Objective:
To determine the prevalence of severe acute respiratory coronavirus virus 2 (SARS-CoV-2) IgG nucleocapsid (N) antibodies among healthcare personnel (HCP) with no prior history of COVID-19 and to identify factors associated with seropositivity.
Design:Prospective cohort study.
Setting:An academic, tertiary-care hospital in St. Louis, Missouri.
Participants:The study included 400 HCP aged ≥18 years who potentially worked with coronavirus disease 2019 (COVID-19) patients and had no known history of COVID-19; 309 of these HCP also completed a follow-up visit 70–160 days after enrollment. Enrollment visits took place between September and December 2020. Follow-up visits took place between December 2020 and April 2021.
Methods:At each study visit, participants underwent SARS-CoV-2 IgG N-antibody testing using the Abbott SARS-CoV-2 IgG assay and completed a survey providing information about demographics, job characteristics, comorbidities, symptoms, and potential SARS-CoV-2 exposures.
Results:Participants were predominately women (64%) and white (79%), with median age of 34.5 years (interquartile range [IQR], 30–45). Among the 400 HCP, 18 (4.5%) were seropositive for IgG N-antibodies at enrollment. Also, 34 (11.0%) of 309 were seropositive at follow-up. HCP who reported having a household contact with COVID-19 had greater likelihood of seropositivity at both enrollment and at follow-up.
Conclusions:In this cohort of HCP during the first wave of the COVID-19 pandemic, ∼1 in 20 had serological evidence of prior, undocumented SARS-CoV-2 infection at enrollment. Having a household contact with COVID-19 was associated with seropositivity.

In this prospective, longitudinal study, we examined the risk factors for severe acute respiratory coronavirus virus 2 (SARS-CoV-2) infection among a cohort of chronic hemodialysis (HD) patients and healthcare personnel (HCPs) over a 6-month period. The risk of SARS-CoV-2 infection among HD patients and HCPs was consistently associated with a household member having SARS-CoV-2 infection.

In a prospective cohort of healthcare personnel (HCP), we measured severe acute respiratory syndrome coronavirus virus 2 (SARS-CoV-2) nucleocapsid IgG antibodies after SARS-CoV-2 infection. Among 79 HCP, 68 (86%) were seropositive 14–28 days after their positive PCR test, and 54 (77%) of 70 were seropositive at the 70–180-day follow-up. Many seropositive HCP (95%) experienced an antibody decline by the second visit.

Background
Growing evidence suggests that air pollution exposure may adversely affect the brain and increase risk for psychiatric disorders such as schizophrenia and depression. However, little is known about the potential role of air pollution in severity and relapse following illness onset.
AimsTo examine the longitudinal association between residential air pollution exposure and mental health service use (an indicator of illness severity and relapse) among individuals with first presentations of psychotic and mood disorders.
MethodWe identified individuals aged ≥15 years who had first contact with the South London and Maudsley NHS Foundation Trust for psychotic and mood disorders in 2008–2012 (n = 13 887). High-resolution (20 × 20 m) estimates of nitrogen dioxide (NO2), nitrogen oxides (NOx) and particulate matter (PM2.5 and PM10) levels in ambient air were linked to residential addresses. In-patient days and community mental health service (CMHS) events were recorded over 1-year and 7-year follow-up periods.
ResultsFollowing covariate adjustment, interquartile range increases in NO2, NOx and PM2.5 were associated with 18% (95% CI 5–34%), 18% (95% CI 5–34%) and 11% (95% CI 3–19%) increased risk for in-patient days after 1 year. Similarly, interquartile range increases in NO2, NOx, PM2.5 and PM10 were associated with 32% (95% CI 25–38%), 31% (95% CI 24–37%), 7% (95% CI 4–11%) and 9% (95% CI 5–14%) increased risk for CMHS events after 1 year. Associations persisted after 7 years.
ConclusionsResidential air pollution exposure is associated with increased mental health service use among people recently diagnosed with psychotic and mood disorders. Assuming causality, interventions to reduce air pollution exposure could improve mental health prognoses and reduce healthcare costs.

Recent advances in research tools for the systematic analysis of textual data are enabling exciting new research throughout the social sciences. For comparative politics, scholars who are often interested in non-English and possibly multilingual textual datasets, these advances may be difficult to access. This article discusses practical issues that arise in the processing, management, translation, and analysis of textual data with a particular focus on how procedures differ across languages. These procedures are combined in two applied examples of automated text analysis using the recently introduced Structural Topic Model. We also show how the model can be used to analyze data that have been translated into a single language via machine translation tools. All the methods we describe here are implemented in open-source software packages available from the authors.

