Background:
The Fontan procedure is the final stage of surgical palliation for a single-ventricle circulation. Significant complications are common including rhythm disturbance necessitating implantation of a permanent pacemaker. This has been widely considered a negative prognostic indicator.
Methods:This single-centre, retrospective case control study involved all patients who underwent the Fontan procedure at the Leeds Congenital Heart Unit between 1990 and 2015 and have had regular follow-up in Yorkshire and Humber, United Kingdom. 167 Fontan patients were identified of which 2 were excluded for having a pre-procedure pacemaker. Of the remainder, 23 patients required a pacemaker. Outcomes were survival, early and late complications, need for further intervention and oxygen saturation in long-term follow-up.
Results:There was no difference in survival (30-day survival pacemaker 92.6%, sinus rhythm 90.5%, p = 0.66, 1-year pacemaker 11.1%, sinus rhythm 10.1%, p = 1). The pacemaker group was more likely to have cerebral or renal complications in the first-year post-procedure (acute kidney injury: sinus rhythm 0.8%, pacemaker 19.1%, p = 0.002). No difference was observed in longer term complications including protein losing enteropathy (sinus rhythm 3.5%, pacemaker 0% p = 1). There was no difference in saturations between the two groups at follow-up. Paced patients were more likely to have required further intervention, with a higher incidence of cardiopulmonary bypass procedures (sinus rhythm 6.3%, pacemaker 35%, p < 0.001).
Conclusions:Despite an increase in early complications and the need for further interventions, pacemaker requirement does not appear to affect long-term survival following the Fontan procedure.

Sexual dimorphism is common in many extant animals, but it is difficult to demonstrate in fossil species. Working with material from the Late Cretaceous of the U.S. Coastal Plain, we herein analyze sexual dimorphism in ostracodes from the superfamily Cytheroidea, a group whose extant members have males that are relatively more elongate than females. We digitized outlines of more than 6000 individual ostracode valves or carapaces, extracted size (area) and shape (length-to-height ratio) information, and used finite mixture models to assess hypotheses of sexual dimorphism. Male and female clusters can be discerned in nearly all populations with sufficient data, resulting in estimates of size and shape dimorphism for 142 populations across 106 species; an additional nine samples are interpreted to consist only of females. Dimorphism patterns varied across taxa, especially for body size: males range from 30% larger to 20% smaller than females. Magnitudes of sexual dimorphism are generally stable within species across time and space; we can demonstrate substantial evolutionary changes in dimorphism in only one species, Haplocytheridea renfroensis. Several lines of evidence indicate that patterns of sexual dimorphism in these ostracodes reflect male investment in reproduction, suggesting that this study system has the potential to capture variation in sexual selection through the fossil record.

Multibeam bathymetry and 3.5-kHz sub-bottom profiler data collected from the US icebreaker Healy in 2003 provide convincing evidence for grounded ice on the Chukchi Borderland off the northern Alaskan margin, Arctic Ocean. The data show parallel, glacially induced seafloor scours, or grooves, and intervening ridges that reach widths of 1000 m (rim to rim) and as much as 40 m relief. Following previous authors, we refer to these features as “megascale glacial lineations (MSGLs).” Additional support for ice grounding is apparent from stratigraphic unconformities, interpreted to have been caused by ice-induced erosion. Most likely, the observed sea-floor features represent evidence for massive ice-shelf grounding. The general ESE/WNW direction of the MSGLs, together with sediment, evidently bulldozed off the Chukchi Plateau, that is mapped on the western (Siberian) side of the plateau, suggests ice flow from the Canada Basin side of Chukchi Borderland. Two separate generations of glacially derived MSGLs are identified on the Chukchi Borderland from the Healy geophysical data. The deepest and oldest extensive MSGLs appear to be draped by sediments less than 5 m thick, whereas no sediment drape can be distinguished within the resolution of the sub-bottom profiles on the younger generation.

Patients with dementia of the Alzheimer's type (DAT) and their matched controls wrote, on a computer graphics tablet, 4 consecutive, cursive letter ‘l’s, with varying levels of visual feedback: noninking pen and blank paper so that only the hand movements could be seen, noninking pen and lined paper to constrain their writing, goggles to occlude the lower visual field and eliminate all relevant visual feedback, and inking pen with full vision. The kinematic measures of stroke length, duration, and peak velocity were expressed in terms of consistency via a signal-to-noise ratio (M value of each parameter divided by its SD). Irrespective of medication or severity, DAT patients had writing strokes of significantly less consistent lengths than controls', and were disproportionately impaired by reduced visual feedback. Again irrespective of medication or severity, patients' strokes were of significantly less consistent duration, and significantly less consistent peak velocity than controls', independent of feedback conditions. Patients, unlike controls, frequently perseverated, producing more than 4 ‘l’s, or multiple sets of responses, which was not differentially affected by level of visual feedback. The more variable performance of patients supports a degradation of the base motor program, and resembles that of Huntington's rather than Parkinson's disease patients. It may indeed reflect frontal rather than basal ganglia dysfunction. (JINS, 1999, 5, 20–25.)

This is the fourth of six volumes designed to explore the history of Japan from prehistoric to modern times. Volume 4 covers the years from 1550 to 1800, a short but surprisingly eventful period in Japanese history commonly referred to as Japan's Early Modern Age. At the start, in the sixteenth century, much of the country was being pulled apart by local military lords engaged in a struggle for land and local hegemony. These daimyo succeeded in dividing Japan into nearly autonomous regional domains. This volume attempts to flesh out the historical tale with insights into the way that people lived and worked. It examines the relationship between peasant and local lord, and between the lord, as a unit of local government, and the emerging shogunate. It offers insights into the evolution of indigenous thought and religion and it also deals with Japan's foreign relations, particularly the impact of the Christian missionary movement.
