Independent contribution of polygenic risk for schizophrenia and cannabis use in predicting psychotic-like experiences in young adulthood: testing gene × environment moderation and mediation

Background
It has not yet been determined if the commonly reported cannabis–psychosis association is limited to individuals with pre-existing genetic risk for psychotic disorders.
MethodsWe examined whether the relationship between polygenic risk score for schizophrenia (PRS-Sz) and psychotic-like experiences (PLEs), as measured by the Community Assessment of Psychic Experiences-42 (CAPE-42) questionnaire, is mediated or moderated by lifetime cannabis use at 16 years of age in 1740 of the individuals of the European IMAGEN cohort. Secondary analysis examined the relationships between lifetime cannabis use, PRS-Sz and the various sub-scales of the CAPE-42. Sensitivity analyses including covariates, including a PRS for cannabis use, were conducted and results were replicated using data from 1223 individuals in the Dutch Utrecht cannabis cohort.
ResultsPRS-Sz significantly predicted cannabis use (p = 0.027) and PLE (p = 0.004) in the IMAGEN cohort. In the full model, considering PRS-Sz and covariates, cannabis use was also significantly associated with PLE in IMAGEN (p = 0.007). Results remained consistent in the Utrecht cohort and through sensitivity analyses. Nevertheless, there was no evidence of a mediation or moderation effects.
ConclusionsThese results suggest that cannabis use remains a risk factor for PLEs, over and above genetic vulnerability for schizophrenia. This research does not support the notion that the cannabis–psychosis link is limited to individuals who are genetically predisposed to psychosis and suggests a need for research focusing on cannabis-related processes in psychosis that cannot be explained by genetic vulnerability.

Background
Abnormalities in reward circuit function are considered a core feature of addiction. Yet, it is still largely unknown whether these abnormalities stem from chronic drug use, a genetic predisposition, or both.
MethodsIn the present study, we investigated this issue using a large sample of adolescent children by applying structural equation modeling to examine the effects of several dopaminergic polymorphisms of the D1 and D2 receptor type on the reward function of the ventral striatum (VS) and orbital frontal cortex (OFC), and whether this relationship predicted the propensity to engage in early alcohol misuse behaviors at 14 years of age and again at 16 years of age.
ResultsThe results demonstrated a regional specificity with which the functional polymorphism rs686 of the D1 dopamine receptor (DRD1) gene and Taq1A of the ANKK1 gene influenced medial and lateral OFC activation during reward anticipation, respectively. Importantly, our path model revealed a significant indirect relationship between the rs686 of the DRD1 gene and early onset of alcohol misuse through a medial OFC × VS interaction.
ConclusionsThese findings highlight the role of D1 and D2 in adjusting reward-related activations within the mesocorticolimbic circuitry, as well as in the susceptibility to early onset of alcohol misuse.

Both psychologists and psychiatrists are trained to write formulations of their patients' illnesses, with some differences in how they do this. Psychologists focus on psychological understanding, while psychiatrists' formulation brings together aetiology, functioning and a management plan. Mohtashemi et al's study records how some psychiatrists understand formulation and its usefulness. Time pressure was an important barrier to making a full formulation, and some believed the medical role of the psychiatrist was a priority. The study illustrates some of the challenges facing psychiatrists working in the NHS in terms of maintaining high clinical standards and a holistic approach to patient care.

Toward a neurobiologically grounded approach to explaining self-control we discuss the case of a patient with a bilateral lesion in frontal ventromedial cortex. Patients with such lesions display a marked deficit in social decision making. Compared with an account that examines the causal antecedents of self-control, Rachlin's behaviorist approach seems lacking in explanatory strength.