Changing environmental conditions in the Canadian Arctic are associated with an increase in marine tourism. A substantial decline in the extent of ice coverage in the summer season has resulted in greater accessibility for all categories of ships, and the tourism sector has been quick to respond to new opportunities. This increase in vessel traffic has raised significant issues for management, and particular concerns about the pleasure craft (non-commercial tourism) sector. This paper reports on research aimed at identifying change in the pleasure craft sector in Canadian Arctic waters since 1990; exploring management concerns held by stakeholders regarding changes in the sector; and, providing recommendations for government stakeholders. The paper is based on material gathered through an examination of existing data sources and stakeholder interviews (n = 22). Analysis was aimed at understanding the rapid development of the sector and potential management strategies, including research needs. Analysis reveals a dramatic increase in annual vessel numbers, particularly from 2010 onwards. Management concerns of interviewees relate to implications of this growth in four areas: visitor behaviour; services, facilities and infrastructure; control; and, planning and development. The paper concludes by describing recommendations in the areas of research needs, regulation, and strategic development.

Background:
Original studies published over the last decade regarding time trends in dementia report mixed results. The aims of the present study were to use linked administrative health data for the province of Saskatchewan for the period 2005/2006 to 2012/2013 to: (1) examine simultaneous temporal trends in annual age- and sex-specific dementia incidence and prevalence among individuals aged 45 and older, and (2) stratify the changes in incidence over time by database of identification.
Methods:Using a population-based retrospective cohort study design, data were extracted from seven provincial administrative health databases linked by a unique anonymized identification number. Individuals 45 years and older at first identification of dementia between April 1, 2005 and March 31, 2013 were included, based on case definition criteria met within any one of four administrative health databases (hospital, physician, prescription drug, and long-term care).
Results:Between 2005/2006 and 2012/2013, the 12-month age-standardized incidence rate of dementia declined significantly by 11.07% and the 12-month age-standardized prevalence increased significantly by 30.54%. The number of incident cases decreased from 3,389 to 3,270 and the number of prevalent cases increased from 8,795 to 13,012. Incidence rate reductions were observed in every database of identification.
Conclusions:We observed a simultaneous trend of decreasing incidence and increasing prevalence of dementia over a relatively short 8-year time period from 2005/2006 to 2012/2013. These trends indicate that the average survival time of dementia is lengthening. Continued observation of these time trends is warranted given the short study period.
Radiofrequency ablation versus cryoablation for atrioventricular nodal re-entrant tachycardia in children: a value comparison

Background
It is unclear whether cryoablation or radiofrequency ablation offers better value for treating atrioventricular nodal re-entrant tachycardia in children. We aimed to compare the value of these procedures for treating atrioventricular nodal re-entrant tachycardia in children, with value being outcomes relative to costs.
MethodsWe performed a retrospective cohort study of all atrioventricular nodal re-entrant tachycardia ablations for children (age⩽18 years) from July, 2009 to June, 2011 at our institution. Costs included fixed costs, miscellaneous hospital costs, and labour costs, and key outcomes were acute and long-term success (6 months) of the ablations. We conducted T-tests and regression analyses to investigate the associations between the ablation procedure type and the cost and success of the ablations.
ResultsOf 96 unique cases performed by three paediatric electrophysiologists, 48 were cryoablation only, 42 radiofrequency ablation only, and six were a combination. Acute success was 100% for the cryoablation only and radiofrequency ablation only cases and 83% for the combination cases. There were no notable adverse events. The average total cost was $9636 for cryoablation cases, $9708 for radiofrequency ablation cases, and $10,967 for combination cases (p=0.51 for cryoablation only versus radiofrequency ablation only). The long-term success rate was 79.1% for cryoablation only, 92.8% for radiofrequency ablation only, and 66.7% for the combination (p=0.01 for cryoablation only versus radiofrequency ablation only), but long-term success varied notably by provider.
ConclusionsCryoablation and radiofrequency ablation offer similar value in the short term for the treatment of atrioventricular nodal re-entrant tachycardia in children. Differences in long-term success may vary substantially by physician, and thus may lead to differences in long-term value.

