Antipsychotic-induced weight gain: exploring the role of psychiatrists in managing patients’ physical health – challenges, current options and direction for future care

Antipsychotics and severe mental illness (SMI) are associated with weight gain, and obesity increases the risks of cardiometabolic disease and premature death. These present management and liability issues for psychiatrists. Physical healthcare for people with SMI is poor, and this may partly be owing to training limitations and lack of proactiveness by psychiatrists. Ethically and legally, psychiatrists have a duty to avoid unnecessary harm and to maintain an acceptable standard of care. This would apply particularly to patients receiving compulsory treatment for their SMI owing to their vulnerability. Discrepancy between psychiatric and non-psychiatric approaches to pharmacological treatment creates ambiguity, and weight gain could demotivate antipsychotic adherence. This article explores how the Mental Health Act could be used to address these issues, and the ethical considerations, and proposes how long-acting glucagon-like peptide-1 receptor agonists could be introduced into existing psychiatric practice as a treatment option for antipsychotic-induced weight gain and obesity in SMI.
Public awareness of tetralogy of Fallot after Jimmy Kimmel Live! television episode: A cross-sectional analysis

Background
Persons with rare disorders, such as tetralogy of Fallot, often feel socially isolated due to poor public awareness of the disorder. On 1 May 2017, Jimmy Kimmel aired a segment on Jimmy Kimmel Live! highlighting the impact of tetralogy of Fallot on his son and how the public can learn more about the disorder.
MethodsWe tracked public interest in tetralogy of Fallot using Google Trends and Twitter after the episode and constructed an autoregressive integrated moving average algorithm to calculate search volumes had Kimmel not aired the episode.
ResultsGoogle searches and the number of Tweets for tetralogy of Fallot increased by 3063.27% and 4672.62%, respectively, above expected.
ConclusionsOur findings indicate that television talk shows may represent strong outlets for increasing public awareness of rare disorders.

Purulent bacterial pericarditis is an uncommon infection that manifests during childhood, and in the post-antibiotic era Streptococcus pneumoniae is an unusual cause. We report a case of purulent bacterial pericarditis in a neonate caused by Streptococcus pneumoniae serotype 7F. Although cases of bacterial pericarditis caused by Streptococcus pneumoniae as a causative agent have been reported, their combination in a neonate is unique and this is, to our knowledge, the first case of this combination in the newborn period.

We report on 2,688 nosocomial infection episodes involving 3,238 pathogens in Taiwan's only children's hospital. The mean annual incidence density was 4.06 cases per 1,000 patient-days. The relative risk was highest in intensive care units, oncology wards, and neonatal observation wards. Very young, low-birth-weight, critically ill, and oncology patients were most at risk.

Dielectrophoresis switching with vertical microelectrodes in the side wall of microchannel have been designed, fabricated and tested. A set of interdigitated electrodes in the side wall of the microchannels is used for the generation of non-uniform electrical field to introduce DEP force to repel or attract beads/cells to the sidewalls. A countering DEP force is generated from another set of electrodes patterning on the opposing side walls. These DEP forces can be adjusted by the voltage and frequency applied. By manipulating the coupled DEP forces, the particles flowing through the microchannel can be positioned at different equilibrium points along the width direction and continue to flow into different outlets. The objects of interest can be switched to desired channel outlets in the flow with no need of any moving parts and the down-stream channel can be more than three outlets. Experimental results for switching cells to two outlets and polystyrene microbeads to five outlets have been achieved. The effect of the geometry and flow rate on the performance of the switching was studied and an analytical solution for the optimal design and operation of DEP electrode arrays has been derived from the proposed model.

The infusion of advanced technology into defense systems is accelerating, by necessity, to keep pace with the exponential growth in information accessibility. Critical to national security and global stability, military forces must sustain their technological edge over potential adversaries. It is obvious that such an advantage cannot be maintained by sole reliance on protecting access to high-tech information.

This paper gives an overview of the recent developments in impact, friction, and wear testing of polycrystalline silicon (polysilicon) based micro electromechanical structures. Impact-friction actuated micromechanisms form a new type of actuators. In this type of actuators, lateral resonant structures are retracted by electrostatic comb drives and are propelled forward toward the actuated micromechanism by elastic forces generated by folded beam flexures at frequencies ranging from 10 kHz to 20 kHz. This sequence generates normal impact and tangential friction contact between the two microstructures, raising wear concerns. Two sorts of impact test structures are introduced in this paper, One with a resonant micro impact bumper (MIB) striking a stationary impact wall anchored ofn the substrate. Another is a testing of two MIBs driven to impact each other. Two types of impact actuators are also described, an impact actuated micro angular oscillator (MAO) and a polysilicon micro vibromotor. The vibromotor is a rotor driven by oblique impact on the rim by a converter pointer tip attached to a lateral resonator. Some initial results of impact wear testing as well as static and dynamic friction done by researchers in the field are also described in the paper. Finally, many areas where material scientists can contribute to this field are suggested.

We call attention to the possible importance of pH-dependent speciation in determining the distribution coefficient of americium in a granitic ground water. We obtained experimental distribution coefficients for americium. We used the geochemical code PHREEQE to calculate the amount present of americium complexes with hydroxides and carbonates over a range of pH, using formation constants taken from the literature and one estimated for AmOHCO3, a neutral complex suspected of playing an important role in the aqueous chemistry of americium, although not experimentally evaluated. Combining the above inputs, we calculated individual distribution coefficients for americium complexes. We are able to show that the batch distribution coefficient for americium can be seen as the sum of individual distribution coefficients of various complexes.
