Clinical risk model to predict 28-day unplanned readmission via the accident and emergency department after discharge from acute psychiatric units for patients with psychotic spectrum disorders

Background
Unplanned readmissions rates are an important indicator of the quality of care provided in a psychiatric unit. However, there is no validated risk model to predict this outcome in patients with psychotic spectrum disorders.
AimsThis paper aims to establish a clinical risk prediction model to predict 28-day unplanned readmission via the accident and emergency department after discharge from acute psychiatric units for patients with psychotic spectrum disorders.
MethodAdult patients with psychotic spectrum disorders discharged within a 5-year period from all psychiatric units in Hong Kong were included in this study. Information on the socioeconomic background, past medical and psychiatric history, current discharge episode and Health of the Nation Outcome Scales (HoNOS) scores were used in a logistic regression to derive the risk model and the predictive variables. The sample was randomly split into two to derive (n = 10 219) and validate (n = 10 643) the model.
ResultsThe rate of unplanned readmission was 7.09%. The risk factors for unplanned readmission include higher number of previous admissions, comorbid substance misuse, history of violence and a score of one or more in the discharge HoNOS overactivity or aggression item. Protective factors include older age, prescribing clozapine, living with family and relatives after discharge and imposition of conditional discharge. The model had moderate discriminative power with a c-statistic of 0.705 and 0.684 on the derivation and validation data-set.
ConclusionsThe risk of readmission for each patient can be identified and adjustments in the treatment for those with a high risk may be implemented to prevent this undesirable outcome.

Clownfish and their associated sea anemones are icons of tropical and subtropical coastal ecosystems, but bleaching and collecting for the aquarium trade has reduced their population densities in several Indo-Pacific locations. We surveyed four Entacmaea quadricolor populations and their associated clownfish Amphiprion clarkii in Hong Kong. These assemblages represented a combination of exposed and sheltered, flat bottomed and sloped conditions. Different from tropical areas, where E. quadricolor is usually found in sheltered shallow waters, the four populations in Hong Kong were distributed in 4–10 m depths, being densest on a 40 × 80 m slope of volcanic rocks (34.2–76.6% substrate cover and 48.7–102.1 ind m−2) at an exposed site. This distribution pattern could be explained by the high turbidities associated with river discharge and sedimentation rates in local waters, which discourage the establishment of E. quadricolor populations in sheltered bays; shallow habitats might not be suitable for E. quadricolor in Hong Kong due to strong wave actions generated by typhoons. Amphiprion clarkii was present at low densities (0.07–0.26 fish m−2) across the sites. Our data provide a baseline for monitoring the changes of these conspicuous members of shallow water communities in Hong Kong.

It has long been recognised that the proportions of Neolithic domestic animal species—cattle, pig and sheep/goat—vary from region to region, but it has hitherto been unclear how much this variability is related to cultural practices or to environmental constraints. This study uses hundreds of faunal assemblages from across Neolithic Europe to reveal the distribution of animal use between north and south, east and west. The remarkable results present us with a geography of Neolithic animal society—from the rabbit-loving Mediterranean to the beef-eaters of the north and west. They also demonstrate that the choices made by early Neolithic herders were largely determined by their environments. Cultural links appear to have played only a minor role in the species composition of early Neolithic animal societies.

Photoluminescence and Electroluminescence Quenching in 8-Hydroxyquinoline Aluminum Chelates

The present communication proposes a mechanism for photoluminescence (PL) and electroluminescence (EL) quenching in aluminum(III) 8-hydroxyquinoline (Alq3) chelates. Our experiments indicate that in the presence of moisture, ligand exchange of 8-hydroxyquinoline with water can occur. At elevated temperatures, the liberated 8-hydroxyquinoline undergoes a condensation reaction in the presence of traces of oxygen, generating a dark, non-emissive polymeric compound. Spectroscopic and electrochemical methods were used to determine the band gap and energy levels (highest occupied molecular orbital (HOMO) and lowest unoccupied molecular orbital (LUMO)) of this byproduct. Steady state PL experiments indicate that (0.5 to 1%) concentration of this byproduct, evenly dispersed in Alq3 films, results in dramatic PL quenching. Deliberate insertion of an approximately 50 Å thick film of this byproduct into the interface of a poly(p-phenylenevinylene)/Alq3 light emitting diode (LED) completely quenches the EL as well. Initial data suggest an energy transfer quenching mechanism.
