Test–Retest Reliability of a Semi-Structured Interview to Aid in Pediatric Traumatic Brain Injury Diagnosis

Objective:
Retrospective self-report is typically used for diagnosing previous pediatric traumatic brain injury (TBI). A new semi-structured interview instrument (New Mexico Assessment of Pediatric TBI; NewMAP TBI) investigated test–retest reliability for TBI characteristics in both the TBI that qualified for study inclusion and for lifetime history of TBI.
Method:One-hundred and eight-four mTBI (aged 8–18), 156 matched healthy controls (HC), and their parents completed the NewMAP TBI within 11 days (subacute; SA) and 4 months (early chronic; EC) of injury, with a subset returning at 1 year (late chronic; LC).
Results:The test–retest reliability of common TBI characteristics [loss of consciousness (LOC), post-traumatic amnesia (PTA), retrograde amnesia, confusion/disorientation] and post-concussion symptoms (PCS) were examined across study visits. Aside from PTA, binary reporting (present/absent) for all TBI characteristics exhibited acceptable (≥0.60) test–retest reliability for both Qualifying and Remote TBIs across all three visits. In contrast, reliability for continuous data (exact duration) was generally unacceptable, with LOC and PCS meeting acceptable criteria at only half of the assessments. Transforming continuous self-report ratings into discrete categories based on injury severity resulted in acceptable reliability. Reliability was not strongly affected by the parent completing the NewMAP TBI.
Conclusions:Categorical reporting of TBI characteristics in children and adolescents can aid clinicians in retrospectively obtaining reliable estimates of TBI severity up to a year post-injury. However, test–retest reliability is strongly impacted by the initial data distribution, selected statistical methods, and potentially by patient difficulty in distinguishing among conceptually similar medical concepts (i.e., PTA vs. confusion).

Identifying gaps in parental support for families of children with hypoplastic left heart syndrome

Purpose:
The purpose of this study is to identify gaps in support for parents of children with Hypoplastic Left Heart Syndrome.
Design and methods:Using a mixed-methods approach, the researchers first studied the parental and care team experience through interviews of Hypoplastic Left Heart Syndrome mothers and members of the inter-professional care team and then conducted an international survey of 690 Hypoplastic Left Heart Syndrome primary caregivers to validate the qualitative findings.
Results:Parental and care team interviews revealed three main gaps in parental support, including lack of open communication, unrealistic parental expectations, and unclear inter-professional team roles. Survey results found that parents whose children were diagnosed with Hypoplastic Left Heart Syndrome after birth indicated significant dissatisfaction with the care team’s open communication and welcoming of feedback (p = 0.008). As parents progress through the stages of surgical intervention, they also indicate significant dissatisfaction with the care team’s anticipation of parental emotional needs and provision of coping resources (p = 0.003).
Conclusions:Parental support interventions should focus on providing resources to help parents cope, helping the care team model open communication, and welcoming feedback on the parental experience.
Practice implications:Interventions should be piloted with parents who are in the later stages of the surgical intervention timeline or whose children were diagnosed after birth as they are the populations who perceived the least support within this study.

Annual grass weeds reduce profits of wheat farmers in the Pacific Northwest. The very-long-chain fatty acid elongase (VLCFA)-inhibiting herbicides S-metolachlor and dimethenamid-P could expand options for control of annual grasses but are not registered in wheat, because of crop injury. We evaluated a safener, fluxofenim, applied to wheat seed for protection of 19 soft white winter wheat varieties from S-metolachlor, dimethenamid-P, and pyroxasulfone herbicides; investigated the response of six varieties (UI Sparrow, LWW 15-72223, UI Magic CL+, Brundage 96, UI Castle CL+, and UI Palouse CL+) to incremental doses of fluxofenim; established the fluxofenim dose required to optimally protect the varieties from VLCFA-inhibiting herbicides; and assessed the impact of fluxofenim dose on glutathione S-transferase (GST) activity in three wheat varieties (UI Sparrow, Brundage 96, and UI Castle CL+). Fluxofenim increased the biomass of four varieties treated with S-metolachlor or dimethenamid-P herbicides and one variety treated with pyroxasulfone. Three varieties showed tolerance to the herbicides regardless of the fluxofenim treatment. Estimated fluxofenim doses resulting in 10% biomass reduction of wheat ranged from 0.55 to 1.23 g ai kg−1 seed. Fluxofenim doses resulting in 90% increased biomass after treatment with S-metolachlor, dimethenamid-P, and pyroxasulfone ranged from 0.07 to 0.55, 0.09 to 0.73, and 0.30 to 1.03 g ai kg−1 seed, respectively. Fluxofenim at 0.36 g ai kg−1 seed increased GST activity in UI Castle CL+, UI Sparrow, and Brundage 96 by 58%, 30%, and 38%, respectively. These results suggest fluxofenim would not damage wheat seedlings up to three times the rate labeled for sorghum, and fluxofenim protects soft white winter wheat varieties from S-metolachlor, dimethenamid-P, or pyroxasulfone injury at the herbicide rates evaluated.
Terrestrial sensitivity to abrupt cooling recorded by aeolian activity in northwest Ohio, USA

