Rapid diagnostic testing (RDT) can provide prompt, accurate identification of infectious organisms and be a key component of antimicrobial stewardship (AMS) programs. However, their use is less widespread in Asia Pacific than western countries. Cost can be prohibitive, particularly in less resource-replete settings. A selective approach is required, possibly focusing on the initiation of antimicrobials, for differentiating bacterial versus viral infections and identifying locally relevant tropical diseases. Across Asia Pacific, more data are needed on RDT use within AMS, focusing on the impact on antimicrobial usage, patient morbidity and mortality, and cost effectiveness. Moreover, in the absence of formal guidelines, regional consensus statements to guide clinical practice are warranted. These will provide a regionally relevant definition for RDT; greater consensus on its role in managing infections; advice on implementation and overcoming barriers; and guidance on optimizing human resource capacity. By addressing these issues, the outcomes of AMS programs should improve.

Objective
To better understand barriers and facilitators that contribute to antibiotic overuse in long-term care and to use this information to inform an evidence and theory-informed program.
MethodsInformation on barriers and facilitators associated with the assessment and management of urinary tract infections were identified from a mixed-methods survey and from focus groups with stakeholders working in long-term care. Each barrier or facilitator was mapped to corresponding determinants of behavior change, as described by the theoretical domains framework (TDF). The Rx for Change database was used to identify strategies to address the key determinants of behavior change.
ResultsIn total, 19 distinct barriers and facilitators were mapped to 8 domains from the TDF: knowledge, skills, environmental context and resources, professional role or identity, beliefs about consequences, social influences, emotions, and reinforcements. The assessment of barriers and facilitators informed the need for a multifaceted approach with the inclusion of strategies (1) to establish buy-in for the changes; (2) to align organizational policies and procedures; (3) to provide education and ongoing coaching support to staff; (4) to provide information and education to residents and families; (5) to establish process surveillance with feedback to staff; and (6) to deliver reminders.
ConclusionsThe use of a stepped approach was valuable to ensure that locally relevant barriers and facilitators to practice change were addressed in the development of a regional program to help long-term care facilities minimize antibiotic prescribing for asymptomatic bacteriuria. This stepped approach provides considerable opportunity to advance the design and impact of antimicrobial stewardship programs.
Neoliberalism and the Musical Entrepreneur

In 2012, the flutist Claire Chase, founder of the International Contemporary Ensemble, received a MacArthur Award for her work as an “arts entrepreneur and flutist.” The award's emphasis on Chase's entrepreneurship reflects the growing demand among classical musicians, educators, and critics for self-driven musical projects, promoted as an engine of classical music's concert culture and as crucial to its renewal in the United States. Entrepreneurship curricula are now in place at almost every music school in the country.
In this article, I offer a critique of the increasingly institutionalized push for musical entrepreneurship, demonstrating that it is rooted in the discourse and ideals of neoliberalism. Drawing on scholarship by economist Guy Standing and political theorist Wendy Brown, I analyze the discourse supporting musical entrepreneurship training, demonstrating the ways it advances neoliberal values through the association of “freedom” and “innovation” with the dismantling of collectivity and valorization of precarious labor structures. This discourse produces an expectation of radical self-sufficiency throughout U.S. society, across multiple economic sectors and including non-economic areas of life. I argue that musical entrepreneurship training serves not as a progressive alternative to other forms of musical career building, but instead habituates musicians to precariousness and insecurity through its rhetoric and institutional endorsement.

We use high-resolution n-body/SPH simulations to study the hydrodynamical interaction between the Large Magellanic Cloud and the hot halo of the Milky Way. We investigate whether the ram-pressure acting on the gaseous disk of the satellite can explain the peculiarities observed in the Hi distribution and the location of the recent star formation activity.

Objective
An increasing focus on legislation, policy and guidance on the nutritional content of school food has in part been in response to the limited impact of more behavioural or educational approaches. However, there is a risk that a sole focus on policy-level action may lead to neglect of the important contribution that more behavioural approaches can make as components of effective, coordinated, multilevel action to improve the dietary intake of schoolchildren. The current paper aims to highlight the potential importance of viewing alternative approaches as complementary or synergistic, rather than competing.
DesignThe socio-ecological and RE-AIM frameworks are used to provide a theoretical rationale and demonstrate the importance of explicitly identifying the interdependence of policies, interventions and contextual structures and processes. School food case study evidence is used to exemplify how understanding and exploiting these interdependencies can maximise impact on dietary outcomes.
SettingCase studies of trials in schools in the UK (South West England and Wales) and Australia (Victoria).

ResultsThe case studies provide examples to support the hypothesis that the reach, effectiveness, adoption, implementation and maintenance of school food policies and interventions can be maximised by understanding and exploiting the interdependence between levels in the socio-ecological framework.
ConclusionsRather than being seen as competing alternatives, diverse approaches to improving the diets of schoolchildren should be considered in terms of their potential to be complementary and synergistic, acting at multiple levels to improve acceptability, fidelity, effectiveness and sustainability.

Summary
Abstract In this chapter we summarize integrated investigations carried out at the Randa rock-slope instability in Matter Valley, Switzerland, between 2000 and 2010. We present a 3D geometric and structural model of the current instability, which consists of 5–6 million m3 of crystalline rock. We also document the complex kinematic behavior and discuss the driving factors for observed slope movements. We show that both the May 1991 failure and the current instability are bounded laterally by the same large-scale fault, and at the base by a planar or stepped rupture surface daylighting at the contact with the Randa orthogneiss. The spatial distribution of current displacements indicates toppling in the upper section of the instability between 2200 and 2400 m asl and sliding in the lower area between 1900 and 2200 m asl. Continuous displacement time series from the surface and deep boreholes show increasing deformation rates when ground surface temperatures decrease in fall and a decrease after snowmelt in spring as the rock warms. We have not detected displacement signatures related to heavy rainstorms or snowmelt. Mapping of the locations of springs and recordings of borehole pore water pressure demonstrate locally perched groundwater in open fractures, and a low regional groundwater table located at or below the basal rupture surface. Numerical modeling results support the hypothesis that thermo-mechanical coupled deformation resulting from annual temperature changes and critically stressed fractures in a complex topography is the primary mechanism driving deep-seated displacements at Randa.

The functions and structure of the pericardial glands (Keber's organ) of bivalves were reviewed by White (1942). The cells of this gland have been considered to have an excretory role and Grobben (1887) suggested that the cells of the pericardial gland could be compared with the chloragogen cells of annelids.
