The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Hypothesis:
Emergency Medical Services (EMS) systems have developed protocols for prehospital activation of the cardiac catheterization laboratory for patients with suspected ST-elevation myocardial infarction (STEMI) to decrease first-medical-contact-to-balloon time (FMC2B). The rate of “false positive” prehospital activations is high. In order to decrease this rate and expedite care for patients with true STEMI, the American Heart Association (AHA; Dallas, Texas USA) developed the Mission Lifeline PreAct STEMI algorithm, which was implemented in Los Angeles County (LAC; California USA) in 2015. The hypothesis of this study was that implementation of the PreAct algorithm would increase the positive predictive value (PPV) of prehospital activation.
Methods:This is an observational pre-/post-study of the effect of the implementation of the PreAct algorithm for patients with suspected STEMI transported to one of five STEMI Receiving Centers (SRCs) within the LAC Regional System. The primary outcome was the PPV of cardiac catheterization laboratory activation for percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG). The secondary outcome was FMC2B.
Results:A total of 1,877 patients were analyzed for the primary outcome in the pre-intervention period and 405 patients in the post-intervention period. There was an overall decrease in cardiac catheterization laboratory activations, from 67% in the pre-intervention period to 49% in the post-intervention period (95% CI for the difference, -14% to -22%). The overall rate of cardiac catheterization declined in post-intervention period as compared the pre-intervention period, from 34% to 30% (95% CI, for the difference -7.6% to 0.4%), but actually increased for subjects who had activation (48% versus 58%; 95% CI, 4.6%-15.0%). Implementation of the PreAct algorithm was associated with an increase in the PPV of activation for PCI or CABG from 37.9% to 48.6%. The overall odds ratio (OR) associated with the intervention was 1.4 (95% CI, 1.1-1.8). The effect of the intervention was to decrease variability between medical centers. There was no associated change in average FMC2B.
Conclusions:The implementation of the PreAct algorithm in the LAC EMS system was associated with an overall increase in the PPV of cardiac catheterization laboratory activation.

Differential thermal analysis of a rapidly reversible transition in carnegieite to 7 kbar shows the transition temperature increases from ⪝ 707°C (on heating) at I bar with initial slope ≈ 8·0 deg kbar−1 and anomalous curvature (d2T/dp2 > o), with hysteresis between heating and cooling signals that decreases with pressure. DTA of rapidly reversible transitions in nepheline shows the very subtle transition near 185°C (on heating) at 1 bar decreases with pressure to ≈ 170°C near 2-3 kbar and hardly varies in temperature up to 6 kbar; and the higher temperature transition increases from ≈ 872°C (on heating) at 1 bar with slope ≈ 25 deg kbar−1, to 5 kbar, with the hysteretic interval remaining essentially unchanged in the investigated range.

Under the auspices of the United States Department of State, The Ford Foundation, Georgetown University, and the African-American Institute, more than 75 scholars and other specialists convened at the Shoreham Hotel, Washington, D. C., from August 17 to 21, 1964, to exchange views on problems of political and social change in francophonic Africa. The program was organized and directed by Dr. William H. Lewis of Georgetown University. The first such conclave ever to be convened in the United States, it brought together more than 500 scholars, government officials, and diplomatic personnel from Africa, Western Europe, Canada, and the United States.
The basic purpose of this special program was to stimulate greater interest among American scholars and graduate students in the unfolding problems of francophonic Africa -- extending from Morocco, Algeria, and Tunisia in the north to the Congo (Leopoldville) and the Malagasy Republic to the south. To this end, the sponsors established a four-week graduate Institute which preceded the Congress. Conducted at Georgetown University, the Institute brought together a faculty of leading African and American scholars, as well as a student body comprising Africans, Europeans, and Americans. The Institute offered a program of instruction in African history, problems of economic development, parameters of social change, West African politics, and nationalism in North and sub-Saharan Africa.