Summary
As constitutional actors, legislatures are accustomed to asserting their understanding of the constitutionality of the legislation they enact. They are also not uncommonly engaged in constitutional disputes with other constitutional actors. At times, they appear to be avoiding their constitutional role, informally “delegating” it to another actor, and for this they are likely to be subject to criticism. But are there times legislatures – and legislators – stand back, suspend judgment, and engage in what I am calling “constitutional agnosticism”? Are there times when they should be?
Whether constitutions are limited to institutional organization or are aspirational, written or unwritten or a hybrid, easy or difficult to amend, one thing they must do is help maintain stability in the country, at least sufficient to allow for orderly transition of power within a recognized government structure. At a higher level, the constitution may play a much greater role in shaping common values or in allowing a regular “conversation” about identifying and defining the values that “matter.” Without saying that both of these are functions the Constitution must fulfill, they are important functions that constitutions do fulfill and the first, if the country is to function at all, must be filled in some manner or under some authority.
The constitution's capacity to meet these goals of stability, orderly transition, and/or shaping of the nation's value structure depends on the extent to which it is recognized as “legitimate” by the public or by those who have been acknowledged as “speaking for” the public.

Introduction
The modern story of attachment began soon after World War II, when John Bowlby (1907–1990) and Rene Spitz (1887–1974) independently began to document the fate of children separated from their parents, reared from infancy in orphanages, or placed in traumatic circumstances in institutions where care might be efficient but impersonal. What they found is familiar to us now: listless, apathetic children who had not had personal attention from an interested adult, and disinhibited, over-friendly children who made shallow relationships with any adult who passed through their world. John Bowlby's contribution to our understanding of child development was his perception that ‘mother love’ had evolutionary value, and that some behaviours that young children show with their parents might be biologically determined to support survival. Bowlby was no biological determinist, however, and fully recognized the importance of the quality of parental care and the potentially harmful effects of its loss early in life. His theory of attachment proposed that the need for safety is an organizing system in the psychosocial development of children, and that this is different and separate from the need for food or sex. The theory posits both innate characteristics, determined by evolution, and a continuing effect of the experienced environment on the developing child.
Two strands have run through the work that has followed Bowlby's original ideas, developmental and clinical.

Background
Approximately 1 in 200 UK pregnancies ends in stillbirth. Although serious psychological effects of stillbirth on mothers are well established, much less is known about the impact of such loss on fathers.
AimsTo assess the psychological morbidity of fathers in the pregnancy and post-partum year subsequentto a stillbirth, to test within-couple effects and to identify risk factors.
MethodThis was a community-based cohort study of 38 pregnant couples whose previous pregnancy had ended in stillbirth, and 38 pair-matched controls. Psychological assessments took place antenatally and at 6 weeks, 6 months and 1 year postnatally.
ResultsFathers in the index group experienced significant levels of anxiety and post-traumatic stress disorder antenatally, but all of their symptoms remitted postnatally (after the birth of a live baby). Fathers' symptom levels were lower than those of mothers at all time points. In contrast to mothers, fathers experienced greater anxiety when a subsequent pregnancy (following stillbirth) was delayed.
ConclusionsThe vulnerability of fathers to psychological distress during the pregnancy after a stillbirth needs to be recognised.

“Everywhere increased complexity and mind-burdening terminology. What is the teacher to do? And more important, what can the poor student do, confronted with so much new knowledge and a Rabelaisian onomatomania?” (Osier, 1913).
The publication of Tomorrow's Doctors(General Medical Council, 1993) opened our minds to the need for a more thoughtful and evaluated approach to education in medicine. The expansion of medical knowledge has shown no sign of slowing, which makes it imperative that we find ways to prioritise and organise what we teach so that students are given a clear structure. Learning objectives are one of the building blocks that give order to what we do with and for our students.

“The Speciality Tutor is a psychiatrist working in the fields of child and adolescent psychiatry, psychotherapy, forensic psychiatry, learning disability, psycho-geriatrics, or the dependencies (drugs and alcohol), who is regarded as the tutor for the organisation of training in that speciality in the Region. There is one tutor for each Region.” (Royal College of Psychiatrists, 1996)
The speciality tutor is recognised but not appointed by the College. He or she may be appointed by the University, the Regional Post graduate Education Committee, or by a local committee, for example, the local Division of Psychiatry. The appointment, however, must have the agreement of colleagues (Royal College of Psychiatrists, 1996).
