, Vanden-Broeck & Tuck had conjectured that a single-cornered piecewise-linear hull will always generate a wake; in this paper, we show how recently developed tools in exponential asymptotics can be used to confirm this conjecture. In particular, we show how the formation of waves near a ship is a necessary consequence of singularities in the ship’s geometry (or its analytic continuation). Comprehensive numerical computations confirm the analytical predictions.

Diary observations of two-year-olds' over-extended word use have been interpreted as arising from the word's underlying semantic feature structure, retrieval error, or limited vocabulary. The first interpretation was tested by analysing whether the same few features systematically characterized most over-extensions of a word (picture-naming task), and whether the same word was over-extended in comprehension (picture-pointing task). Four instances of over-extended use were studied in each of 5 children aged 1; 9 to 2; 3. A semantic feature interpretation is rejected, as is the view that overextensions are used by the child deliberately, to refer to objects which he knows are not properly named by the words. The need to construct models of early word meaning which reflect the early instability of conceptual organization, the probability of retrieval error in recall tasks, and the developmental history of individual words – particularly parental acceptance or rejection of use – is discussed.

The problem of irrotational inviscid incompressible free-surface flow is examined in the limit of small Froude number. Since this is a singular perturbation, singularities in the flow field (or its analytic continuation) such as stagnation points, or corners in submerged objects or on rough beds, lead to a divergent asymptotic expansion, with associated Stokes lines. Recent techniques in exponential asymptotics are employed to observe the switching on of exponentially small gravity waves across these Stokes lines.
As a concrete example, the flow over a step is considered. It is found that there are three possible parameter regimes, depending on whether the dimensionless step height is small, of the same order, or large compared to the square of the Froude number. Asymptotic results are derived in each case, and compared with numerical simulations of the full nonlinear problem. The agreement is remarkably good, even at relatively large Froude number. This is in contrast to the alternative analytical theory of small step height, which is accurate only for very small steps.

Summary
Clinicians have provided rich descriptions of schizophrenia-prone individuals. These descriptions are primarily anecdotal and speculative, but they provide a valuable source of hypotheses for testing in more rigorous research. Both Kraepelin ([1913] 1919) and Bleuler (1924), working from case history information as well as from observations of relatives of schizophrenic patients, described schizophreniaprone individuals as having deviant but nonpsychotic symptoms. The symptoms were often milder or transient versions of the types of symptoms displayed by psychotic patients. They interpreted these symptoms as early signs of the disorder but reported that only a portion of persons who show these symptoms of schizophrenia proneness actually develop full-fledged schizophrenia.
The construct of schizophrenia proneness
“Schizophrenia proneness” refers to a predisposition or diathesis to schizophrenia. Most researchers in the field guide their work by a diathesis-stress model. Well-known examples include those of Gottesman (1991), and Meehl (1962), and Slater and Cowie (1971). It is usually assumed that the diathesis is genetic, but that assumption is not necessary for the logic of the model. Although there is clearly a strong genetic predisposition to schizophrenia (Gottesman 1991), other contributions to the predisposition could be environmental. The stress component of diathesis-stress is considered to be only environmental, although the concept of environmental stress is broad, and includes much more than interpersonal stress. For example, one variety of environmental stress is the difficulty of coping with the problems of life for a person with a poor genetic endowment, such as genetically weak physique or low intelligence.

The Eysencks suggested that sociopaths and criminals are high on psychoticism and schizophrenics are the highest of all. Accordingly, the Eysencks built their Psychoticism Scale primarily around aspects of personality often found in sociopaths and criminals. Nielsen and Petersen developed a scale of schizophrenism, which they defined as a subschizophrenic trait indexed by withdrawal and cognitive peculiarities. Venables et al. needed a schizotypy scale for a research project in Mauritius and found that none of the previously published scales quite met their needs. They wanted a scale to measure positive and negative symptoms of schizotypy, types of symptoms that they believed might correspond to positive and negative schizophrenia. A number of researchers have investigated the usefulness of various Minnesota Multiphasic Personality Inventory (MMPI) scales and indicators for the detection of schizotypy. Schizoidia is the underlying syndrome that gives rise to schizotypal symptoms.

Every day psychiatrists and clinical psychologists must make vital decisions:
What is his problem? Should he be committed to a mental hospital? Is he a suicide-risk or a homicide-risk? Is this patient well enough to be discharged from the hospital or should he stay?
For help with their decisions the clinicians almost always use psychological tests.
According to a survey by Norman Sundberg, the two most widely used tests of any kind are the Rorschach inkblot test and the Draw-a-Person test (DAP). Both are projective tests, based on the premise that a person projects part of his personality when he responds to an ambiguous, unstructured situation. For example, since there are no objective shapes in an inkblot, anything a person sees in one presumably reflects his own drives, conflicts and personality. Similarly, when one draws a picture of a person on a blank sheet of paper, he is thought to project a bit of himself into his creation.

Our recent research suggests that the Rorschach and DAP may be projective tests in more ways than one. In interpreting the results of these tests, the average clinician may project his own preconceptions and assumptions into his description of the patient.
Our first studies in this area were with the Draw-a-Person test, in which a clinician gives the subject a pencil and a blank sheet of paper and asks him to draw a person. Karen Machover published the test in 1949.

This series of four papers has outlined a method for predicting the bactericidal efficiency of an ultra-high-temperature sterilizing plant. The method has been illustrated with reference to the performance of an A.P.V. 200 gal./hr. plate-type sterilizing plant, treating at different temperatures milk heavily inoculated with suspensions of B. subtilis and B. stearothermophilus spores.
The method involves the determination of the temperature and flow time distributions in the plant, and of the thermal death characteristics of the organism to be considered. The investigations of plant performance are considered in Part I. The determination of the sporicidal effect of the plant from this information is described in Part II. The effect is expressed in terms of the slope of the thermal death line for the organism considered, at the operation temperature of the plant. By expressing the result in this form it becomes general and applicable to any type of organism, at any operating temperature close to that for which the plant is designed.
The thermal death time of any organism at the operating temperature may be obtained by laboratory experiment, by extrapolation if necessary. In Parts III and IV, laboratory data for B. subtilis spores and B. Stearothermophilus spores are used to calculate the performance of the plant, and the calculated results are compared with the results of direct plant experiment. The agreement is satisfactory.
The interpretation of the results with spores of B. stearothermophilus is complicated by their very marked inhibition by u.h.t.-treated milk. Only about one spore per million germinates and grows in such milk. A false impression of the number of truly surviving organisms after the heat treatment may therefore be obtained. It is not known to what extent this effect occurs with spores of other strains of B. stearothermophilus or with other organisms.
It is not suggested that the theoretical method of estimation of performance will give reliable information on the spoilage level to be expected from a plant under practical dairy conditions. The uncertainties as to the number and types of incident organisms are too great. Comparisons can be made between different plants, however, by comparing the results of the theoretical analyses for the plants. No bacteriological data are then required. This may be a less difficult procedure for manufacturers than a direct bacteriological experiment using heavily inoculated milk. The analysis also enables the contribution to the overall lethal effect of the different part of a sterilizing plant to be assessed.

The previous papers of this series (1,2) have shown how the bactericidal efficiency of a flow-type heat exchanger may be predicted from physical measurements, and have given the necessary calculations for a small plate-type ultra-high-temperature sterilizing plant made by the A.P.V. Co. The purpose of this paper is to show the degree of agreement between practice and prediction from theory.
