Structured Psychological Support for people with personality disorder: feasibility randomised controlled trial of a low-intensity intervention

Background
National guidance cautions against low-intensity interventions for people with personality disorder, but evidence from trials is lacking.
AimsTo test the feasibility of conducting a randomised trial of a low-intensity intervention for people with personality disorder.
MethodSingle-blind, feasibility trial (trial registration: ISRCTN14994755). We recruited people aged 18 or over with a clinical diagnosis of personality disorder from mental health services, excluding those with a coexisting organic or psychotic mental disorder. We randomly allocated participants via a remote system on a 1:1 ratio to six to ten sessions of Structured Psychological Support (SPS) or to treatment as usual. We assessed social functioning, mental health, health-related quality of life, satisfaction with care and resource use and costs at baseline and 24 weeks after randomisation.
ResultsA total of 63 participants were randomly assigned to either SPS (n = 33) or treatment as usual (n = 30). Twenty-nine (88%) of those in the active arm of the trial received one or more session (median 7). Among 46 (73%) who were followed up at 24 weeks, social dysfunction was lower (−6.3, 95% CI −12.0 to −0.6, P = 0.03) and satisfaction with care was higher (6.5, 95% CI 2.5 to 10.4; P = 0.002) in those allocated to SPS. Statistically significant differences were not found in other outcomes. The cost of the intervention was low and total costs over 24 weeks were similar in both groups.
ConclusionsSPS may provide an effective low-intensity intervention for people with personality disorder and should be tested in fully powered clinical trials.

Background
Different diagnostic interviews are used as reference standards for major depression classification in research. Semi-structured interviews involve clinical judgement, whereas fully structured interviews are completely scripted. The Mini International Neuropsychiatric Interview (MINI), a brief fully structured interview, is also sometimes used. It is not known whether interview method is associated with probability of major depression classification.
AimsTo evaluate the association between interview method and odds of major depression classification, controlling for depressive symptom scores and participant characteristics.
MethodData collected for an individual participant data meta-analysis of Patient Health Questionnaire-9 (PHQ-9) diagnostic accuracy were analysed and binomial generalised linear mixed models were fit.
ResultsA total of 17 158 participants (2287 with major depression) from 57 primary studies were analysed. Among fully structured interviews, odds of major depression were higher for the MINI compared with the Composite International Diagnostic Interview (CIDI) (odds ratio (OR) = 2.10; 95% CI = 1.15–3.87). Compared with semi-structured interviews, fully structured interviews (MINI excluded) were non-significantly more likely to classify participants with low-level depressive symptoms (PHQ-9 scores ≤6) as having major depression (OR = 3.13; 95% CI = 0.98–10.00), similarly likely for moderate-level symptoms (PHQ-9 scores 7–15) (OR = 0.96; 95% CI = 0.56–1.66) and significantly less likely for high-level symptoms (PHQ-9 scores ≥16) (OR = 0.50; 95% CI = 0.26–0.97).
ConclusionsThe MINI may identify more people as depressed than the CIDI, and semi-structured and fully structured interviews may not be interchangeable methods, but these results should be replicated.

Managing Alien Plants for Biodiversity Outcomes—the Need for Triage

Recognition that alien plants pose a significant threat to biodiversity has not always translated into effective management strategies, policy reforms, and systems to establish priorities. Thus, many alien plant management decisions for the protection of biodiversity occur with limited knowledge of what needs to be protected (other than biodiversity in a generalized sense) or the urgency of actions. To rectify this, we have developed a triage system that enables alien plant management decisions to be made based on (1) the urgency of control relative to the degree of threat posed to biodiversity, compared with (2) the likelihood of achieving a successful conservation outcome as a result of alien plant control. This triage system is underpinned by a two-step approach, which identifies the biodiversity at risk and assesses sites to determine priorities for control. This triage system was initially developed to manage the threat posed by bitou bush to native species in New South Wales (NSW), Australia. It has subsequently been improved with the national assessment of lantana in Australia, and the adaptation from a single to multiple alien plant species approach on a regional scale. This triage system identifies nine levels of priority for alien plant management aimed at biodiversity conservation, ranging from immediate, targeted action to limited or no action. The development of this approach has enabled long-term management priorities to be set for widespread alien plants that are unlikely to be eradicated. It also enables control to occur in a coordinated manner for biodiversity conservation at a landscape scale, rather than as a series of individual unconnected short-term actions.

We present an update of the ‘key points’ from the Antarctic Climate Change and the Environment (ACCE) report that was published by the Scientific Committee on Antarctic Research (SCAR) in 2009. We summarise subsequent advances in knowledge concerning how the climates of the Antarctic and Southern Ocean have changed in the past, how they might change in the future, and examine the associated impacts on the marine and terrestrial biota. We also incorporate relevant material presented by SCAR to the Antarctic Treaty Consultative Meetings, and make use of emerging results that will form part of the Intergovernmental Panel on Climate Change (IPCC) Fifth Assessment Report.