Summary
INTRODUCTION
Each day humans generate massive volumes of data in a variety of different forms (Lazer et al., 2009). For example, digitized texts provide a rich source of political content through standard media sources such as newspapers, as well as newer forms of political discourse such as tweets and blog posts. In this chapter we analyze a corpus of 13,246 posts that were written for six political blogs during the course of the 2008 U.S. presidential election. But this is just one small example. An aggregator of nearly every document produced by the U.S. federal government, voxgov.com, has collected more than eight million documents from 2010–2014, including over a million tweets from members of Congress. These data open new possibilities for studies of all aspect of political life from public opinion (Hopkins and King, 2010) to political control (King, Pan, and Roberts, 2013) to political representation (Grimmer, 2013).
The explosion of new sources of political data has been met by the rapid development of new statistical tools for meeting the challenges of analyzing “big data.” (National Research Council, 2013; Grimmer and Stewart, 2013; Fan, Han, and Liu, 2014). A prominent example in the field of text analysis is latent Dirichlet allocation (LDA) (Blei, Ng, and Jordan, 2003; Blei, 2012), a topic model that uses patterns of word co-occurrences to discover latent themes across documents. Topic models can help us deal with the reality that large data sets of text are also typically unstructured. In this chapter we focus on a particular variant of LDA, the structural topic model (STM) (Roberts et al., 2014), which provides a framework to relate the corpus structure we do have (in the form of document-level metadata) with the inferred topical structure of the model.
Techniques for automated text analysis have been thoroughly reviewed elsewhere (Grimmer and Stewart, 2013).We instead focus on a less often discussed feature of topic models and of latent variable models more broadly: multimodality. That is, the models discussed here give rise to optimization problems that are nonconvex. Thus, unlike workhorse tools such as linear regression, the solution we find can be sensitive to our starting values (in technical parlance, the function we are optimizing has multiple modes). We engage directly with this issue of multimodality, helping the reader understand why it arises and what can be done about it.

Researchers have long been interested in whether particular temperamental traits in childhood connote risk for depressive disorders. For example, children characterized as having high negative emotionality (NE; sadness, fear, anger) and low positive emotionality (PE; anhedonia, listlessness, and lack of enthusiasm) are hypothesized to be at risk for depression. Few studies, however, have examined whether (and how) these two temperamental dimensions interact to confer risk. In a sample of 329 preschoolers, the present study addressed this question by examining the relation between PE and NE and asymmetry in resting EEG activity in frontal and posterior regions, which are putative biomarkers for depression. Using a laboratory battery to define temperament, we found an interaction of PE and NE on posterior asymmetry. Specifically, when PE was high, NE was associated with greater relative right activity. When PE was low, NE was not related to posterior asymmetry. These results were driven by differences in EEG activity in right posterior regions, an area associated with emotional processing and arousal, and were specific to girls. We found no relation between temperament and frontal asymmetry. These findings suggest that, at least for girls, PE and NE may have an interactive effect on risk for depression.

In response to Canada's pressing need for effective evidence-based policy, services, and practices specific to seniors, national leaders representing all concerned stakeholders designed and implemented a National Consensus Process to promote spread, exchange, choice, and uptake of research evidence on social and health issues associated with an aging population. This article presents the innovative methods and evaluation of this three-year project, illuminating for all constituencies the challenges and opportunities associated with promoting seniors' independence through collaborative knowledge transfer efforts. A total of 198 organizations and 65 individuals were surveyed at baseline, throughout the intervention, immediately post-intervention, and one year post-intervention. Knowledge from 783 studies was spread to 63,387 people, 90 per cent of whom reported knowledge exchange. Over 50 per cent of stakeholders reported using the research evidence, although processes for facilitating knowledge choice did not achieve consensus. Significant knowledge uptake occurred in two of the four research theme areas.

The resistance of spores of B. subtilis var. niger produced in liquid synthetic medium and exposed to ethylene oxide on a nylon surface, has been shown to be almost identical to that for spores produced on a traditional solidified complex medium with exposure to the sterilant on aluminium foil. The use of short lengths of nylon tube as carriers allowed easy production and handling, with self-protection of the spore-bearing surface. Addition of a dye provided visual evidence of inoculation without affecting resistance to ethylene oxide. Such a monitor is suitable for use as a standardized biological challenge in routine ethylene oxide sterilization cycles.