In March 1962, France and Algeria, parties to seven years of conflict, agreed upon the need to destroy an illusion. For more than a century Algeria had been regarded as an integral part of metropolitan France, despite the patently non-Gallic character of its indigenous population. Bloody insurrection and the realization in France that the Mother Country could not undo the wrongs of previous generations brought the illusion into disrepute; it was interred in July 1962 with the acquisition of full independent status by Algeria. This separation restored France to a reasonable semblance of well-being; Algeria, on the other hand, suffered a far less happy fate.
At independence Algeria found itself on the threshold of renewed conflict. The principals in this second act of violence were not the European settlers (colons) who had dominated Algeria's political life for generations as baronial overlords. The vast majority of settlers, approximately 850,000 out of a total of one million, had scuttled and run out of fear of vengeful retribution by Algeria's nine million Muslims. Nor were the half-million French soldiers remaining on Algerian soil participants in the new drama; the overwhelming majority were anxious to depart this melancholy land without molestation. The principals were the founders and leaders of the Algerian revolution, men who, at the hour of triumph, fell into disagreement over the distribution of power and ignored a more compelling imperative - - the rehabilitation of a wartorn society.

Objective
Evaluation of a dietary Na reduction trial in a community setting.
DesignCommunity-based randomized trial. Ten-week nutrition intervention activities focused on lifestyle modification to decrease dietary Na intake, under the supervision of a registered dietitian. Twenty-four hour urine specimens were collected at baseline and follow-up visits to determine 24 h urinary Na excretion.
SettingThe University of Pittsburgh Center for Healthy Aging, Key to Life Nutrition Program.
SubjectsHypertensive adults at least 65 years of age.
ResultsMean age of participants was 75 years. Twenty-four hour mean urinary Na excretion at baseline was 3174 mg/d. This reduced to 2944 mg/d (P = 0·30) and 2875 mg/d (P ≤ 0·03) at 6- and 12-month follow-ups, respectively. In a sub-sample (urine volume of ≥1000 ml, baseline to 12 months), mean urinary Na excretion decreased from 3220 mg/d to 2875 mg/d (P ≤ 0·02).
ConclusionsSignificant reductions in mean 24 h urinary Na were reported, but results fell short of the recommended guidelines of 1500 mg/d for at-risk individuals. Our results reiterate the difficulty in implementing these guidelines in community-based programmes. More aggressive public health efforts, food industry support and health policy changes are needed to decrease Na levels in older adults to the recommended guidelines.

Africa, an area of contrasts and contradictions, is beginning to assume new importance in Western eyes. Its more than 200 million people, in many stages of economic and political evolution, are evincing a new vitality and demonstrating a growing consciousness of Africa's place in the modem world. The rapid changes on this vast continent are most fully reflected on the political plane. Since World War II, Libya, Morocco, Tunisia, the Sudan, and the Gold Coast (Ghana) have become independent states. Former Italian Somaliland, Nigeria, and Uganda are taking uncertain strides in that direction. France is offering the hope of a new political future to its remaining African possessions. In addition, new and modern cities have been carved out of jungles, diversified skills imparted to Africans, and the values of a scientific age made more comprehensible.

