In this paper we review the design and development of a 100 J, 10 Hz nanosecond pulsed laser, codenamed DiPOLE100X, being built at the Central Laser Facility (CLF). This 1 kW average power diode-pumped solid-state laser (DPSSL) is based on a master oscillator power amplifier (MOPA) design, which includes two cryogenic gas cooled amplifier stages based on DiPOLE multi-slab ceramic Yb:YAG amplifier technology developed at the CLF. The laser will produce pulses between 2 and 15 ns in duration with precise, arbitrarily selectable shapes, at pulse repetition rates up to 10 Hz, allowing real-time shape optimization for compression experiments. Once completed, the laser will be delivered to the European X-ray Free Electron Laser (XFEL) facility in Germany as a UK-funded contribution in kind, where it will be used to study extreme states of matter at the High Energy Density (HED) instrument.

Environmental issues have been politically salient in Queensland throughout recent decades of Labor dominance. While in general Labor has benefited politically from being regarded as the ‘greener’ of the major parties, state Labor governments have also sometimes found environmental policy difficult to deal with. This is partly a reflection of the political passions that environmental and resource use conflicts can generate and is partly due to the pressures on environmental assets from population and economic growth in Queensland; primarily, however, it reflects the difficulty of reconciling environmental and sustainability policy goals with the historically dominant model of development in Queensland.

A community survey and subsequent clinical assessment of 192 Cree aged 65 years and over registered in two Reserves in Northern Manitoba identified only one case of probable Alzheimer's disease among eight cases of dementia, giving a prevalence of 0.5% for Alzheimer's disease and 4.2% for all dementias. This contrasted with an age-adjusted prevalence of 3.5% for Alzheimer's disease and 4.2% for all dementias in an age-stratified sample of 241 English-speaking residents of Winnipeg. Although it was not so for all dementias, the difference between the groups for prevalence of Alzheimer's disease was highly significant (p < .001). The age-specific patterns of all dementias in the two groups were significantly different, however (p = .0254).

Summary
Introduction
Miss Lake attends complaining of a third episode of sore throat within the period of a year. Before making the appointment she had had an argument with her mother, a chronic invalid, who told her there was nothing wrong with her. Under other circumstances Miss Lake might have struggled on, but fed up with her mother's demands and stung by her lack of sympathy, she presents herself to the doctor in pursuit of the barely conscious goals of getting the sympathy she feels she deserves and at the same time arming herself for revenge on her mother with the proof that she is genuinely ill.
Clinical transactions between patient and doctor do not take place in a vacuum. Each participant is influenced in sometimes subtle and, at other times, penetrating ways by factors that have their origins in contexts external to the transaction itself and may serve to make it complicated. Of these contexts, multiple and overlapping, within which doctor and patient operate, some will be shared and others not. Community, race, gender, social class, position in family, for example, all form contexts that may bring influences to bear on patient and doctor in terms of the dynamics of their interaction. At the public level these might include the shared values and expectations conferred by the social roles of doctor and patient. But those expectations and values will themselves be modified in quality and degree by the personal contexts of each participant, by any idiosyncratic, eccentric or unconventional aspects of doctor and patient as individuals.

Introduction
Mrs Black, a mother of three children and a part-time telephonist, developed a painful throat and swollen glands during the course of an evening. She had a headache and fever, for which she took paracetamol. The next day her symptoms were more severe and, unable to work, she decided to consult her doctor, making an appointment to do so. When Mrs Black was seen and examined, the doctor, who knew her well and liked her, diagnosed acute tonsillitis and, communicating this to her, prescribed an appropriate antibiotic. Mrs Black complied with this treatment and recovered over the next few days.
This simple vignette is a common enough example of a clinical transaction in primary care with an outcome satisfactory to both doctor and patient. The term ‘clinical transaction’ is introduced here to refer specifically to the two-person, interactive system of doctor and patient. It embraces both the public and personal domains of their interaction. To ‘transact’ means: ‘to carry through negotiations: to have dealings; do business; to treat; to manage or settle affairs’ (Shorter Oxford English dictionary). A clinical transaction, therefore, is defined as: an appropriately negotiated, goal-orientated interaction between doctor and patient, comprising both public and personal components.
The process of the clinical transaction, considered as a negotiation, implies that patient and doctor share a more equal status than that of the traditionally portrayed doctor-patient interaction, even if certain inequalities remain. This differs from the traditional conception of the consultation, wherein the patient is regarded as a relatively passive recipient of advice or treatment.

Introduction
Potentially, the doctor–patient interaction, being an open system, is crucially affected by its immediate physical, emotional and social environment. In this chapter, aspects of the structure and organisation of the wider clinical setting are examined, as they may influence the clinical transaction so as to increase the risk of complication. Certain features of clinics are shared by both primary care and hospital-based out-patient settings. However, issues of size and scale are important and not all the aspects considered apply equally to each setting. Clinical transactions, especially complicated ones, influence the function of the supra-system of the clinic. Thus, the relationship between clinical transaction and the wider clinical context is interactive. Ways in which this interaction can be manifest are examined. Organisational structures that can reduce the potential for complicated transactions are described.
The physical setting
The physical setting of clinics forms part of the ‘frame’ in which the clinical transaction unfolds and should be acknowledged as being part of the therapist's communication with the patient (Langs, 1978). The setting not only communicates something attributable to the particular doctors who work there but it also conveys something about the system of health care as a whole, of which the clinic is only a part or subsystem. This potentially powerful form of non-verbal communication therefore makes its statement and has its effect at both public and personal levels.
The physical aspects of a clinical setting include the buildings and their location, the external and internal decoration, and the arrangements for reception, waiting areas, consulting and treatment rooms.

