Aim:
To explore higher education institution (HEI) perspectives on the development and implementation of trainee nursing associates (NAs) in the primary care workforce in England.
Background:Current shortages of primary health care staff have led to innovative skill mix approaches in attempts to maintain safe and effective care. In England, a new level of nursing practice, NAs, was introduced and joined the workforce in 2019. This role was envisaged as a way of bridging the skills gap between health care assistants and registered nurses and as an alternative route into registered nursing. However, there is limited evidence on programme development and implementation of trainee NAs within primary care settings and HEI perspectives on this.
Methods:This paper draws from a larger qualitative study of HEI perspectives on the trainee NA programme. Twenty-seven staff involved in training NAs, from five HEIs across England, were interviewed from June to September 2021. The interview schedule specifically included questions relating to primary care. Data relating to primary care were extracted and analysed using a combined framework and thematic analysis approach.
Findings:Three themes were developed: ‘Understanding the trainee role and requirements’, ‘Trainee support in primary care’ and ‘Skills and scope of practice’. It is apparent that a more limited understanding of the NA programme requirements can lead to difficulties in accessing the right support for trainees in primary care. This can create challenges for trainees in gaining the required competencies and uncertainty in understanding what constitutes a safe scope of practice within the role for both employers and trainees. It might be anticipated that as this new programme becomes more embedded in primary care, a greater understanding will develop, support will improve and the nature and scope of this new level of practice will become clearer.
Does the impact of bereavement vary between same and different gender partnerships? A representative national, cross-sectional study

Background
Data suggest poorer bereavement outcomes for lesbian, gay and bisexual people, but this has not been estimated in population-based research. This study compared bereavement outcomes for partners of same-gender and different-gender decedents.
MethodsIn this population-based, cross-sectional survey of people bereaved of a civil partner or spouse 6–10 months previously, we used adjusted logistic and linear regression to investigate outcomes of interest: (1) positive screen on Inventory of Complicated Grief (ICG), (2) positive screen on General Health Questionnaire (GHQ), (3) grief intensity (ICG) and (4) psychiatric symptoms (GHQ-12).
ResultsAmong 233 same-gender partners and 329 of different-gender partners, 66.1% [95% confidence interval (CI) 60.0–72.2] and 59.2% [95% CI (53.9–64.6)] respectively screened positive for complicated grief on the ICG, whilst 76.0% [95% CI (70.5–81.5)] and 69.3% [95% CI (64.3–74.3)] respectively screened positive on the GHQ-12. Same-gender bereaved partners were not significantly more likely to screen positive for complicated grief than different-gender partners [adjusted odds ratio (aOR) 1.56, 95% CI (0.98–2.47)], p = 0.059, but same-gender bereaved partners were significantly more likely to screen for psychiatric caseness [aOR 1.67 (1.02, 2.71) p = 0.043]. We similarly found no significant association of partner gender with grief intensity [B = 1.86, 95% CI (−0.91to 4.63), p = 0.188], but significantly greater psychological distress for same-gender partners [B = 1.54, 95% CI (−0.69–2.40), p < 0.001].
ConclusionsSame-gender bereaved partners report significantly more psychological distress. In view of their poorer sub-clinical mental health, clinical and bereavement services should refine screening processes to identify those at risk of poor mental health outcomes.

Background
Contrasting the well-described effects of early intervention (EI) services for youth-onset psychosis, the potential benefits of the intervention for adult-onset psychosis are uncertain. This paper aims to examine the effectiveness of EI on functioning and symptomatic improvement in adult-onset psychosis, and the optimal duration of the intervention.
Methods360 psychosis patients aged 26–55 years were randomized to receive either standard care (SC, n = 120), or case management for two (2-year EI, n = 120) or 4 years (4-year EI, n = 120) in a 4-year rater-masked, parallel-group, superiority, randomized controlled trial of treatment effectiveness (Clinicaltrials.gov: NCT00919620). Primary (i.e. social and occupational functioning) and secondary outcomes (i.e. positive and negative symptoms, and quality of life) were assessed at baseline, 6-month, and yearly for 4 years.
ResultsCompared with SC, patients with 4-year EI had better Role Functioning Scale (RFS) immediate [interaction estimate = 0.008, 95% confidence interval (CI) = 0.001–0.014, p = 0.02] and extended social network (interaction estimate = 0.011, 95% CI = 0.004–0.018, p = 0.003) scores. Specifically, these improvements were observed in the first 2 years. Compared with the 2-year EI group, the 4-year EI group had better RFS total (p = 0.01), immediate (p = 0.01), and extended social network (p = 0.05) scores at the fourth year. Meanwhile, the 4-year (p = 0.02) and 2-year EI (p = 0.004) group had less severe symptoms than the SC group at the first year.
ConclusionsSpecialized EI treatment for psychosis patients aged 26–55 should be provided for at least the initial 2 years of illness. Further treatment up to 4 years confers little benefits in this age range over the course of the study.

Aims and method
In three localities in a mental health trust in England, an enhanced bed management team was established to improve patient flow and reduce out-of-area placements. Trusted assessments were provided to support risk management and conflict resolution. Two measures of flow were compared before and after the team was established.
ResultsThe trusted assessment recommendation was for discharge in 70% of cases. The number of out-of-area placements was significantly reduced (P < 0.05), saving £616 876 over a 12-month period. Patient flow was significantly improved in one of the three localities as measured by patients/bed/6-month period (P < 0.05). In one of the other localities increased use of trusted assessment input and reduced numbers of patients being transferred in are recommended to improve flow.
Clinical implicationsMental health trusts should consider the establishment of an enhanced bed management team, including trusted assessment, as a safe and cost-effective approach to improving patient flow and reducing the need for out-of-area placement.

