Great Basin Survivance (USA): Challenges and Windfalls of the Neoglaciation / Late Holocene Dry Period (3100–1800 cal BP)

The Late Holocene Dry Period (LHDP) was a one-plus millennial megadrought (3100–1800 cal BP) that delivered challenges and windfalls to Indigenous communities of the central Great Basin (United States). New pollen and sedimentation rate studies, combined with existing tree-ring data, submerged stump ages, and lake-level evidence, demonstrate that the LHDP was the driest Great Basin climate within the last 6,000 years—more extreme than the well-known Medieval Climatic Anomaly. New evidence reported here documents that most Great Basin archaeological sites south of 40° N latitude were abandoned during the long dry phase of the LHDP (3100–2200 cal BP), sometimes reoccupied during a wet interval (2200–2000 cal BP), and abandoned again during the most extreme drought (2000–1800 cal BP). Even in the face of epic drought, this is a story of remarkable survivance by some people who adjusted to their drought-stricken landscape where they had lived for millennia. Some moved on, but other resilient foragers refused to abandon their homeland, taking advantage of glacier-fed mountain springs with cooler alpine temperatures and greater moisture retention at high altitude, a result of early Neoglaciation conditions across many Great Basin ranges, despite epic drought conditions in the lowlands.

Current scholarship suggests that Neo-Eneolithic systems of settlement and subsistence in Eastern Europe were defined by short-to-medium range migration, while sparsely populated land in peripheral regions allowed for the continual colonization of new territories. We address the Eastern Tripolye Culture (ETC), a sub-group of the Cucuteni-Tripolye cultural complex that flourished ca. 4300–2950 BC by expanding into the forest-steppe ecozone of Central Ukraine. While a general lack of multi-layer sites complicates regional chronology, we resolve several longstanding questions in Ukrainian archaeological discourse by combining traditional relative chronologies of ceramic types with high-precision AMS dating of material from key sites. We offer a revision of the chronology of Tripolye BI and BI-II, which, rather than consisting of distinct “early” and “late” temporal periods, instead constitute a single period characterized by stylistic diversity in material culture. With an absolute chronology established, we then analyze the space-time distribution of sites, revealing a southwest-to-northeast migratory vector across Central Ukraine characterized by punctuated episodes of “leapfrog” colonization. The establishment of this vector by the ETC presages larger-scale population movements by the Western Tripolye Culture (WTC), which led to the establishment of the giant-settlement phenomenon during the first part of the 4th millennium BC.
Clinical, microbiological, and genomic characteristics of clade-III Candida auris colonization and infection in southern California, 2019–2022

Background:
Candida auris is an emerging fungal pathogen causing outbreaks in healthcare facilities. Five distinctive genomic clades exhibit clade-unique characteristics, highlighting the importance of real-time genomic surveillance and incorporating genotypic information to inform infection prevention practices and treatment algorithms.
Methods:Both active and passive surveillance were used to screen hospitalized patients. C. auris polymerase chain reaction (PCR) assay on inguinal-axillary swabs was performed on high-risk patients upon admission. All clinical yeast isolates were identified to the species level. C. auris isolates were characterized by both phenotypic antifungal susceptibility tests and whole-genome sequencing.
Results:From late 2019 to early 2022, we identified 45 patients with C. auris. Most had a tracheostomy or were from a facility with a known outbreak. Moreover, 7 patients (15%) were only identified through passive surveillance. Also, 8 (18%) of the patients had a history of severe COVID-19. The overall mortality was 18%. Invasive C. auris infections were identified in 13 patients (29%), 9 (69%) of whom had bloodstream infections. Patients with invasive infection were more likely to have a central line. All C. auris isolates were resistant to fluconazole but susceptible to echinocandins. Genomic analysis showed that 1 dominant clade-III lineage is circulating in Los Angeles, with very limited intrahost and interhost genetic diversity.
Conclusions:We have demonstrated that a robust C. auris surveillance program can be established using both active and passive surveillance, with multidisciplinary efforts involving the microbiology laboratory and the hospital epidemiology team. In Los Angeles County, C. auris strains are highly related and echinocandins should be used for empiric therapy.

The post-mortem manipulation of human bodies is documented in many regions of the world, including South America. Recent archaeological fieldwork in the Chincha Valley, Peru, adds to this catalogue nearly 200 examples of the threading of human vertebrae onto reed posts. Here, the authors report on the distribution and composition of these ‘vertebrae-on-posts’, which are radiocarbon-dated to the Late Horizon (AD 1400–1532) and Colonial (AD 1532–1825) periods. The authors argue that these modified remains represent a social process that reconstructed the dead in response to Colonial-period looting. This manipulation of human remains reflects protracted relationships between the living and the dead, and the enduring social lives of human remains.

