This preregistered study replicates and extends studies concerning emotional response to wartime rally speeches and applies it to U.S. President Donald Trump’s first national address regarding the COVID-19 pandemic on March 11, 2020. We experimentally test the effect of a micro-expression (ME) by Trump associated with appraised threat on change in participant self-reported distress, sadness, anger, affinity, and reassurance while controlling for followership. We find that polarization is perpetuated in emotional response to the address which focused on portraying the COVID-19 threat as being of Chinese provenance. We also find a significant, albeit slight, effect by Trump’s ME on self-reported sadness, suggesting that this facial behavior served did not diminish his speech, instead serving as a form of nonverbal punctuation. Further exploration of participant response using the Linguistic Inventory and Word Count software reinforces and extends these findings.

The rise of climate change on the global political agenda coincided with the growth of partisan polarization in US politics and, in many ways, their trajectories mirror one another. When the climate crisis first began to attract political attention 30 years ago, Republicans and Democrats responded with similar levels of interest and concern. Today, partisan division overwhelms all other aspects of climate-change politics and environmental politics more broadly (Egan, Konisky, and Mullin 2022; Egan and Mullin 2017).

Background: Patients presenting to hospitals often arrive with peripherally inserted central catheters (PICC) in place upon admission. The admitting facility may not be familiar with that device’s history and the unknown risk for bloodstream infection associated with it often prompts requests for device replacement. A blanket approach to “change all lines” must be balanced with the potential for patient discomfort and insertion-related complications. To better inform our approach to prevention, we determined the incidence of central-line–associated bloodstream infection (CLABSI) in adult patients presenting to hospitals in our health system with a PICC present on admission (POA), relative to those who have a PICC placed after admission (PAA). Methods: This retrospective cohort study included all adult hospital encounters at 11 Cleveland Clinic acute-care hospitals lasting > 2 days in 2018 with electronic medical record nursing care flowsheet documentation of a PICC during the stay. Patients whose admission diagnosis was related to intravascular catheter infection, children aged <18 years, and observation unit encounters were excluded. Patients were categorized as having a PICC POA if a nurse selected that option on a PICC flowsheet, otherwise the patient was categorized has having a PICC PAA. Surveillance for CLABSI was performed in all inpatient locations at all hospitals according to the NHSN protocol. Patients with ≥1 CLABSI were matched to encounters by name and date of admission. Repeat infections occurring to the same patient were excluded. Results: Of the 8,827 eligible hospital encounters, 1,799 (20%) involved a PICC POA and 7,028 (80%) had PICCs PAA. Across 11 hospitals, the median proportion of PICC-associated encounters with a device POA was 15% (range, 8%–25%). Moreover, 23 of the 112 CLABSIs (21%) in our cohort occurred in patients with a PICC POA and 89 (79%) occurred in patients with a PICC PAA (Table 1). The overall relative risk of CLABSI, whether the PICC was placed before or after admission, was 1.00 (95% CI, 0.64–1.60). Conclusions: Patients with a PICC present on admission to our hospitals were no more likely to experience a CLABSI than patients who had a PICC placed after admission. Replacing vascular catheters that are POA may not reduce the risk of CLABSI. With up to 25% of PICC-associated encounters having the device POA, universal device replacement at admission would involve hundreds of patients per year at our multihospital health system.

Summary
A valuable though unintended consequence of the important debate now under way on the future of Australian cities, particularly on how to plan metropolitan areas, is the way it has highlighted how little we know about the role played by households in urban development. Surprisingly little is published on the household's place in urban social structures, both in Australia and elsewhere, and even less is available on the way households act as a force to help form and transform cities and towns. With the household being the major institution upon which ‘social’ (as against ‘economic’ and ‘political’) aspects of urban growth and change are based, knowing little about this institution inevitably thwarts a fuller understanding of urban development and this, in turn, blocks the formulation of more adequate urban planning.
Such ignorance is particularly disconcerting at this time because the current debate on Australian cities cites the actions of households as a major cause of today's urban problems. This is especially apparent in the urban consolidation debate, where advocates of the policy are expressing alarm at the continuing movement of large numbers of households to the metropolitan fringe and at their disinterest in living in well-established, well-serviced, but depopulating, inner and middle suburbs.

Various benefits claimed for methadone maintenance in the treatment of opioid drug takers are reviewed. It is said to stop illicit drug use, maintain treatment contact, reduce morbidity, mortality and crime, and improve social adjustment. Little firm evidence is found to support these claims. Results are reported comparing 26 drug takers on methadone prescriptions with 16 illicit opioid takers. The measures used are Stimson's (1972) ‘Patterns of Behaviour’ questionnaire, and direct behavioural measures of social functioning. The groups are found not to differ in terms of treatment contact, work status and involvement with other drug takers. There is some evidence that the methadone group is involved in less criminal activity. However, no association is found between amounts of drugs prescribed and numbers of local pharmacy thefts. It is concluded that the benefits of methadone maintenance have been exaggerated.