Summary
I have spent more than forty years in clinical practice, nearly all of that time as a general practitioner. The arena in which I perform (compete?) as a general practitioner is the clinical consultation, and the environment in that arena depends upon the nature and quality of my relationship with each person who consults me. The study of the process of consultations and of the quality of doctor–patient relationships is, then, a proper occupation for any clinician, but most especially for a generalist. The motivation for undertaking such study is a pervasive sense of discomfort experienced by the empathic doctor who has just conducted an unsatisfactory (unsatisfying?) encounter with a patient. For the patient there is escape into the real world since his or her performance in consultation is not a professional obligation. As one humorist worded it: ‘My doctor is nice; every time I see him I feel ashamed of what I think of doctors in general.’
As generation succeeds generation, the proper study of humans produces fresh theories and models to explain human behaviour in dyads and in groups large and small. Kingsley Norton and Sam Smith have taken human encounters and applied to them an appropriate range of new ideas and old. You may share in my experience of pleasure when I survive a difficult consultation without harming the patient or of dealing with my lack of satisfaction without being punitive to client or staff. Definition
According to the International Classification of Diseases (WHO, 1992), personality disorder refers to conditions which comprise:
deeply ingrained and enduring behaviour patterns, manifesting themselves as inflexible responses to a broad range of personal and social situations. They represent either extreme or significant deviations from the way the average individual in a given culture perceives, thinks, feels and particularly relates to others. Such behaviour patterns tend to be stable and to encompass multiple domains of behaviour and psychological functioning. They are frequently, but not always, associated with various degrees of subjective distress and problems in social functioning and performance.
The other major classification scheme – Diagnostic and statistical manual of mental disorders(APA, 1987) – utilises very similar criteria. Thus, personality disorder is defined as:
behaviours or traits that are characteristic of the person's recent (past year) and long-term functioning (generally since adolescence or early adulthood). The constellation of behaviours or traits causes either significant impairment in social or occupational functioning or subjective distress. Behaviours or traits limited to episodes of illness are not considered in making a diagnosis of Personality Disorder.
Personality disorder subtypes
Subclassification is currently limited to the description of a series of types and subtypes that are not mutually exclusive and that overlap in some of their characteristics. In making a diagnosis of personality disorder, the clinician should consider all aspects of personal functioning, although the diagnostic formulation will refer only to those dimensions or traits for which suggested thresholds for severity are reached. The assessment should be based, ideally, on as many sources of information as possible.

Introduction
The majority of clinical transactions in primary care settings are routine and straightforward enough to warrant no special attention or modification to their management. The professional relationship seems able to cope with any tendency to wander away from important clinical goals, so that whatever else is also dealt with in the transaction, these are not neglected. But probably at least once in every clinic list the doctor will be left after a consultation with the uncomfortable feeling that something, even if it cannot be exactly defined, was not as it should have been. Sometimes, whole clinic lists can feel like this. Afterwards, the doctor might resentfully think that had it not been for a particular patient all would have been well. Was it, however, the patient's unreasonable behaviour or the receptionist's interruption in the previous consultation or the quarrel before leaving home, that sowed the seed of discontent?
The clinical transaction represents a process that takes place over time, either within a single consultation, or over more prolonged periods and multiple meetings. The doctor must be aware, therefore, of the particular stage of the transaction at any given point, since this bears on the requirements of the interaction (public and personal) at that moment. In the early stages of the transaction, establishing a rapport and a meaningful therapeutic alliance with the patient are important tasks to be addressed. Later, the need to impart information or instructions may take precedence. Later still, disengagement and any associated difficulties with endings may become issues that need attention. To an extent, the shorter the overall length of the transaction the more important are its beginning and ending.

The best kind of patient... is one who from great suffering and danger of life or sanity responds quickly to a treatment that interests his doctor and thereafter remains completely well; but those who recover only slowly or incompletely are less satisfying.

Mr Taylor entered the consulting room almost stealthily and took his seat. The doctor's greeting was received silently with a somewhat jeering smile. Mr Taylor had a shifty appearance, accentuated by the wandering of his gaze, which was hardly ever allowed to engage the doctor's. He began to detail his minor complaints, which were numerous. The doctor, looking at his own notes, saw uncomfortably that Mr Taylor was regularly prescribed five different medications, three of which might potentially interact in a harmful way. As he listened, endeavouring to make contact with Mr Taylor's wandering gaze, the doctor's shoulders drooped as if in resignation. From long experience he sensed nothing would change as a result of this clinical encounter. Mr Taylor left with a prescription to continue all five drugs, a prescription written guiltily and received with an almost sadistic smile, the only steady eye contact of the consultation.
The above clinical vignette based on an actual doctor–patient interaction (videotaped and discussed by trainer and trainee) represents a consultation with which most doctors are familiar. Mr Taylor is a so-called ‘problem patient’. He does not appear to experience great suffering and certainly does not have a life-threatening disease. In spite of repeated consultations little progress is made in alleviating his multiple complaints.