Objective
To assess variability in antimicrobial use and associations with infection testing in pediatric ventilator-associated events (VAEs).
DesignDescriptive retrospective cohort with nested case-control study.
SettingPediatric intensive care units (PICUs), cardiac intensive care units (CICUs), and neonatal intensive care units (NICUs) in 6 US hospitals.
PatientsChildren≤18 years ventilated for≥1 calendar day.
MethodsWe identified patients with pediatric ventilator-associated conditions (VACs), pediatric VACs with antimicrobial use for≥4 days (AVACs), and possible ventilator-associated pneumonia (PVAP, defined as pediatric AVAC with a positive respiratory diagnostic test) according to previously proposed criteria.
ResultsAmong 9,025 ventilated children, we identified 192 VAC cases, 43 in CICUs, 70 in PICUs, and 79 in NICUs. AVAC criteria were met in 79 VAC cases (41%) (58% CICU; 51% PICU; and 23% NICU), and varied by hospital (CICU, 20–67%; PICU, 0–70%; and NICU, 0–43%). Type and duration of AVAC antimicrobials varied by ICU type. AVAC cases in CICUs and PICUs received broad-spectrum antimicrobials more often than those in NICUs. Among AVAC cases, 39% had respiratory infection diagnostic testing performed; PVAP was identified in 15 VAC cases. Also, among AVAC cases, 73% had no associated positive respiratory or nonrespiratory diagnostic test.
ConclusionsAntimicrobial use is common in pediatric VAC, with variability in spectrum and duration of antimicrobials within hospitals and across ICU types, while PVAP is uncommon. Prolonged antimicrobial use despite low rates of PVAP or positive laboratory testing for infection suggests that AVAC may provide a lever for antimicrobial stewardship programs to improve utilization.

OBJECTIVE
Adult ventilator-associated event (VAE) definitions include ventilator-associated conditions (VAC) and subcategories for infection-related ventilator-associated complications (IVAC) and possible ventilator-associated pneumonia (PVAP). We explored these definitions for children.
DESIGNRetrospective cohort

METHODSWe identified patients with pediatric VAC based on previously proposed criteria. We applied adult temperature, white blood cell count, antibiotic, and culture criteria for IVAC and PVAP to these patients. We matched pediatric VAC patients with controls and evaluated associations with adverse outcomes using Cox proportional hazards models.
RESULTSIn total, 233 pediatric VACs (12,167 ventilation episodes) were identified. In the cardiac ICU (CICU), 62.5% of VACs met adult IVAC criteria; in the pediatric ICU (PICU), 54.2% of VACs met adult IVAC criteria; and in the neonatal ICU (NICU), 20.2% of VACs met adult IVAC criteria. Most patients had abnormal white blood cell counts and temperatures; we therefore recommend simplifying surveillance by focusing on “pediatric VAC with antimicrobial use” (pediatric AVAC). Pediatric AVAC with a positive respiratory diagnostic test (“pediatric PVAP”) occurred in 8.9% of VACs in the CICU, 13.3% of VACs in the PICU, and 4.3% of VACs in the NICU. Hospital mortality was increased, and hospital and ICU length of stay and duration of ventilation were prolonged among all pediatric VAE subsets compared with controls.
CONCLUSIONSWe propose pediatric AVAC for surveillance related to antimicrobial use, with pediatric PVAP as a subset of AVAC. Studies on generalizability and responsiveness of these metrics to quality improvement initiatives are needed, as are studies to determine whether lower pediatric VAE rates are associated with improvements in other outcomes.

Acute gastrointestinal illness in two Inuit communities: burden of illness in Rigolet and Iqaluit, Canada

Food- and waterborne disease is thought to be high in some Canadian Indigenous communities; however, the burden of acute gastrointestinal illness (AGI) is not well understood due to limited availability and quality of surveillance data. This study estimated the burden of community-level self-reported AGI in the Inuit communities of Rigolet, Nunatsiavut, and Iqaluit, Nunavut, Canada. Cross-sectional retrospective surveys captured information on AGI and potential environmental risk factors. Multivariable logistic regression models identified potential AGI risk factors. The annual incidence of AGI ranged from 2·9–3·9 cases/person per year in Rigolet and Iqaluit. In Rigolet, increased spending on obtaining country foods, a homeless person in the house, not visiting a cabin recently, exposure to puppies, and alternative sources of drinking water were associated with increased odds of AGI. In Iqaluit, eating country fish often, exposure to cats, employment status of the person responsible for food preparation, not washing the countertop with soap after preparing meat, a homeless person in the house, and overcrowding were associated with increased odds of AGI. The results highlight the need for systematic data collection to better understand and support previously anecdotal indications of high AGI incidence, as well as insights into unique AGI environmental risk factors in Indigenous populations.