Optically stimulated luminescence dated sand dunes and Pleistocene beach ridges in northwest Ohio are used to reconstruct landscape modification more than 5000 yr after deglaciation. Four of the OSL ages (13.3–11.1 ka) cluster around the Younger Dryas cold event, five ages (10.8–8.2 ka) cluster around the Preboreal, one young age (0.9–0.7 ka) records more recent aeolian activity, and one age of 15.1–13.1 ka dates a barrier spit in Lake Warren. In northwest Ohio, both landscape instability recorded by aeolian activity and a vegetation response recorded by pollen are coeval with the Younger Dryas. However, the climate conditions during the Preboreal resulting in aeolian activity are not recorded in the available pollen records. From this, we conclude that aeolian dunes and surfaces susceptible to deflation are sensitive to cooler, drier episodes of climate and can complement pollen data. Younger Dryas and Preboreal aged aeolian activity in northwestern Ohio coincides with aeolian records elsewhere in the Great Lakes region east of the prairie–forest ecotone.
Abnormalities in serum biomarkers correlate with lower cardiac index in the Fontan population

Background
Fontan survivors have depressed cardiac index that worsens over time. Serum biomarker measurement is minimally invasive, rapid, widely available, and may be useful for serial monitoring. The purpose of this study was to identify biomarkers that correlate with lower cardiac index in Fontan patients.
Methods and resultsThis study was a multi-centre case series assessing the correlations between biomarkers and cardiac magnetic resonance-derived cardiac index in Fontan patients ⩾6 years of age with biochemical and haematopoietic biomarkers obtained ±12 months from cardiac magnetic resonance. Medical history and biomarker values were obtained by chart review. Spearman’s Rank correlation assessed associations between biomarker z-scores and cardiac index. Biomarkers with significant correlations had receiver operating characteristic curves and area under the curve estimated. In total, 97 cardiac magnetic resonances in 87 patients met inclusion criteria: median age at cardiac magnetic resonance was 15 (6–33) years. Significant correlations were found between cardiac index and total alkaline phosphatase (−0.26, p=0.04), estimated creatinine clearance (0.26, p=0.02), and mean corpuscular volume (−0.32, p<0.01). Area under the curve for the three individual biomarkers was 0.63–0.69. Area under the curve for the three-biomarker panel was 0.75. Comparison of cardiac index above and below the receiver operating characteristic curve-identified cut-off points revealed significant differences for each biomarker (p<0.01) and for the composite panel [median cardiac index for higher-risk group=2.17 L/minute/m2 versus lower-risk group=2.96 L/minute/m2, (p<0.01)].
ConclusionsHigher total alkaline phosphatase and mean corpuscular volume as well as lower estimated creatinine clearance identify Fontan patients with lower cardiac index. Using biomarkers to monitor haemodynamics and organ-specific effects warrants prospective investigation.

Background:
Patient registries represent an important method of organizing “real world” patient information for clinical and research purposes. Registries can facilitate clinical trial planning and recruitment and are particularly useful in this regard for uncommon and rare diseases. Neuromuscular diseases (NMDs) are individually rare but in aggregate have a significant prevalence. In Canada, information on NMDs is lacking. Barriers to performing Canadian multicentre NMD research exist which can be overcome by a comprehensive and collaborative NMD registry.
Methods:We describe the objectives, design, feasibility and initial recruitment results for the Canadian Neuromuscular Disease Registry (CNDR).
Results:The CNDR is a clinic-based registry which launched nationally in June 2011, incorporates paediatric and adult neuromuscular clinics in British Columbia, Alberta, Ontario, Quebec, New Brunswick and Nova Scotia and, as of December 2012, has recruited 1161 patients from 12 provinces and territories. Complete medical datasets have been captured on 460 “index disease” patients. Another 618 “non-index” patients have been recruited with capture of physician-confirmed diagnosis and contact information. We have demonstrated the feasibility of blended clinic and central office-based recruitment. “Index disease” patients recruited at the time of writing include 253 with Duchenne and Becker muscular dystrophy, 161 with myotonic dystrophy, and 71 with ALS.
Conclusions:The CNDR is a new nationwide registry of patients with NMDs that represents an important advance in Canadian neuromuscular disease research capacity. It provides an innovative platform for organizing patient information to facilitate clinical research and to expedite translation of recent laboratory findings into human studies.