This review discusses the historical aspects, current state of the art, and potential future advances in the areas of nomenclature and databases for the analysis of outcomes of treatments for patients with congenitally malformed hearts. We will consider the current state of analysis of outcomes, lay out some principles which might make it possible to achieve life-long monitoring and follow-up using our databases, and describe the next steps those involved in the care of these patients need to take in order to achieve these objectives. In order to perform meaningful multi-institutional analyses, we suggest that any database must incorporate the following six essential elements: use of a common language and nomenclature, use of an established uniform core dataset for collection of information, incorporation of a mechanism of evaluating case complexity, availability of a mechanism to assure and verify the completeness and accuracy of the data collected, collaboration between medical and surgical subspecialties, and standardised protocols for life-long follow-up.
During the 1990s, both The European Association for Cardio-Thoracic Surgery and The Society of Thoracic Surgeons created databases to assess the outcomes of congenital cardiac surgery. Beginning in 1998, these two organizations collaborated to create the International Congenital Heart Surgery Nomenclature and Database Project. By 2000, a common nomenclature, along with a common core minimal dataset, were adopted by The European Association for Cardio-Thoracic Surgery and The Society of Thoracic Surgeons, and published in the Annals of Thoracic Surgery. In 2000, The International Nomenclature Committee for Pediatric and Congenital Heart Disease was established. This committee eventually evolved into the International Society for Nomenclature of Paediatric and Congenital Heart Disease. The working component of this international nomenclature society has been The International Working Group for Mapping and Coding of Nomenclatures for Paediatric and Congenital Heart Disease, also known as the Nomenclature Working Group. By 2005, the Nomenclature Working Group crossmapped the nomenclature of the International Congenital Heart Surgery Nomenclature and Database Project of The European Association for Cardio-Thoracic Surgery and The Society of Thoracic Surgeons with the European Paediatric Cardiac Code of the Association for European Paediatric Cardiology, and therefore created the International Paediatric and Congenital Cardiac Code, which is available for free download from the internet at [http://www.IPCCC.NET].
This common nomenclature, the International Paediatric and Congenital Cardiac Code, and the common minimum database data set created by the International Congenital Heart Surgery Nomenclature and Database Project, are now utilized by both The European Association for Cardio-Thoracic Surgery and The Society of Thoracic Surgeons. Between 1998 and 2007 inclusive, this nomenclature and database was used by both of these two organizations to analyze outcomes of over 150,000 operations involving patients undergoing surgical treatment for congenital cardiac disease.
Two major multi-institutional efforts that have attempted to measure the complexity of congenital heart surgery are the Risk Adjustment in Congenital Heart Surgery-1 system, and the Aristotle Complexity Score. Current efforts to unify the Risk Adjustment in Congenital Heart Surgery-1 system and the Aristotle Complexity Score are in their early stages, but encouraging. Collaborative efforts involving The European Association for Cardio-Thoracic Surgery and The Society of Thoracic Surgeons are under way to develop mechanisms to verify the completeness and accuracy of the data in the databases. Under the leadership of The MultiSocietal Database Committee for Pediatric and Congenital Heart Disease, further collaborative efforts are ongoing between congenital and paediatric cardiac surgeons and other subspecialties, including paediatric cardiac anaesthesiologists, via The Congenital Cardiac Anesthesia Society, paediatric cardiac intensivists, via The Pediatric Cardiac Intensive Care Society, and paediatric cardiologists, via the Joint Council on Congenital Heart Disease and The Association for European Paediatric Cardiology.
In finalising our review, we emphasise that analysis of outcomes must move beyond mortality, and encompass longer term follow-up, including cardiac and non cardiac morbidities, and importantly, those morbidities impacting health related quality of life. Methodologies must be implemented in these databases to allow uniform, protocol driven, and meaningful, long term follow-up.

A transitive finite permutation group is called elusive if it contains no nontrivial semiregular subgroup. The purpose of the paper is to collect known information about elusive groups. The main results are recursive constructions of elusive permutation groups, using various product operations and affine group constructions. A brief historical introduction and a survey of known elusive groups are also included. In a sequel, Giudici has determined all the quasiprimitive elusive groups.
Part of the motivation for studying this class of groups was a conjecture due to Marušič, Jordan and Klin asserting that there is no elusive 2-closed permutation group. It is shown that the constructions given will not build counterexamples to this conjecture.

With increasing emphasis in the meat sector on better and more consistent quality, carcass leanness and conformation is now an important issue for sheep breeders. In 1999, only 47% of all carcasses in the UK met the target specifications for weight, fat and conformation (MLC, 2000), highlighting the potential for improvement. In the current stratified crossbreeding system, crossbred wether lambs are a by-product of the production of dam line ewes for the lowland sector. If their carcass quality is sufficient, they can give a valuable boost to the economics of the breeding programme. Genetic improvement of carcass quality in crossing sire breeds would benefit the crossbred wethers, as well as filter through to the terminal sire cross lambs produced by the crossbred ewes. This work aims to assess the influence of selection index and live conformation score of crossing sires (in this case Bluefaced Leicesters) on growth and carcass quality traits of their crossbred progeny, as a first step towards designing a genetic improvement programme for crossing sire sheep.
Diagnosis of Twin Zygosity by Self-Assessment and by Genetic Analysis

For 173 pairs of like-sex adult twins, self-assessment of zygosity was verified by laboratory diagnosis. Seventeen percent of twins who were very likely monozygous (MZ) believed themselves dizygous (DZ), frequently citing two placentas at their delivery as “evidence.” We suggest that twins be asked what leads them to their assessment of their own zygosity. For 93% of Caucasian and 89% of American Black like-sex twins in our sample, DZ twins could be differentiated based on six polymorphic markers retrievable from frozen sera. MZ twins who believe themselves DZ can be considered “environmentally DZ, genetically MZ” twins, and might be used to study genetic and environmental influences on the treatment of twins and on twins' choices of social characteristics.