The land crab, Discoplax celeste (Gecarcinidae) is endemic to Christmas Island in the Indian Ocean. Due to a freshwater-dependant life history, in which the megalopae migrate from the ocean up freshwater streams to their adult terrestrial/freshwater habitat, D. celeste inhabits only a few isolated locations on the island. This restricted distribution is one of a number of factors which has previously highlighted the vulnerability of this species to outside threats. A number of anthropogenic factors including the introduction of multiple invasive species and habitat destruction have led to drastic ecosystem change on Christmas Island. The aim of this study was to investigate whether the restricted geographical distributions of D. celeste populations contribute to significant genetic structuring across Christmas Island, with an objective to inform future conservation strategies for this species on Christmas Island. Fragments of the mitochondrial cytochrome oxidase I gene and the control region were sequenced from 95 individuals collected from all five locations on Christmas Island known to be inhabited by D. celeste. Analyses using analysis of molecular variance revealed no evidence of population sub-structuring, indicating that despite any geographical isolation, there is a single population of D. celeste on Christmas Island. This lack of population differentiation is probably explained by the oceanic dispersal of larvae, rather than terrestrial migration of D. celeste. Therefore, based on these results, for conservation purposes, D. celeste on Christmas Island can be considered a single management unit.

Bulk silicon-germanium (SiGe) alloys and two SiGe thick films (4 and 5 μm) on Si wafers were tested with the electron probe microanalyzer (EPMA) using wavelength dispersive spectrometers (WDS) for heterogeneity and composition for use as reference materials needed by the microelectronics industry. One alloy with a nominal composition of Si0.86Ge0.14 and the two thick films with nominal compositions of Si0.90Ge0.10 and Si0.75Ge0.25 on Si, evaluated for micro- and macroheterogeneity, will make good microanalysis reference materials with an overall expanded heterogeneity uncertainty of 1.1% relative or less for Ge. The bulk Ge composition in the Si0.86Ge0.14 alloy was determined to be 30.228% mass fraction Ge with an expanded uncertainty of the mean of 0.195% mass fraction. The thick films were quantified with WDS-EPMA using both the Si0.86Ge0.14 alloy and element wafers as reference materials. The Ge concentration was determined to be 22.80% mass fraction with an expanded uncertainty of the mean of 0.12% mass fraction for the Si0.90Ge0.10 wafer and 43.66% mass fraction for the Si0.75Ge0.25 wafer with an expanded uncertainty of the mean of 0.25% mass fraction. The two thick SiGe films will be issued as National Institute of Standards and Technology Reference Materials (RM 8905).

The mechanical properties of healthy and diseased bone tissue were extensively studied in mechanical tests. Most of this research was motivated by the immense costs of health care and social impacts due to osteoporosis in post-menopausal women and the aged. Osteoporosis results in bone loss and change of trabecular architecture, causing a decrease in bone strength. To address the problem of assessing local failure behavior of bone, we combined mechanical compression testing of trabecular bone samples with high-speed photography. In this exploratory study, we investigated healthy, osteoarthritic, and osteoporotic human vertebral trabecular bone compressed at high strain rates. Apparent strains were found to transfer into to a broad range of local strains. Strained trabeculae were seen to whiten with increasing strain. Comparison of whitened regions seen in high-speed photography sequences with scanning electron micrographs showed that the observed whitening was due to the formation of microcracks. From the results of a motion energy filter applied to the recorded movies, we saw that the whitened areas are, presumably, also areas of high deformation. In summary, high-speed photography allows the detection of microdamage in real time, leading toward a better understanding of the local processes involved in bone failure.

From the results of a motion energy filter applied to the recorded movies, we saw that the whitened areas are, presumably, also of high deformation. We believe that this method will allow further insights into bone failure mechanisms, and help toward a better understanding of the processes involved in bone failure.
Limited Impact of Sustained Simple Feedback Based on Soap and Paper Towel Consumption on the Frequency of Hand Washing in an Adult Intensive Care Unit

Objective:
To determine whether hand washing would increase with sustained feedback based on measurements of soap and paper towel consumption.
Design:Prospective trial with a nonequivalent control group.
Setting:Open multibed rooms in the Omaha Veterans Affairs Medical Center's Surgical Intensive Care Unit (SICU) and Medical Intensive Care Unit (MICU).
Subjects:Unit staff.
Intervention:Every weekday from May 26 through December 8,1998, we recorded daytime soap and paper towel consumption, nurse staffing, and occupied beds in the SICU (intervention unit) and the MICU (control unit) and used these data to calculate estimated hand washing episodes (EHWEs), EHWEs per occupied bed per hour, and patient-to-nurse ratios. In addition, from May 26 through June 26 (baseline period) and from November 2 through December 8 (follow-up period), live observers stationed daily for random 4-hour intervals in the MICU and the SICU counted actual hand washing episodes (CHWEs). The intervention consisted of posting in the SICU, but not in the MICU, a graph showing the weekly EHWEs per occupied bed per hour for the preceding 5 weeks.
Results:Directly counted hand washing fell in the SICU from a baseline of 2.68 ± 0.72 (mean ± standard deviation) episodes per occupied bed per hour to 1.92 ± 1.35 in the follow-up period. In the MICU, episodes fell from 2.58 ± 0.95 (baseline) to 1.74 ± 0.69. In the MICU, the withdrawal of live observers was associated with a decrease in estimated episodes from 1.36 ± 0.49 at baseline to 1.01 ± 0.36, with a return to 1.16 ± 0.50 when the observers returned. In the SICU, a similar decrease did not persist throughout a period of feedback. Estimated hand washing correlated negatively with the patient-to-nurse ratio (r = -0.35 for the MICU, r = -0.46 for the SICU).
Conclusions:Sustained feedback on hand washing failed to produce a sustained improvement. Live observers were associated with increased hand washing, even when they did not offer feedback. Hand washing decreased when the patient-to-nurse ratio increased.
