The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.
Risk of Surgical Site Infection (SSI) following Colorectal Resection Is Higher in Patients With Disseminated Cancer: An NCCN Member Cohort Study

BACKGROUND
Surgical site infections (SSIs) following colorectal surgery (CRS) are among the most common healthcare-associated infections (HAIs). Reduction in colorectal SSI rates is an important goal for surgical quality improvement.
OBJECTIVETo examine rates of SSI in patients with and without cancer and to identify potential predictors of SSI risk following CRS
DESIGNAmerican College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) data files for 2011–2013 from a sample of 12 National Comprehensive Cancer Network (NCCN) member institutions were combined. Pooled SSI rates for colorectal procedures were calculated and risk was evaluated. The independent importance of potential risk factors was assessed using logistic regression.
SETTINGMulticenter study
PARTICIPANTSOf 22 invited NCCN centers, 11 participated (50%). Colorectal procedures were selected by principal procedure current procedural technology (CPT) code. Cancer was defined by International Classification of Disease, Ninth Revision, Clinical Modification (ICD-9-CM) codes.
MAIN OUTCOMEThe primary outcome of interest was 30-day SSI rate.
RESULTSA total of 652 SSIs (11.06%) were reported among 5,893 CRSs. Risk of SSI was similar for patients with and without cancer. Among CRS patients with underlying cancer, disseminated cancer (SSI rate, 17.5%; odds ratio [OR], 1.66; 95% confidence interval [CI], 1.23–2.26; P=.001), ASA score ≥3 (OR, 1.41; 95% CI, 1.09–1.83; P=.001), chronic obstructive pulmonary disease (COPD; OR, 1.6; 95% CI, 1.06–2.53; P=.02), and longer duration of procedure were associated with development of SSI.
CONCLUSIONSPatients with disseminated cancer are at a higher risk for developing SSI. ASA score >3, COPD, and longer duration of surgery predict SSI risk. Disseminated cancer should be further evaluated by the Centers for Disease Control and Prevention (CDC) in generating risk-adjusted outcomes.

We describe the design and performance of the Engineering Development Array, which is a low-frequency radio telescope comprising 256 dual-polarisation dipole antennas working as a phased array. The Engineering Development Array was conceived of, developed, and deployed in just 18 months via re-use of Square Kilometre Array precursor technology and expertise, specifically from the Murchison Widefield Array radio telescope. Using drift scans and a model for the sky brightness temperature at low frequencies, we have derived the Engineering Development Array’s receiver temperature as a function of frequency. The Engineering Development Array is shown to be sky-noise limited over most of the frequency range measured between 60 and 240 MHz. By using the Engineering Development Array in interferometric mode with the Murchison Widefield Array, we used calibrated visibilities to measure the absolute sensitivity of the array. The measured array sensitivity matches very well with a model based on the array layout and measured receiver temperature. The results demonstrate the practicality and feasibility of using Murchison Widefield Array-style precursor technology for Square Kilometre Array-scale stations. The modular architecture of the Engineering Development Array allows upgrades to the array to be rolled out in a staged approach. Future improvements to the Engineering Development Array include replacing the second stage beamformer with a fully digital system, and to transition to using RF-over-fibre for the signal output from first stage beamformers.

Low heart rate variability (HRV) predicts sudden cardiac death. Long-chain (LC) n-3 PUFA (C20–C22) status is positively associated with HRV. This cross-sectional study investigated whether vegans aged 40–70 years (n 23), whose diets are naturally free from EPA (20 : 5n-3) and DHA (22 : 6n-3), have lower HRV compared with omnivores (n 24). Proportions of LC n-3 PUFA in erythrocyte membranes, plasma fatty acids and concentrations of plasma LC n-3 PUFA-derived lipid mediators were significantly lower in vegans. Day-time interbeat intervals (IBI), adjusted for physical activity, age, BMI and sex, were significantly shorter in vegans compared with omnivores (mean difference −67 ms; 95 % CI −130, −3·4, P<0·05), but there were no significant differences over 24 h or during sleep. Vegans had higher overall HRV, measured as 24 h standard deviation of normal-to-normal intervals (SDNN) (mean adjusted difference 27 ms; 95 % CI 1, 52, P=0·039). Conversely, vegans presented with decreased 8 h day-time HRV: mean adjusted difference in SDNN −20 ms; 95 % CI −37, −3, P=0·021, with no differences during nocturnal sleep. Day-time parameters of beat-to-beat HRV (root of the mean of the sum of the squares of differences between adjacent normal-to-normal intervals, percentage of adjacent normal-to-normal intervals that differ by >50 % and high-frequency power) were similarly lower in vegans, with no differences during sleep. In conclusion, vegans have higher 24 h SDNN, but lower day-time HRV and shorter day-time IBI relative to comparable omnivores. Vegans may have reduced availability of precursor markers for pro-resolving lipid mediators; it remains to be determined whether there is a direct link with impaired cardiac function in populations with low-n-3 status.