The catalytic activity of polycrystalline and nanocrystalline CeO2-supported Pd (Pd/pCeO2 and Pd/nCeO2) has been determined as a function of temperature and Pd loading. While the untreated nCeO2 support gives 50% methane conversion at 420°C, the untreated pCeO2 support exhibits little activity under the conditions examined due to its low surface area. A Pd loading of 5 wt% increases the activity of pCeO2 to 50% conversion at 260°C, while a 40 wt% Pd loading on nCeO2 exhibits a relatively smaller activity increase, yielding 50% conversion at 240°C. On a mass basis the 40 wt% Pd/nCeO2 catalyst is the most active tested in this study, but it is less active than the 5 wt% Pd/pCeO2 catalyst on a surface-area basis. Furthermore, the activity of the 40 wt% Pd/nCeO2 catalyst does not decrease during 100 hrs of exposure to CH4 and O2 at 250°C.
X-ray photoelectron spectroscopy (XPS) and ion scattering spectroscopy (ISS) have been used to characterize the surfaces of both bare supports and Pd-containing catalysts before and after exposure to reactor conditions. The XPS results reveal that the Pd surface concentration is more than an order of magnitude higher for 5 wt% Pd/pCeO2 than for 5 wt% Pd/nCeO2 due to the larger surface area of nCeO2 and that the 40 wt% Pd/nCeO2 catalyst has a lower Pd loading on a surface-area basis than the 5 wt% Pd/pCeO2 catalyst. Most of the supported Pd is in the form of PdO, but higher PdO2/PdO ratios are observed for both CeO2 supports compared to Pd supported on ZrO2 or CO3O4. Furthermore, a significant amount of metallic Pd forms on Pd/nCeO2 but not on Pd/pCeO2 during reaction. The nanocrystalline and polycrystalline CeO2 behave differently chemically which is consistent with the fact that the nanocrystalline catalysts are less active on a surface-area basis. Accumulation of H20 on the Pd/pCeO2 surface during reaction is significant but not on the Pd/nCeO2 surface. This suggests that the rate limiting step may be H2O desorption on Pd/pCeO2 while for Pd on nCeO2 adsorption of methane appears to be the slow step. The ISS data indicate that the outermost atomic layer of Pd/nCeO2 consists mostly of O and C, which is not the case for Pd/pCeO2. Site blockage by these species may also contribute to the lower activity on a surface-area basis of Pd/nCeO2 compared to Pd/pCeO2.

Dehydrin proteins (late embryogenesis abundant (LEA) D11 family) are produced in a wide variety of plant species in response to environmental stimuli with a dehydrative component, including drought, low temperature, salinity, and developmental stages such as seed and pollen maturation. Despite their widespread occurrence and abundance in cells under dehydrative conditions, the biochemical role of dehydrins remains elusive. The subcellular location of dehydrins is consistent with a biochemical role as an intracellular stabilizer, possibly with surfactant characteristics, acting upon targets in both the nucleus and cytoplasm. In some species, dehydrin loci are located within quantitative trait loci (QTL) intervals for important phenotypic traits including winter hardiness in barley (Hordeum vulgare L.) and anthesis-silking interval in maize (Zea mays L.). Dehydrin loci tend to be multigenic and occur in clusters on more than one chromosome. Investigations are currently under way in our laboratory and others' to move beyond protein accumulation studies and correlations with QTL to uncover direct cause-and-effect relationships between dehydrin (dhn) genes and phenotypes associated with physiological responses to stress.
Oxidation Dynamics of Nanophase Aluminum Clusters: A Molecular Dynamics Study

Oxidation of an aluminum nanocluster (252,158 atoms) of radius 100Å placed in gaseous oxygen (530,727 atoms) is investigated by performing molecular-dynamics simulations on parallel computers. The simulation takes into account the effect of charge transfer between Al and 0 based on the electronegativity equalization principles. We find that the oxidation starts at the surface of the cluster and the oxide layer grows to a thickness of ∼28Å. Evolutions of local temperature and densities of Al and 0 are investigated. The surface oxide melts because of the high temperature resulting from the release of energy associated with Al-O bondings. Amorphous surface-oxides are obtained by quenching the cluster. Vibrational density-of-states for the surface oxide is analyzed through comparisons with those for crystalline Al, Al nanocluster, and α-Al2O3

Fracture in thin films of silicon nitride and alumina is investigated with large-scale multiresolution molecular-dynamics (MD) simulations on parallel computers. The simulations for alumina include dynamic charge transfer effects based on electronegativity equalization. Results for structural and dynamic correlations and the effects of temperature and orientation of film on fracture dynamics and morphology of fracture surfaces are presented.