This article focuses on the way that staff and guardians in the rural Nottinghamshire workhouse of Southwell sought to exert control and containment over pauper inmates. Fusing together local and central records for the period 1834–71, including locally held punishment books and correspondence at The National Archives, Kew (TNA), we argue that the notional power of the workhouse authorities was heavily shaded. Most paupers most of the time did not find their behaviour heavily and clumsily controlled. Rather, staff focused their attention in terms of detecting and punishing disorderly behaviour on a small group of long-term and often mentally ill paupers whose actions might create enmities or spiral into larger conflicts and dissent in the workhouse setting. Both inmates and those under threat of workhouse admission would have seen or heard about punishment of ‘the usual characters’. This has important implications for how we understand the intent and experience of the New Poor Law up to the formation of the Local Government Board (LGB) in 1871.

The collective response of electrons in an ultrathin foil target irradiated by an ultraintense (${\sim}6\times 10^{20}~\text{W}~\text{cm}^{-2}$) laser pulse is investigated experimentally and via 3D particle-in-cell simulations. It is shown that if the target is sufficiently thin that the laser induces significant radiation pressure, but not thin enough to become relativistically transparent to the laser light, the resulting relativistic electron beam is elliptical, with the major axis of the ellipse directed along the laser polarization axis. When the target thickness is decreased such that it becomes relativistically transparent early in the interaction with the laser pulse, diffraction of the transmitted laser light occurs through a so called ‘relativistic plasma aperture’, inducing structure in the spatial-intensity profile of the beam of energetic electrons. It is shown that the electron beam profile can be modified by variation of the target thickness and degree of ellipticity in the laser polarization.

The U. S. Department of Energy’s Ames Laboratory in Ames, Iowa was a coalition partner for outreach activities connected with NOVA’s Making Stuff television series on PBS. Volunteers affiliated with the Ames Laboratory and Iowa State University, with backgrounds in materials science, took part in activities including a science-themed Family Night at a local mall, Science Cafés at the Science Center of Iowa, teacher workshops, demonstrations at science nights in elementary and middle schools, and various other events. We describe a selection of the activities and present a summary of their outcomes and extent of their impact on Ames, Des Moines and the surrounding communities in Iowa.
In Part 2, results of a volunteer attitude survey are presented, which shed some light on the volunteer experience and show how the volunteers’ participation in outreach activities has affected their views of materials education.

This article addresses the relative dearth of work on courtship and marriage motivations for early nineteenth-century England. Focusing on rural-industrial Lancashire, the article draws on a rare conjunction of sources: an autobiography, a series of love-letters and letters from friends, relating to the nascent textile entrepreneur David Whitehead and his intended wife Betty Wood. Triangulating these sources suggests that some of the seemingly dominant influences on courtship and marriage seen in other studies, such as the economic status of partners, family and kin, had little part in this drama. Rather, issues of love, destiny and, above all, religious suitability dictated the pace, content and outcome of the courtship process. Against this backdrop, it was Betty Wood, rather than David Whitehead, who held the levers of power in the courtship. The article also explores other aspects of courtship, most especially the relationship between courtship intensity/fragility and the spatial dynamics of the marriage market.

This article addresses the way in which officials in the last decades of the Old Poor Law thought about and addressed the problems posed to the poor relief process by the migration of paupers. Focusing on the so called out-parish relief system, the article uses rich overseer correspondence and supplementary pauper letters from the northwest of England to explore several key themes in the period 1800–1840: the nature of money transmission where allowances had to be paid at a distance, issues of administrative competence and incompetence, the nature of relationships between parishes and between parishes and their distant poor under the out-parish relief system, and issues of trust and reputation between parishes and between parishes and paupers. The article will show that the out-parish system was vital to the stability of the Old Poor Law and that its apparent fragility and susceptibility to fraud and mistrust is to some extent belied by the fact that robust and long term relationships developed between parishes under the out-parish system.

Following on from the article appearing in Children Australia Vol 30 No 2 (King 2005), this short paper develops a checklist for organisations working with fathers for assessing the programs they provide to men. The checklist uses four categories – environment, language, initial contact and marketing, and service provision – to assess their programs’ strengths and weaknesses.
The paper provides an opportunity for organisations to benchmark their approach in working with male service users and provides guidance on how program delivery can be improved.

This article first examines the recent historiography of the Poor Law, notes the dearth of historical writing on this topic with respect to Wales and then uses an incident which took place in the rural Welsh town of Llantrisant in the early 1840s which clearly exemplifies both particularly Welsh characteristics and those of the medical services of the New Poor Law. It is contended that further study of the welfare regime in nineteenth-century Wales is important for both Welsh history and for the broader historical understanding of the Poor Laws in rural areas.

This article uses court records, overseers' accounts, pauper examinations and other records from several counties, including Huntingdonshire and Staffordshire, to look at the experiences of poor relief in early modern England. It shows the varied circumstances under which the poor ‘encountered’ the poor law in the seventeenth and early eighteenth centuries, highlighting the often contradictory attitudes that poor people encountered. More than this, it argues that there were many sorts of ‘poor’ people with very different capacities to negotiate about relief and to help themselves and each other. These features compounded enduring regional differences in the nature and extent of relief to generate a complex patchwork of experiences for the poor in early modern England.