Objective:
To assess management of epistaxis at a tertiary ENT referral hospital against a recently published standard of best practice.
Methods:Fifty consecutive cases of acute epistaxis that required admission to Guy's Hospital in 2009 were evaluated. Epistaxis education sessions were held to introduce our algorithm of best practice in tandem with an emphasis on emergency department care. Similar retrospective reviews were carried out in both 2010 and 2011 (on groups of 50 patients).
Results and conclusion:The first audit cycle demonstrated that only 8 per cent of patients underwent a suitable nasal examination in the emergency department prior to transfer, with no documented attempts at nasal cautery. Surgical intervention procedures were performed on only 40 per cent of eligible patients. The audit cycles that followed the introduction of the epistaxis algorithm demonstrated continued improvement in initial evaluation and management of epistaxis. In select patients, sphenopalatine artery ligation can provide timely, definitive management of refractory epistaxis.

Introduction
Aquatic ecosystems have been fertile ground for understanding the extent to which animals can alter nutrient cycling. Although animals have been included in ecosystem models for years (for example, Teal, 1962), it is only more recently that investigators have looked at animals, either as individuals, single species, or assemblages, as agents regulating nutrient cycling (Kitchell et al., 1979; Meyer, Schultz & Helfman, 1983; Grimm, 1988; Jones & Lawton, 1995). A recent review details how animals can affect nutrient cycling in freshwater ecosystems (Vanni, 2002), but the next step is to understand the controls on which animals are important regulators of nutrient dynamics in ecosystems. One controlling factor is determined by attributes of the animals themselves, such as their body size.
Animals can regulate nutrient cycling directly or indirectly (Kitchell et al., 1979; Vanni, 2002). Direct regulation is the transformation and transportation of nutrients by animal ingestion, egestion, production and excretion. For example, animal excretion can constitute the largest source of plant-available nitrogen (N) within an ecosystem (Hall, Tank & Dybdahl, 2003) and animals can move nutrients between habitats (Meyer et al., 1983). Perhaps more common are indirect controls, whereby animals alter nutrient cycling by changing the biomass, production or distribution of the plants or microbes that take up nutrients. For example, predatory fish can regulate phosphorus (P) dynamics or nitrogen retention via a trophic cascade (Elser et al., 1998; Simon et al., 2004).
Observations of a nonlinear solitary wave packet in the Kelvin wake of a ship

Thirty data sets of wavestaff measurements of a solitary feature in the Kelvin wake of the Coast Guard cutter Point Brower are analysed. The average characteristics of the feature between 1 and 4 km aft of the ship are shown to be consistent with those of an oblique nonlinear solitary wave packet. The ship speed is 7.7 m/s (Froude number 0.49) and the waves that comprise the packet have an average frequency of 3.28 rad/s. The ship speed and the wave frequency, together with Kelvin wake kinematics, imply that the feature appears at an average wake half-angle of 10.9°. The packet does not exhibit linear dispersion beyond 1 km aft of the ship and its average width is 8.9 m (measured at e−1 of the peak variance). The average amplitude is 1.1 times the theoretical amplitude of an oblique nonlinear solitary wave packet with the observed width. There is considerable variability from run to run, and there is evidence of dispersive spreading before 1 km aft of the ship. An aerial photograph shows a sinuous fluctuation of the feature; possible explanations for the fluctuation include small variations in initial conditions or a sinuous instability. The solitary feature is a possible explanation for the long bright lines observed in SEASAT SAR images in light to moderate winds and observed in sun glitter photos taken from the space shuttle.

Novel polymers are synthesized by attaching amphiphilic dye groups along a hydrophilic polyether backbone. These polymers are fabricated into noncentrosymmetric films by the Langmuir-Blodgett (L/B) technique. Stacks of molecular bi-layers are assembled by γ-type deposition, interleaving two different dye-substituted polymers. One polymer has the dye's molecular dipole pointing toward the backbone, while in the other polymer, the dye points in the opposite direction. Light from a Nd:YAG laser is passed through these films and the second harmonic is detected.
